골밀도검사에서 피검자의 장기선량 측정 및 차폐기구의 효용성 평가 (Evaluation of the Effectiveness of the Shielding Device and the Organ Dose of Subject During Bone Mineral Density)

  • Bone mineral density is a examination to measure the amount of bone in patients with metabolic bone disease. It is a low dose, but may cause unnecessary exposure to the gonads and other organs located in the periphery when examining the lumbar and proximal femurs. Therefore, the purpose of this study was to evaluated the exposure dose for each organ exposed during the bone mineral density through simulation, and analyzed the applicability of the subject to radiation shielding devices using 3D printing materials. As a result, the highest dose was shown at 11.47 uSv in the breast during lumbar examination and 8.98 uSv in the testis during proximal femur examination. Also, the farther away from the examination site, the lower the effect of the scattering-ray. The shielding effect of using 3D printing shielding device showed high results in proportion to the effective atomic number and specific gravity of the printing material. Among the printing materials, ABS + W showed an effect of at least 78.72 to 96.3 9% compared to the existing lead material.

간호대학생의 신체조성과 골다공증 자기효능감이 골밀도에 미치는 영향 (Effect of Body Composition and Osteoporosis Self-efficacy on Bone Mineral Density of Female Nursing Students)

  • Purpose: This study was done to identify the relationship among bone mineral density (BMD), body composition and osteoporosis self-efficacy and to identify predictors of BMD in female nursing students. Method: Participants were 154 nursing students. Osteoporosis self-efficacy was determined by a self-report questionnaire. BMD was measured by ultrasound bone densitometry and body composition by a body composition analyzer. Data were collected between April 1 and 27, 2013 and analyzed using descriptive statistics, ANOVA, Scheff$\acute{e}$ test, Pearson correlation coefficient, and multiple regression with SPSS 18.0. Results: Mean BMD at the calcaneus site was $0.58{\pm}1.31$ (T-score). Incidence of osteopenia was 11.7%. Percentage of body fat (PBF)-defined obesity had higher prevalence than body mass index (BMI)-defined obesity. BMD had significant positive correlations with skeletal muscle mass (r=.226, p=.005) and fat free mass (r=.225, p=.005). The factor predicting BMD was skeletal muscle mass with 4.7% of explained variance. Conclusion: Study results indicate that of body composition components, skeletal muscle mass is the prime predicting factor for BMD. Thus to promote healthy bones, it is important to strengthen the muscles using a program, based on balanced development of all muscles.

  • Peak bone mass is established predominately during childhood and adolescence. It is an important determinant of future resistance to osteoporosis and fractures to gain bone mass during growth. The issue of low bone density in children and adolescents has recently attracted much attention and the use of pediatric dual-energy X-ray absorptiometry (DXA) is increasing. The process of interpretation of pediatric DXA results is different from that of adults because normal bone mineral density (BMD) of children varies by age, body size, pubertal stage, skeletal maturation, sex, and ethnicity. Thus, an appropriate normal BMD Z-score reference value with Z-score should be used to detect and manage low BMD. Z-scores below -2.0 are generally considered a low BMD to pediatrician even though diagnoses of osteoporosis in children and adolescents are usually only made in the presence of at least one fragility fracture. This article will review the basic knowledge and practical guidelines on pediatric DXA based on the International Society for Clinical Densitometry (ISCD) Pediatric Official Positions. Also discussed are the characteristics of normal Korean children and adolescents with respect to BMD development. The objective of this review is to help pediatricians to understand when DXA will be useful and how to interpret pediatric DXA reports in the clinical practice for management of children with the potential to develop osteoporosis in adulthood.

이중에너지 방사선흡수 골밀도 장치의 품질관리 현황 (A Status Report on Dual Energy X-ray Absorptiometry Quality Control in Korea)

  • DEXA는 골다공증 진단에 가장 많이 사용되는 표준 측정법이다. 현재 국내에서 운영되고 있는 DEXA 장치는 5,671대이다. 본 연구에서는 국내에서 설치 운영되고 있는 DEXA장비의 품질관리 실태를 조사하였다. 3,000개 기관을 대상으로 한 설문 조사에서 12.6%의 응답을 받아 그 결과를 분석하였다. 결과에서 77.5%가 일일 품질관리를 시행하고 있으나 Shewhart chart는 68.5%, CUSUM chart는 86.0%가 모르거나 사용하지 않는다고 답하여 정확한 품질관리가 되지 않는 것으로 조사되었다. 정확한 골밀도 검사는 환자의 골절의 위험도를 산정하기 위한 자료로 활용된다. 하지만 부적절한 검사는 오진을 유발할 가능성을 높여준다. 따라서 정확한 골밀도 검사를 위해서는 적절한 보조기구를 사용하고 장치의 품질관리를 반드시 시행하여 장치와 자세에 대한 오차를 줄이고 검사 의료진의 교육을 통해 오류 요인을 줄이는 것이 필수적이다.

초음파방식으로 측정된 골밀도와 신체조성 및 배근력의 관계 (Relationship of Bone Mineral Density Measured by Ultrasound Bone Densitometry and Body Composition or Backmuscle Strength)

  • 본 연구에서는 20대 초반의 남, 여를 대상으로 초음파 방식으로 측정된 골밀도와 신체조성 및 배근력의 관계에 대해 알아보았다. D대학에 재학 중인 61명의 학생에 대해 IRB 승인과 참여자 동의 후 설문조사, 초음파방식 골밀도 및 신체조성, 배근력을 측정하였다. 골밀도는 주로 사용하는 측 종골을 측정하여 T-값을 두 그룹($T-score{\geq}-1.0$; T-score<-1.0)으로 나누었다. 생체전기 저항법을 이용하여 신체조성을 측정하였고, 배근력은 2회 측정하여 평균값을 구하였다. 모든 통계분석은 SPSS ver. 22.0 을 사용하여 실시하였다. 신체활동 또는 체질량지수가 증가할수록 골밀도 및 신체조성의 근육량도 증가하였다. T-값이 높은 그룹이 낮은 그룹 보다 신체조성의 근육량 및 골격근량, 신체발달이 높았지만 통계학적인 유의성은 없었다(p>0.05). 배근력에서도 T-값이 높은 그룹이 낮은 그룹 보다 높았다(101.1 vs. 92.1, p=0.308). 남, 여그룹에서 골밀도 T-값과 신체조성의 골격근과 근육량은 보통 수준의 유의한 양의 상관성을 보였고(p<0.05), 배근력은 신체조성의 골격근과 근육량, 신체발달에서 골밀도 보다 높은 유의한 양의 상관성을 보였다(p<0.001). 여자그룹이 남자그룹 보다 골밀도 T-값과 신체조성의 골격근과 신체발달에서 높은 상관성을 보였다. 남자 그룹에서 배근력과 골밀도의 T-값과는 유의한 상관성을 보이지 않았지만(r=0.176, p=0.319), 여자 그룹에서는 보통 수준의 유의한 상관성을 보였다(r=0.591, p=0.001). 이상과 같이 초음파 방식으로 측정된 골밀도와 신체조성, 배근력은 상호 유의한 관련성을 보였다.

Age-related Reference Intervals for Total Collagen-I-N-terminal Propeptide in Healthy Korean Population

  • Background: Procollagen type I N-terminal propeptide (PINP) is one of the most clinically useful bone formation biomarkers. Therefore, the purpose of this study was to independently evaluate the performance of automated total PINP assay and established age- and gender- specific reference intervals for PINP in healthy Korean population. Methods: The imprecision, linearity, and detection capability of Elecsys total PINP assay was determined and reference interval was established using 599 serums from Korean population with normal bone mineral densities based on bone densitometry. Age groups were divided into 20s, 30s, 40s, 50s, 60s and over. Results: Elecsys total PINP had excellent performance in imprecision, linearity, and detection capability. When partitioning age groups in Korean male and female populations, there was significant difference in total PINP between different age groups. In male populations, PINP level was decreased with increasing age, then it remained steady after middle-age. In female populations, there was a decreasing tendency similar to that in the male population with a sharp increase in the 50 to 59 age group. Conclusions: Elecsys total PINP assay showed precise and reliable performance in our study. We established age-related PINP reference intervals for Korean male and female population with normal bone mineral densities.

40세 이후 남성의 골밀도 관련 요인과 골다공증의 유병률 (Factors Influencing in the Bone Mineral Density and the Incidence of the Osteoporosis among Male Older than 40 Years Old)

  • 의학과 과학의 발전으로 인한 고령인구의 증가로 여성에게 뿐만이 아니라 남성에서도 골다공증에 의한 골절의 빈도가 증가하면서 남성 골다공증에 대한 관심이 높아지고 있다. 이에 본 연구는 40세 이후 남성의 골밀도에 관련된 요인을 알아보고 이러한 병인으로 인한 골다공증의 유병률을 조사하여 남성 골다공증의 예방과 치료대책을 위한 기초 자료를 제공하고자 한다. 건강 검진에서 골밀도 검사를 시행한 40세 이후 남성을 대상으로 한 본 연구에서 L-spine의 골감소증과 골다공증은 각각 45%, 12.9%의 유병률을 보였고, Femur에서는 51.9%, 7.63%의 유병률을 보여 미국이나 유럽에서 50세 이상 남성을 대상으로 한 연구 결과 수치보다 높게 관찰되었다. L-spine의 경우 연령과 골감소증 및 골다공증 유병률이 유의한 차이를 보였고, Femur에서 역시 연령과 골다공증의 유병률이 유의한 연관성을 가지는 것으로 조사되었다. 연령이 증가할수록 L-spine, Femur 두 부위 모두 평균 BMD가 낮아지는 것으로 조사되었고, 체중이 증가할수록, 체질량지수(BMI, kg/$m^2$)가 높을수록 골다공증의 유병률은 감소하는 음의 상관관계를 가졌다. 운동, 흡연, 음주는 골다공증의 유병율과 유의한 연관성이 없는 것으로 조사되었으나, 운동의 횟수, 흡연기간, 음주횟수는 골 양을 변화시켜 남성 골밀도에 영향을 주는 인자가 되고 있다.

소아의 골밀도에 관한 연구 (A Study of Bone Mineral Density in Children)

  • Objectives : Osteoporosis is generally occurred in old age, especially postmenopausal women therefore, it is relatively overlook osteoporosis in children, but the authors have paid attention to bone density in children because maximizing peak bone mass of childhood is advocated as a way to prevent osteoporosis and environmental factor of childhood play role in the regulation of bone density. the present study was performed to measure bone density of children and to assess the influence of age, sex, body size, exercise, weak symptoms on bone density during the period of bone growth. Methods : We have measured bone density of calcaneus bone in 283 children (male 144 female 139, ranged from3 to 18) who visited in growth clinic of Pediatrics, Dongguk University Bundang Oriental Hospital, using quantitative ultra sound densitometry and they were divided into 3 groups by age(3-10, 11-15, 15-18 years). Pearson's correlation was used to assessed the influence of age, body size and Student's t-test was used to detect differences in sex, exercise, weak symptoms between the groups on bone density Results : The bone density correlated with age, height, weight, body mass index in this study. Bone density were significantly higher in males than in female. Digestive weak children's bone density were lower than normal children. Conclusion : Weight-bearing exercise and food rich in nutrition are necessary to increase maximizing peak bone mass in children.

국산 이중에너지 방사선흡수 골밀도 장치와 GE Lunar Prodigy의 교차분석 식 도출에 관한 연구 (Cross-Calibration of Domestic Devices and GE Lunar Prodigy Advance Dual-Energy X-Ray Densitometer Devices for Bone Mineral Measurements)

  • Reliable follow-up of bone mineral density (BMD) by dual energy X-ray absorptiometry (DXA) is essential in clinical practice. When there is a difference in the BMD values from DXA systems in the same patient, cross calibration equation is required for the reliable follow-up. Unfortunately, no equation is existed in BMD measure between GE Lunar Prodigy Advance (US, GE Healthcare; LPA) and Osteosys Dexxum T (Korea, Osteosys; ODT) DXA systems. In this study, we evaluate the agreement of BMD values between LPA and ODT and suggest the cross calibration equation using European spine phantom (ESP) with two systems. We performed BMD measurements using ten scans with ESP in each DXA systems. We compared BMD values and calculated cross calibration equation by linear regression analysis. The comparison between the LPA and ODT bone densitometers used the ESP. Compared to the ESP BMD values, ODT underestimated 14.36% and LPA overestimated 12.96%. The average of total BMD measurement values acquired with ODT were 21.44% lower than those from LPA. Cross-calibration equation for LPA and ODT was derived from ESP. We calculated simple cross calibration equation for LPA and ODT DXA systems. Cross-calibration equation is necessary for the reliable follow-up of BMD values in two different systems.

Short Segment Fixation for Thoracolumbar Burst Fracture Accompanying Osteopenia : A Comparative Study

  • Objective : The purpose of this study was to compare the results of three types of short segment screw fixation for thoracolumbar burst fracture accompanying osteopenia. Methods : The records of 70 patients who underwent short segment screw fixation for a thoracolumbar burst fracture accompanying osteopenia (-2.5< mean T score by bone mineral densitometry <-1.0) from January 2005 to January 2008 were reviewed. Patients were divided into three groups based on whether or not bone fusion and bone cement augmentation procedure 1) Group I (n=26) : short segment fixation with posterolateral bone fusion; 2) Group II (n=23) : bone cement augmented short segment fixation with posterolateral bone fusion; 3) Group III (n=21) : bone cement augmented, short segment percutaneous screw fixation without bone fusion. Clinical outcomes were assessed using a visual analogue scale and modified MacNab's criteria. Radiological findings, including kyphotic angle and vertebral height, and procedure-related complications, such as screw loosening or pull-out, were analyzed. Results : No significant difference in radiographic or clinical outcomes was noted between patients managed using the three different techniques at last follow up. However, Group I showed more correction loss of kyphotic deformities and vertebral height loss at final follow-up, and Group I had higher screw loosening and implant failure rates than Group II or III. Conclusion : Bone cement augmented procedure can be an efficient and safe surgical techniques in terms of achieving better outcomes with minimal complications for thoracolumbar burst fracture accompanying osteopenia.
