  • In diabetics, daily blood glucose testing is generally required at home, and thus, performing blood collection several times a day using a blood line is essential. Blood collection in the home and in the hospital is a source of pain and is the second most common cause of infection. In blood collecting device generally consists of four major parts: inner-case, outer case, main shaft and triger, and the most import part among those for necessary functionality is the main shaft. Filling time and injection pressure, filling balance, strain-rate analysis of change based on availability of the product. The Moldflow of FEM simulation is used for the analysis of injection molding process. In this study, aims to create a technique for injection molding and manufacturing of a main shaft of a high-performance blood-collecting apparatus capable of automatically extracting a lancet to relieve pain through depth control of the lancet.

A Study on the Appropriate Manpower Estimation according to the Evaluation of the Blood Collection Workload of Medical Technologists (임상병리사의 채혈 업무량 평가에 따른 적정 인력 산정에 관한 연구)

  • This study explored the method of determining the appropriate size of the workforce according to the assessment of the workload of medical technologists (also called medical and clinical laboratory technologists, and medical and clinical laboratory scientists) in order to present a standard production model for the appropriate manpower in blood collection rooms. The eleven university hospitals selected for this study had between 600 and 2,000 beds. The 14-steps standard blood collection time was 4 minutes and 8 seconds for the outpatients aged between 20 to 60 years old (57%) except for children and the elderly (43%). Assuming that there were 8 hours per day for mechanically collecting blood, the maximum number of blood donations by one clinical laboratory scientist was analyzed to be 100 cases. In conclusion, it is appropriate to have fewer than 100 cases of daily blood collection by a medical technologist engaged in blood collection. Since the proper number of blood collection workers (100% of blood collection work)=the number of annual working days/(one day's work hours/time per case)×the number of working days per year, then the proper number of blood collection workers (one day's work hours)=the number of working days per year/100×the number of working days).

Blood Collection Workload and Safety of Medical Technologists at Korean Hospitals (한국 의료기관에서 임상병리사의 채혈 업무 현황과 안전 실태 조사)

  • This study investigates the current situation of medical technologists for blood collection, which is considered the most important step for diagnosis. The survey enrolled 650 medical technologists working in hospitals and medical check centers in Busan, Ulsan, and Gyeongnam. We found that each medical technologist performed blood collection for about 100 patients. There was more than one blood collection failure per day, with more than one case of pain and filing of civil complaints per year. Hence, there was a high work burden on the medical technologists. Cases where a medical technologist was stabbed with a used needle occurred more than once a year, and about 15% of them received infection control and treatment because of stab wounds. Additionally, more than half of the participants suffered from musculoskeletal disorders and mental stress due to blood collection work. Unlike administering intravenous and intramuscular injections using the same needle, no fee is charged for blood collection. Based on the results of this study, it will be possible to improve the safety and rights of medical technologists by calculating the actual fee for blood collection work and assigning a relative value score.

Effect of Repeated Blood Collection on the Amount of Vitamin E in Serum and Liver of the Rats Fed Standard Diet (반복된 채혈이 일반식이 흰쥐의 간, 혈청, Vitamin E 함량에 미치는 영향)

  • This study was conducted to investigate the effect of repeated blood collection on the amount of vitamin E in serum and liver of the rats fed standard diet. Rats (Sprague Dawley strain) were fed standard were fed standard diet for several weeks. Blood was collected with on improved cardiac puncture method at regular intervals . The amount of vitamin E analogs were measured by the HPLC. 1) The growth curve of all groups was normal, but bodyweight gains of group II were significantly decreased in case of 7 week- rats whose blood was collected at intervals of two weeks. 2) The amount of vitamin E analogs in serum of rats during growth increased, but continuos blood collection could not affect on the amount of vitamin E in serum. Especially, in the group I serum vitamin E equivalent were significantly higher in case of 7 weeks than first week. 3) The amount of liver vitamin E analogs were changed irregularly, and especially, $\alpha$-Tocopherol equivalent were significantly higher in group II then other groups.

Daily Profiles of Blood Insulin, Insulin-Like Growth Factor-I, Thyroxine and Triiodothyronine in Ewes Under Three Levels of Feed Intake

  • Daily variation in the serum concentrations of insulin and insulin-like growth factor-I and in the plasma concentrations of thyroxine and triiodothyronine were evaluated in ewes fed 30%, 100% and 200% of theoretical maintenance energy requirements. The single daily meal has had significant effects (p<0.05) on almost all profiles. In general, serum or plasma hormone concentrations have increased after the meal, in particular at the two higher levels of energy intake. In the group submitted to the lowest level of energy intake, the consequences of the meal on circulating levels were almost imperceptible. The effects of feeding levels on serum or plasma concentrations have widely varied among hormones, not showing any objective pattern or relationship. Because these variations may affect the interpretation of these blood indicators, knowledge of daily profiles and of the effect of feed level must be considered. In order to maximize the diagnostic value of those indicators, the most suitable times for blood collection seem to be 16 h after the meal and (or) just before the meal. The collection 16 h after the meal apparently allows the characterization of a relatively steady metabolic state, intermediate between the close effects of food intake and the final phase of the intensification of body reserves mobilization. The collection just before the meal will give a good indication of the level of activity of those mobilization mechanisms.

Factors Influencing Peripheral Blood Stem Cell Collection (자가 말초혈액 조혈모세포 채집에 영향을 주는 관련요인)

  • Purpose: Peripheral blood stem cell transplantation (PBSCT) has been widely used. The optimal time for collection is a critical factor to obtain proper counts of CD34 cell by peripheral blood stem cell collection (PBSC). The purpose of this study was to identify the factors influencing peripheral blood stem cell collection in order to figure out the more effective timing for PBSC. Method: The subjects of this study were 189 patients undergoing 3 leukapheresis from January 28, 2005 to December 31,2006. Group's characteristics, checkup opinion of pre-peripheral blood on the day of harvest & outcome of PBSC were analyzed and evaluated using SAS statistics program after grouping patients as below; group 1-CD34 cell counts $<2{\times}10^6/kg$ (n=97); group $2-2{\times}10^6/kg$ ${\leq}CD34$ cell counts $<4{\times}10^6/kg$ (n=26); group 3-CD34 cell counts ${\geq}4{\times}10^6/kg$ (n=63). Results: Based on outcome of peripheral blood stem cell according to diagnosis, acute myelocytic leukemia (AML) was 65.5% at Group 1, Lymphoma was 21.7% at Group 2 and multiple myeloma (MM) was 70.8% at Group 3. There were significant differences in CD34 cell counts according to diagnosis (p=0.00004). Type of cytokine mobilization according to diagnosis, Lenograsim was using 62.5% of MM & 38.2% of AML and filgrastim is using 22.0% of AML only. Circular peripheral blood CD34 cell counts prior to harvest was $258.1/{\mu}L$ at Group 3 which was much higher comparing to Group 1 ($10.5/{\mu}L$) and Group 2 ($39.9/{\mu}L$) (p<0.001). TNC counts of collected peripheral blood stem cell was $15.36{\times}10^6/kg$ at Group 3 and it's much higher than Group 2 ($13.16{\times}10^6/kg$) and Group 1 ($12.36{\times}10^6/kg$) (p=0.083). There was no significant difference in MNC counts inbetween 3 groups. Conclusions: Circular peripheral blood CD34+ cell counts prior to harvest was much higher at Group 3 than Group 1 and Group 2. Therefore, the number of CD34+ cells on the day of harvest can be used as an accurate predictor for peripheral blood stem cell.

Isolation of Anaerobic Bacteria from Oral Pyogenic Infections (구강 화농성 감염에서 혐기성 세균의 배양분리)

  • Strict anaerobic procedures and anaerobic chamber were employed in order to improve the isolation of obligate anaerobes from oral pyogenic infections. Also different culture media were utilized to maximize bacterial recovery. Two blood media: nalidixic acid Tween blood agar (NATB) and plain blood agar (BA), two non-blood media: nalidixic acid Tween agar (NAT) and Gifu anaerobic medium (GAM) were used and compared for their isolation efficacy. Specimens from seven patients were collected with syringe. After collection, the needle was sealed with sterilixed silicone rubber and brought to labortory. The time spent from specimen collection to its processing in anaerobic chamber usually was 15 min. Identification of isolated bacterial strains was done with the API-20A system. Increase in isolation of anaerobic vacteria was achieved. Obligate anaerobic bacteria isolated were 3.3 strains per specimen. This figure falls within the range of 1.9-4.4 strains per specimen reported in other countries. With respect to the media utilized, blood media were superior to non-blood media. NATB medium was effective especially for the isolation of Gram-positive cocci. A total of 15 species of Gram-negative rods was isolated and 12 of them belonged to Bacteroides.

Umbilical Cord Blood Transplantation

  • The number of umbilical cord blood transplantation is increasing worldwide as it has expanded the ability of the transplantaion community to meet the growing needs of their patients. Clinical data over the last decade show promising results in transplantation using both related as well as unrelated cord bloods. Cord blood banks are essential for the clinical use for transplantation and are now established around the world with the major efforts to standardize banking in collection, processing and distribution of cord blood for providing the highest quality stem cells for the patients. In Korea, Medipost, Histostem and some regional cord blood banks were established some years ago and collected thousands of cord blood for public but it had some limitations and was not expanded as the cord blood transplantation was not covered by medical insurance. Recently with the change in the policy of medical insurance to cover the cord blood transplantation, several venture companies are showing great interests in cord blood banking and trying to establish private cord blood banks in Korea. This review article discusses the current status of cord blood transplantaion and also the clincial use of stem cells from cord blood.

Relationship Among Dietary Intake, Blood Level, and Urinary Excretion of Minerals and Blood Pressure in Korean Rural Adult Men and Women (농촌 성인 남녀의 무기질 섭취량, 혈액수준 및 소변중 배설량과 혈압과의 관계에 대한 연구)

  • This study was carried out to estimate the relationship between dietary intake, blood level, and urinary excretion of minerals and blood pressure in 30 healthy adults living in rural area of Korea(12 males and 18 females). Analysis for the nutritional status of the subjects were performed by 3-day dietary intake record, duplicated diet collection, 24-hour urine collection, and venous blood sampling before measuring blood pressure. The mean blood pressure of subjects was 117.50/80.83mmHg in males and 110.00/73.89mmHg in females. The mean daily intakes of Na, K, Ca, P, Mg, Fe, Cu, Zn estimated for 3 days were 199.97mEq, 49.56mEq, 452.50mg, 725.57mg, 240.40mg, 12.48mg, 3.41mg, 8.28mg, respectively. The serum concentration of Na, K, Ca, P, Mg, Fe, Cu, Zn were 139.83mEq/dl, 4.06mEq/dl, 8.86mg/dl, 3.28mg/dl, 2.13mg/dl, 0.12mg/dl, 0.12mg/dl, 0.14mg/dl, respectively. The 24-hour urinary excretions of Na, K, Ca, p, Mg, Fe, Cu, Zn estimated for 169.60mEq, 39.37mEq, 80.40mg, 398.97mg, 64.77mg, 0.21mg, 0.07mg, 0.29mg, respectively. No significant correlation was found between dietary intake, serum concentration, and urinary excretion of minerals and blood pressure. But, the serum Ca/Mg ratio showed negative correlation with the systolic and diastolic blood pressure at the level of significance of 5%. The study verifies the need for more systematic studies on interrelationship among minerals and mineral requirements in normotensive and hypertensive subjects.

A Determination of the Optimal Blood-Issuing Polices (최적 혈액 유출 정책의 결정)

  • Human blood is a perishable product : it has a legal lifetime of 21 days from collection, during which it can be used for transfusion to a Patient of the same type, and after which it has to be discarded. Therefore, blood must be supplied safely and effectively because it is one of the medical resources which keep humanlife. In this study, the effects of blood issuing policies on average inventory levels and average age of blood at transfusion are determined by simulation applied the theory of absorbing Markov chains. And as a practical study, the daily demand distribution of blood is estimated by using the data of B General Hospital. The distribution estimated follows poisson distribution and the estimator of parameter estimated from the poisson distribution is 0.762. Simulation is done by using the parameter. The most important problem when control blood is the amount of outdata. So we compared random policy with Modified LIFO and Modified FIFO by using outdata. As a results it is shown that Modified LIFO and Modified FIFO by using outdata. As a results it Is shown that Modified LIFO and Modified FIFO present better issuing policy than Random Policy.
