젖소의 개체인식 및 형상 정보화를 위한 컴퓨터 시각 시스템 개발 (I) - 반문에 의한 개체인식 - (Development of Computer Vision System for Individual Recognition and Feature Information of Cow (I) - Individual recognition using the speckle pattern of cow -)

  • Cow image processing technique would be useful not only for recognizing an individual but also for establishing the image database and analyzing the shape of cows. A cow (Holstein) has usually the unique speckle pattern. In this study, the individual recognition of cow was carried out using the speckle pattern and the content-based image retrieval technique. Sixty cow images of 16 heads were captured under outdoor illumination, which were complicated images due to shadow, obstacles and walking posture of cow. Sixteen images were selected as the reference image for each cow and 44 query images were used for evaluating the efficiency of individual recognition by matching to each reference image. Run-lengths and positions of runs across speckle area were calculated from 40 horizontal line profiles for ROI (region of interest) in a cow body image after 3 passes of 5$\times$5 median filtering. A similarity measure for recognizing cow individuals was calculated using Euclidean distance of normalized G-frame histogram (GH). normalized speckle run-length (BRL), normalized x and y positions (BRX, BRY) of speckle runs. This study evaluated the efficiency of individual recognition of cow using Recall(Success rate) and AVRR(Average rank of relevant images). Success rate of individual recognition was 100% when GH, BRL, BRX and BRY were used as image query indices. It was concluded that the histogram as global property and the information of speckle runs as local properties were good image features for individual recognition and the developed system of individual recognition was reliable.

Prevalence of Low Back Pain and Associated Risk Factors among Farmers in Jeju

  • Background: We aimed to investigate the prevalence of low back pain (LBP) and its associated agricultural work-related, biomechanical factors among this population. Methods: We analyzed initial survey data from the Safety for Agricultural Injury of Farmers cohort study involving adult farmers in Jeju Island. The prevalence of LBP was calculated with associated factors. Results: In total, 1,209 participants were included in the analysis. The overall prevalence of LBP was 23.7%. Significant associations for LBP were the type of farming activity, length of farming career, prior agricultural injury within 1 year, and stress levels. Multivariate logistic regression analysis revealed three biomechanical factors significantly related to LBP: repetitive use of particular body parts; the inappropriate posture of the lower back and neck. Conclusions: Some occupational, and biomechanical risk factors contribute to LBP. Therefore, postural education, injury prevention education, and psychological support will be needed to prevent LBP.

대학생의 배가로근과 뭇갈래근 두께와 척추정렬간의 상관관계 (Correlations between the Muscle Thickness of the Transverse Abdominis and the Multifidus Muscle with Spinal Alignment in College Students)

  • Purpose: The transverse abdominis and themultifidus muscle are located in the core. They surround one's trunk and help in body stabilization. Specifically, they control spine articulation to maintain posture and balance. Therefore, weakened deep muscle in the trunk may cause spinal malalignment. This study aims to compare the correlation between the thickness of the transverse abdominis and the multifidus muscle and the spine alignment among college students in their 20s. Methods: This study measured the thickness of the transverse abdominis and the multifidus muscle of 42 healthy college students in their 20s using ultrasonic waves. The thickness of the muscle was measured for the length of the cross-section except for fascia. The thickness of the left and right muscles was measured, and the mean value was calculated. As the thickness of the transverse abdominis can increase because of pressure during exhalation, it was measured at the last moment of exhalation. Spinal alignment was measured by the kyphosis angle, lordosis angle, pelvic tilt, trunk inclination, lateral deviation, trunk imbalance, and surface rotation using Formetric III, which is a three-dimensional imaging equipment. They were measured for three times, and the mean values were calculated. The general characteristics of the subjects were analyzed using descriptive statistics. The correlations between each factor were analyzed using Pearson's correlation analysis. Results: The transverse abdominis showed asignificant correlation with trunk inclination (p<.05). The multifidus muscle showed a significant positive correlation with pelvic tilt and a negative correlation with surface rotation (p<.05). Conclusion: The thickness of transverse abdominis and the multifidus muscle appears to influence spinal alignment. Specifically, the multifidus muscle, which plays an important role on the sagittal plane, influences surface rotation, thus making it an important muscle for scoliosis patients. Therefore, a strengthening training program for the transverse abdominis and the multifidus muscle is necessary according to specific purposes among adults with spinal malalignment.

폐쇄성 수면 무호흡증 환자에 있어서 하악 재위치 장치 장착과 체위에 따른 상기도 구조와 근활성도의 변화에 관한 EMG 및 두부방사선학적 연구 (EMG AND CEPHALOMETRIC STUDY ON CHANCES IN UPPER AIRWAY STRUCTURES AND MUSCLE ACTIVITIES ACCORDING TO THE USE OF MANDIBULAR REPOSITIONING APPLIANCE AND BODY POSTURE IN OSA PATIENTS)

  • 폐쇄성 수면 무호흡증(obstructive sleep apnea, 이하 OSA로 표시)은 수면 도중 계속 반복 되어 일어나는 상기도 폐쇄로 인하여 무호흡증을 나타내는 장애(disorder)이다. 근래의 연구에 의하면 상기도 구조의 해부학적 요인뿐 아니라 생리적 요인도 무호흡 발생에 기여한다고 하였으며 또한 이설근이 상기도 유지에 중요한 역할을 한다고 하였다. OSA의 치료를 위해 다양한 술식이 시행되었으며 하악 재위치 장치를 통한 치료 역시 양호한 결과를 나타낸다고 하였다. 그러나 하악 재위치 장치 장착에 따른 구조적 생리적 작용기전에 대한 연구는 미비한 상태이다. 이에 본 저자는 연세대학교 치과대학병원 교정과에 내원한 OSA 환자 26명 (남 17명 여 9명)과 일반 정상 성인 20명 (남 10명 여 10명)을 대상으로 앙와위에서 장치 장착 전후의 두부방사선사진을 채득하여 장치에 따른 상기도의 구조 변화를 연구하고 두군 사이의 장치 장착에 따른 효과를 비교하였으며, 또한OSA환자 14명(남 10명 여 4명)을 대상으로 체위 변화와 장치장착에 따른 이설근 근전도 변화를 연구하여 다음과 같은 결론을 얻었다. 1. 상기도 구조에 대한 두부계측학적 측정치 중 연구개의 길이, 연구개 최대 두께와 SPAS, MAS, VAL, H-H1, MP-H 에서 정상군과 OSA군 사이의 통계적 유의차를 보였으며, IAS와 EAS는 두 군간에 통계적 유의차를 보이지 않았다. 2. 정상군과 OSA군 모두에서 장치 장착에 따라 후두개가 전방 이동하면서 후두개 수준(epiglottis level)의 기도 폭경은 증가하였고 연구개의 최대 두께가 변하였으며 설골은 전방 이동하였으나 두 군 모두 IAS에서는 다양한 반응을 나타내었다. 장치 장착에 따른 상기도 구조에 대한 효과는 두 군 사이에 서로 차이를 나타내었다. 3. 체위 변화에 따라 앙와위에서 이설근 근전도가 증가하는 경향을 나타내었으나 통계적 유의차는 없었으며, 직립위와 앙와위 모두에서 하악 재위치 장치 장착시에 통계적으로 유의하게 이설근 근전도가 증가하였다. 하악 재위치 장치는 상기도의 해부학적 구조뿐 아니라 상기도 생리에도 영향을 미치는 것으로 생각되며 장치에 대한 반응은 정상군과 OSA군 사이에 차이를 나타내었고, 두 군은 상기도 생리에 있어서도 서로 다르다고 사료된다.

측후방융합술을 시행한 요추관협착증 환자의 Critical Pathway 적용효과 (Effects of a Critical Pathway of Posterolateral Fusion in Patients with Lumbar Spinal Stenosis)

  • The case management has been applied to improve the quality of care and the cost-effectiveness in the most health care institutions. In a way of case management, the critical pathway(CP) has been executed in many acute care settings, focused on the diagnoses with high cost, high volume, and high risk. This study was conducted to develop a case management program using CP as an intervention of patients with lumbar spinal stenosis for the surgery of posterolateral fusion, and to find out the effects of the critical pathway on the quality of nursing care, patient satisfaction as an outcome of care, length of stay and medical charge, and nurses' job satisfaction. At the same time, patients' functional states were checked with the Oswestry Low Back Pain Index, to show that the CP would not decrease the patients' function compared to the control group. The subjects were 25 control patients with a usual operation of lumbar fusion and 25 experimental patients with CP. They were all female, aged $50s{\sim}70s$, admitted in the Orthopedic surgery ward of a university hospital. Also nurses on the floor using CP were asked to respond to measurement tool of job satisfaction before and after the application of CP, and compared with other nurses on the different wards. Data were analyzed with t-test for continuous variables and chi-square for non-parametric variables in addition to the reliability test of the measurement tools. The results of this study were as followings: 1. Patients' functional states The differences in Oswestry scores of the experimental and control groups assessed at preoperation and at discharge were not statistically significant. The change in scores of the experimental group measured at preoperation and at discharge was larger than that of the control group, however the difference was not statistically significant. The results indicate that the CP did not decrease the patients' functional status. 2. The quality of nursing care The total of quality of nursing care given to the experimental group was better than that of the control group(P=.000). In addition, the experimental group showed better scores of quality of every item of care than the control group(P=.000 -.004). 3. Patient satisfaction Patients of the experimental group were not more satisfied with general care than the control group. But they were more satisfied with discharge care of 'explanation about medication, body posture, and brace application' and 'explanation about the adjustment of daily living and exercise during recovery'(P= .047, P=.028). 4. Nurses' job satisfaction Nurses working with the CP showed more job satisfaction than before the CP introduction(P=.048). But the control group of nurses on a different floor showed no change in job satisfaction at the same period of time. 5. Length of stay and medical charge The mean length of stay of the experimental group was shorter than that of the control group without statistical significance. The charge of medication and treatment of the experimental group were smaller than that of the control group(P=.011, P=.000). The results of the study support that the case management using critical pathway enables to improve the quality of care and job satisfaction, to reduce the medical charge, and consequently to increase satisfaction with care. However, the case management should be instituted focusing on the quality improvement of nursing and the client satisfaction, not just for the purpose of cost-effectiveness of health care facilities.

고령자를 대상으로 12주간 운동이 대요근 및 대퇴부 근황단면적에 미치는 영향 (Changes in Psoas Major and Quadriceps Cross Sectional Area in Elderly People after 12 Weeks of Exercise)

  • 본 연구는 남녀 노인을 대상으로 12주간의 트레이닝을 실시한 후, 대요근과 대퇴부 근육의 근 횡단면적의 변화를 검토하였다. 연구 대상자는 65세에서 80세까지의 노인 58명(남성 30명, 여성 28명)이었으며, 대조군 19명과 운동군 39명으로 나누어 실시하였다. 측정항목은 신장, 체중, 체질량, 대요근과 대퇴부의 근 횡단면적을 측정하였다. 근육의 평가는 자기공명영상법(MRI)을 사용하여 실험 전, 후 실시하였다. 운동군은 대요근과 대퇴부 근육을 증가시키기 위하여 특별히 고안된 기계를 사용하여 평균 주 2일, 12주 동안 23번의 트레이닝을 실시하였다. 대요근의 측정부위는 L4와 L5의 중간부위를 분석하고, 대퇴부는 대퇴골과 무릎까지 길이를 나누어 50%부위에서 분석하였다. 트레이닝 후, 운동군의 대요근 근횡단면적은 9.4% 증가하였다(남성 11.5%, 여성 8.4%). 그러나 대퇴부 근 횡단면적은 남녀 모두 운동 후 증가경향은 나타나지 않았다. 또한 같은 기간 대조군의 근 횡단면적에서도 유의한 변화는 나타나지 않았다. 대요근은 자세 유지와 허리, 골반 및 대퇴부의 연결에 중요한 역할을 담당하기 때문에, 특히 노인의 이동활동에 중요하다고 사료된다. 본 연구에서는 새로이 개발된 트레이닝 기계를 사용하여 단기간트레이닝 후, 대요근 근 횡단면적의 증가를 확인하였으며, 이것은 노인들의 근력 및 이동능력의 향상 가능성이 시사되었다.
