The purpose of this study was to verify whether Mediterranean diet, which proved to have a significant effect on preventing dementia for people aged 65 or older, could be well modified to be a Korean-style Mediterranean diet. This study was performed as a randomized-controlled trial for 6 weeks. Functional physical changes, cognitive scores, depression scores and dietary changes were all assessed. The walking speed (P<0.001) and the cognitive scores were statistically improved in only the experimental group (P<0.001), and the depression scores were also significantly improved in only the experimental group (P<0.01). The dietary intake showed a 30% improvement for consuming more than 7 cups per day of vegetables and fruits. When the participants were contacted four months after the end of the study, 90% of them said that the Korean-style Mediterranean diet was feasible and 100% said the nutrition interventions helped them maintain the diet during their daily lives. The results suggest that although the Mediterranean diet can be difficult to apply, any limitations of this healthy diet can be overcome.
Objective: The purpose of this study was to compare with different distance across three conditions in stroke survivors with the grading of cognitive state. Design: Cross-sectional study. Methods: Twelve stroke patients who agreed to active participation were included. Participants were allotted to normal cognitive (CN) group (n=7) and cognitive impairment (CI) group (n=5) and then walked on a self-paced walkway at three conditions on the Time Up and Go (TUG) test and the 6 minute walk test (6MWT): 1) walking with your comfortable speed, 2) walking while carrying a tray with glasses, 3) walking with a verbal cognitive task. The TUG test was repeated three successful times on each condition. For the 6MWT, participants were tested one time. Results: The CI group walked slower than the CN group at the three conditions on the TUG test. However, there was no significant difference between two groups to each condition. A significant effect of dual tasking was found only in error of verbal cognitive task condition for the TUG test (p<0.05). On the 6MWT, the participants in the CI group walked short distance rather than the CN group (p<0.05). There were significant differences between two groups not only at all conditions but also at error of verbal cognitive task condition as well (p<0.05). Conclusions: To consider the results of different distances such as the TUG test and the 6MWT, we think that exercises in long distance would be more effective to patients with CI. Those would be improved patient's endurance in cognitive problem.
Objective: The purpose of this study was to determine the effect of the abdominal bracing (AB) and abdominal bracing combined with ankle dorsiflexion (ABDF) on abdominal muscle thickness and strength in patients with chronic low back pain (LBP). Design: Two group pretest posttest design. Methods: Sixteen subjects were divided randomly into two group: ABDF group (n=8), and alone AB group (n=8). The ABDF group practiced AB exercise with additional ankle dorsiflexion. AB group practiced only AB exercises. Subjects in both groups received ABDF exercise and AB exercise for 40 min per day, three days per week during a period of three weeks, respectively. All the subjects were evaluated for abdominal muscle thickness and strength before and after intervention using ultrasonography and MedX machine. Results: The external oblique (EO), internal oblique, transverse abdominis (TrA) muscle thickness and the strength produced at $48^{\circ}$, $60^{\circ}$, $72^{\circ}$ showed a significant increase in the ABDF group after intervention, with a more significant improvement in EO and TrA muscle thickness in the ABDF group compared with the AB group (p<0.05). Also, the strength at $48^{\circ}$ strength showed a significant improvement in the ABDF group than the AB group (p<0.05). Conclusions: The study results showed that abdominal muscle contraction exercises with AD in patients with LBP had an influence on abdominal muscle thickness and strength. Therefore, these findings suggest that ABDF may be useful approach for enhancement of abdominal muscle thickness and strength in patients with chronic low back pain.

Purpose: To grasp public opinion accurately, we conducted an opinion poll on beliefs and attitudes toward physician-assisted suicide (PAS). Methods: A randomized telephone survey ensuring a representative sample was conducted, 1,007 participants aged 18 years or older (response rate, 9.5%). Results: The main results are as follows: i) 61.1% of participants thought that the current social support system for terminally ill patients and their families is insufficient; ii) 60% of participants did not recognize the term "hospice and palliative care"; iii) 81.7% of participants would not like to receive life-sustaining treatment if there is no possibility of recovery; iv) 58.4% of participants would like to receive hospice and palliative care if they are diagnosed with a terminal illness; v) the priorities for dignified dying were preparing a support system to reduce the burden of care (28.6%), economic support including reduction of medical expenses (26.7%), expansion of hospice and palliative care services (25.4%), and legalization of PAS (13.6%); and vi) 58.3% of participants agreed that the expansion of hospice and palliative care should precede the legalization of PAS. Conclusion: Koreans currently want other efforts, including expansion of hospice and palliative care services, instead of the legalization of PAS.
Purpose: Incontinence-associated dermatitis (IAD) is widespread and a common problem in healthcare settings, It is defined as irritation of the perineal skin due to contact with urine, feces, or both. It is one of the major risk factors for the development of pressure injuries are regarded as a quality indicator of nursing care. It is important to understand the knowledge and attitudes of nurses who are primarily responsible for providing appropriate nursing care that includes differentiation of IAD from other skin breakdowns, and appropriate management. Therefore, the purpose of this study was to determine the knowledge, attitudes, and performance of IAD among clinical nurses in Korea. Methods: Data about IAD knowledge, attitudes, and performance were collected from August to November 2022. 656 nurses from six hospitals participated in the study. Results: The mean scores of nurses' knowledge, attitudes, and performance were 17.98±2.91, 76.34±7.85, and 91.53±16.18, respectively. Nurses' knowledge showed a positive correlation in both attitudes (r=0.27, p<.001) and performance (r=0.09, p=.02), and attitudes and performance (r=0.34, p<.001) also showed a positive correlation. Conclusion: To improve the knowledge and performance of nurses, an educational program consisting only of incontinence-related dermatitis is needed, and case-based education and clinical skills education are needed to enhance the ability to differentiate from pressure injury.
목적: 호스피스 완화의료 전문인력을 위한 기본단계 교육과정으로 개발된 표준교육 프로그램 시범교육의 결과를 평가하기 위함 방법: 표준교육 프로그램을 개발하기 위하여 미국의 Education in Palliative and End-of-life Care (EPEC)교육을 벤치마킹하여 국내 실정에 맞게 수정 보완하였다. 표준교육 프로그램은 총 19개의 세부 주제로 구성하였으며, 강의와 쌍방향식 강의, 소그룹 토론, 역할극 등의 다양한 교육방법을 적용하도록 설계하였다. 개발한 표준교육 프로그램의 내용과 교육방법의 타당성을 검증하기 위하여 3개 주제를 선택하여 호스피스 고위과정을 수료한 23명을 대상으로 시범교육을 실시하였다. 결과: 표준교육 프로그램 시범교육 대상자들은 피교육자 중심의 교육방법과 현장에서 활용할 수 있는 교육 내용에 대하여 만족하였으며, 강의를 위주로 하는 기존의 교육보다 효과적인 교육이라고 평가하였다. 결론: 표준교육 프로그램 시범교육은 그 효과가 긍정적이었으며 호스피스 완화의료 교육의 표준화와 교육 방법의 발전에 기여할 것으로 기대된다. 앞으로 표준교육 프로그램을 성공적으로 실행하기 위해서는 교육자 (Trainer)를 위한 교육이 실행되어야한다.
본 연구는 일상생활이 가능한 65세 이상 노인에게 연명치료중단 교육프로그램을 적용한 후 대상자들의 심폐소생술 금지(Do-Not-Resuscitate, DNR)에 대한 지식, 태도 및 시행의지에 미치는지 효과를 규명하기 위한 목적으로 시도 되었다. 연구대상은 S시에 소재한 J복지관출석 노인 총 40명 이었고 자료수집은 2012년 10월 2일에서 동년 11월 9일까지 진행되었다. 실험군에게는 연명치료중단 교육프로그램을 1회당 50분, 주 2회, 3주에 걸쳐 실시하였으며, 대조군에게는 시간차를 두고 설문지만 작성하게 하였다. 연구결과 연명치료중단 교육프로그램을 실시 후 DNR 지식점수(F=4.158;p=.049)와 DNR 태도점수(F=39.60; p=.001)는 실험군이 대조군에 비해 유의하게 높았다. 또한 연명치료중단 교육프로그램 실시 후 DNR 시행의지 점수는 형제자매(p=.344), 자녀(p=.109)를 제외한 본인(p=.006), 배우자(p=.039), 부모님(p=.006)에서 실험군이 대조군에 비해 유의하게 높았다. 이상의 연구결과로 연명치료중단 교육프로그램이 일반 노인의 DNR 지식, DNR 태도 및 형제자매, 자녀를 제외한 DNR 시행의지에 영향을 미치는 것으로 나타났으며 추후 노인들의 DNR 의사결정을 돕기위한 자료로 활용될 수 있겠다.
목적: 말기암환자들이 보완대체요법을 선택하게된 주관적 경험을 파악하여, 대상자 입장에서의 당위성을 이해하고 이용과정에서 발생할 수 있는 현상들을 이해함으로써 좀 더 바람직하고 효율적인 이용을 위한 호스피스 완화의료 중재를 하는데 기여하고자 실시하였다. 방법: 심층면담한 내용을 Colaizzi(1976)의 방법을 이용하여 귀납적이고 서술적인 기술과 분석을 실시하였다. 결과: 말기암환자와 가족들이 보완대체요법을 선택한 4가지 범주는 첫째 현대의학의 한계 인식, 둘째 보완대체요법의 효과에 대한 신뢰, 셋째 가족의 정서적 욕구 충족 넷째, 의사의 부정적인 태도로 인한 불신이었다. 주제모음은 9가지로 '불가능한 완치에 대한 기대', '병원치료에 대한 불확실성', '항암치료의 부작용을 해결하기 위한 보조적 수단', '증상완화와 생명연장을 위함', '암치료의 부작용에 대한 두려움', '체험과 정보에 대한 신뢰', '타인의 권유', '가족으로서 도리를 다함', '의사의 부정적 태도에 불만족'이었다. 결론: 암환자의 치료 결정과정에서 대상자와 의사가 바람직한 의사소통과정을 통하여 현대의학 치료에 대한 설명이 충분히 이루어져서 대상자가 일반적인 호스피스 완화의료 서비스를 이용하고 과학적인 근거에 바탕을 둔 보완대체요법에 대한 정확한 정보 제공이 필요하다고 본다.
목적 : 본 연구는 한국의 암 환자에서 나타나는 증상 발병율과 고통과의 관계를 파악하여 암환자의 통증조절을 위한 초기 평가 도구를 개발하기 위해서 실시하였다. 방법 : 2003년 1월 6일부터 20일 사이에 국내 5개 종합병원에 내원한 암 환자 1%명을 대상으로 하였다. 이들 환자들은 MSAS (Memorial symptom assessment scale)을 이용하여 나타난 증상에 대해 기록하였다. 결과 : 가장 흔한 증상은 무기력(82.1%), 구갈(73.5%), 통증(73.5%) 등이었고 흔한 증상 24가지 중에서 고통 지수가 높은 증상은 구갈(2.11), 가려움증(2.07), 통증(2.03) 등이었다. 통증과 정신 심리학적 증상을 제외하고 빈도와 고통 지수가 높았던 10가지 증상을 평가 항목으로 선정하였다. 결론 : 이들 10가지 증상은 한국의 암 환자에서 발병율과 고통 지수가 높은 증상들이다. 따라서 이들 증상들에 대한 평가가 암 환자에서 통증 조절을 위한 통증 초기 평가 도구를 개발하는데 도움이 될 것으로 생각된다.
배경 : 통증은 암환자가 겪는 가장 흔한 증상 중 하나로, 정확한 통증 평가는 적절한 통증 치료를 위해 필수적이다. 한편 통증은 주관적인 문제이기 때문에 환자의 표현은 통증 상태의 가장 믿을만한 지표이다. 이에 실제 암 환자의 통증 표현 어휘를 조사하구 통증의 성격에 따른 통증어휘를 분석함으로 효율적인 통증평가 및 조절에 도움이 되고자 하였다. 방법 : 2002년 9월부터 11월 사이 서울과 경기도에 위치한 6개 대학병원에서 암진단을 받고 치료 중인 75명의 환자를 대상으로 조사자가 직접 면담하여 환자가 표현하는 통증 어휘를 조사하였고, 통증의 성격에 따른 어휘를 분류하였다. 수집된 통증 어휘를 7차례의 자문회의와 실무회의를 거쳐 정리하였고, 표준형 성인 암성 통증평가 도구'를 위해 통증의 성격 범주당 각 5개의 통증 어휘를 선정하였다. 결과 : 183개(중복포함)의 통증 표현 어휘가 추출되었고, 52종류의 어휘로 표준화되었다. 체성 통증, 내장성 통증, 신경병증성 통증 모두에서 '쑤시다'가 가장 빈도가 높았고, '결리다', '끊어지는 듯하다', '지긋이 아프다', '채찍으로 후리는 듯하다'는 체성 통증에서만 나타나면서 빈도가 높은 표현이었고, '쓰리다', '뒤틀리다', '묵직하다', '짓누르다'는 내장성 통증에서, '따갑다', '저리다'는 신경병증성 통증에서만 나타나면서 빈도가 높았다. 결론 : 통증의 성격을 대표하는 어휘를 각각 5개씩 선택하였는데, 체성 통증의 경우 '쑤시다', '결리다', '찢어지는 듯하다', '뻐개지는 듯하다', '찌르다'가, 내장성 통증의 경우 '쑤시다', '뻐근하다', '쓰리다', '뒤틀리다', '쥐어짜는 듯하다'가, 신경병성 통증의 경우 '화끈거리다', '저리다', '뻗치다', '찌릿찌릿하다', '피부가 닿기만해도 아프다'가 선정되었다.
