Management of Traumatic Cerebrospinal Fluid Rhinorrhea using External Ethmoidectomy Approach (비외사골동수술법을 이용한 외상성 뇌척수액 비루의 치료)

  • Cerebrospinal fluid (CSF) rhinorrhea usually occurs as a result of trauma including operation. Unheated CSF rhinorrhea may induce major morbidity such as meningitis and brain abscess, etc. This paper presents a review of four cases of traumatic CSF rhinorrhea Sites of CSF leakage were easily found out by intrathecal fluorescent dye injection. Surgery was performed by external ethmoidectomy approach and dural tear and bone defect was repaired with abdominal fat and free mucosal graft taken from amputated middle turbinates. We conclude that repair using free fat and mucosal graft via external ethmoidectomy approach could be accepted as the intial method of CSF rhinorrhea management.

  • This case report describes the symptoms and clinical course of a 35-year-old female patient who was diagnosed with a temporo-sphenoidal encephalocele. It is characterized by herniation of cerebral tissue of the temporal lobe through a defect of the skull base localized in the middle fossa. At the time of first presentation the patient complained about recurrent nasal discharge of clear fluid which had begun some weeks earlier. She also reported that three months earlier she had for the first time suffered from a generalized seizure. In a first therapeutic attempt an endoscopic endonasal approach to the sphenoid sinus was performed. An attempt to randomly seal the suspicious area failed. After frontotemporal craniotomy, it was possible to localize the encephalocele and the underlying bone defect. The herniated brain tissue was resected and the dural defect was closed with fascia of the temporalis muscle. In summary, the combination of recurrent rhinorrhea and a first-time seizure should alert specialists of otolaryngology, neurology and neurosurgery of a temporo-sphenoidal encephalocele as a possible cause. Treatment is likely to require a neurosurgical approach.

  • Objectives : The paper reviews the texts related to Naosuxiaoshuo in Huangdineijing, and investigates its cause, mechanism, prognosis and symptom expressions. Methods : The paper searches for the texts within Huangdineijing that deal with Naosuxiaoshuo, and tries to clarify the significance and the characteristics of Naosuxiaoshuo using the annotators comments regarding this issue. Moreover, the paper tries to search for similarities between the symptoms of Naosuxiaoshuo and the relevant diseases in modern medicine. Results : Naosuxiaoshuo is a serious disease where the diminishing of the brain's parenchyma can even lead to death. The cause is yin-deficiency based on the lack of vital essence and body fluid, and it also can be caused by the external pathogen or other stimulations. Moreover, it shows some similarities with brain atrophy and cerebrospinal fluid diseases. Conclusions : Naosuxiaoshuo should be treated with a focus on yin-tonifying dealing with spleen related to production of body fluid and the kidney related to storage of vital essence. It is also important to prevent external pathogens or stimulations damaging the bone marrow.

Gingival crevicular fluid levels of sclerostin in chronic periodontitis and healthy subjects

  • Objectives: Chronic periodontitis is a common inflammatory disease of the oral cavity that causes destruction of periodontal tissues and bone around the teeth. Sclerostin is a protein encoded by the SOST gene. In this study, gingival crevicular fluid (GCF) levels of sclerostin in patients with chronic periodontitis were compared with those of healthy subjects. Materials and Methods: In this case-control study, a total of 40 subjects were enrolled and divided into the healthy group (n=23) and chronic periodontitis group (n=17). GCF samples were collected, and the concentration of sclerostin was evaluated using enzyme-linked immunosorbent assay. Comparison of significance between groups was assessed using Mann-Whitney U test. Results: Sclerostin concentration was significantly higher in the chronic periodontitis group compared with the healthy group (P<0.005). Conclusion: Despite the limitations of this study, sclerostin can be a possible marker for assessment of periodontal health status.

  • One six-year-old male Cocker spaniel with chief complaints of mandibular swelling and anorexia was referred. This dog was diagnosed into lymphosarcoma by the results of the examination with peripheral blood and fluid of the lymph nodes and bone marrow aspirated. This patient was multicentric form clinically and was cytomorphologically identified into lymphoblastic form of lymphosarcoma.

Mesenchymal stem cells for restoration of ovarian function

  • With the progress of regenerative medicine, mesenchymal stem cells (MSCs) have received attention as a way to restore ovarian function. It has been reported that MSCs derived from bone marrow, adipose, umbilical cord blood, menstrual blood, and amniotic fluid improved ovarian function. In light of previous studies and advances in this field, there are increased expectations regarding the utilization of MSCs to restore ovarian function. This review summarizes recent research into potential applications of MSCs in women with infertility or primary ovarian insufficiency, including cases where these conditions are induced by anticancer therapy.

IN VIVO EFFECTS OF DENTIN BONDING AGENTS ON DENTINAL FLUID MOVEMENT AND INTRADENTAL NERVE ACTIVITY (In vivo에서 상아질 접착제 도포가 상아세관액 이동과 치수신경활동에 미치는 영향)

  • The effect of application of dentin bonding agent to the exposed dentin on the intradental nerve activity (INA), dentinal fluid movement and sealing of the dentinal tubules, was investigated in this study. The INA was recorded from the single pulp nerve unit dissected from the inferior alveolar nerve. And specimen of dentin was observed by SEM. Dentinal fluid 'movement through exposed dentin surface was measured before and after the application of dentin bonding agent. 1. Eight Ao-fiber units (conduction velocity: $8.0{\pm}4.0m$/sec) were identified. 4M NaCl evoked an irregular burst of action potentials which ceased immediately after washing. 2. In 4 $A{\delta}$-fiber units, appliction of All Bond 2 completely abolished the INA induced by 4M NaCl. Also, application of Scotchbond Multipurpose(SBMP) totally abolished the INA induced by 4M NaCl in 4 $A{\delta}$-fiber units. 3. Before the application of dentin bonding agent, outward dentinal fluid movement of $10.2{\pm}5.7\;pl{\cdot}s^{-1}{\cdot}mm^{-2}$ was obsered. But after the application of dentin bonding agent the movement of dentinal fluid was stopped. 4. The gap width of 2-$3{\mu}m$ was formed between exposed dentin and adhesive resin in the specimens applied with dentin bonding agents of All Bone 2 and SBMP. But the formation of hybrid layer and the penetration of resin into were dentinal tubules were not clearly observed in interface between dentin and adhesive resin.

Enhanced compatibility and initial stability of Ti6Al4V alloy orthodontic miniscrews subjected to anodization, cyclic precalcification, and heat treatment

  • Objective: To evaluate the bioactivity, and the biomechanical and bone-regenerative properties of Ti6Al4V miniscrews subjected to anodization, cyclic precalcification, and heat treatment (APH treatment) and their potential clinical use. Methods: The surfaces of Ti6Al4V alloys were modified by APH treatment. Bioactivity was assessed after immersion in simulated body fluid for 3 days. The hydrophilicity and the roughness of APH-treated surfaces were compared with those of untreated (UT) and anodized and heat-treated (AH) samples. For in vivo tests, 32 miniscrews (16 UT and 16 APH) were inserted into 16 Wistar rats, one UT and one APH-treated miniscrew in either tibia. The miniscrews were extracted after 3 and 6 weeks and their osseointegration (n = 8 for each time point and group) was investigated by surface and histological analyses and removal torque measurements. Results: APH treatment formed a dense surface array of nanotubular TiO2 layer covered with a compact apatite-like film. APH-treated samples showed better bioactivity and biocompatibility compared with UT and AH samples. In vivo, APH-treated miniscrews showed higher removal torque and bone-to-implant contact than did UT miniscrews, after both 3 and 6 weeks (p < 0.05). Also, early deposition of densely mineralized bone around APH-treated miniscrews was observed, implying good bonding to the treated surface. Conclusions: APH treatment enhanced the bioactivity, and the biomechanical and bone regenerative properties of the Ti6Al4V alloy miniscrews. The enhanced initial stability afforded should be valuable in orthodontic applications.

  • The purpose of the present study is to evaluate the clinical and diagnostic significance of incidental findings of renal and urinary tract abnormalities, and extraosseous uptake of bone scans. The authors analyzed bone scans using $^{99m}Tc-MDP$(methylene diphosphonate) in 1238 cases of bone disease from April, 1979 to March, 1981. The results obtained were as follows. 1. Total extraosseous abnormalities were 112 cases (9%), which include 64 cases (5%) of renal and urinary tract abnormalities and 48 case(4%) of other extraosseous uptakes. 2. Renal and urinary tract abnormalities were 32 cases(50%) of obstruction, 14 cases(22%) of nonvisualization, 6 cases of space occupying lesion in kidney, 8 cases of kidney displacement and 4 cases of urinary bladder deformities. 3. Other extraosseous uptakes were 16 cases (33%) of body fluid collection, 15 cases of tumor uptake, 9 cases of free pertechnetate uptake and 8 others.

Cytologic Features of an Angiosarcoma in Pleural Fluid - A Case Report - (흉수의 혈관육종의 세포학적 소견 - 1예 보고 -)

  • Angiosarcomas are rare tumors, seen most commonly in the skin and soft tissue of head and neck legion. But it has been described in numerous body sites including thyroid, breast, liver, spleen, bone, etc.. Their biological behaviors depend on the microscopic grade, site of origin, and multifocality. We report the unique cytological features of an angiosarcoma in pleural fluid. A 61-year-old woman presented with a 6 month history of dyspnea on exertion and chest pain. Chest computerized tomography(CT) revealed multiple subpleural small nodules in the right lung and widespread all space consolidation and pleural effusion in the left lung. CT of liver revealed multiple small low attenuated lesion. The smears obtained from pleural fluid showed hypocellularity with a hemorrhagic background. The tumor cells were highly pleomorphic oval or spindle in shape and presented singly, in loose groups, in knitted syncytial aggregates, and in acinar pattern. Their nuclei had vesicular chromatin with delineated, thick nuclear membranes and occasionally a large eosinophilic, prominent nucleolus. The cytoplasm was plump, thin or protected in spindly fashion. Almost ail tumor cells showed variable sized intracytoplasmic vacuoles and their nuclei were sometimes crescentic by a huge vacuole. Occasional binucleated tumor cells and mitotic figures were present. Cellular debris and streaky materials were identified. Needle biopsy specimen from the pleura revealed anastomosing slit-like spaces lined by pleomorphic tumor cells. The tumor cells showed a strong reactivity for CD31 and vimentin and focal weak reactivity for factor VIII-related antigen.
