Intradiploic epidermoid cysts, like epidermoid cysts in other cranial locations, are rare. Approximately 100 intradiploic epidermoids have been reported, involving all of the cranial bones in proportion to their relative sizes. Over half erode through both tables of the cranium, creating variably sized areas of unprotected brain beneath the soft tumor. We report a case of an intradiploic epidermoid cyst of the right parietal bone that was found after minor head trauma.
골내 표피낭종은 드문 양성 낭성 병변이다. 이는 선천적인 요인이나 외상의 결과로 사료되며, 골 주변의 연부 조직에서 발생하기 때문에 골 파괴를 유발할 수 있다. 골내 표피낭종의 단순방사선 소견은 피질골의 팽대를 동반한, 경계가 명확한 방사선 투과성 병변이다. 임상적, 방사선학적 소견이 유사하기 때문에 원위 지골에서 발생한 다른 질환들과 골내 표피낭종을 감별하는 것이 중요하다. 저자들은 원위 지골에서 발생한 골내 표피낭종의 2개의 드문 증례를 보고하고자 한다.
The aim of this study was to evaluate the clinical, radiographic and histopathologic features of 23 cases of solitary bone cyst by means of the analysis of radiographs and biopsy specimens in 23 persons visited the Department of Oral and Maxillofacial Radiology, School of Dentistry, Kyung Hee University and Chunbuk National University. The obtained results were as follows; 1. The incidence of solitary bone cyst was almost equal in males(52.2%) and in females(47.8%) and the prevalent age of the solitary bone cyst were the second decade(47.8%) and the third decade (21.7%). 2. In the signs and symptoms of solitary bone cyst, pain or tenderness revealed in 17.4%, swelling revealed in 13.0%, pain and swelling revealed in 21.7%, paresthesia revealed in 4.4% and 43.5% were asymptom and the tooth vitality involved in the solitary bone cyst, 76.5% were positive and 23.5% were either positive or negative. 3. In the location of the solitary bone cyst, 47.8% present posterior region, 21.7% present anterior region, 21.6% present anterior and posterior region, 4.4% present condylar process area. 4. In the hyperostotic border of the solitary bone cyst, 47.8% were seen entirely, 21.8% were seen partialy, and 30.4% were not seen. 5. In the change of tooth, 59.1% were intact, 18.2% were loss of the alveolar lamina dura, 13.6% were root resorption 4.55% were tooth displacement, 4.55% were root resorption and tooth displacement. 6. In the change of cortical bone of the solitary bone cyst, 39.1% were intact and 60.9% were thinning and expansion of cortical bone. 7. In the histopathologic findings of 9 cases, 33.3% were thin connective tissue wall, 11.1% were thickened myxofibromatous wall, 55.6% were thickened myxofibromatous wall with dysplastic bone formation.
목적: 골수에서 분리한 기질 세포를 단독으로 또는 동종 망상골과 함께 이식하여 치료했던 양성 골 병변 소파 후 발생한 골 결손에 대한 치료 결과를 보고 하고자 한다. 대상 및 방법: 1996년 7월 이래 장관골 양성 골 병변에 대해 병변이 작거나 소아여서 소파술과 함께 자가 골수 기질 세포를 단독으로 이식하였던 2명 3례와, 자가 골수 기질 세포와 동종 망상골 이식을 병행하여 치료하였던 6명 6례 등 총 8명 9례를 대상으로 수술 후 임상증상의 해소, 신생 골 형성 및 주변 정상골과의 유합 과정을 추시하였다. 남자가 6례, 여자가 2례였으며 평균 연령은 24세(8세~47세)였다. 조직학적 진단은 섬유성 골 이형성증이 5례로 많았고, 단순 골 낭종 2례, 연골모세포종과 섬유성 피질 결손이 각각 1례씩 이었다. 수술 후 평균 추시기간은 16.3개월(3개월~84개월) 이었다. 결과: 수술 전 병소 부위에 통증을 호소했던 4례 모두에서 술 후 2주 이내에 증상이 소실되었으며, 수술 후 촬영한 단순 방사선 사진 상 약 4주경부터 병변 부위에 신생 골 형성이 관찰되고 약 8주경에는 골 결손 부위가 신생 골로 충전되고 주변 정상골과 유합된 소견을 보였다. 합병증으로는 1례에서 술 후 3주에 병소 부위를 통과하는 골절이 발생했으며, 단순 골 낭종 1례에서 술 후 5개월에 병소가 재발하였다. 결론: 장관골 양성 골 병변에 대해 소파 후 골수에서 분리한 골수 기질 세포를 단독으로 또는 동종 망상골과 함께 이식할 때 빠른 치유과정을 보여, 비록 단 기간의 추시 결과이기는 하나 골수 기질 세포 이식술이 향후 골 낭종이나 양성 골 병변의 치료에 크게 이용될 수 있을 것으로 보인다. 골 낭종 수술 후 재발 원인은 낭종내벽 섬유조직의 불완전 소파의 결과로 보이며, 골절 발생 가능성이 높은 대퇴골 전자간 골 낭종 등에 대하여는 금속판 등을 이용한 예방적인 고정이 필요할 것으로 판단되었다.
Two thousand-two hundred-forty-three cases of various lesions of the oral and maxillofacial region in children under 16 years of age were reviewed. The lesions were classified by 10 groups; osteomyelitis, cysts, benign tumors, malignant tumors, fibro-osseous lesions, developmental disturbances, antral lesions, TMJ lesions, salivary gland lesions, and other lesions. The obtained results were as follows; 1. Developmental disturbances, especially cleft alveolus and palate, was the most common lesions (25.7%), followed by cysts (22.5%), antral lesions (12.7%), benign tumors (11.9%) and osteomyelitis (9.3%). 2. With the time, the incidence of osteomyelitis and malignant tumors has been decreased, but that of developmental disturbances, cysts and antral lesions has been increased. 3. The sex distribution was relatively equal for the entire series, with male predominence in cysts, malignant tumors, developmental disturbanes and salivary gland lesions and with female predominence in TMJ lesions. 4. Children with malignant tumors and osteomyelitis were somewhat younger and those with TMJ lesions were somewhat older. 5. Heading the list of each group except osteomyelitis was dentigerous cyst, odontoma, malignant lymphoma, fibrous dysplasia, cleft alveolus and palate, inflammatory change, degenerative change of condylar process, sialolithiasis and simple bone cyst respectively.

This case series study demonstrates the possibility of successful implant rehabilitation without bone augmentation in the atrophic posterior maxilla with cystic lesion in the sinus. Sinus lift without bone graft using the lateral approach was performed. In one patient, the cyst was aspirated and simultaneous implantation under local anesthesia was performed, whereas the other cyst was removed under general anesthesia, and the sinus membrane was elevated in a second process, followed by implantation. In both cases, tapered 11.5-mm-long implants were utilized. With all of the implants, good stability and appropriate bone height were achieved. The mean bone level gain was 5.73 mm; adequate bone augmentation around the implants was shown, the sinus floor was moved apically, and the cyst was no longer radiologically detected. Completion of all of the treatments required an average of 12.5 months. The present study showed that sufficient bone formation and stable implantation in a maxilla of insufficient bone volume are possible through sinus lift without bone materials. The results serve to demonstrate, moreover, that surgical treatment of mucous retention cyst can facilitate rehabilitation. These techniques can reduce the risk of complications related to bone grafts, save money, and successfully treat antral cyst.

Objectives: Idiopathic bone cavity (IBC) is an uncommon intra-osseous cavity of unknown etiology. Clinical features of IBC are not well known and treatment modalities of IBC are controversial. The purpose of this study was to investigate the clinical characteristics of 27 IBC patients who underwent surgical exploration. Materials and Methods: A total of 27 consecutive patients who underwent surgery due to a jaw bone cavity from April 2006 to February 2016 were included in this study. Nine male and 18 female patients were enrolled. Patients were examined retrospectively regarding primary site, history of trauma, graft material, radiographic size of the lesion, presence of interdental scalloping, erosion of the inferior border of the mandible, complications, results of bone graft, and recurrence. Results: Female dominance was found. Maxillary lesion was found in one patient, and bilateral posterior mandibular lesions were found in two patients. The other patients showed a single mandibular lesion. The posterior mandible (24 cases) was the most common site of IBC, followed by the anterior mandible (5 cases). Two patients with anterior mandibular lesion reported history of trauma due to car accident, while the others denied any trauma history. Radiographic cystic cavity length over 30 mm was found in 10 patients. Seven patients showed erosion of the mandibular inferior border. The operations performed were surgical exploration, curettage, and bone or collagen graft. One bilateral IBC patient showed recurrence of the lesion during follow-up. Grafted bone was integrated into the native mandibular bone without infection. One patient reported necrosis of the mandibular incisor pulp after operation. Conclusion: Differential diagnosis of IBC is difficult, and IBC is often confused with periapical cyst. Surgical exploration and bone graft are recommended for treating IBC. Endodontic treatment of involved teeth should be evaluated before operation. Bone graft is recommended to reduce the healing period.
To present a rare case of a cystic giant schwannoma of the sacrum mimicking aneurysmal bone cyst (ABC). A 54-year-old man visited our institute complaining left leg weakness and sensory change for several years. Magnetic resonance imaging revealed a large multilocular cystic mass with canal invasion and bone erosion confined to left S1 body. The lesion showed multiple septal enhancement without definite solid component. Initially the tumor was considered as ABC. The patient underwent grossly-total tumor resection with lumbosacral reconstruction via posterior approach. The tumor was proved to be a cystic schwannoma. The postoperative course was uneventful and the patient was relieved from preoperative symptoms. We present a rare case of pure cystic giant schwannoma confined to sacrum mimicking ABC. The surgical treatment is challenging due to the complex anatomy of the sacrum. Schwannoma should be considered in the differential diagnosis of osteolytic sacral cysts.

Fibrous dysplasia is a benign fi bro-osseous lesion wherein normal bone is replaced with an excessive proliferation of cellular fibrous connective tissue intermixed with irregular bony trabeculae. Fibro-osseous lesions like fibrous dysplasia are often associated with non-epithelial cysts, such as simple bone cyst. The etiologic and pathogenic relationships between fi brous dysplasia and simple bone cyst have not been conclusively established. Nonetheless, the mechanism of cyst formation in fibro-osseous lesion associated with simple bone cyst can be said to differ from that of the typical simple bone cyst of the jaws. This article reports a case of bilateral lesions including fi brous dysplasia and simple bone cyst on each site and reviews the pathogenesis of cyst formation in the fibro-osseous lesion.

Keratocystic odontogenic tumor (KCOT) is a benign odontogenic tumor with a potentially aggressive and infiltrative behavior. KCOT is most commonly occurred in mandible and demonstrate a unilocular, round, oval, scalloped radiolucent area, while large lesions may appear multilocular. An important characteristic of KCOT is its propensity to grow in an antero-posterior direction within medullary cavity of bone causing minimal expansion. Definitive diagnosis relies on histological examination. In this report, a KCOT that had an expansion both buccal and lingual cortical bone is described including its features in computed tomography and ultrasonographic exams. The lesion was removed surgically via an intraoral approach under local anesthesia and histologically reported as a KCOT.
