Osteoporosis Prevalence of Radius and Tibia and Related Factors Using Multiple Bone Sites Quantitative Ultrasound Measurement of the Korean Health and Genome Study Cohort Women (지역사회 코호트 여성의 요골과 경골에서의 골다공증 유병률과 관련 요인 분석)

  • This study was conducted to investigate osteopenia and osteoporosis prevalence of radius and tibia using Quantitative Ultrasound (QUS) and to identify affecting factors of osteoporosis. A total of 4,340 women aged 40 - 69 years, living in Ansung (rural) and Ansan (mid-sized) area, and free of illnesses affecting bone metabolism participated in the community-based cohort study. Among them 4,059 subjects measured radius bone density and 4,089 measured tibia. The T-score threshold, defined as < -1.0 and $\le$-2.5, was used to identify subjects with osteopenia and osteoporosis by WHO criteria. The crude prevalence of osteoporosis in radius and tibia was $8.4\%$ and $23.3\%$ respectively; after adjustment for age, it changed $6.3\%$ and $18.8\%$. In simple logistic regression analysis, the prevalence of osteoporosis increased by aging, non-marital status, low education, low income. Otherwise, high intakes of Ca/P, thiamin, riboflavin, vitamin B6, and vitamin E were decreased osteoporosis prevalence. Compared to the normal BMI (body mass index) group 08.5 $\le$ BMI < 23), the odds ratio (ORs) of the low BMI group (BMI < 18.5), and high BMI groups (BMl25-30, BMI $\ge$ 30) were significantly increased. The OR of osteoporosis decreased across increasing quartiles of intakes of Ca, P and Ca/P. Therefore, maintaining normal BMI and increasing Ca intake and Ca/P ratio may have a beneficial effect on bone health of Korean women.

Correlations of Lumbar and Femoral Bone Mineral Densities with Calcaneal Speed of Sound in Osteoporotic Woman (골다공증 여성에서 요추골 밑 대퇴골 부위의 골밀도와 종골 음속 사이의 상관관계)

  • In this in vivo study, correlations of lumbar and femoral bone mineral densities (BMDs) with calcaneal speed of sound (SOS) were investigated in 36 osteoporotic women. Areal BMDs of the L2-L4 lumbar spine and the right femoral neck were measured by using dual energy X-ray absorptiometry (DEXA). SOS of the right calcaneus was measured by using ultrasound bone densitometry. Pearson's correlation coefficient (r) and level of significance (p) were used to evaluate the correlations between measurements. Lumbar BMD was highly correlated with femoral BMD (r=0.81). Lumbar and femoral BMDs exhibited similar comparable negative correlations with age (r=-0.52 and r=-0.55). A moderate negative correlation was found between calcaneal SOS and age (r=-0.45). Calcaneal SOS was significantly correlated with lumbar and femoral BMDs, with a higher correlation with femoral BMD rather than with lumbar BMD (r=0.54 and r=0.62). However, calcaneal SOS may not be an optimum index for the estimation of BMD of the most important fracture sites, such as the lumbar and the femur, because it showed lower correlations with lumbar and femoral BMDs compared to that with calcaneal BMD. Therefore, the development of a quantitative ultrasound technology for the direct measurement of acoustic properties at the lumbar and the femur is required to estimate BMD of these sites more accurately.

Analysis of Women with Low Back Pain and Bone mineral density (일부 여성에 대한 요통의 관련인자 분석)

  • Objectives: The objective of this study was to investigate the associated factors of low back pain in measurement of women. Methods: The data were collected from women who visited Physical Examination Center of hospital located in Daegu from July 20, 2000 to September 29, 2000. Data from 36 normal in the women. Results: The experience rate for low back pain was 58.3%. The mean age is 56 years. The the lumbar spine of healthy women in age($50\sim59$) was 73.4%, the lumbar spine of women low back pain in age($50\sim59$) was 66.7%, Variables significantly associated with low back pain were weight, education, Exercise time, menopause existence, occupation(p<0.05). The experience for LBP increased as weight increased(Odds ratio = 999.000). The experience for LBP increased as Exercise time decreased(Odds ratio = 1.090), The experience for LBP increased as menopause existence increased(Odds ratio = 0.7111), However all three variables had significant relationship. Conclusions: Results from this study indicated that a statistically significant association between LBP and weight, education. Exercise time, menopause existence, occupation, smoking in $x^2$-test. In logistic regression test. there were related variables.

A study on dose attenuation in bone density when TBI using diode detector and TLD (전신방사선조사(TBI)시 다이오드 측정기(Diode detector) 및 열형광선량계(TLD)를 이용한 골조직 선량감쇄에 대한 고찰)

  • I. Purpose Uniform dose distribution of the whole body is essential factor for the total body irradiation(TBI). In order to achieved this goal, we used to compensation filter to compensate body contour irregularity and thickness differences. But we can not compensate components of body, namely lung or bone. The purpose of this study is evaluation of dose attenuation in bone tissue when TBI using diode detectors and TLD system. II. Materials and Methods The object of this study were 5 patients who undergo TBI at our hospital. Dosimetry system were diode detectors and TLD system. Treatment method was bilateral and delivered 10MV X-ray from linear accelerator. Measurement points were head, neck, pelvis, knees and ankles. TLD used two patients and diode detectors used three patients. III. Results Results are as followed. All measured dose value were normalized skin dose. TLD dosimetry : Measured skin dose of head, neck, pelvis, knees and ankles were $92.78{\pm}3.3,\;104.34{\pm}2.3,\;98.03{\pm}1.4,\;99.9{\pm}2.53,\;98.17{\pm}0.56$ respectably. Measured mid-depth dose of pelvis, knees and ankles were $86{\pm}1.82,\;93.24{\pm}2.53,\;91.50{\pm}2.84$ respectably. There were $6.67\%{\sim}11.65\%$ dose attenuation at mid-depth in pelvis, knees and ankles. Diode detector : Measured skin dose of head, neck, pelvis, knees and ankles were $95.23{\pm}1.18,\;98.33{\pm}0.6,\;93.5{\pm}1.5,\;87.3{\pm}1.5,\;86.90{\pm}1.16$ respectably. There were $4.53\%{\sim}12.6\%$ dose attenuation at mid-depth in pelvis, knees and ankles. IV. Conclusion We concluded that dose measurement with TLD or diode detector was inevitable when TBI treatment. Considered dose attenuation in bone tissue, We must have adequately deduction of compensator thickness that body portion involved bone tissue.

Effect of Contrast-Enhanced $^{18}F$-FDG PET/CT on Measurements of Whole Body Bone Mineral Density and Body Composition by Dual-Energy X-Ray Absorptiometry (조영증강 $^{18}F$-FDG PET/CT가 이중에너지 X-선 흡수 계측법을 이용한 골밀도 및 체성분 측정에 미치는 영향)

  • Purpose : The effect of concomitant use of $^{18}F$-FDG and intravenous contrast agent (CA) on dual-energy X-ray absorptiometry (DXA), was rarely reported. We had investigated these potentially confounding effects. Materials and Methods : Twenty-two patients had undergone DXA before and immediately after $^{18}F$-FDG PET/CT scans. Two DXA and 1 PET/CT scans had performed within one-day. $^{18}F$-FDG PET/CT scans had been performed with CA in 17 patients and without CA in 5 patients. Whole body bone mineral content (BMC), whole body bone mineral density (BMD), total fat mass (TFM), and lean body mass (LBM) were measured by DXA scanner before and after the $^{18}F$-FDG PET/CT scans. Results : BMC, BMD, TFM and LBM had significantly affected by $^{18}F$-FDG PET/CT with CA (BMC, +13.7%, from $2061.3{\pm}393.7$ to $2343.4{\pm}373.3$; BMD, +9.3%, from $1.07{\pm}0.09$ to $1.17{\pm}0.08$; TFM, -34.1%, from $17052.1{\pm}4049.9$ to $11237.1{\pm}2990.3$; LBM, +13.6%, from $45834.5{\pm}5662.1$ to $52094.0{\pm}6335.4$). However, $^{18}F$-FDG PET/CT without CA had no effect on the measurement of DXA (BMC, +2.4%, from $2197.7{\pm}391.6$ to $2251.5{\pm}380.9$; BMD, +1.8%, from $1.13{\pm}0.09$ to $1.15{\pm}0.07$; TFM, -6.8%, from $14585.6{\pm}3455.9$ to $13591.3{\pm}4351.4$; LBM, +2.2%, from $47360.5{\pm}8381.8$ to $48441.1{\pm}8488.1$). Conclusion : The measurements of DXA are affected by using CA. However, DXA scans might be unaffected by the presence of $^{18}F$-FDG administered for PET/CT.

Bone Mineral Density Measurement of Rats Using Dual-energy X-ray Absorptiometry: Precision of In Vivo Measurements for Various Skeletal Sites with or without Repositioning (쥐에서 이중에너지 방사선 흡수법을 이용한 골밀도의 측정: 다양한 골부위에서 재위치 여부에 따른 생체내 측정의 정밀도)

  • Purpose: Bone mineral density (BMD) measurements need to be precise enough to be capable of detecting small changes in bone mass of rats. Using a regular dual-energy X-ray absorptiometry (DXA), we measured many BMD of various skeletal sites in rats to examine precision of DXA in relation to the repositioning on the bones of rats. Materials and Methods: Using DXA and small animal software, scans were performed 4 times in all 12 male rats without repositioning (Group 1a). Another four scans for 6 of 12 rats were done with repositioning between scans (Group 2). Customized regions of interest (ROIs), encapsulate the right hind limb, L1-4, skull and pelvic bones were drawn at each measurement. The precision of the measurements was evaluated by measuring the coefficient of variation (CV) of four measurements of BMD at each skeletal site of all rats with or without repositioning. Significance of differences between group 1b (six rats out of group 1a, which were come under group 2) and group2 were evaluated with Wilcoxon Signed Rank Sum Test. Results: CVs obtained at different skeletal sites of all measurements in Group 1b and 2. It was $3.51{\pm}1.20$, $ 2.62{\pm}1.20$ for the hindlimb (p=0.173), $3.83{\pm}2.02$, $4.59{\pm}2.02$ for L1-4 (p=0.600), $3.73{\pm}1.87$, $1.53{\pm}0.89$ for skull (p=0.046), and $2.92{\pm}0.60$, $1.45{\pm}0.60$ for pelvic bones (p=0.075). Conclusion: Our study demonstrates that the DXA technique has the precision necessary when used to assess BMD for various skeletal sites in rats regardless of repositioning.

Discordance in Diagnosis of Osteoporosis by Skeletal Site in Korean Women: KNHANES 2008-2010 (한국 여성의 골다공증 검진부위에 따른 진단결과 불일치: 국민건강영양조사 2008-2010)

  • Based on an analysis on the difference on the measurement sites and their relations to osteoporosis diagnosis and prevalence, this study attempts to investigate the patterns of prevalence in skeletal sites in the aging process. The samples were driven by the National Health and Nutrition Examination Survey (2008-2010) on Korean females (n = 4,449) aged from 50 to 89 years. The result shows that the differences on affected sites in the aging process were detected as follow: the osteoporosis prevalence in lumbar spine was relatively high in 50s, 60s and 70s; however, the prevalence in the proximal femur (total hip and femoral neck) was higher in 80s. In terms of the diagnosis identified in a single site, the prevalence in lumbar spine seemed to decrease as age increases; but, the rate went up in the figure of femoral neck. Overall, the osteoporosis prevalence for the diagnosis detected in multiple sites was increased as age goes up. In general, the increase in lumbar osteoporosis prevalence starts earlier than that of femoral neck. The findings of this study also suggests that if the proximal femur site is only used for diagnosing women age in 50s, 60s and 70s, the significant portion of osteoporosis patients could be misdiagnosed as normal.

Study on the Cathodic Protection Characteristics of Hot Water Boiler by Mg-Alloy Galvanic Anode(1) (Mg 합금 유전양극에 의한 온수Boiler의 음극방식특성에 관한 연구(1))

  • Corrosion damage of boiler, factory equipment and so forth occur quickly due to using of the polluted water, resulting in increasing leak accident. Especially, working life of hot water boiler using the polluted water becomes more short, and energy loss increases. The cathodic protection method is the most economical and reliable one to prevent corrosion damage of steel structures. Mg-base alloys galvanic anode protection of cathodic protection methode is suitable for the application of hot water boiler using water with high specific resistance such as tap water. This paper is studied on the cathodic protection characteristics of hot water boiler. In tap water solution, the measurement of cathodic protection potential according to the time elapsed is carried out, and behavior of cathodic polarization with current change is investigated. The main results obtained are as follows. In hot water boiler shell, the open circuit potential of base metal become less noble than that of weld Bone, and the current density of base metal becomes low than that of weld zone. The further distance from Mg-alloy galvanic anode, the higher cathodic protection potential of hot water boiler appears. And protective potential becomes high according to pass cathodic protection time and after 6∼10 days become stable.

Studies on the Comparative Analysis Between GE Prodigy and $FRAX^{TM}$ Tool in Absolute Fracture Risk Assessment Tool (골절의 절대위험도 평가방법에서 GE Prodigy와 FRAX Tool의 비교분석에 관한 고찰)

  • Purpose: World Health Organization (WHO) have suggested that an individual's 10-year absolute fracture risk is more reliable than Bone Mineral Density (BMD) measurement as the predictor of osteoporotic fracture. In 2008, Fracture Risk Assessment Tool ($FRAX^{TM}$) was developed by WHO to evaluate fracture risk of patients based on individual's clinical risk factors. The purpose of this study is to offer the comparative analysis of the existing GE prodigy and $FRAX^{TM}$ Tool in Absolute Fracture Risk Assessment Tool. Materials and Methods: 201 women ($55{\pm}3.5$ years) underwent femoral neck BMD measurement using GE Prodigy. The 10-year probability (%) of hip fracture (or a major osteoporosis-related fracture) was estimated using T-scores of GE prodigy and $FRAX^{TM}$. We made a comparative analysis of these data using SPSS (Ver.12). Results: There was a significant difference statistically between T-score ($-0.52{\pm}0.97$) of GE prodigy and T-score ($-1.45{\pm}0.81$) of $FRAX^{TM}$ (r=0.977, p=0.000). Also, there was a significant difference statistically between a major osteoporosis- related fracture ($9.15{\pm}3.71$) of GE prodigy and a major osteoporosis-related fracture ($4.87{\pm}1.51$) of $FRAX^{TM}$ (r=0.909, p=0.000). Moreover, a statistically significant difference was found in the 10-year probability of hip fracture of GE prodigy ($1.56{\pm}1.48$) and of hip fracture ($0.53{\pm}0.61$) of $FRAX^{TM}$ (r=0.905, p=0.000). Conclusions: There was a significant difference statistically between GE prodigy and $FRAX^{TM}$ Tool in Absolute Fracture Risk Assessment Tool. Especially, T-score, a major osteoporosis-related fracture and the 10-year probability of hip fracture that were estimated using GE prodigy tended to show the higher results than one evaluated by $FRAX^{TM}$ Tool. In conclusion, $FRAX^{TM}$ Tool may provide a better tool. The application of $FRAX^{TM}$ Tool as a fracture predictor remains to be clarified.

Quantitative Analysis of Bone Mineral Measurements in Different Types of Dual-energy Absorptiometry Systems: Comparison of CT vs DEXA (이중 에너지 조사 방식의 장비별 골밀도 측정의 정량적 비교 분석: CT vs DEXA 비교)

  • Generally assessing bone mineral density (BMD) were performed on dual energy X-ray absorptiometry (DEXA) the same as dual energy CT (DECT) with a rapid-kVp switching. The purpose of this study is to compare the different of BMD value between DEXA and DECT method, and evaluate usefulness of DECT method. Using scanner for BMD measurements were GE, Healthcare Discovery 750 HD for DECT and Hologic QDR 4500W for DEXA. For compare BMD value in each method, scanned lumbar spine phantom and subjects visiting Korean National Cancer Center from April 2015 to December 2015, records of 50 patients. This study was approved by the Institutional Review Board. The mean BMD value measures for spine phantom and for subjects in each scanners presented strong correlation (r=0.948 with p<0.05 for phantom; r=0.635 with p<0.05 and Kendall's tau $({\tau})=0.46$ with p<0.05 for subjects) and linear relationship between DECT and conventional DEXA. DECT technique for BMD measurement will provide a very useful methodology without additional radiation dose.
