One Case of Diabetic Peripheral Polyneuropathy Improved by Binsosan-gamibang (빈소산가미방(檳蘇散加味方)으로 호전된 당뇨병성 말초신경병증 치험1례)

  • Objectives : This study it designed to evaluate the effects of an oriental medicine therapy, namely Binsosan-gamibang, on diabetic peripheral polyneuropathy. Methods : The clinical data was analyzed on a patient with diabetic peripheral polyneuropathy due to subyeolongsung(濕熱壅盛), gihyeoloeche(氣血瘀滯) whose main symptoms were pain and numbness in both legs. The patient was admitted at the internal medicine department of Dae-Gu Hanny University Dae-Gu Oriental Medicine Hospital on December 22, 2003, and was treated with herbal medicine(Binsosan-gamibang), acupuncture and physical therapy. Results : After treatment, improvement in pain and numbness in both legs was seen in the subsection of the pain rating score(PRS) and the visual analogue scale(VAS). Conclusions : This study suggests that Binsosan-gamibang is significantly effective in treatment of diabetic peripheral polyneuropathy.

The Effects of a Neck Exercise using a PNF Neck Pattern on the Balance and Numbness of Both the Upper Extremities and Neck Motions in Patients with Cervical Myelopathy - Single Subject Design - (PNF 목 패턴을 이용한 목 운동이 경추척수증 환자의 균형, 양팔의 저린감 및 목의 움직임에 미치는 영향 - 단일 사례 연구 -)

  • Purpose: The purpose of this study was to investigate the effects of a neck exercise using a proprioceptive neuromuscular facilitation (PNF) neck flexion and extension pattern on body balance in a progressive range of positions (supine, prone on elbow, and sitting), on numbness in the upper extremities, and on neck flexion motions in cervical myelopathy patients. Methods: One participant who was diagnosed with cervical myelopathy participated in this study. A reversal design (A-B-A') was used. The A and A' were the baseline period (no intervention), and B was the intervention period. The intervention used a neck extension pattern with a hold-relax technique and a neck flexion pattern with a combination of isotonic techniques in the supine position. Then, neck flexion and extension patterns were applied together with a reversal technique for stabilization, followed by a neck extension pattern with a combination of isotonic techniques in the prone position on the elbows. Finally, a neck flexion and extension pattern was used with a stabilizing reversal technique, and a neck extension pattern was applied with a combination of isotonic techniques in the sitting position for 60 minutes per day, 3 times per week for 8 weeks. To measure balance, numbness, and neck motion during neck flexion, the one-leg stand test and the visual analogue scale were used. Results: The right and left one-leg stand tests showed increased balance ability in the intervention phase. Upper extremity numbness was decreased in the intervention phase, and neck flexion motion was increased in the intervention phase. These increases were maintained after the intervention (Baseline II). Conclusion: These results suggest that a neck exercise using a PNF neck pattern with additional techniques in a progressive range of positions has a positive effect on cervical myelopathy patients for balance, numbness, and neck motion.

Clinical Study of Sobokchugeo-tang's Effects on 4 Cases of Complications of Post-hysterecomy (자궁절제술 후 후유증상을 소복축어탕을 중심으로 치료한 증례 보고)

  • Objectives: This study was reported to confirm the effect of Sobokchugeo-tang (SC) on complications of post-hysterecomy. Methods: Four women who were operated a hysterectomy within a month were enrolled in this study. They complained of abdominal pain, lumbago, both leg numbness, dizziness, etc. And received oriental treatments to decrease pain and improve the symptoms. Sobokchugeo-tang 3 times a day was prescribed, their complcation were decreased compared with first visit. Results: A hysterecomy can lead to comlications. They complained of various symptoms. After the treatment, abdominal pain, both leg numbness were rapidly decreased and lumbago, dizziness were improved too. Conclusions: These clinical cases indicated that sobokchugeo-tang was effective in the treatment of women who undertaken a hysterecomy.

  • Diabetes mellitus is a metabolic disorder characterized by hyperglycemia which results from one or both of decreased insulin secretion and increased insulin resistance. Chronic hyperglycemia causes damage to the eyes, kidneys, nerves, heart and blood vessels. The major goal in treating diabetes mellitus is controlling elevated blood sugars without causing abnormally low levels of blood sugar. A 67-year-old woman was admitted with hyperglycemia, thirst, Lt. leg numbness and both leg weakness. The patient was diagnosed as So-gal(消渴) due to dry ness-heat based on yin-deficiency(陰虛燥熱) through oriental medicine and NIDDM through western medicine. The patient was treated with oriental and western medicines. Specifically herbal medicine(Gamijihwangtang), acupuncture, moxa theraphy were used as well as oral medications. Clinical symptoms improved and hyperglycemia dramatically stablized.

  • Symptoms of Psychosomatic disease are various. Mamok(麻木), one of that symptoms, is a kind of sensory disorder and similar to numbness but more complicated. In this report, we described two men diagnosed as psychosomatic disease and undifferentiated somatoform disorder. One complained Mamok of his tongue and the other complained it of his right arm and leg. And both of their symptoms were disappeared quickly after treated by Hwa-acupuncture.

Effect of Stroke Health Education on Knowledge of Stroke for Rural Elders (일 지역 농촌 노인의 뇌졸중 교육이 뇌졸중 지식정도에 미치는 효과)

  • Purpose: This study was conducted to test the effect of a 5 weeks health education program on stroke for rural elderly people. Methods: A total of 55 rural elders in Kok Sung County were participated in the health education on stroke program. Descriptive statistics, and paired t-test were used for statistical analysis with SPSS 12.0. Results: The results are summarized as follows : Mean age was 67.3 years and 88.2% of participants were female. The mean pre-test scores for knowledge of stroke were 0.35 and post-test, 0.85 (p< .001). Percentage of accurate response rates of warning signs of stroke were sudden dizziness pre-test 45.5% post-test 80.0%, sudden trouble speaking or understanding, pre-test 27.3% post-test 41.8%, sudden trouble walking, loss of balance or coordination, pre-test 32.7% post-test 70.9%, sudden numbness or weakness of the arm or leg, especially on one side of the body, pre-test 47.3% post-test 72.7%, sudden confusion, pre-test 27.3% post-test 81.8%, sudden numbness or weakness of the face especially on one side of the body, pre-test 40.0% post-test 76.4%, sudden severe headache with no known cause, pre-test 40.0% post-test 70.9%, Sudden trouble seeing in one or both eyes, pre-test 38.1% post-test 66.0%, nausea or vomiting pre-test 16.4% post-test 43.6%. The increases after the health education on stroke were statistically significant. Conclusions: Health education on stroke for rural elderly people are needed and should focus on community health care programs, especially for those who are older, had a low level of education, and low socio-economical status.

Associated Factors of Clinical sign in Herniated Lumbar Intervertebral Disc (요추간판탈출증의 임상증상에 관련요인)

  • Purpose: This study was undertaken to evaluate the effect analysis of clinical sing in 40 patients with lumbar intervertebral disc. Methods: All patients were assessed by the low back pain protocol which was composed of symptom, clinical sing before and after the treatment. The effect of clinical sing was analyzed by age, occupation, duration of symptom. Results: Occupation by clinical sing of after the treatment was desking job s from 3.6 to 5.2 scores(p<0.05). Duration of symptoms by clinical sing of after the treatment was over 4 months from 3.8 to 5.5 scores(p<0.05). Clinical sing of after the treatment was straight leg raising test from 1.275 to 1.850 scores, sensory disturbances from 1.425 to 2.575 scores, manual muscle testing from 1.525 to 4.625 scores. Conclusion: Herniated disks are most common in middle age, especially between 35 and 45, due to aging-related degeneration of the disks. Significant or increasing pain, numbness or weakness spreading to one or both legs. Progressive loss of sensation in areas that would touch a saddle (inner thighs, back of legs and area around the rectum).

Two Case Reports and an Updated Review of Spinal Intraosseous Schwannoma

  • We report two rare cases of spinal intraosseous schwannoma (SIS) with sustained myelopathy symptoms and provide an updated review regarding SIS in the literature. A 71-year-old man experienced right lumbocrural pain and gait disturbance accompanied with paresthesia and right leg weakness. Imaging examinations revealed a mass with lesions in L4 vertebral body causing bone destruction and spinal cord compression. Complete resection of the well-demarcated tumor and posterior fusion were performed. A 54-year-old female reported bilateral gait disturbance, paresthesia, and numbness without weakness, and imaging revealed a posterior mass from T9 causing spinal cord compression and bone erosion. The tumor was completely separated from the spinal nerve root. The tumors from both patients were confirmed as schwannomas. Tumor recurrence was not observed at the 2-4 year follow-up. Although rare, SIS should be considered during differential diagnosis and can affect treatment planning. SIS symptoms vary depending on tumor location, and fusion is frequently necessary for spinal reconstruction after complete tumor resection.

  • After this study: I report the following result from it. 1. Beriberi is defined as sensory or motor disorder of leg with a painful-swelling and pyrexia. If it progress severely, the disease transmits to heart and cause numbness of the lower abdomen, vomiting, anorexia, palpitation, chest distress and confusion. 2. The external etiologic factors of Beriberi are wind-toxic pathogen, dampness, improper diet and climate sickness, the internal etiologic factors are deficiencies of the kidney, both Gi and blood and original vital energy. 3. The treatment of Beriberi is free-going of blockage, because it is a blocked disease. 4. Danggwiy-juntongtang and Ganghwar-dochetang is the most in the order of frequency of use. Mahwang-juagyongtang, Banha-juagyongtang, Daehwang-juagyongtang, Shinbi-juagyongtang are used in sequence. 5. The prescriptions of Beriberi are made up of the herbs that clear heat, for example, Ganghwar, Changchul, Mahwang and Bangpung, and the herbs that reinforces Gi, for example, Insam and Bakchul.

Gender Differences in Cardiac Knowledge and Symptoms Recognition in Patients with Acute Coronary Syndromes (일 대학병원에 내원한 급성 관상동맥증후군 환자의 성별에 따른 질병지식 및 증상인지 비교)

  • Purpose: The purpose of this study was to compare the cardiac knowledge and symptoms recognition between men and women with acute coronary syndrome (ACS). Methods: Cross-sectional survey research design and convenience sampling were used in this study. 64 men and 42 women from a university medical center were participated in this study. Data collection were used with self reported questionnaires and medical records. Results: There was no difference in cardiac knowledge by gender. Women marked the higher score in symptoms recognition than men. Significant gender differences were observed in the reports of several symptoms (headache, nausea, palpitation, hand paresthesia, and leg numbness) and with ACS. There was significant correlation between cardiac knowledge and symptom recognition on both men and women. Conclusion: These findings suggest that health care professionals should pay attention to the differences in clinical symptoms between women and men. Furthermore, tailored information about possible symptoms of coronary artery disease according to the patient's gender is needed.
