The Effect of Bangpungtongsungsan on Insulin Resistance Induced by Non-alcoholic Fatty Liver Disease (NAFLD) (방풍통성산의 지방대사조절을 통한 비알콜성 지방간 유도 인슐린저항성 개선 효과)

  • Objectives Bangpungtongsungsan is an herbal medicine that treats obesity and dampness-phlegm. The aim of this study was to investigate the efficacy of Bangpungtongsungsan on insulin resistance induced by non-alcoholic fatty liver disease. Methods Male 6-week-old C57BL/6 male mice were divided into four groups: control group (Ctrl), high fat diet group (HFF), high fat diet with Bangpungtongsungsan extract administration group (BT1), and high fat diet with double concentration of Bangpungtongsungsan extract administration group (BT2). Each 10 mice were allocated to each group (total of 40 mice). All mice were allowed to eat fat rich diet freely throughout the experiment. To examine the effect of Bangpungtongsungsan, we observed weight changes, lipid blot distributions, PGC-1, $p-I{\kappa}B$, 8-OHdG, p-JNK, total cholesterol and glucose levels. Results Comparing of body weight measurements between 4 groups, weight gain was significantly lower in BT1 and BT2 group than the HFF group. The distribution of lipid blots and positive reaction of PGC-1 were significantly lower in BT1 and BT2 group. The positive reaction of $p-I{\kappa}B$ and 8-OHdG in hepatic tissues was significantly lower in BT1 and BT2 group. The positive reaction of p-JNK in hepatic tissues was significantly lower in BT1 and BT2 group. Total cholesterol and glucose levels were significantly lower in BT1 and BT2 group. Conclusions Bangpungtongsungsan has the effect of improving non-alcoholic fatty liver induced insulin resistance through regulation of lipid metabolism.

Gluteal Region Reshaping of Massive Weight Loss Patients-A Decision-Making Strategy

  • Background Massive weight loss (MWL) is a very common presentation that you may face as a plastic surgeon. Each patient has his own individual criteria, so, you should work according to a well-organized plan, especially when such cases have concerns about their gluteal area contour that were neglected before by many surgeons. A decision-making strategy was used to give a personalized treatment for targeting gluteal region reshaping of MWL patients. Methods This study considered all patients with MWL subjected to buttock reshaping. There was no randomization in treatment; there was a case-by-case assessment. We analyzed the features of the buttocks, the type of surgery performed, the outcomes, and the complications. Results Fifty two patients were included (41 females and 11 males), ages ranged between 21 and 66 years. Demographic data, preoperative body mass index (BMI), duration of surgery, type of surgery, and postoperative complications were collected. Statistically significant improvements were observed in gluteal ptosis and patient satisfaction grades. Conclusion Aesthetic improvement of the buttocks involves either augmentation or contouring that may be obtained by liposculpture, surgical lifting, or combination. Patients with MWL have high expectations and are often treated with multiple procedures. Thus, an easy strategic approach personalized on each patient to treat multiple adjacent areas in one operation is necessary. Adipose tissue distribution, gluteal skin status, and BMI were the main factors that can forcefully affect our plan to guarantee reduction of unpleasant results and complications and improve patient satisfaction.

  • The effect of magnesium on the electrolytes distribution and transport in mice was studied using isocalorie and isonitrogenous diets added magnesium salt. The results are summarized as follows: 1. The body weight was the highest value in group C fed with basal diet and 0.4 mg of magnesium per day rather than others. 2. In blood plasma tile content of sodium of group B was higher than others in both male and female. The content of potassium of group B was higher than others in female, and the content of calcium of group B in female and group D in male was higher than others in blood plasma. 3. In case of muscle the content of sodium of group B was higher than others in both male and female. The content of potassium of group B was higher than others in female, but of group C was higher than others in male. The content of calcium of group B was higher than others in female, but of group D was higher than others in male. 4. Magnesium effect on the distribution electrolytes such as sodium, potassium and calcium ions in muscle and blood vessel.

Evaluation of Biomechanical Stability of Newly Developed Revision Total Knee Arthroplasty through Strain and Stress Distribution Analysis within the Tibia: Finite Element Analysis (경골 내 변형률 및 응력 분포 특성 분석을 통한 새로이 개발된 재치환용 인공슬관절의 생체역학적 안정성 평가: 유한요소해석)

  • In this study, biomechanical stability of the newly developed revision total knee arthroplasty (rTKA) was evaluated through strain and stress distribution analysis within the implanted proximal tibia using a three-dimensional finite element (FE) analysis. 2000N of compressive load (about 3 times body weight) was applied to the condyle surface on spacer, sharing by the medial (60%) and lateral (40%) condyles simulating a stance phase before toe-off. The results showed that PVMS within the revision total knee arthroplasty and the proximal tibia were less than yield strength considering safe factor 4.0 (rTKA: less than 10%, Cortical bone: less than 70%, Cancellous bone: less than 70%). The materials composed of them and the strain and stress distributions within the proximal tibia were generally well matched with those of a traditional revision total knee arthoplasty (Scorpio TS revision system, Stryker Corp., Michigan, USA) without the critical damage strain and stress, which may reduce the capacity for bone remodeling, leading to bone degeneration. This study may be useful to design parameter improvement of the revision total knee arthoplasty in biomechanical stability point of view beyond structural stability of revision total knee arthoplasty itself.

Comparative evaluation of supplemental zilpaterol hydrochloride sources on growth performance, dietary energetics and carcass characteristics of finishing lambs

  • Objective: We compare the effects of three different approved sources of supplemental zilpaterol on growth-performance responses and carcass characteristics of finishing lambs. Methods: Twenty four Pelibuey${\times}$Katahdin lambs ($46.75{\pm}2.43kg$) were used in a 33-day feeding trial. Lambs were fed a dry rolled corn-based finishing diet. Treatments consisted of the non-supplemental basal diet (Control) versus the basal diet supplemented with 125 mg zilpaterol/kg of diet (as fed basis) from three commercial sources marketed in Mexico: Zilmax (ZIL), Grofactor, and Zipamix. Results: Compared to controls, zilpaterol (ZH) supplementation did not affect dry matter intake (DMI), but increased carcass adjusted daily weight gain (ADG, 36.7%), gain efficiency (34.2%), and dietary net energy (26.0%), and decreased (23.4%) the ratio of observed:expected DMI. Compared to controls, supplemental ZH increased hot carcass weight (6.4%), dressing percentage (3.2%), m. longissimus thoracis (LM) area (15.6%), and shoulder muscle:fat ratio (28.7%), but decreased kidney-pelvic-heart fat, and fat thickness. Supplemental ZH increased 10.9% and 14.3% whole cut weight of loin and leg, respectively, and the proportion (as percentage of cold carcass weight) of leg (4.3%). These increases were reflected in greater forequarter and hindquarter weights. Lambs fed ZH increased (4.6%) empty body weight (EBW) and reduced (14.7%) liver/spleen weight (as g/kg EBW). Likewise, ZH supplementation tended (p = 0.08) to lower (8.9%) visceral fat. Growth performance, energetic efficiency, hot carcass weight, dressing percentage, LM area and whole cuts were not different across supplemental ZH sources. However, compared with non-supplemented controls, only ZIL appreciably decreased carcass fat distribution, including fat thickness, percentage kidney pelvic and heart fat, shoulder fat, and visceral fat. Conclusion: Supplemental ZH increases ADG, gain efficiency, carcass dressing percentage, and LM area. The magnitude of these responses was similar among ZH sources. Nevertheless, compared with non-supplemented controls, only ZIL appreciably decreases carcass fat. The basis for this is uncertain, but indicative that some practical differences in zilpaterol bio-equivalency may exist across commercial sources tested.

Spawning and Growth of eightspine stickleback, Pungitius sinensis kaibarae (Tanaka) (Gasterosteidae, Pisces), in the Chavans Stream, a tributary of Kumho River, Korea (잔가시고기 Pungitius sinensis koibaroe (Tanaka) (큰가시고기과, 어강)의 산란과 성장)

  • The spawning and growth of eightspine stickleback,Pungitius sinensis kaiborae (Tanaka, 19151, were investigated in the Chavang Stream from March 1990 to february 1991 as a part of study on its life history. Nest building, spavming and parental behaviors were similar to those known on the sticklebacks of Europe, North America and Japan. Bht f sinensis kaibarae in the Chayang Stream, peculiarly, showed only superficial gluing behavior in nest building phase and made two nurseries iust behind the nest in Parental Phase. The individuals with fully mature eggs were found from late February to late June. The fecundity of an adult female was about 21 to 110 (mean 47.07) eggs and the diameter of kllv matured ovarian eggs ranged 1.3 to 1.5 mm. The smallest female with fully mature eggs was 25.8 mm in body length and had 21 eggs. The relationship between the number of mature eggs in ovary and body length was No. of eggs = 2.857BL -55.134 (r = 0.890). The number of deposited eggs in a nest was 27 to 637 and thew were composed of several clusters of various developmental stages. The distribution pattern of gonadsomatic index (GSl) against the body length shown that in females 1+ year-old fishes mainly participate in spawning but in males 11 and 2+ year-old fishes equally participate. From the fluctuat지n pattern of GSI and the number of mature eggs in ovary, it was assumed that the spawning season was from February to June and the peak of spawning was early March to late April. The newly hatched vouns was found at late April and their body length was about 10 mm. The vouns (O+) grew rapidly until late August. Then their srowth was slowed down and finallY ceased in midautumn. The young reached about 35 mm BL in their first year of life. The halting of growth was lasted to next Sune when the spawning season would be nearly closed. They (1+) began to grow asain from late june, grew rapidly until tate september and reached about 415 mm BL. Then there was no more grouvth until to die as 2+ yearold fish. Therefore it was assumed that the life span of P. sinensis kaibarae in the Chavang Stream would be about ko years. The relationship between the body length and the body weight was logBW = 2.9541ogBL -4.802 (r = 0.998).

Relationship of vitamin D status and obesity index in Korean women (한국 성인 여성의 비타민 D 영양상태와 비만지표와의 관계)

  • Purpose: Vitamin D status is associated with several chronic diseases related to obesity. In this study, we evaluate the nutritional status of vitamin D and its relation to obesity indices in Korean women. Methods: A total of 156 healthy women participated. Vitamin D status (serum $25-OH-vitamin\;D_3$ level) and obesity indices (body mass index, body fat mass, waisthip ratio, and body fat percentage etc.) and serum lipid profiles and serum adipokine (leptin and adiponectin) levels were analyzed. Results: The $25(OH)D_3$ level showed an extremely skewed distribution from 4.1 ng/ml to 24.4 ng/ml and mean $25(OH)D_3$ level was $9.0{\pm}4.0ng/ml$. With cut-off level for vitamin D deficiency (< 12.0 ng/ml), insufficiency (12-19.9 ng/ml) and sufficiency (${\geq}20ng/ml$), 77.6%, 19.2%, and 3.2% of subjects showed vitamin D deficiency, insufficiency, and sufficiency status, respectively. The $25(OH)D_3$ level showed positive correlation with weight (r = 0.2461, p < 0.01), body mass index (r = 0.2913, p < 0.001), body fat contents (r = 0.1691, p < 0.05), fat free mass (r = 0.2330, p < 0.01), and waist hip ratio (r = 0.1749, p < 0.05) after adjusted by age. The $25(OH)D_3$ level showed no significant correlation with serum lipid profiles and adipokine levels. Conclusion: Most subjects (76.6%) in this study, who had a vitamin D deficient status and serum $25(OH)D_3$ level, showed positive correlation with several obesity indices, however further research based on a large Korean population is needed to confirm the relationship.

Study on the Diet Style According to the Sasang Constitution (사상체질(四象體質)에 따른 식이(食餌)습관에 관(關)한 연구(硏究))

  • In oriental medicine, food are regarded as important one like medicine. Especially Sasang medicine put emphasis on diet, it regarded food as a important one by taking food according to their own constitution in the point of improving health condition and preventing diseases. In this report, the author try to find out the relationship between food preference and each suitable food according to Sasang constitution by using survey data. The results are like this. 1. In constitutional distribution patterns among 203 person(male : 118 person, female : 85 person) reveal Soyang Group 58person(28.6%), Taeum Group 61(30%) and Soeum Group 84(41.4%). And their were no significant difference in their height in all group according to gender. Compared with the other groups, When compared with the other groups on body weight and BMI, Taeum Group indicate significantly high body weight and BMI(Body Mass Index) in both gender. 2. Taeum Group show significantly high smoking rate. But alcohol drinking rates reveal no significant difference in all groups. 3. In the Survey about food preference according to the constitution, rice, Soju, watermelon show significantly high preference in case of suitable food to each group. But perilla seeds, coffee, ginger tea show insignificant results. 4. In the survey about food preference according to the constitution and sex, welsh onion, crab preserved with soysauce, beer show significantly high preference in case of suitable food to each group. But glutinous rice, cooked barley, water dropwort muchim, lettuce, dog meat, egg, yellow croaker, coffee, ginger tea, and Soju show insignificant results. 5. In the survey about food preference according to the constitution and age, rice, watermelon, onion, garlic, salt and Soju show significantly high preference in case of suitable food to each group. But sugar and perilla seeds show insignificant results. 6. In the survey about food preference according to constitution, sex and age, glutinous rice, soybean milk, banana, crab preserved with soysauce, sea cucumber, sea mustard, Soju, beer, onion, garlic, salt show high preference in case of suitable food to each group. But cooked barley, sugar, water dropwort muchim, dog meat, puffer soup and perilla seeds show insignificant results.

Testicular Toxicity of 2-Bromopropane in Male Sprague-Dawley Rats (2-Bromopropane의 수컷 랫트를 이용한 고환 독성시험)

  • 2-Bromopropane, important industrial chemical, specially in electronic industry at Yangsan in Korea has been reported to cause amenorrhea for female and azoospermia, oligozoospermia or reduced sperm motility for male. 2-BP was investigated through 21 days of repeated dose in male Sprague-Dawley rats. The dose levels per body weight were 0 (control), 250,500 and 1,000 mg/kg. 2-BP dissolved in vehicle olive oil was injected into the intraperitoneum 6 times per week for 3 weeks, but 1,000 mg/kg dose group was 2 weeks because of serious illness. Male rats showed significant decreases in body weight and right and left testis showed typical weight losses depending on the 2-BP. The number of white blood cell and red blood cell , percentage of monocytes, and hemoglobin decreased significantly in high dose (P< 0.05). Red cell volume distribution width increased significantly in the high dose (P< 0.05). Histopathological findings of testes showed a decrease of spermatogenic cells, exfoliation of spermatid and spermatocyte, vacuolization of Sertoli cells and hyperplasia of Leydig cells. Protein band density between 113,000 dalton ($\beta$-galactosidase) and 53,900 dalton (ovalbumin) has decreased in 250 mg/kg dose group, but it has gradually increased to the higher density in 1,000 mg/kg dose group than in control group.

A Simple Dosing Scheme for Intravenous Busulfan Based on Retrospective Population Pharmacokinetic Analysis in Korean Patients

  • Busulfan is an antineoplastic agent with a narrow therapeutic window. A post-hoc population pharmacokinetic analysis of a prospective randomized trial for comparison of four-times daily versus once-daily intravenous busulfan was carried out to search for predictive factors of intravenous busulfan (iBu) pharmacokinetics (PK). In this study the population PK of iBu was characterized to provide suitable dosing recommendations. Patients were randomized to receive iBu, either as 0.8 mg/kg every 6 h or 3.2 mg/kg daily over 4 days prior to hematopoietic stem cell transplantation. In total, 295 busulfan concentrations were analyzed with NONMEM. Actual body weight and sex were significant covariates affecting the PK of iBu. Sixty patients were included in the study (all Korean; 23 women, 37 men; mean [SD] age, 36.5 [10.9] years; weight, 66.5 [11.3] kg). Population estimates for a typical patient weighing 65 kg were: clearance (CL) 7.6 l/h and volume of distribution (Vd) 32.2 l for men and 29.1 L for women. Inter-individual random variabilities of CL and $V_d$ were 16% and 9%. Based on a CL estimate from the final PK model, a simple dosage scheme to achieve the target $AUC_{0-inf}$ (defined as median AUC0-inf with a once-daily dosage) of 26.18 $mg/l{\cdot}hr$, was proposed: $24.79{\cdot}ABW^{0.5}mg$ q24h, where ABW represents the actual body weight in kilograms. The dosing scheme reduced the unexplained interindividual variabilities of CL and Vd of iBu with ABW being a significant covariate affecting clearance of iBU. We propose a new simple dosing scheme for iBu based only on ABW.
