원발성 비성 NK/T 세포 림프종: 조기 진단을 위한 임상적, 방사선학적, 조직학적 특징 (Extranodal NK/T cell Lymphoma, nasal type: clinical, radiological, histological features for early diagnosis)

  • Primary nasal type natural killer (NK)/T cell (NKTC) lymphoma, a specific form of malignant lymphoma, has a higher geographic incidence in Oriental, Mexican, and South American populations than the Western population. In Koreans, it comprises 9-12% of all cases of non-Hodgkin's lymphoma. This type of lymphoma has also been named as angiocentic lymphoma and lethal midline granuloma because the most common site is the upper airway area and its clinical aggressiveness presents with a necrotic and destructive pattern. NKTC lymphoma can also be detected in different organs (testis, spleen, parotid gland, skin, gastroinstinal tract, central nervous system, lungs, bone marrow, etc.) other than the upper airway including the oral cavity. The lymphoma detected in the oral cavity shows various destructive and inflammatory changes, similar to the signs of inflammation and infection from periodontitis and pulpal disease, making a diagnosis difficult with just the clinical signs. For early detection, clinical, radiological, and pathological examinations are required. This report describes the clinical, radiological and histological characteristics with a case report for the early detection of NKTC lymphoma in the oral cavity.

영구 계승치의 조기 맹출에 관한 증례 보고 (PREMATURE ERUPTION OF PERMANENT SUCCESSORS : A CASE REPORT)

  • 유치의 탈락과 영구치의 맹출은 정상적이고 예측 가능한 생리적인 과정이다. 치아 발육의 정확한 예측은 어린이의 치료 계획의 수립에 있어서 중요하다. 어린이의 연령과 그에 해당하는 치아 발육의 정상치가 여러 연구자들에 의해 조사되었다. 그러나 다양한 요소에 의해 치아의 정상 발육이 영향받을 수 있고 치아의 맹출을 촉진하거나 지연시킬 수 있다. 특히 농양이 있던 유치의 조기 발거나 탈락은 영구 계승치의 맹출에 뚜렷한 영향을 미칠 수 있다. 유치의 농양은 영구치를 덮은 골을 흡수시키고 따라서 발육상태에 관계 없이 영구치는 맹출할 수 있다. 유구치가 일찍 탈락 또는 발거되고 영구 계승치의 맹출이 조기에 일어나면 치아의 편향과 회전, 맹출 순서의 변경, 이웃 치아의 매복, 중심선 변위 등의 다양한 중요한 사건이 발생할 수 있다. 이에 저자는 서울대학교 병원 소아치과에 내원한 환아들의 조기 맹출한 영구 계승치의 임상적, 방사선학적 특징과 그로 인한 영향을 알아보고, 그에 따른 적합한 처치와 추후 관찰 사항을 보고하고자 한다.

Polymorphism of Exon 2 of BMP15 Gene and Its Relationship with Litter Size of Two Chinese Goats

  • Polymorphisms of BMP15 gene exon 2 and its relationship with prolificacy of goats were detected by PCR-SSCP and DNA sequencing methods in Chinese two local goat breeds. The results showed that the product amplified by the primers displayed polymorphisms. Three genotypes (AA, BB and AB) were detected in Funiu white goats, and their frequency was 0.071, 0.715, 0.214, respectively. Two genotypes (AB and BB) were detected in Taihang black goats, and their frequency was 0.342 and 0.658, respectively. Sequencing revealed that four mutations (456T${\rightarrow}$G, 466C${\rightarrow}$G, 510C${\rightarrow}$T, 511T${\rightarrow}$C) occurred in genotype BB of Funiu white goat, which resulted in amino acid substitution of V155G and S171P. No mutation was detected in Taihang black goat. The Funiu white goat with genotype BB had 0.91 or 0.82 kids, more than those with AB or AA, respectively. The difference of the least squares means for litter size between BB and AB was not significant (p>0.05) in Taihang black goat. It is concluded that the BMP15 gene may be a major gene which affects the prolificacy in Funiu white goats. This study could provide basic molecular data on the reproductive characteristics of local breeds of Henan province in China, and a scientific basis for the conservation and utilization of those two goat breeds.

  • 이상적인 회전근 개 봉합술은 봉합 초기 높은 고정 강도로 봉합 부위 건-골간 간격 형성을 최소화시키며, 재활 과정 중 발생하는 반복적인 부하에도 견디는 기계적 강도를 나타내어 궁극적으로 건-골 조직의 생물학적 치유를 얻을 수 있는 방법이다. 현재 사용되는 회전근 개 봉합술 중 교량형 봉합술식은 회전근 개 부착 부위를 해부학적으로 복원할 수 있으며, 건-골간 압력 접촉 면적을 증가시키고, 방사형의 봉합 형태를 통하여 회전근 개 전체에 균등하게 압력을 분포함으로 부하를 분산시키며 생물학적 치유를 향상시킨다. 또한 건-골간 간격 형성을 최소화하며 전단 및 회전 응력에 저항력을 주어 정상과 동일한 해부학적 복원력으로 빠른 재활 운동을 가능하게 한다. 그러나 비록 교량형 봉합술식이 다른 술식에 비해 우수한 생역학적 특성을 나타내도 임상적으로 더 좋은 결과를 초래한다는 증거는 없으며, 이열 봉합술과는 비슷한 재파열율이 보고되고 있다. 회전근 개봉합술의 선택은 회전근 개 파열 크기, 파열 양상 및 건의 상태 등을 고려하여 적절하게 선택하여야 할 것으로 사료된다.

초음파골밀도측정기 정도관리 방안제시를 위한 한국 초음파골밀도 사용현황 조사 (Survey on Usage of Korean Quantitative Ultrasound for Proposing Quantitative Ultrasound Quality Control Guideline)

  • This study was investigated quantitative ultrasound (QUS) usage in Korea for the QUS quality control guidelines. A total of 344 questionnaires collected from July 24th to August 25th 2017 were analyzed. Questionnaires were created through user interviews, expert group advice, literature review and field observation. As a result of the general characteristics of quantitative ultrasound holding amounted to 81.98% of clinic and 6.69% of hospitals. The main user was radio-logical technologists as 31.39%. The contact methods of the gel pad (balloon) were the most used at 56.68% and the scan region was 91.9% of calcaneus. The quantitative ultrasound quality control cycle was 67.37% when the abnormality was found in the equipment, and 63.66% when the accuracy control was implemented according to the manual. The phantoms of QUS were 34.30% of the manufacturer's own phantoms. User of QUS had never received education for quality control of quantitative ultrasound as 62.20%. This study was expected to be useful when creating detailed quality control guidelines in the future, as well as guidelines for the quality control of Korea's standard quantitative ultrasound.

Genetic and biological characteristics of recent Korean isolates of avian influenza virus subtype H9N2

  • The worldwide distribution and continuing genetic mutation of avian influenza virus (AIV) has been posed a great threat to human and animal health. A comparison of 3 isolates of AIV H9N2, A/chicken/Korea/KBNP-0028/00 (H9N2) (KBNP-0028), A/chicken/Korea/SNU8011/08 (H9N2) (SNU 8011) and an inactivated oil vaccine strain A/chicken/Korea/01310/01 (H9N2) (01310), was performed. The former 2 AIVs were isolated from field cases before and after the application of an inactivated H9N2 vaccine in 2007, respectively. The antigenic relationship, viral shedding, tissue tropism and genetic analysis were examined. The comparison of virus shedding from the cloaca and the oropharynx revealed that both isolates were more frequently isolated from the upper respiratory tract (90~100%) 1 day post inoculation (DPI) compared with isolation 5 DPI from gastrointestinal tracts (10~60%). Moreover, the isolate KBNP-0028 were recovered from all organs including bone marrow, brain and kidneys, indicating higher ability for broad tissue dissemination than that of SNU 8011. KBNP-0028 replicated earlier than other strains and with a higher titer than SNU 8011. In full-length nucleotide sequences of the NA gene and a partial sequence of the HA gene of SNU 8011, we found that there might be significant changes in tissue tropism, virus replication and genetic mutation in AIV H9N2 isolates.

Microstructure and Mechanical Properties of Ti-35Nb-7Zr-XCPP Biomaterials Fabricated by Rapid Sintering

  • Ti-6Al-4V ELI (Extra Low Interstitial) alloy have been widely used as alternative to bone due to its excellent biocompatibility, although it still has many problems such as high elastic modulus and toxicity. Therefore, biomaterials with low elastic modulus and non toxic characteristics have to be developed. A novel ${\beta}$ Ti-35wt%Nb-7wt%Zr-Calcium pyrophosphate (CPP) composite that is a biocompatible alloy without elemental Al or V was fabricated by spark plasma sintering (SPS) at $1000^{\circ}C$ under 70 MPa using high energy mechanical milled (HEMM) powder. The microstructure and phases of the milled powders and the sintered specimens were studied using SEM, TEM, and XRD. Ti-35wt%Nb-7wt%Zr alloy was transformed from ${\alpha}$ phase to ${\beta}$ phase in the 4h-milled powder by sintering. The sintered specimen using the 4h-milled powder showed that all the elements were distributed very homogeneously and had higher density and hardness. ${\beta}$ Ti alloy-CPP composite, which has nanometer particles, was fabricated by SPS using HEMMed powder. During the sintering process, $CaTiO_3$, TixOy, and CaO were formed because of the reaction between Ti and CPP. The Vickers hardness of the composites increases with the increase of the milling time and the addition of CPP. The biocompatibility of the Ti-Nb-Zr alloys was improved by addition of CPP.

비비안 웨스트우드의 패션디자인과 정치 성향의 관계 (The Relation Between Fashion Design of Vivienne Westwood and Her Political Attitude)

  • Vivienne Westwood could be placed in a unique position in the contemporary fashion design. She has never belonged in a core group of mainstream fashion designer but not been always in an outsider position. She got from mainstream fashion designer circles enormous appreciation, even established fashion industry was influenced by her fashion design. She began her fashion design with rebellious T-shirts like 'destroy' T-shirt, chicken-bone T-shirt, and nipple-zipper T-shirt, all of which revealed her disgust against establishment. All these T-shirts testify her total negation tendency in her youth. However, she did not continue to keep such a kind of total negation attitude against establishment In 1980s she changed her political attitude towards establishment, and this change also found a reflection in her fashion design. In her pirate-collection the dark image of her fashion in the 1970s changed into a totally different bright image with full of gold colour. Although this collection had radiated brightness, it contained still outsider character from mainstream fashion design. The following fashion design in 1980s and 1990s evolved further on the line of Pirate-collection, but they continued to contain certain outsider characteristics. Vivienne Westwood kept some kind of anti-establishment attitude, and this attitude more or little came to the surface of her design. She was always political and critical to the establishment. In 2005, in her 64, she designed a liberty T-shirt, which showed her political attitude and her engagement in social issues. Also it showed the change from the early total negation to the constructive critical affirmation. In this paper the relation between the fashion design of Vivienne Westwood and her political attitude and the influence of her political attitude to her fashion design is analyzed.

공격성 섬유종증에서 PTEN 종양 억제 인자의 발현 (Expression of PTEN, Tumor Suppressor Protein, in Aggressive Fibromastosis)

  • 공격성 섬유종증은 비교적 드문 연부 조직 종양이지만, 국소 증식 및 침윤성이 강한 종양으로 알려져 있다. 그렇지만 종양 증식에 관한 정확한 기전은 아직 보고되어 지지 않고 있다. 한편, 종양 억제 유전자 PTEN은 국소 침윤성과 전이를 보이는 여러 암 조직에서 발현의 감소 또는 돌연 변이가 보고되고 있다. 본 연구에서는 공격성 섬유종증에서 면역조직화학적 염색과 면역탁본법을 통해 종양 억제 유전자 PTEN의 발현의 양상을 알아보고자 하였다. 면역 탁본법과 면역조직화학적 염색상 PTEN의 발현은 정상 근막 조직에서는 균일하게 발현되었으나, 공격성 섬유종증 종양조직에서는 발현이 감소하였다. 공격성 섬유종증에서 PTEN발현의 감소는 급속한 증식과 재발 및 주위 조직의 침윤이 특징인 공격성 섬유종증의 병인에 관련이 있을 것으로 사료된다.

Articulation error of children with adenoid hypertrophy

  • Purpose: Adenoid hypertrophy is a physical alteration that may affect speech, and a speech disorder can have other negative effects on a child's life. Airway obstruction leads to constricted oral breathing and causes postural alterations of several oro-facial structures, including the mouth, tongue, and hyoid bone. The postural modifications may affect several aspects of speech production. Methods: In this study, we compared articulation errors in 19 children with adenoid hypertrophy (subject group) to those of 33 children with functional articulation disorders independent of anatomical problems (control group). Results: The mean age of the subject group was significantly higher (P=0.016). Substitution was more frequent in the subject group (P=0.003; odds ratio [OR], 1.80; 95% confidence interval [CI], 1.23- 2.62), while omission was less frequent (P<0.001; OR, 0.43; 95% CI, 0.27-0.67). Articulation errors were significantly less frequent in the palatal affricative in the subject group (P=0.047; OR, 0.25; 95% CI, 0.07-0.92). The number of articulation errors in other consonants was not different between the two groups. Nasalization and aspiration were significantly more frequent in the subject group (P=0.007 and 0.014; OR, 14.77 and 0.014; 95% CI, [1.62-135.04] and NA, respectively). Otherwise, there were no differences between the two groups. Conclusion: We identified the characteristics of articulation errors in children with adenoid hypertrophy, but our data did not show the relationship between adenoid hypertrophy and oral motor function that has been observed in previous studies. The association between adenoid hypertrophy and oral motor function remains doubtful.
