A Study on Optical Properties in Biological Tissue Using A Photon Path Diffusion Model (광 항적경로 모델을 이용한 피하조직에서의 광 특성에 관한 연구)

  • This paper proposes a method of noninvasive reflectance light to measure the blood fractional volume (Vb) and oxygen saturation ($SO_2$) of biological tissue. We chose the red light of 660nm and infrared light of 880nm. In Vivo reflectance data were obtained by the physiological changes front the surface of the skin over the calf in human subject. The reflected light intensity from different layers within a biological tissue was measured by specially designed reflectometer to apply photon path diffusion model. The collected data represent the changes of blood (ractional volume and oxygen saturation at each reflected light wavelengths. The data evaluation was assessed by examining the slopes of the plotted indices for the changes in oxygen saturation and blood (ractional volume. The results presented in this paper claim that light reflectance can separately discriminate the change of blood volume and that of oxygenation in muscle and also in skin.

  • This paper is the study of the reflectance of light from biological tissue for red and Infrared wavelengths and relates the acquired reflectance data to expected physiological changes within the skin and muscle layers associated with heat and exercise. The instrument was disigned to collect data from the calf muscle in human subjects with probe located at the surface of skin. Rapid data acquisition method allowed monitoring of rapid changes in reflecttance due to a stimulus. This study demonstrates that changes in O2 saturation and blood fractional volume expected within the dermis and muscle layers were asserted by examining the slopes of the plotted index for heat and exercise. The results presented in thls study support the claim that reflectance can separately discriminate between changes of blood volume and oxygenation in muscle and in skin. The data demonstrate the ability to measure consistent changes In tissue optical properties during exercise and heat.

  • Optical measurements of turbid biological tissue have provided a means to study tissue metabolism, tissue blood perfusion and blood oxygenation non-invasively. We used the red light of 660nm and infrared of 880nm to measure the blood fractional volume and oxygen saturation of biological tissue. In vivo reflectance data were obtained the physiological change from the deep tissue in human subject. The data evaluation was assessed by examining the slopes of the plotter index for the changes in oxygen saturation and blood fraction volume. The index is the natural logarithm of the ratio of reflected light intensity from measured medium to reference intensity at each wavelength. According to the experimental results, oxygen index changes significantly in the muscle of calf during exercise.

  • Background: Echocardiography is a primary tool used by veterinarians to evaluate heart diseases. In recent years, various studies have targeted standard echocardiographic values for different breeds. Reference data are currently lacking in Maltese dogs and it is important to fill this gap as this breed is predisposed to myxomatous mitral valve disease, which is a volume overload disease. Objectives: To establish the normal echocardiographic parameters for Maltese dogs. Methods: In total, 23 healthy Maltese dogs were involved in this study. Blood pressure measurements, thoracic radiography, and complete transthoracic echocardiography were performed. The effects of body weight, age and sex were evaluated, and the correlations between weight and linear and volumetric dimensions were calculated by regression analysis. Results: The mean vertebral heart size was 9.1 ± 0.4. Aside from the ejection fraction, fractional shortening, and the left atrial to aorta root ratio, all the other echocardiographic parameters were significantly correlated with weight. Conclusion: This study describes normal echocardiographic parameters that may be useful in the echocardiographic evaluation of Maltese dogs.

Clinical Features of Chronic Obstructive Pulmonary Disease with High Fractional Exhaled Nitric Oxide

  • Background: The fractional exhaled nitric oxide (FENO) test is useful in asthma patients. However, a few studies on its usefulness in chronic obstructive pulmonary disease (COPD) patients have been reported. We analyzed the FENO level distribution and clinical characteristics according to the FENO level in COPD patients. Methods: From December 2014 to June 2019, COPD patients who underwent pulmonary function and FENO tests at Chonnam National University Hospital were retrospectively evaluated for FENO, comorbidities, asthma history, blood eosinophil, and pulmonary function test. The high FENO group was defined as those with FENO level>25 parts per billion (ppb). Results: A total of 849 COPD patients (mean age, 70.3±9.4 years) were included. The mean forced expiratory volume at 1 second was 66.5±21.7% and the mean FENO level was 24.3±20.5 ppb. Patients with FENO ≤25 ppb were 572 (67.4%) and those with FENO >25 ppb were 277 (32.6%). Blood eosinophil percentage was significantly higher (4.2±4.8 vs. 2.7±2.5, p<0.001) in patients with the high FENO group than the low FENO group. The high FENO group revealed a significantly higher frequency of patients with blood eosinophil percentage >3% (46.9% vs. 34.8%, p=0.001) and asthma history (25.6% vs. 8.6%, p<0.001) than the lower FENO group. Asthma history, blood eosinophil percentage >3%, and positive bronchodilator response (BDR) were independent risk factors for the high FENO level (adjusted odds ratio [aOR], 3.85; p<0.001; aOR, 1.46; p=0.017; and aOR, 1.57, p=0.034, respectively) in the multivariable analysis. Conclusion: The FENO level distribution varied in COPD patients and the mean FENO value was slightly elevated. Asthma history, eosinophil percent, and positive BDR were independent risk factors for the high FENO level.

Effects of Supplemental Recombinant Bovine Somatotropin (rbST) and Cooling with Misters and Fans on Renal Function in Relation to Regulation of Body Fluids in Different Stages of Lactation in Crossbred Holstein Cattle

  • The aim of this study was to investigate the effect of supplemental recombinant bovine somatotropin (rbST) and cooling with misters and fans on renal function in relation to regulation of body fluids in different stages of lactation in crossbred Holstein cattle. Ten, 87.5% crossbred Holstein cattle were divided into two groups of 5 animals each, housing in a normal shaded barn (NS) and in a shaded barn with a mister-fans cooling system (MF). The experiment in each group was divided into 3 phases, early- (Day 75 postpartum), mid- (Day 135 postpartum), and late stage of lactation (Day 195 postpartum). The pre-treatment study was conducted on the starting day of each stage of lactation and the treatment study was performed after the end of the pre-treatment, during which the animal was injected with 500 mg of rbST (POSILAC) every 14 days for three times. During the study, ambient temperature at the hottest period daily in the MF barn was significantly lower, while relative humidity was higher than that of the NS barn. The temperature humidity index (THI) in both barns ranged from 79-85 throughout the periods of study. Cows in the MF barn showed a lower rectal temperature and respiration rate as compared with cows in the NS barn. The effect of rbST administration increased both rectal temperature and respiration rates of cows housed in either the NS or MF barn. Milk yield significantly increased in cows treated with rbST in all stages of lactation. Increases in mammary blood flow, accompanied by increases of total body water (TBW), extracellular fluid (ECF), blood volume (BV) and plasma volume (PV), were observed in both groups of cows receiving rbST in all stages of lactation. No alterations of renal blood flow and glomerular filtration rate were observed in cows receiving rbST, but decreases in urinary excretion and fractional excretion of sodium, potassium and chloride ions appeared to correlate with reduction in the rate of urine flow and osmolar clearance during rbST administration. These results suggest that the effect of rbST supplementation to cows housed either in NS or MF barns on body fluid volume expansion is attributable to changes in the rate of electrolyte excretion by the kidney. The increased availability of renal tubular reabsorption of sodium, potassium and chloride ions during rbST treatment was a major factor in retaining body water through its colligative properties in exerting formation of an osmotic force mechanism.

Effects of Thyroid Hormone on Left Ventricular Volume and Function in Hyperthyroidism (갑상선기능항진증(甲狀線機能亢進症)에서 좌심실용적(左心室容積) 및 기능변화(機能變化)에 관한 연구(硏究))

  • The purpose of this study is to investigate the effects of thyroid hormone on the left ventricular(LV) volume arid function in man with untreated hyperthyroidism and to determine the effects of successful therapy for thyrotoxicosis on the ventricular pathophysiology. In the present study, equilibrium radionuclide cardiac angiography was performed and LV volume index, ejection phase indexes of LV performance, serum thyroid hormone levels and other hemodynamic parameters were measured in 28 normal subjects and 39 patients with hyperthyroidism before treatment and again every 4 weeks for the first 2 months after the initiation of effective therapy. The results obtained were as follows; 1) In the untreated hyperthyroid state heart rate, blood volume, cardiac index and stroke volume index($97{\pm}14$ beats/min, $73.5{\pm}11.8ml/kg,\;6.9{\pm}1.4\;l/min/m^2$ and $77.6{\pm}13.8ml/m^2$, respectively) were increased significantly compared to those in normal control($74{\pm}12beats/min$, $66.6{\pm}14.8ml/kg,\;3.8{\pm}1.2\;l/min/m^2$ and $56.6{\pm}13.2ml/m^2$ respectively). $(Mean{\pm}SD)$ 2) There was a significant increase in LV end-diastolic volume index in patients with hyperthyroidism ($30.5{\pm}7.5$ for hyperthyroid group compared to a normal control of $22.2{\pm}6.5$; p<0.001), whereas end-systolic volume index remained unchanged $9.6{\pm}3.6\;and\;8.8{\pm}3.3$ respectively).3) In patients with hyperthyroidism, LV ejection fraction was $70.0{\pm}5.6%$, fractional shortening $32.9{\pm}5.1%$, mean velocity of circumferential fiber shortening(mean Vcf) $1.34{\pm}0.31$ circ/sec and maximum ejection rate $3.47{\pm}0.80$. All the ejection phase indexes were significantly greater than those in normal control($65.2{\pm}5.7%,\;28.8{\pm}3.2%,\;0.88{\pm}0.37$ circ/see and $2.27{\pm}0.50$, respectively; p<0.001). 4) Effective therapy produced significant decrease in all the values of serum thyroid hormone concentrations(p<0.00l), hemodynamic parameters(p<0.001), end-diastolic volume index(p<0.01) and ejection phase indexes of LV contractility in patients with hyperthyroidism and after one to two months, when the patients were euthyroid, these measurements were in the range of normal. 5) A significant linear correlation between mean Vcf and serum thyroxine level(r=0.63, p<0.001) as well as between mean Vcf and serum triiodothyronine level(r=0.62, p<0.001) was found. The lesser degree of correlation was also noted between other ejection phase indexes and serum thyroid hormone concentrations. The results indicate that the major effects of excess thyroid hormone on the LV in human beings with hyperthyroidism are an enhancement of LV function and an increase in LV enddiastolic volume and that these effects cause predictable reversible cardiac alteration which are changed dramatically and immediately after effective therapy.

Effects of Renal Arterial Trifluoperazine on the Renal Function in Conscious Rabbit (무마취가토 신동맥내 투여에 의한 Trifluoperazine의 신장기능에 미치는 영향)

  • Renal arterial infusion of renotropic agents has been a very useful technique in the renal function studies. This type of experiments have usually been conducted in the large animals such as dogs and sheep. In these animals a catheter can be placed in the site without much disturbances of renal blood flow. Rabbits as an experimental model, however, caused a disturbances of renal blood flow by a catheterization of renal artery by its properties. Therefore we have developed a new technique that allows a simple and selective access to one side of renal arteries and the other as a control, without any disturbances of renal function. The distance between the both bifurcations of renal arteries on abdominal aorta is about 7 mm. To locate the tip of catheter on one side renal artery, ascending cannulation performed via femoral artery was done. We did an experiment with the technique to clarify the effect of calmodulin inhibitor on the renal function. One of the phenothiazine derivatives, trifluoperazine known as a powerful calmodulin inhibitor. Trifluoperazine, actual dose ranges of $2.76-5.20\;ug\;{\cdot}\;kg^{-1}\;{\cdot}\;min^{-1}$, increased urine volume and glomerular filtration rate significantly. Significant increases in urinary excretion of sodium, chloride and potassium were found. Fractional excretion of sodium and free water clearance increased significantly. These data suggest that this new technique is very useful in field of renal physiology and that striking effect of trifluoperazine on the renal function may be caused by increasing the renal hemodynamics, and by the inhibition of sodium, chloride and potassium reabsorption in the renal tubules.

Acute Kidney Injury after Dose-Titration of Liraglutide in an Obese Patient (비만 환자에서 리라글루티드 증량 과정에서 발생한 급성 신손상)

  • Liraglutide (SaxendaR) is prescribed to induce and sustain weight loss in obese patients. The starting dose of liraglutide is 0.6 mg/day for 1 week, which is increased by 0.6 mg/day every week until the full maintenance dose of 3 mg/day is achieved. Such dose titration is needed to prevent side effects, which primarily include gastrointestinal problems such as nausea, diarrhea, constipation, vomiting, dyspepsia, and abdominal pain. A 35-year-old, reportedly healthy obese man receiving liraglutide treatment for obesity visited the emergency room complaining of generalized weakness and dizziness accompanied by repeated diarrhea and vomiting. He reported over 20 episodes of diarrhea starting the day after liraglutide dose escalation from 1.2 mg/day to 1.8 mg/day. Laboratory findings suggested pre-renal acute kidney injury, including serum creatinine 4.77 mg/dl, blood urea nitrogen (BUN) 37 mg/dl, estimated glomerular filtration rate (eGFR) 15 ml/min/1.73 m2, and Fractional excretion of sodium 0.08. After volume repletion therapy, his renal function recovered to a normal range with laboratory values of creatinine 1.08 mg/dl, BUN 14 mg/dl, and eGFR 88 ml/min/1.73 m2. This case emphasizes the need for caution when prescribing glucagon-like peptide-1 receptor agonists, including liraglutide, given the risk of serious renal impairments induced by volume depletion and dehydration through severe-grade diarrhea and vomiting.

Studies on the Hemodynamic Changes in Cirrhosis of the Liver (간경변증(肝硬變症)에서의 혈역학적(血力學的) 변화(變化)에 관(關)한 연구(硏究))

  • Cardiac output, plasma volume and renal plasma flow were determined to evaluate hemodynamic changes in 29 patients with cirrhosis of the liver. The results obtained were as follows. 1. The mean plasma volume was 3793+895ml and it was significantly higher than the normal controls. The mean blood volume ($5266{\pm}1222ml$) and blood volume per kg body weight ($95.7{\pm}23.41ml$) were also increased significantly. The mean plasma volume per kg body weight ($69.1{\pm}19.1ml$) showed increased tendency and the mean difference between blood volume and plasma volume per kg body weight ($26.4{\pm}7.05ml$) was in lower limit of normal range. 2. The mean cardiac output was $7708{\pm}2652ml/min$ and it was significantly increased. The mean cardiac index ($4924{\pm}1998ml/min/M^2$), stroke volume ($96.2{\pm}34.2ml/beat$), stroke index ($62.3{\pm}27.34ml/M^2$) and fractional cardiac index ($1.54{\pm}0.577$) were also increased significantly. The mean total -peripheral resistance was $1664{\pm}753.8\;dynes\;sec\;cm^{-5}M^2$ and it was significantly lower than the normal controls. 3. The mean renal plasma flow was $537{\pm}146.8ml/min/1.73M^2$ and it was normal to decreased tendency. The mean endogenous creatinine clearance ($66.7{\pm}23.0ml/min/1.73M^2$) was significantly decreased. Filtration fraction was variable, but it was slightly lower than normal in most cases. The mean renal fraction of cardiac output ($11.4{\pm}6.27%$) was relatively decreased. 4. Although renal plasma flow was normal or decreased in general, it was definitely diminished in patients with creatinine clearance less than $60ml/min/1.73M^2$, resistant ascites, and signs of azotemia (elevated BUN and serum creatinine). 5. Diminished glomrular filtration rate with low filtration fraction and decreased renal fraction of cardiac output observed strongly supported increased renal afferent arteriolar resistance. 6. Renal circulatory impairment preceded azotemia or oroliguria in cirrhosis. 7. Clinical findigns and liver function were not correlated with hemodynamic changes, except for esophageal varices associated with high cardiac output obsedved. 8. No definite correlation of renal hemodynamics with plasma volume or cardiac output was found.
