Deep Neck Space Infection Caused by Keratocystic Odontogenic Tumor

  • Keratocystic odontogenic tumor (KCOT) is a benign cystic intraosseous tumor of odontogenic origin. An infection of a KCOT is not common because KCOT is a benign developmental neoplasm. Moreover, a severe deep neck space infection with compromised airway caused by infected KCOT is rare. This report presents a 60-year-old male patient with a severe deep neck space infection related to an infected KCOT due to cortical bone perforation and rupture of the exudate. Treatment of the deep neck space infection and KCOT are reported.

  • Hydatid cyst is usually located in the liver and lungs, rare cases showing localization in other organs or tissues. In the unusual location, echinococcosis is an excluding diagnosis that is established only after microscopic evaluation. Our first case occurred in a 67-year-old female previously diagnosed with pulmonary tuberculosis and hospitalized with persistent pain in the hip joint. The clinical diagnosis was tuberculosis of the joint, but the presence of the specific acellular membrane indicated a hydatid cyst of the synovial membrane, without bone involvement. Fewer than 25 cases of joint hydatidosis have been reported in literature to date. In the second case, the intramural hydatid cyst was incidentally discovered at autopsy, in the left heart ventricle of a 52-year-old male hospitalized for a fatal brain hemorrhage, as a result of rupture of an anterior communicating artery aneurysm. The conclusion of our paper is that echinococcosis should be taken into account for the differential diagnosis of cystic lesions, independently from their location.

  • 말초 신경에 발생하는 결절종은 일부 보고된 바는 있으나, 발생 빈도는 드문 것으로 되어있으며 발생 부위는 비골 신경이 가장 흔한 것으로 보고되고 있다. 이외에도 요골 신경, 척골 신경, 정중 신경, 좌골 신경, 경골 신경 및 후 골간신경에 발생한 경우도 보고되고 있다. 저자들은 내측 족저 신경에 발생한 신경 내 결절종 1예와 총 비골 신경에 발생한 신경 내 결절종 1예를 신경학적 후유증 없이 치료하여 이를 보고하고자 한다.

  • Epidermoid cyst is a cystic form of teratoma and believed to be derived from trapped embryonic cells along the lines of embryonic closure. A 28-year-old woman presented with a painless swelling over the left mandibular area. On panoramic view, the mandible revealed a $5.5{\times}2.0\;cm^2$ multilocular radiolucent lesion of the left mandibular body and a computed tomography scan showed expansion of both the buccal and lingual plates in the same area. Microscopy found stratified squamous epithelium of the cystic wall and cystic contents of keratinized material. The histological diagnosis wasan epidermoid cyst. The most common location of epidermoid cyst at the head and neck is in the orbit (47%), followed by the mouth floor (23%) and the cervical area (9~24%), but in the jaw bone, it is considered very infrequent. We report the uncommon epidermoid cyst in the mandibular body that had a good healing outcome after treatment with a conservative marsupialization during the 40 months follow-up.

감압술과 국소마취하 적출술을 통한 하악골 낭종의 치험례 (Treatment of huge mandibular cyst with enucleation after decompression under local anesthesia)

  • Various treatment methods for huge cystic lesion of the jaw exist, such as, resection of the involved bone, enucleation and decompression. Among these methods, enucleation after decompression is a conservative technique that decreases the size of the cystic cavity and reduces the risk of intrabony defects, which could be induced by primary enucleation. In addition, it can save the adjacent anatomic structures. In these cases, the decompression combined with partial enucleation, which was performed before complete cyst enucleation was performed on huge cystic lesions of the mandible. During the process, a decrease in the size of the lesion and the growth of normal oral tissues was observed. The size of the lesion decreased until the time of complete enucleation, and surgery could be performed under local anesthesia. No damage to inferior alveolar nerve was observed. We report these cases with a review of the relevant literature.

  • PURPOSE. The aim of this study was to determine the frequency of significant panoramic radiographic findings and eventual treatment requirements before conventional or implant supported prosthetic treatment in asymptomatic edentulous patients. MATERIALS AND METHODS. A total of 743 asymptomatic edentulous patients were retrospectively evaluated using a digital panoramic system. We analyzed the radiographic findings, including impacted teeth, retained root fragments, foreign bodies, severe atrophy of the posterior maxillary alveolar bone, mucous retention cysts, soft tissue calcifications and radiopaque-radiolucent conditions. RESULTS. Four-hundred-eighty-seven (65.6%) patients had no radiographic finding. A total of 331 radiographic findings were detected in 256 (34%) patients. In 52.9% (n=175) of these conditions, surgical treatment was required before application of implant-supported fixed prosthesis. However, before application of conventional removable prosthesis surgical treatment was required for 6% (n=20) of these conditions. CONCLUSION. The edentulous patients who will have implant placement for implant-supported fixed prosthesis can frequently require additional surgical procedures to eliminate pathological conditions.

  • Keratocystic odontogenic tumors can occur in any area of the maxilla or mandible. According to their size, location, and relations with surrounding structures, they are treated by cyst enucleation or enucleation after either marsupialization or decompression. Enucleation is performed when cysts are not large and when only minor damage to adjacent anatomical structures is expected. Although marsupialization and decompression follow the same basic bone-regeneration principle, which is to say, by reducing the pressure within the cyst, the former leaves a large defect after healing due to the large fistula necessary to induce the conversion of the cyst-lining epithelia to oral epithelia; the latter leaves only a relatively small defect, because of the continuous washing carried out by means of a tube inserted into a small hole in the cyst. In the latter case too, a decompressor appropriate for the focal position is required, owing to the importance of maintaining the device and controlling for oral hygiene. We report herein decompression treatment with a patient-customized device for an extensive cyst in the anterior region of the mandible.

  • 말초신경초 내에 발생하는 결절종은 일부 보고된 바는 있으나, 발생빈도는 드문 것으로 되어있으며, 발생 부위는 슬관절과 근위 경비관절 부위의 비골신경에서 가장 흔한 것으로 보고 되어 있다. 저자들은 신경학적 증상없이 슬관절부 비골신경 내에 발생한 결절종에 대하여 후유증 없이 치료하여 이를 보고하고자 한다.

  • A fibrin adhesive have been widely used in oral and maxillofacial surgery for microvascular anastomosis, autogenous chip bone grafts, many kinds of soft tissue surgery (vestibuloplasty, bleeding control after extraction, primary healing by covering of suture of a gum after the extirpation of large cysts). There are two principal components in adhesive systems biologically: lyophilized human fibrinogen and bovine thrombin. The fibrinogen component contains coagulation factor XIII and enhance the initial wound healing, which polymerizes soluble fibrin monomers into an insoluble clot. The thrombin is dissolved in a solution of calcium chloride to provide the second component. We applied fibrin adhesive, Beriplast (Behring, Behringwerke AG, D-3350, Marburg, FRD), to 4 patients for fixation of free skin grafting donors who had facial scar around eye, nose, mouth corner which received from accidents, or burn. We have experienced initial accelerated graft fixation between donor and recipient sites with no additional fixation. And It's made easy bleeding control and easy manipulation during operation. But two cases showed partial hypertrophic scar engrowth in above 3 months follow up, but no significant. Histopathological reviews in general were showed similar scar findings such as abundant collagen bundles in H&E, M/T stain, but slight positive signs in elastic and collagen antibody immunopathologic findings in hypertrophic scar cases.

  • 상갑선 설관낭종 및 설관루는 선천성 경부질환중 가장 많은 빈도를 차지하는 질환으로 설저에서 강상선에 이르기까지 갑상선설관의 경로중 어디서나 발견될 수 있으며, 다른 종양이나 낭종과 구별이 용이치 않을 경우가 많다. 단순한 낭종 또는 결절로 오인되어 근치절제가 되지 않았을 경우 흔히 재발을 잘 일으키며 또한 갑상선 조직이 포함될 수 있다. 경우에 따라서는 이것이 유일한 갑상선 조직일 수 있어 절제시에 더욱 신중을 요한다. 저자는 최근 2년동안 본병원 이비인후과에서 낭종또는 루를 가진 갑상선설관 환자 8명을 수술적 요법으로 치료하였던바 이에 대한 문헌고찰과 아울러 보고하는 바이다.
