폐경전 성인 여성에서 운동과 칼슘 섭취량이 혈압과 혈중 지질에 미치는 영향 (Effects of Exercise and Calcium intake on Blood Pressure and Blood Lipids in Premenopausal Women)

  • The purpose of this study was to confirm that regular physical exercise habit and calcium intake play a role reducing the risk of cardiovascular diseae, particularly to confrim that regular exercise is important in reducing serum lipid levels. Subjects were classified into regular exercise group(more than 3 times/wk, more than 30 min per exercise) and non-regular exercise group. A convenient frequency method was used to assess the nutritional intake of subjects. Anthropometric measurement such as bodyweight and hight, and blood pressure were measured. There was no significant difference between nonexercisers and exerciser in energy intake, calcium intake and blood lipid levels. The strength of frequency of exercise may not by adequate to modify lipid profiles in premenopausal woman with normal lipid level, Although we found no significant difference in blood lipid levels, this result does not imply there are no benefits of exercise subjects. There were no signigicant correlations between age or weight with blood lipids in regular exercise group, while there were significant positive correlations between age of weight with blood lipids in non-regular exercise group. The levels of serum cholesterol, and triglyceride, blood pressure and atherogenic index increased with age in nonexercise women, Especially, atherogenic index was lower in regular exercise group. The blood pressure in nonexercise group was significantly higher than that in regular exercise group There was a highly significant negative correlation between calcium intake and blood pressure in nonexercise women, There was a highly significant negative correlation between calcium intake and blood pressure in nonexercise women. The results suggest that increased habitual physical activity and calcium intake may have desirable effects on serum lipid levels and blood pressure in premenopausal women.(Korean J Nutrition 34(1):62-68, 2001)

오실로메트릭 혈압 측정에서 커패시턴스 센서와 적응필터를 이용한 새로운 잡음제거방법에 관한 연구 (A New Method for Artifact Reduction Based on Capacitive Sensor and Adaptive Filter in Oscillometric Blood Pressure Measurement)

  • In this study, a new method using a capacitive sensor and an adaptive filter was proposed to deal with artifacts contaminating an oscillation signal in oscilometric blood pressure measurement. The proposed method makes use of a variation of the capacitance between an electrode fixed to a cuff and an external object to detect artifacts caused by the external object bumping into the cuff. The proposed method utilizes the adaptive filter based on linear prediction to remove the detected artifacts. The conventional method using linear interpolation and the proposed method using the adaptive filter were applied to three types of the artifact-contaminated oscillation signals(no overlap, non-consecutive overlap, and consecutive overlap between artifacts and oscillations) to compare them in terms of the artifact reduction performance. The proposed method was more robust than the conventional method in the case of consecutive overlap between artifacts and oscillations. The proposed method could be useful for measuring blood pressure in such a noisy environment that the subject is being transported.

  • There are two methods in blood pressure measurement ; Invasive methode and Non-invasive method. The Invasive methode can get the 띠cod pressure measurement but, patient feel uncomfortable. So Non-invasive methode used generally. The Oscillometric method is typical Non-Invasive method. This method is commonly used to measure BP in electric sphygmomanometer and has various algorithm. In this paper it is described about a algorithm, it controls, determinates the cuff pressure, and fillers the measured BP data. This system can interface with PC(personal computer) by RS-232 and save the measured data in PC. This system deflates the cuff pressure by Solenoid valve. The main algorithm are oscillometric and maximum amplitude algorithm(MAA). MAA has various measured oscillation, it depends on patient's age, height, weight and arm circumference size. In this paper proposed system can measure Systolic BP, Diastolic BP, and Mean BP using Interpolation, Auto Reinflation algorithm.

일부 성인남녀의 무기질 (Ca, P, Fe, Na, K, Zn) 섭취와 혈압 및 혈중 지질과의 관련성 (Relation among Mineral (Ca, P, Fe, Na, K,Zn) Intakes, Blood Pressure, and Blood Lipids in Korean Adults)

  • The purpose of this study was to investigate the relation among mineral intakes, blood pressure, and blood lipids for health management of Korean adults over 20 years old. Subjects were recruited from the district where they lived, which included rural (Yoju, n = 137) , coastal (Taean, n = 100) , and urban district (Suwon, n : 117) . hnthropornekical measurement, blood pressure measurement, dietary intake assessment using 24-hour recall method, and blood collection and analysis of blood lipids were conducted. The average age, height, weight, and BMI were 54.1 years, 168.5 cm, 67.3 kg, and 24.5 kg/$m^{2}$ for men and 53.9 years,153.8 cm,59.2 kg, and 25.0 kg/$m^{2}$ for women, respectively. The mean daily intakes of total food and energy were 1219.1 g and 1740.9 kcal for men and 1071.3 g and 1432.6 kcal for women. The mineral intakes of men and women were 448.5 mg and 400.4 mg for calcium,955.3 mg and 803.7 mg for phosphorus, 13.3 mg and 11.1 mg for iron, 4302.8 mg and 3393.3 mg for sodium,2588.0 mg and 2264.9 mg for potassium, and 8.9 mg and 7.3 mg for zinc. The daily intakes of energy, calcium, zinc, vitamin $B_{2}$, and vitamin I were $76.1\%$, $60.1\%$, $73.1\%$, $68.6\%$, and $80.4\%$ of RDAs, respectively. The intakes of energy, calcium, zinc, vitamin $B_{2}$ and I were not meet Korean RDAs. Especially, calcium, zinc, and vitamin $B_{2}$ were less than $75\%$ of RDAs. Blood pressure of the subjects was 128.3/75.5 mmHg for men and 124.1/73.7 mmHg for women. Serum lipids of men and women were 180.2 mg/dL and 184.9 mg/dL for total cholesterol, 160.8 mg/dL and 137.6 mg/dL for triglyceride,41.5 mg/dL and 44.7 mg/dL for HDL-cholesterol,106.6 mg/dL and 112.7 mg/dL for LDL-cholesterol, and 3.5 and 3.3 for atherogenic index. While serum triglyceride and AI of men was significantly higher, HDL-cholesterol was lower than those of women. In the relation among mineral intakes, blood pressure, and serum lipids, we found a significantly negative correlation of intakes of calcium, calcium/phosphorus, calcium density with systolic blood pressure. There were significantly negative correlation between intake of calcium/phosphorus and serum cholesterol, calcium density and serum cholesterol, sodium intake and serum HDL-cholesterol. Based on these results, we concluded that high intakes of calcium, calcium/phosphorus and low sodium intake were associated with favorable blood pressure and serum lipid profiles in adult men and women.   • 혈압을 측정하는 방법에는 침습적(Invasive)인 방법과 비침습적(Non-invasive)인 방법이 있는데 침습적인 방법에 의한 혈압측정의 경우 정확한 값을 얻는 반면에 그 사용법이 복잡하고 환자에게 불편하므로 중환자의 경우를 제외한 경우 대부분 비침습적인 방법에 의해 혈압을 측정한다. 현재 전자 혈압계의 경우 대부분이 오실로메트릭(Oscillometric) 방법을 사용하여 기본적인 시스템을 구성하고 여기에 여러 가지 알고리즘을 부가적으로 적용하고 있다. 본 연구에서 이러한 혈압 측정 시스템을 개발하기 위해 커프(Cuff)의 압력을 제어, 측정하고 측정된 데이터를 필터링하여 혈압을 측정할 수 있도록 알고리즘을 구성하였다. 통신방식으로는 개발된 시스템의 모든 기능이 개인용 컴퓨터(Personal Computer)와의 통신을 통해 이루어지도록 구성되어져 있으며, 사용된 통신방식은 비동기 직렬 통신 방식(RS-232)인 패킷방식을 사용하였고 솔레노이드(Solenoid) 밸브를 이용한 자동 또는 급속 배기 방법을 이용하여 커프의 압력을 감압(Deflation)하였다. 혈압측정 알고리즘은 기본적인 오실로메트릭 방법과 최대 진폭 알고리즘(Maximum Amplitude Algorithm)을 사용하였다. 최대진폭 알고리즘에서는 측정된 오실레이션(Oscillation)중 최고의 진폭을 가지는 오실레이션 같은 오차의 원인이 되는 연령이나 체중, 팔의 두께에 따라 다양하게 나타난다. 본 연구에서는 오실레이션의 간에 획일적인 비율을 적용하지 않고 여러 가지 요인들에 대한 가변적인 특정 비율(Characteristic Ratio)반영하여 보다 정확한 최고혈압(Systolic Blood Pressure), 최저혈압(Diastolic Blood Pressure), 평균혈압(Mean Blood Pressure)의 값을 구하는 연구를 하였다. 최고진폭 알고리즘에서 가변적인 특성비율의 적용은 혈압간의 오차를 줄일 수 있을것으로 기대된다.

Validation of the mobile wireless digital automatic blood pressure monitor using the cuff pressure oscillometric method, for clinical use and self-management, according to international protocols

  • The purpose of this study was to evaluate the accuracy of a mobile wireless digital automatic blood pressure monitor for clinical use and mobile health (mHealth). In this study, a manual sphygmomanometer and a digital blood pressure monitor were tested in 100 participants in a repetitive and sequential manner to measure blood pressure. The guidelines for measurement used the Korea Food & Drug Administration protocol, which reflects international standards, such as the American National Standard Institution/Association for the Advancement of Medical Instrumentation SP 10: 1992 and the British Hypertension Society protocol. Measurements were generally consistent across observers according to the measured mean ${\pm}SD$, which ranged in $0.1{\pm}2.6mmHg$ for systolic blood pressure (SBP) and $0.5{\pm}2.2mmHg$ for diastolic blood pressure (DBP). For the device and the observer, the difference in average blood pressure (mean${\pm}$SD) was $2.3{\pm}4.7mmHg$ for SBP and $2.0{\pm}4.2mmHg$ for DBP. The SBP and DBP measured in this study showed accurate measurements that satisfied all criteria, including an average difference that did not exceed 5 mmHg and a standard deviation that did not exceed 8 mmHg. The mobile wireless digital blood pressure monitor has the potential for clinical use and managing one's own health.

중년 남성에서 혈압과 관련한 허리-엉덩이 둘레비 산출 (A Distribution of Waist-hip Ratio Associated with the Blood Pressure in Middle-aged Men)

  • Backgroud: Excess abdominal fat, expressed as an increased ratio of waist to hip circumferences (WHR), is independently associated with higher levels of blood pressure. Although a WHR greater than 1.0 in men has been shown to predict complications from obesity, the WHR has not been evaluated in all ethnic groups. Methods: In order to ascertain the association between WHR and classification of blood pressure and to investigate the critical value of WHR as a predictive factor of hypertension in Korean middle-aged men, we compared the mean of WHRs according to the classification of blood pressure in Seoul Cohort participants. Results: Through a surrey of direct measurement of waist and hip girth, 452 subjects were recruited from the cohort. The mean of WHR was 0.88 and its standard deviation was 0.04. The mean of WHRs was higher in the systolic blood pressure group (above 140 mmHg), diastolic blood pressure group (above 90 mmHg), and hypertension group than in the systolic blood pressure group (below 140 mmHg), diastolic blood pressure group (below 90 mmHg), and normotensive group, respectively. And WHR of above 0.89 was associated with hypertension (z-value =6.66). Conclusion: It is necessary for Korean males with WHR greater than 0.89 to recommend the primary prevention and early defection of hypertension.

Continuous Blood Pressure Monitoring using Pulse Wave Transit Time

  • In this paper, we describe the method of non-invasive blood pressure measurement using pulse wave transit time(PWTT). PWTT is a new parameter involved with a vascular that can indicate the change of BP. PWTT is measured by continuous monitoring of ECG and pulse wave. No additional sensors or modules are required. In many cases, the change of PWTT correlates with the change of BP. We measure pulse wave using the photo plethysmograph(PPG) sensor in an earlobe and we measure ECG using the ECG monitoring device our made in the chest. The measurement device for detecting pulse wave consists of infrared LED for transmitted light illumination, pin photodiode as light detector, amplifier and filter. We composed 0.5Hz high pass, 60Hz notch and 10Hz low pass filter. ECG measurement device consists of multiplexer, amplifier, filter, micro-controller and RF module. After amplification and filtering, ECG signal and pulse wave is fed through micro-controller. We performed the initial work towards the development of ambulatory BP monitoring system using PWTT. An earlobe is suitable place to measure PPG signal without the restraint in daily work. From the results, we can know that the dependence of PWTT on BP is almost linear and it is possible to monitoring an individual BP continuously after the individual calibration.

Radial Electrical Impedance: A Potential Indicator for Noninvasive Cuffless Blood Pressure Measurement

  • Noninvasive, cuffless, and continuous blood pressure (BP) monitoring is essential to prevent and control hypertension. A well-known existing method for this measurement is pulse transit time (PTT), which has been investigated by many researchers as a promising approach. However, the fundamental principle of the PTT method is based on the time interval taken by a pulse wave to propagate between the proximal and distal arterial sites. Consequently, this method needs an independent system with two devices placed at two different sites, which is a problem. Even though some studies attempted to synchronize the system, it is bulky and inconvenient by contemporary standards. To find a more sensitive method to be used in a BP measurement device, this study used radial electrical bioimpedance (REB) as a potential indicator for BP determination. Only one impedance plethysmography channel at the wrist is performed for demonstrating a ubiquitous BP wearable device. The experiment was evaluated on eight healthy subjects with the ambulatory BP monitor on the upper arm as a reference. The results demonstrated the potential of the proposed method by the correlation of estimated systolic (SBP) and diastolic (DBP) BP against the reference at $0.84{\pm}0.05$ and $0.83{\pm}0.05$, respectively. REB also tracked the DBP well with a root-mean-squared-error of $7.5{\pm}1.35mmHg$.

  • In this paper we present an implantable pressure sensor to measure real-time blood pressure by monitoring mechanical movement of artery. Sensor is composed of inductors (L) and capacitors (C) which are formed by microfabrication and direct bonding on two biocompatible substrates (quartz). When electrical potential is applied to the sensor, the inductors and capacitors generates a LC resonance circuit and produce characteristic resonant frequencies. Real-time variation of the resonant frequency is monitored by an external measurement system using inductive coupling. Structural and electrical simulation was performed by Computer Aided Engineering (CAE) programs, ANSYS and HFSS, to optimize geometry of sensor. Ultrafast laser (femto-second) cutting and MEMS process were executed as sensor fabrication methods with consideration of brittleness of the substrate and small radial artery size. After whole fabrication processes, we got sensors of $3mm{\times}15mm{\times}0.5mm$. Resonant frequency of the sensor was around 90 MHz at atmosphere (760 mmHg), and the sensor has good linearity without any hysteresis. Longterm (5 years) stability of the sensor was verified by thermal acceleration testing with Arrhenius model. Moreover, in-vitro cytotoxicity test was done to show biocompatiblity of the sensor and validation of real-time blood pressure measurement was verified with animal test by implant of the sensor. By integration with development of external interrogation system, the proposed sensor system will be a promising method to measure real-time blood pressure.
