골종양에서 탈륨 스캔의 정량적 분석 (Quantitative Analysis of Thallium-201 Scintigraphy in Bone Tumor)

  • 목적: 골종양이 의심되는 환자들에게 탈륨 스캔을 시행하여 섭취 정도를 정량적으로 측정하고 비교 분석하여 탈륨 스캔의 악성 골종양 조직에 대한 판별 능력을 규명하고자 한다. 대상 및 방법: 골종양이 의심되는 82명의 환자에 대하여 조직 생검 전에 다른 영상검사와 함께 전향적으로 탈륨 스캔을 시행하였다. 스캔의 결과는 정성적 판독과 정량적 탈륨 섭취율을 측정하였으며, retention index(delayed/early phase의 탈륨 섭취량)를 구하였다. 결과의 분석을 위하여 고등급 악성 골종양 군, 양성 골종양 군, 거대세포종 군 및 저등급 악성 골종양 군으로 나누고 각 그룹간의 통계적 유의성을 조사하였다. 결과: 탈륨 섭취율의 정량적 측정에서는 고등급 악성 골종양 군에서는 early phase에 평균 4.14, delayed phase에서는 평균 2.26였으며, 양성 골종양 군에서는 각각 1.16과 1.09, 거대세포종 군에서는 3.15와 1.94, 저등급 악성 골종양 군에서는 1.41과 1.31이었다. Retention index는 고등급 악성 골종양 군에서는 평균 0.62, 양성 골종양 군에서는 0.97, 거대세포종 군에서는 0.66, 저등급 악성 골종양 군에서는 0.93이었다. 고등급 악성 골종양 군은 early phase와 delayed phase의 탈륨 섭취율이 거대세포종을 제외한 양성 골종양 군 보다 유의하게 높았으며(p<0.001), retention index는 유의하게 낮았다(p<0.001). 결론: 탈륨 스캔은 악성 골종양과 대부분의 양성 골종양을 구분 할 수 있는 유용한 검사라고 사료되지만, 거대세포종의 경우 악성 골종양 같이, 저등급 악성 골종양의 경우 양성 골종양 같이 나타나는 특성에 유의하여 사용하여야 할 것이다.

  • Purpose : This study was performed to evaluate the diagnostic value of both planar and SPECT imagings by comparing the results with the clinical and other imaging modalities findings. Materials and Methods: A total of 578 joints with TMJ complaints were examined using planar scintigraphy and SPECT. The planar scintigraphy and SPECT findings were analyzed and compared to the clinical findings of pain, clicking, crepitus and limitation of mouth opening. Moreover we compared the accuracy of the planar scintigraphy and SPECT methods with the one of the panoramic, transcranial, tomographic and MR imaging methods. Results: The planar scintigraphy and SPECT methods showed a high sensitivity of 0.76-0.84 and low specificity of 0.25-0.45 toward the clinical findings: pain, clicking, crepitus and mouth opening restriction. Simple radionuclide uptake ratio was high in each group of patients with pain, crepitus, limitation of mouth opening (p < 0.05) complaints, in each group with positive bone changes on panoramic, transcranial or tomographic images, and in disc displaced group. Conclusion: Although both planar and SPECT imagings have limited specificity, these techniques are sensitive for detection of internal derangement of the TMJ.

Cerebral Infarction Mimicking Skeletal Metastases on Tc-99m MDP Bone Scintigraphy

  • A 6000-year-old male with carcinoma of the prostate and cerebral infarction underwent a Tc-99m MDP bone scintigraphy for the evaluation of skeletal metastases. Bone scintigraphy (Fig. 1) showed multiple areas of increased uptake of Tc-99m MDP in the skull, spine, and ribs representing skeletal metastases. Two different patterns of uptake occurred in the skull region (Fig. 1A-C); one represents bony metastasis and the ether represents cerebral infarction. The shape, size, location, intensity, and border of the increased uptake differed between the two lesions. An oval-shaped pattern with smaller size, greater intensity and more sharply defined border in the frontal region was consistent with bony metastasis. A rectangular-shaped pattern with larger size, lesser intensity and relatively indistinct border in the temporo-parieto-occipital region was consistent with cerebral infarction. Increased uptake of bone-seeking radiotracers in cerebral infarction has been reported previously.$^{1-4)}$ A suggested mechanism by which bone-seeking radiotracers accumulate in the necrotizing cerebral tissue is an alteration of the blood-brain barrier induced during cerebral infarction, which results in entry of the radiotracers into the extracellular space of the brain.$^{4)}$ Brain CT (Fig. 2) performed 7 days before and one month after the bone scintigraphy revealed lesions on the right temporo-parieto-occipital region consistent with acute hemorrhagic and chronic cerebral infarction, respectively.

  • One of common causes of cold defects on bone scintigraphy is an attenuation artifact. Cold defects can be produced by belt buckles, buttons, earring, necklaces, or other metal objects. Cold lesions in the abdomen are also produced by retained barium in the gastrointestinal tract after recent barium studies. We observed artifacts due to retained barium in the colon on bone scintigraphy, which simulated cold bone metastases in two patients with extraskeletal cancer.

  • Osteopoikilosis is a rare, benign hereditary disease, which presents multiple osteosclerotic, and small round nodules in the bone. It is usually detected incidentally by radiological examination. A radionuclide bone scintigraphy is essential in distinguishing osteopoikilosis from osteoblastic metastases, because scintigraphic findings are usually normal in patients with osteopoikilosis. However, there have been no reports about F-18 fluorodeoxyglucose (FDG) PET findings in osteopoikilosis. Herein, we wish to report a case of osteopoikilosis with breast cancer, which could not be seen in either bone scintigraphy or F-18 FDG PET/CT.

Multiple Inputs Deep Neural Networks for Bone Age Estimation Using Whole-Body Bone Scintigraphy

  • The cosmetic and behavioral aspects of aging have become increasingly evident over the years. Physical aging in people can easily be observed on their face, posture, voice, and gait. In contrast, bone aging only becomes apparent once significant bone degeneration manifests through degenerative bone diseases. Therefore, a more accurate and timely assessment of bone aging is needed so that the determinants and its mechanisms can be more effectively identified and ultimately optimized. This study proposed a deep learning approach to assess the bone age of an adult using whole-body bone scintigraphy. The proposed approach uses multiple inputs deep neural network architectures using a loss function, called mean-variance loss. The data set was collected from Chonnam National University Hwasun Hospital. The experiment results show the effectiveness of the proposed method with a mean absolute error of 3.40 years.

골 신티그라피를 시행한 환자의 림프구에서 방사선 적응반응의 유도에 관한 연구 (Induction of Radiation Adaptive Response in Lymphocytes of patients Undergoing Bone Scintigraphy)

  • 목적: 핵의학과에서 가장 흔하게 이용되는 골 신티그라피가 인체에 방사선 장해를 일으키는지 아니면 오히려 적응반응을 통하여 유리한 작용을 하는지 여부를 알기 위하여 이 연구를 시행하였다. 대상 및 방법: 골 신티그라피를 시행한 22명의 환자(남자 6명, 여자 16명, 평균 연령 $50{\pm}14$세)를 대상으로 Tc-99m-methylene diphosphonate (Tc-99m-MDP) 주사 전과 주사 후 4시간에 각각 5 ml씩 채혈하여 배양하고 배양 46시간 후에 Cs-137조사기(central dose rate=654 Gy/h, Gammacell 3000 Elan, Nordion, Canada)를 이용하여 2 Gy의 감마 방사선을 조사하였다. 대조군 10명의 혈액을 채혈하여 Tc-99m-MDP 주사전에 채혈한 군과 같은 방법으로 조사하고 배양하였다. Colcemid 처리 2시간 후에 수확하여 세포 600개당 불안정 염색체인 반지형과 이 중 중심체형 염색체의 숫자를 계수하여 각 군 간의 불안정염색체계수를 분산 분석으로 비교하였다. 결과: Tc-99m-MDP를 주사하기 전에 채혈하고 2 Gy를 조사한 군과 대조군에서 2 Gy를 조사한 군의 불안정염색체의 수는 유의한 차이가 없었다(각각 $385.1{\pm}30.5$, $367.8{\pm}36.6$, p>0.05). 하지만 Tc-99m-MDP를 주사한 후에 채혈하고 2 Gy를 조사한 군에서 불안정 염색체의 수가 유의하게 감소하였다($192.6{\pm}22.1$, p=0.0001). 결론: 골 신티그라피에 의한 저선량의 방사선 조사에 의해서 말초혈액 림프구가 적응반응을 나타내어 다음에 이어지는 방사선 장해에 대해 저항성을 갖는 다는 사실을 알 수 있었다.

즉시형과 지연형 치아 임플란트에서 Tc-99m-MDP의 Bone Uptake 평가 (Evaluation of Bone Uptake on Tc-99m-MDP in Immediate and Delayed Dental Implants)

  • This investigation aimed to determine the relative merit of osseointegration in immediate and delayed implantation in the dog mandible using radiography and bone scintigraphy. five adult mongrel dogs with a mean weight of 8.5 kg were used in this investigation. During the entire study period. all dogs were fed with a soft commercial diet and water ad libitum to minimize functional loading of the implant. Twenty titanium alloy systems 4 mm in diameter and 10 mm in length blasted with calcium phosphate were prepared for insertion. The second and third left mandibular premolars in each dog were extracted for the delayed implant insertion. Twelve weeks later, the second and third right mandibular premolars were extracted for the immediate implant insertion. Before the delayed and immediate implantation procedures and 0, 4, 8, and 12 weeks after the insertions, radiography and bone scintigraphy were conducted. Bone scans were obtained using a large field of view gamma camera equipped with a collimator about 3 hours after intravenous injection of Tc-99m-MDP to the dogs. All the dogs were evaluated weekly for inflammation, necrosis, and other of the bone or sort tissue. Significant macroscopic lesions were not detected. Radioisotope scintigraphy with Tc-99m-MDP hat proved to be a reliable method for measuring increased bone activity at specific skeleton tissue sites. In conclusion, osseointegration in peri-implant bone did not differ significantly between the immediate and delayed implant procedures during the experimental period. The immediate implant may be an alternative treatment of implant insertion in animals.

근골격계 종양에서 탈륨 스캔의 역할 (The Role of Thallium-201 Scintigraphy in Bone and Soft Tissue Tumor)

  • Thallium-201 scintigraphy is used to discriminate the malignant bone tumor from the benign by qualitatively and quantitatively, and to predict the response of preoperative chemotherapy in osteosarcoma, by comparing the changes of thallium uptake ratio after chemotherapy to the tumor necrosis ratio. Thallium-201 scintigraphy scan should be done prior to surgical biopsy. PICKER Prism 2000 gamma camera with high resolution parallel hole collimator is usually used for scanning. The patient is injected with 2-3mCi of Tl-201 and the early phase is checked in 30 minutes and delayed phase in 3 hours. The scan images are visually evaluated by a blinded nuclear medicine physician. We could evaluate true positive, true negative, false positive and false negative by the comparison of results with those of biopsy, and calculate positive and negative predictive value(%), sensitivity(%), specificity(%) and diagnostic accuracy(%). For the quantitative analysis of thallium uptake, we drew the region of interest on the tumor side and contralateral normal side as mirror image, and calculated the uptake ratio with dividing the amount of gamma count in tumor side by normal side. We could calculate the percent changes of thallium uptake ratio in early and delayed phase, and compare them to the ratio of tumor necrosis. Thallium-201 scintigraphy proved as useful imaging study to discriminate malignant bone tumor from benign, but had exception in giant cell tumor and low grade malignant bone tumors. We can use T1-201 scan to differentiate the benign from the malignant tumor, and to evaluate the response of preoperative chemotherapy or radiotherapy, and to determine the residual tumor or local recurrence. For the better result, we need to have a more detail information about false positive cases and a more objective and quantitative reading technique.

  • 횡문근융해증은 다양한 원인에 의해서 발생하며, 생명이 위독할 수도 있는 질환이다. 저자들은 횡문근융해증이 있는 환자에서 진단과 치료에 있어 MRI와 골주사의 중요성을 경험하였기에, 이에 그 특징을 보고하는 바이다.
