달리기 시 발의 인체측정학적 변인과 운동역학적 변인의 관계 (The Relationship between Anthropometric Parameters of the Foot and Kinetic Variables during Running)

  • Objective: The aim of this study was to investigate the correlation coefficients between anthropometric parameters of the foot and kinetic variables during running. Method: This study was conducted on 21 healthy young adults (age: $24.8{\pm}2.1yes$, height: $177.2{\pm}5.8cm$, body mass: $73.3{\pm}7.3kg$, foot length: $256.5{\pm}12.3mm$) with normal foot type and heel strike running. To measure the anthropometric parameters, radiographs were taken on the frontal and sagittal planes, and determined the length and width of each segment and the navicular height. Barefoot running was performed at a preferred velocity ($3.0{\pm}0.2m/s$) and a fixed velocity (4.0 m/s) on treadmill (Bertec, USA) in order to measure the kinetic variables. The vertical impact peak force, the vertical active peak force, the braking peak force, the propulsion peak force, the vertical force at mid-stance (vertical ground reaction when the foot is fully landed in mid-stance or at the point where the weight was uniformly distributed on the foot) and the impact loading rate were calculated. Pearson's correlation coefficient was used to investigate the relationship between anthropometric variables and kinetical variables. The significance level was set to ${\alpha}=.05$. Results: At the preferred velocity running, the runner with longer forefoot had lower active force (r=-.448, p=.041) than the runner with short forefoot. At the fixed velocity, as the navicular height increases, the vertical force at full landing moment increases (r= .671, p= .001) and as the rearfoot length increases, the impact loading rate decreases (r=- .469, p= .032). Conclusion: There was a statistically significant difference in the length of fore-foot and rearfoot, and navicular height. Therefore it was conclude that anthropometric properties need to be considered in the foot study. It was expected that the relationship between anthropometric parameters and kinetical variables of foot during running can be used as scientific criteria and data in various fields including performance, injury and equipment development.

Effects of heme oxygenase-1 upregulation on isoproterenol-induced myocardial infarction

  • The present study was designed to examine the effect of heme oxygenase-1 (HO-1) induction by cobalt protoporphyrin (CoPP) on the cardiac functions and morphology, electrocardiogram (ECG) changes, myocardial antioxidants (superoxide dismutase [SOD] and glutathione [GSH]), and expression of heat shock protein (Hsp) 70 and connexin 43 (Cx-43) in myocardial muscles in isoproterenol (ISO) induced myocardial infarction (MI). Thirty two adult male Sprague Dawely rats were divided into 4 groups (each 8 rats): normal control (NC) group, ISO group: received ISO at dose of 150 mg/kg body weight intraperitoneally (i.p.) for 2 successive days; ISO + Trizma group: received (ISO) and Trizma (solvent of CoPP) at dose of 5 mg/kg i.p. injection 2 days before injection of ISO, with ISO at day 0 and at day 2 after ISO injections; and ISO + CoPP group: received ISO and CoPP at a dose of 5 mg/kg dissolved in Trizma i.p. injection as Trizma. We found that, administration of ISO caused significant increase in heart rate, corrected QT interval, ST segment, cardiac enzymes (lactate dehydrogenase, creatine kinase-muscle/brain), cardiac HO-1, Hsp70 with significant attenuation in myocardial GSH, SOD, and Cx-43. On the other hand, administration of CoPP caused significant improvement in ECG parameters, cardiac enzymes, cardiac morphology; antioxidants induced by ISO with significant increase in HO-1, Cx-43, and Hsp70 expression in myocardium. In conclusions, we concluded that induction of HO-1 by CoPP ameliorates ISO-induced myocardial injury, which might be due to up-regulation of Hsp70 and gap junction protein (Cx-43).

Clinical Characteristics of Spinal Epidural Abscess Accompanied by Bacteremia

  • Objective : The treatment of choice for spinal epidural abscess (SEA) generally is urgent surgery in combination with intravenous antibiotic treatment. However, the optimal duration of antibiotic treatment has not been established to date, although 4-8 weeks is generally advised. Moreover, some researchers have reported that bacteremia is a risk factor for failure of antibiotic treatment in SEA. In this study, we investigated the clinical characteristics of SEA accompanied by bacteremia and also determined whether the conventional 4-8 weeks of antibiotic treatment is sufficient. Methods : We retrospectively reviewed the medical records and radiological data of 23 patients with bacterial SEA who underwent open surgery from March 2010 to April 2020. All patients had bacteremia preoperatively and underwent weeks of perioperative antibiotic treatments based on their identified organisms until all symptoms of infection disappeared. All patients underwent microbiological studies of peripheral blood, specimens from SEA and concomitant infections. The mean follow-up duration was 35.2 months, excluding three patients who died. Results : The male : female ratio was 15 : 8, and the mean age was 68.9 years. The SEA most commonly involved the lumbar spinal segment (73.9%), and the mean size was 2.9 vertebral body lengths. Mean time periods of 8.4 days and 16.6 days were required from admission to diagnosis and from admission to surgery, respectively. Concomitant infections more frequently resulted in delayed diagnosis (p=0.032), masking the symptoms of SEA. Methicillin-sensitive Staphylococcus aureus was the most commonly identified pathogen in both blood and surgical specimens. Seventeen patients (73.9%) showed no deficits at the final follow-up. The overall antibiotic treatment duration was a mean of 66.6 days, excluding three patients who died. This duration was longer than the conventionally advised 4-8 weeks (p=0.010), and psoas or paraspinal abscess required prolonged duration of antibiotic treatment (p=0.038). Conclusion : SEA accompanied by bacteremia required a longer duration (>8 weeks) of antibiotic treatment. In addition, the diagnosis was more frequently delayed in patients with concomitant infections. The duration of antibiotic treatment should be extended for SEA with bacteremia, and a high index of suspicion is mandatory for early diagnosis, especially in patients with concomitant infections.

패션 CAD 시스템을 활용한 성인남성의 타이트 핏 토르소원형 설계방법 연구 - 휜체형과 숙인체형을 중심으로 - (A study on the design method of tight fit Thorso patterns for adult males using fashion CAD system - Focusing on the sway back somatotype and the bend forward somatotype -)

  • This study tried to suggest a drafting method to draft tight fit torso patterns suitable for adult males with a sway back somatotype and a bend forward somatotype by setting and distributing the ease through the completion of the somatic surface pattern using the 3D body surface segment method. It was intended to be presented as a drafting method. As for the research method, the suitability of the somatic surface patterns and the tight fit torso patterns were confirmed by the evaluation of virtual wear, and the patterns were modified and supplemented. The research results are as follows. In the first evaluation of the tight fit torso patterns, the average, for 55 evaluation items, was 3.92 points for the sway back somatotype and 3.89 points for the bend forward somatotype. In the second evaluation, the bend forward somatotype was 4.51 points and the sway back somatotype was 4.62 points. The chest circumference ease amount for the bend forward somatotype and the sway back somatotype are 6.5% (6.8 cm) and 7% (6.8 cm) of the chest circumference, respectively, and the distribution of the front and back ease is the same at 4:6 (2.72 cm:4.08 cm). The waist circumference ease amount is 6.5% (5.8cm) and 6.5% (5.6 cm) of the waist circumference dimensions, and the distribution of the front and backease are 5:5 (1.45 cm: 1.45 cm) and 4:6 (2.24 cm: 3.36 cm), respectively. The completed tight fit torso patterns were converted into institutional formulas and presented as a drafting method.

A Study on $Na^+$ and Water Reabsorption in the Nephron Segment Beyond Proximal Tubule Measured by Lithium Clearance

  • During the past few years it has been proposed that lithium clearance can be used as a reliable measure for the outflow of tubular fluid from the proximal tubule. This study was aimed to characterize the inflow dependent reabsorption of Na in renal tubule beyond the proximal tubule. For this purpose, lithium clearance was used as a measure for the inflow from the proximal tubule and the changes in reabsorption fraction of Na and water were determined in rabbits. Rabbits were pretreated with hypotonic saline solutions for an hour (50 mM/L NaCl, 20 ml/hr/kg). And then a hypertonic solution of 500 mM/L NaCl (20 ml/kg) was administered intraperitoneally in conjunction with a bolus of LiCl solution (2 mM/kg, i.v.) for conditioning the $C_{Li}$ and urine flow rate. To rule out the effect of $Li^+$ on tubular functions, a bolus of NaCl solution (2 mM/kg, i.v.) was administered. Fifteen, thirty, and sixty minutes after injection of hypertonic saline arterial blood and urine samples were taken. Urinary and plasma concentrations as well as urinary output of $Li^+,\;Na^+\;and\;K^+$ were measured. From these $C_{Li},\;C_{Na}$ and the reabsorption fraction of Na and water $(Fr_{Na}\;&\;FrH_2O)$ were calculated. These results were compared with those from control groups in which the same amount of isotonic saline (145 mM/L NaCl) and of 15% dextran solution were administered in the same way as that in experimental group. Followings are the results obtained. 1) The plasma concentration of $Na^+$ in rabbits injected with hypertonic saline reached the peak value after 15 min and thereafter no significant change was observed. Hematocrit values did not show any change, while urinary excretion of $Na^+$ increased markedly during the first 15 min and decreased thereafter. These results were not affected by an injection of a small amount of LiCl. 2) The clearances of $Li^+,\;Na^+\;and\;K^+$ in rabbits injected with hypertonic saline and LiCl solution decreased. 3) In spite of the variation in $C_{Li},\;Fr_{Na}$ did not show any significant change while $FrH_2O$ increased gradually. 4) $C_{Li}$ decreased also in rabbits received isotonic saline. $Fr_{Na}$ tended to be higher than that in hypertonic saline group, while $FrH_2O\;and\;Fr_{Na}$ did not associated with the decrease in $C_{Li}$. 5) $C_{Li}$ of the rabbits received dextran solution fluctuated persistently and $Fr_{Na}\;and\;FrH_2O$ did not change in along with $C_{Li}$ although $Fr_{Na}$ had a tendency to be higher than that in hypertonic saline group. 6) From the above results it was concluded that: (a) In rabbits with normal body store of $Na^+$, the $Fr_{Na}$ of renal tubule beyond proximal tubule. calculated from $C_{Li}$ as a measure of inflow from proximal tubule is constant in spite of variations in $C_{Li}$. (b) The $FrH_2O$ calculated from $C_{Li}$ is dependent largely upon ADH rather than inflow from proximal tubule. (c) When there is a decrease in plasma $Na^+$ concentration or ineffective body fluid. $Li^+$ reabsorption may occur in the thick segnent of Henle's loop and hence the determination of $Fr_{Na}$ and $FrH_2O$ will not be easy one, but $Fr_{Na}$ is constant under the same experimental conditions.

진단용 고선량 CT를 이용한 $^{18}F$-FDG PET/CT 촬영시 정맥 조영제가 SUV 값에 미치는 영향 (The Effect of Intravenous Contrast on SUV Value in $^{18}F$-FDG PET/CT using Diagnostic High Energy CT)

  • 목적 : 현재 PET/CT에서 CT의 역할은 단순히 감쇠 보정만을 위한 도구에서 진단을 위한 도구로 그 역할이 발전해 가고 있다. 따라서 양질의 CT 영상을 얻기 위한 정맥 조영제의 사용과 고선량의 CT 촬영이 요구되고 있으며 최근 조영제가 감쇠 보정한 PET 영상에 크게 영향을 미치지 않는다는 보고가 나오고 있다. 이에 본 연구에서는 고선량 CT에서 정맥 조영제의 사용이 감쇠 보정에 미치는 영향을 알아보았다. 대상 및 방법 : 총 13명의 환자에게 PET/CT 검사를 시행하였다. $^{18}F$-FDG를 주사하고 1시간 후에 촬영하였고, 첫 번째로 조영제의 사용 없이 CT(NECT-120 kVp, 130 mAs)를 촬영하고 이어서 바로 조영제를 사용하여 CT(CECT)를 촬영한 후 PET을 시행하였다. 각 환자마다 NECT와 CECT에 의해 각각 보정된 PET 영상에서 10곳의 신체 부위(병변이 없는 폐첨부, 폐기저부, 상행대동맥, 간의 상부 및 하부, 비장, 척추, 대퇴골두, 장요근과 10번 척추와 척추 인접 근육) 에서 HU와 maxSUV 값을 구하였고, 종양과 임파선 병변의 값도 함께 측정하였다. 결과: 조영제를 사용하였을 때 국소적으로 섭취가 증가된 곳은 관찰되지 않았다. 총 130개의 정상 조직의 maxSUV값을 측정하여 비교하였을 때, 조영제를 사용한 군과 사용하지 않은 군의 평균 maxSUV 값은 각각 $1.1{\pm}0.5,\;1.0{\pm}0.5$이고 두 군 사이에 p<0.001로 통계적으로 유의한 차이를 보였다. 정상 조직에 대해 Bland-Altman 분석을 시행하였을 때 일치의 한계 범위는 $0.1{\pm}0.3$ 이었다. 10개의 대상 영역 중 폐첨부, 폐하부, 상행대동맥, 간의 상부와 하부, 비장에서 maxSUV 값이 두군에서 유의한 차이(p<0.05)가 있었다. 총 39개 병변의 maxSUV 값을 측정하여 비교하였을 때, 조영제를 사용한 군과 사용하지 않은 군의 평균 maxSUV값은 각각 $4.7{\pm}2.0,\;4.4{\pm}2.0$이고 두 군 사이에 p<0.001로 통계적으로 유의한 차이를 보였다. 병변에 대해 Bland-Altman 분석을 시행하였을 때 일치의 한계 범위는 $0.4{\pm}0.8$ 이었다. 결론: 조영제로 인해 maxSUV의 값은 증가되었으나 정상 조직과 병변에서 일치의 한계 범위가 매우 좁았다. 따라서 진단용 고선량 CT를 이용한 PET/CT 촬영 시에 조영제의 사용이 임상 상황에서 판독에 큰 영향을 미치지 않는 것으로 생각된다.

철봉 내리기 공중 동작의 운동학적 분석(I) (Kinematic Analysis of Airborne Movement of Dismount from High Bar(I))

  • 본 연구는 철봉운동에서 기본이 되는 동작인 몸펴 한번 뒤돌아 내리기 동작을 단계적으로 분석한 후, 이를 토대로 하여 현재 시합상황 중 가장 널리 사용되는 응용동작인 몸펴 두 번 뒤돌아 내리기 동작과 몸펴 두 번 뒤돌며 한번 비틀어 내리기 동작의 운동학적 분석을 수행하여 개개분절 간의 상호작용을 해부학적 3차원 각운동과 각속도로 설명하고 이해할 수 있는 운동학적 자료를 제시하는데 있다. 피험자들은 현재 K대학교에 재학중이며 대학 대표선수인 남자 기계체조 선수 7명을 선정하였으며, 연구에 사용된 인체의 모델은 Zatsiorsky와 Seluyanov(1983, 1985)이 사용한 16개의 분절로 이루어진 인체의 모델을 사용하였다. 신체무게중심이 이동방향을 설명할 수 있는 투사각도 및 투사속도는 공중동작의 회전수가 증가할수록 신체무게중심이 투사되는 각도가 증가되며, 이렇게 증가된 신체무게중심의 투사각도는 신체무게중심의 최고점을 증가시키는 경향을 보였다. 3차원 투사속도를 살펴본 결과 Z방향(수직방향)은 공중돌기 회전수가 증가할수록 증가하는 경향이 나타났으나, 운동진행 방향인 Y방향 속도와 좌우측 기울기를 설명할 수 있는 X방향 속도에서는 의미 있는 차이를 보이지 않았다. 철봉 내리기 공중동작에서의 신체분절 및 각도 변화도 중요하지만 각운동량을 만들어내기 위한 동작준비구간의 각도 변화가 더욱더 중요하다고 할 수 있다. 즉, 상체가 철봉 아래 봉과 수직될 때부터 릴리즈 순간까지의 각도 변화에 주목해야 하는 데, 회전수가 증가할수록, 어깨관절 각도와 엉덩관절 각도 변화가 두드러지게 나타나 준비구간의 추기기 동작(Whip swing)의 주된 관절로 작용을 한다. 관성좌표계에 대한 상체의 움직임을 나타내는 3차원 방향의 각도 즉, 뒤돌기(somersault)각도, 틀기(twist)각도 그리고 기울기(tilt)각도로 설명이 되는데, 본 연구의 결과 릴리즈시 뒤돌기 각은 세가지 내리기 동작 유형에 따라 평균 57,7도, 38.8도 그리고 39.7도로 나타났으며, 기울기 각은 평균 -1.5도, -5.4도 그리고 -8.4도로 유의한 차이를 보이고 있으며, 틀기각도는 평균 13.4도, 10.6도 그리고 23.3도로 몸펴 두번 뒤돌며 한번 비틀어 내리기 경우 가장 큰 수치를 나타냈다.

방패광대노린재 국내 분포, 기주식물 및 기초 생태 보고 (A Report of Basic Ecology and Distribution, Host Plants of Cantao ocellatus (Hemiptera: Scutelleridae) in Korea)

  • 방패광대노린재는 2012년 제주도, 진도, 통영, 광주, 태안에서 분포가 확인되었다. 성충은 6월 하순에서 7월 초순경에 예덕나무에 출현하며 10월 말까지 관찰이 가능하다. 실내 사육은 온도 $25^{\circ}C({\pm}2)$, 습도 65%(${\pm}2$), 광주기 16L:8D의 조건에서 성충의 암 수 형태 측정, 발육특성, 기주 및 산란선호성을 조사하였다. 체장(female: 26.20 mm, male: 23.88 mm), 체폭(female: 11.35 mm, male: 10.57 mm ), 두폭(female: 3.84 mm, male: 3.64 mm), 주둥이(female: 7.90 mm, male: 7.27 mm). 더듬이(female: 9.87 mm, male: 9.69 mm), 앞다리(female: 12.50 mm, male: 12.27 mm), 가운뎃다리(female: 14.61 mm, male: 13.12 mm), 뒷다리(female: 16.90 mm, male: 16.53 mm), 생체중(female: 0.46 g, male: 0.31 g)으로 각각 조사되었다. 우리나라에는 앞가슴등판 끝에 가시가 있는 개체가 있고, 가시의 유무는 마지막 5령 탈피시 생겨난다. 암 수 구분은 생식기와 배마디의 점무늬로 구별된다. 기주선호성은 예덕나무, 무늬예덕나무, 피마자, 털인동, 감귤나무, 느티나무의 열매, 엽병, 주맥, 가지를 흡즙하였다.

도마 Lopez 동작의 운동학적 분석: YHS 선수의 성공과 실패 사례 비교 (Kinematical Analysis of Lopez Motion in Horse Vault: Comparison between Successful and Failed Trials)

  • 본 연구는 세계적인 선수를 대상으로 남자 기계체조 도마 종목에서 로페즈 기술을 수행했을 때 성공 동작과 실패 동작에 따라 운동학적 차이를 비교 분석하였다. 연구대상은 올림픽 금메달리스트 YHS 선수(나이: 27세, 신장: 160cm, 체중: 53kg중)이였고, 14대의 적외선 고속카메라를 이용하여 도마 동작을 촬영하였다. 원하는 동작 자료획득과 분석을 위해 19mm 반사 마커 26개를 해부학적 주요지점에 부착시켰고, 15개 분절 신체모델을 사용하여 운동학적 변인 계산을 실시하였다. 분석 결과 첫째, 구름판 접촉구간에서 성공 동작의 소요시간이 실패 동작보다 길었으며, 성공 동작은 슬관절의 가동범위를 실패 동작보다 크게 하였다. 둘째, 제1비약에서 성공 동작과 실패 동작의 소요시간은 동일하였다. 성공 동작은 실패 동작보다 짧은 수평이동을 했으며, 높은 수직위치와 도마 착지각을 보였다. 셋째, 도마 접촉구간에서 성공 동작의 소요시간이 실패 동작보다 짧았다. 성공 동작은 도마접촉 순간에 실패 동작보다 좌측 견관절각을 크게 하였다. 넷째, 제2비약에서는 성공 동작의 소요시간이 실패 동작보다 길었으며, 긴 수평이동을 나타냈다. 성공 동작은 도마이륙 순간 우측 견관절각을 크게 하였고, 비틀기 각속도는 빨랐다. 종합하면 YHS 선수가 안정적인 로페즈 기술을 구사하기 위해서는 도마접촉 순간에 높은 자세로 진입하여 도마이륙 순간에서는 우측 견관절각을 크게하여 빠른 비틀기 각속도를 만들어야 한다. 최대높이와 착지 순간에서는 좌·우견관절각을 작게하여 관성모멘트를 줄이고 신체중심을 낮춰야 할 필요가 있다.

Segmental Deformity Correction after Balloon Kyphoplasty in the Osteoporotic Vertebral Compression Fracture

  • Objective : Balloon kyphoplasty can effectively relieve the symptomatic pain and correct the segmental deformity of osteoporotic vertebral compression fractures. While many articles have reported on the effectiveness of the procedure, there has not been any research on the factors affecting the deformity correction. Here, we evaluated both the relationship between postoperative pain relief and restoration of the vertebral height, and segmental kyphosis, as well as the various factors affecting segmental deformity correction after balloon kyphoplasty. Methods : Between January 2004 and December 2006, 137 patients (158 vertebral levels) underwent balloon kyphoplasty. We analyzed various factors such as the age and sex of the patient, preoperative compression ratio, kyphotic angle of compressed segment, injected PMMA volume, configuration of compression, preoperative bone mineral density (BMD) score, time interval between onset of symptom and the procedure, visual analogue scale (VAS) score for pain rating and surgery-related complications. Results : The mean postoperative VAS score improvement was $4.93{\pm}0.17$. The mean postoperative height restoration rate was $17.8{\pm}1.57%$ and the kyphotic angle reduction was $1.94{\pm}0.38^{\circ}$. However, there were no significant statistical correlations among VAS score improvement, height restoration rate, and kyphotic angle reduction. Among the various factors, the configuration of the compressed vertebral body (p=0.002) was related to the height restoration rate and the direction of the compression (p=0.006) was related with the kyphotic angle reduction. The preoperative compression ratio (p=0.023, p=0.006) and injected PMMA volume (p<0.001, p=0.035) affected both the height restoration and kyphotic angle reduction. Only the preoperative compression ratio was found to be as an independent affecting factor (95% CI : 1.064-5.068). Conclusion : The two major benefits of balloon kyphoplasty are immediate pain relief and local deformity correction, but segmental deformity correction achieved by balloon kyphoplasty does not result in additional pain relief. Among the factors that were shown to affect the segmental deformity correction, configuration of the compressed vertebral body, direction of the most compressed area, and preoperative compression ratio were not modifiable. However, careful preoperative consideration about the modifiable factor, the PMMA volume to inject, may contribute to the dynamic correction of the segmental deformity.
