This study was started for the purpose of providing the basic data for continous managment of kidney transplantation patients after discharge. This study was conducted on 180 patients who received renal transplants at three hospital( B, M, P) pusan, korea. The data collection was done for june 1, to August 31, 2002. General characteristics, renal transplantaton characteristics, physical characteristics, the level of compliance and the degree of educational demand were done by the number and percentage, the mean, standard deviation. The level of compliance and educational demand followed by the characteristics of general and kidney transplantation were analyzed by t-test and ANOVA. The result were as fallows; 1. Man was higher than woman such as 60.0%, Mean age was 42.5 years old, Average total duration of after operation was 5.5 years. 2. Cases of systolic blood pressure over 140mmHg were 10.0%, cases of diastolic blood pressure over 90mmHg were 22.8% and obesity factor in BMI was 15.6%. The person who daily water intake amount is 5000cc was 0.6%, the case that daily urine output is below 1000cc was 8.9%, and the case that urine output is zero was 0.6%. 3. The mean score of compliance was 77.47 point, The score in medication part was highest such as 4.67 point, that in stress situation was lowest such as 3.50 point. 4. The average score of educational demand was 154.02 point, and physical state part was 4.36 points highest, activation part was 3.48 points lowest. As a role of nurse Confirmation of compliance is very important encourage to make good through regular hospital visitation, point out the noncompliance part and then increase compliance of renal transplantation patient As well there will be maintain the normal kidney function to satisfy educational demand through continous education.

This clinical experience reports the effect of wood and fire acupoints therapy on a hypertensive patient. Wood acupoint indicates Jing point in yin channel or Shu point in yang channel. Fire acupoint is Jing in yin channel or Jing in yang channel among the five acupoints of Jing, Xing, Shu, Jing and He. The patient was treated with indicated acupoints of 7 channels: lung, large intestine, spleen, pericardium, triple warmer, heart and liver, for 5 weeks. In case of necessity, another acupoints were added with wood and fire acupoints. After 5 weeks treatment, the blood pressure improved by 125/80 which is in normal level. This result suggests that wood and fire acupoints therapy is effective in the treatment of hypertension.

Journal of the Korean Society of Clothing and Textiles Vol. 18, No. 3 (1994) p. 387∼394 In this study, the restriction of slacks (blue jean) versus box pleats skirt was investigated, from the view point of peripheral blood flow at the toe according to motion variation and E.M.G. analysis of leg muscles after walking on the flat or going up and down stairways. Besides, the pressure of slacks on the lower extrimity was measured statically and dynamically. The main results were summerized as follows; 1. The clothing pressure applied by slacks was; each one of thigh and lower leg was 18.2 g/ cm2 and 22.1 g/cm2 in upright, 63.4 g/cm2 and 26.6 g/cm2 in sitting on the chair, on both sides of thigh and lower leg 272.0 g/cm2 over in squatting. 2. When the motion starts from upright, the dynamic clothing pressure reach their peak before the motions end. When the motion ends and the body comes to a still condition, the clothing pressure grow lower and indicate a constant value. but when the body starts moving again to return to upright, the pressures once grow higher and go to zero value after reaching the peak. The pressure on the knee show much greater than those on the hip. This can be because these pressures depend on the degree of skin stretching motion and of its curvature. 3. The surface E.M.G. in leg muscles M. rectus femoris and M. gastrocnemius were recoreded. In the case of wearing slacks, two muscles were activated much more than wearing skirt. 4. The peripheral blood flow at the toe by wearing slacks was lower than wearing skirt. Also the case when squatting, the peripheral blood flow at the toe was low.
A 23-year-old male patient complained dyspnea on exertion and orthopnea since December 1977. On examination, he was tall and slender. There was grade IV/VI to-and-fro murmur on the left sternal border especially on Erb`s point. The liver was descended 2 fingers breadth below right costal margin. There were no signs of Marfan`s syndrome. Echocardiography demonstrated partial closure of aortic valve and dilated aortic root with enlargement of ascending aorta. Left heart cardiac catheterization revealed moderately elevated pulmonary wedge pressure and right ventricular pressure. The left ventricular end diastolic pressure was markedly elevated to 26 mmHg. On aortography, the aortic regurgitation was severe and it was belonged to angiographically Grade IV. The aortic valve was replaced with Carpentier-Edwards valve without excision and replacement of ascending aorta, under the impression of rheumatic valvular heart disease. After closure of aortotomy, blood pressure was transiently elevated and bleeding from the site of inserting air vent needle of ascending aorta was developed. The bleeding was not controlled by any means. On postmortem microscopic study, the histologic changes were strikingly limited to the ascending aorta from the region of the aortic valve ring.

The current study was performed to observe the effects of Panax ginseng (PG) and P. quinquefolia (PQ) on hemodynamics such as blood flow rate (BF), blood flow velocity (BV), heart rate (HR), systolic blood pressure (SBP) and diastolic blood pressure (DBP, and body temperature (BT) in healthy young men. After testing equality of variance, Student's t-test using PROC TTEST was examined to. prove statistical differences between control and ginseng conditions at each time point. It was found that the BF data were fluctuated by personal deviation. In order to minimize the deviation, the results obtained for 6 hrs were reconstituted after dividing them into two periods of the first half from 1 to 3 hrs and of the latter half from 3.5 to 6 hrs. And then the reconstitution data and dose-response curves were obtained. Blood flow such as BF and BV shows significant increases both two periods in the dose of PG 2.25 and PG 9.0, whereas significant decrease in the dose of PG 4.5. However, in the PQ groups, the middle dose PQ 4.5 shows the highest significant increase among the three doses. Except for PG 2.25 in HR, other doses show significant decreases both in the first half and latter half. SBP of PQ 9.0 shows only a significant decrease in the first half; on the other hand, in the latter half, PG 4.5, PG 9.0 and PQ 9.0 significantly increase SBP. In addition, DBP of PG 2.25 and PG 4.5 show significant increase in the both periods. In the BT, PQ groups show gradual decrease from PQ 2.25 to PQ 9.0; however, PG groups show differently. PG 4.5 shows significant decrease, but PG 9.0 shows a increase without statistical meanings. In summary, PG is more effective in respect to keeping homeostasis of hemodynamics.
Background: This study was conducted to compare the effectiveness of low-dose ketamine versus ketorolac in pain control in patients with acute renal colic presenting to the emergency department (ED). Methods: This is a double-blind randomized clinical trial. The initial pain severity was assessed using the numerical rating scale (NRS). Then, ketamine or ketorolac was administered intravenously at a dose of 0.6 mg/kg and 30 mg respectively. The pain severity and adverse drug reactions were recorded 5, 15, 30, 60, and 120 min thereafter. Results: The data of 62 subjects in the ketamine group and 64 patients in the ketorolac group were analyzed. The mean age of the patients was $34.2{\pm}9.9$ and $37.9{\pm}10.6\;years$ in the ketamine and ketorolac group, respectively. There was no significant difference in the mean NRS scores at each time point, except for the 5 min, between the two groups. Despite a marked decrease in pain severity in the ketamine group from drug administration at the 5 min, a slight increase in pain was observed from the 5 min to the 15 min. The rate of adverse drug reactions, including dizziness (P = 0.001), agitation (P = 0.002), increased systolic blood pressure (> 140 mmHg), and diastolic blood pressure (> 90 mmHg) was higher in the ketamine group. Conclusions: Low dose ketamine is as effective as ketorolac in pain management in patients with renal colic presenting to the ED. However, it is associated with a higher rate of adverse drug reactions.
Purpose: The aim of this study is to evaluate the effects of community based chronic disease management program for managing hypertension and diabetes mellitus. Method: The subjects of this study were 1,000 clients, who participated in a city community based chronic disease management program. The data were collected by a phone survey directly, and selected from DB of community based chronic disease management program, indirectly. The data were analyzed by the STATA program, using descriptive statistics, t-test, and Multiple regression analysis. Results: The mean score of knowledge of hypertension was 4.33 and diabetes mellitus was 2.52. The affecting factors of knowledge of hypertension were the clinic visiting point and blood sugar. However, the affecting factors of knowledge of diabetes mellitus was core lab point, diastolic blood pressure, and blood sugar. Conclusion: the results of this study showed that knowledge of hypertension increased with higher level of education, and younger ages, while knowledge of diabetes mellitus increased with higher level education, more core lab point, younger ages, and in males. Therefore, to control the chronic diseases more effectively in a community, it is important to develop a more systematic education and self- managing programs, using collaboration of health centers and local clinics.
This study was made to find out how stress affect on nutrition status of the college freshmen who were experiencing physical growth and development as well as drastic emotional change. 400 male and female freshmen in 4 year colleges were surveyed respectively through the health check-up procedure for college entrance in February, In order to find out the stress in each group frustration, deprivation, lack of self efficacy, type A behavior and anxiety response were surveyed through 10 questions with total 40 points by assigning 4 points for each question. Diet Status was expressed by DDS (Dietary Diversity Score by 5 food groups) and DVS(Dietary Variety Score). 24-hrs recall method was used to find out the quantity of daily nutrient of EAR(estimated adquacy ratio) by KDRIs(Korean Dietary Recommended Intakes). Nutrition level was analyzed by Can-Pro for professionals (Korea Nutrition Association). And for the quality intake, percentage was calculated and MAR(Mean Adequacy Ratio) were produced. Highest point was obtained in the stress of anxiety with the total 40 score of 30.20, and the scores were 29.79, 28.67, and 28.39 for deprivation, type A behavior and frustration respectively. There was no difference of blood components in accordance with stress type. Stress type was divided into less sensitive group and highly sensitive one and the relationship with the blood nutrient status was observed. The difference of blood component and blood pressure in sensitive and highly sensitive groups was observed in deprivation and anxiety. The index of blood pressure(p<0.05), hemoglobin(p<0.01), HDL-cholesterol(p<0.05), and Fe(p<0.05) was high in the deprivation of sensitive group. Blood pressure and hemoglobin was high in type A of sensitive group(p<0.05). And the contents of blood triglyceride was high in the anxiety of sensitive group(p<0.001) The result of nutrition intake analysis according to stress type showed that there was low intake for energy, riboflavin, and niacin. When the degree of deprivation was high there was a lack of riboflavin intake and there was no significant difference of nutrition intake in lack of self efficacy, type A behavior and anxiety response. Thus, it is necessary for colleges to educate the students to maintain mental stability through various programs and activities after catching a kind and extent of the stress college students we meeting with like the confusion of value system, open heterosexual relationship, and the employment difficulties linked with political uncertainty and economic recession.

Objective: This study examined the incidence and amount of air inflow during central venous catheter (CVC) insertion. Methods: This study was an experimental study aimed at designing an apparatus to implement blood vessel and blood flow in the human body. A 1.5-m long core tube with a Teflon tube, suction rubber tube, and polyvinyl chloride tube were made. This core tube was assumed to be the blood vessel of the human body. Blood was replaced with a saline solution. The saline solution was placed higher than the core tube and flowed into the inside of the tube by gravity. The CVC was injected 15-cm deep into the core tube. The air was collected through a 3-way valve into the upper tube. The experiments were carried out by differentiating the pressure in the tube, CVC insertion step, and diameter of the end of the catheter. The experiment was repeated 10 times under the same conditions. Results: The amount of air decreased with increasing pressure applied to the tube. Air was not generated when the syringe needle was injected, and the amount of air increased with increasing size of the distal end catheter. Conclusion: To minimize the possibility of air embolism, it is necessary to close the distal end catheter at the earliest point as soon as possible.

Stenosis is the drastic reduction in the cross-sectional area of blood vessel caused by accumulations of cholesterol. It affects the blood flow property and structure from the fluid dynamic point of view. To understand the flow phenomenon more clearly, a particle image velocimetry method is used and the fluid dynamic characteristics in a circular channel containing stenosis structure is investigated experimentally in this study. Different stenotic-structured models made of acrylic material are subjected to a pulsatile flow generated by an in-house designed pulsatile pump. The inner diameter of the tube inlet is 20 mm and the length of reduced area for stenosis ranges between 35mm and 40mm. It is circulated continuously through a circular channel by the pump system. Pressure is measured at four different sections during systolic and diastolic phase changes. The phase-averaged velocity field distribution shows a recirculation regime after the stenotic structure. The effects of the stenotic obstructions are found to be more severe when the aspect ratio is varied.
