Extended Blood Drug Concentrations in Extended Release Formulated Acetaminophen Overdose Patients (서방형 아세트아미노펜 중독 환자에 대한 고찰)

  • Purpose: The Rumack-Matthew nomogram cannot be applied in managing overdose by extended release (ER) preparation acetaminophen (AAP). This study analyzed the clinical characteristics of ER preparation AAP overdose in order to develop a treatment recommendation. Methods: We retrospectively reviewed the medical records of patients presented to the emergency department as a result of AAP overdose from Jan 2008 to Dec 2010. Only those patients who ingested an ER preparation of AAP were included in the study. Their blood AAP concentrations were measured at 4 and 8 hours after ingestion. Clinical variables related to AAP intoxication were analyzed. Results: Of the total 108 AAP overdose patients identified during the 3-year period, 20 suffered specifically with ER preparation AAP overdose. The mean estimated ingestion amount was 167.5 mg/kg. Treatments including gastric lavage, activated charcoal, and N-acetyl cysteine (NAC) were performed on 10, 14, and 11 patients, respectively. Hepatotoxicity was diagnosed in only one patient who was then successfully treated with NAC. In another case, blood AAP concentration continued to increase until at least 11-hours after ingestion. Conclusion: This study suggested that blood AAP concentrations associated with ingestion of ER formulations of AAP, may increase in an extended manner. Therefore, multiple sampling and longer periods between samples assessing AAP blood concentration may be required for incidences of extended release overdose.

Impact Factors and Validity of Blood Variables on Death in COVID-19 patient: Using Data of Korea Disease Control and Prevention Agency

  • In this paper, we propose impact factors and validity of blood variables on death of COVID-19 patients. The clinical-epidemiological data of 5628 COVID-19 patients, provided from Korea Disease Control and Prevention Agency as day of 30th April 2020, were used. As results, impact factors of death were dementia, older age, high lymphocyte, cancer, dyspnea, COPD, change of consciousness, heart disease, high platelets, abnormal diastolic pressure and fever. The validities of blood variables for death were high in the order of lymphocyte, hemoglobin, hematocrit, platelet and WBC. Therefore, risk factors such as initial clinical characteristics, underlying disease and blood test results, could be regarded for efficient management of COVID-19 patients.

  • Arteriosclerosis is a disease in which blood circulation is impaired due to loss of elasticity as blood vessels become narrower, and is a potential cause of recently increasing cardiovascular and cerebrovascular diseases. Carotid ultrasound is used as a predictor of cardiovascular and cerebrovascular diseases by evaluating the degree of atherosclerosis. Therefore, this paper attempted to investigate the correlation between the increase in the thickness of the inner media and various clinical variables in carotid ultrasound. Patients with carotid ultrasound findings were classified into three stages: mild intima thickening, sclerosis, and significant stenosis. CAVI (Carotid Ankle Vascular Index: vascular age) data measured in the degree and physical characteristics of the carotid artery(gender, age, body mass index, blood pressure), blood test(total cholesterol, triglycerides, high-density lipoprotein, low-density lipoprotein, creatine phosphokinase, fasting blood sugar), and arteriosclerosis test were collected. It was confirmed that the carotid intima thickness was correlated with variables such as hypertension, hyperlipidemia, and BMI, and also correlated with the risk factors of cardiovascular disease as CAVI increased.

Effects of a specific blend of essential oils on apparent nutrient digestion, rumen fermentation and rumen microbial populations in sheep fed a 50:50 alfalfa hay:concentrate diet

  • Objective: An experiment was conducted to investigate the effects of a specific mixture of essential oils (MEO), containing thyme, clove and cinnamon EO, on rumen microbial fermentation, nutrient apparent digestibility and blood metabolites in fistulated sheep. Methods: Six sheep fitted with ruminal fistulas were used in a repeated measurement design with two 24-d periods to investigate the effect of adding MEO at 0 (control), 0.8, and 1.6 mL/d on apparent nutrient digestibility, rumen fermentation characteristics, rumen microbial population and blood chemical metabolites. Animals were fed with a 50:50 alfalfa hay:concentrate diet. Results: Ruminal pH, total volatile fatty acids (VFA) concentration, molar proportion of individual VFA, acetate: propionate ratio and methane production were not affected with MEO. Relative to the control, Small peptides plus amino acid nitrogen and large peptides nitrogen concentration in rumen fluid were not affected with MEO supplementation; while, rumen fluid ammonia nitrogen concentration at 0 and 6 h after morning feeding in sheep fed with 1.6 mL/d of MEO was lower (p<0.05) compared to the control and 0.8 mL/d of MEO. At 0 h after morning feeding, ammonia nitrogen concentration was higher (p<0.05) in sheep fed 0.8 mL/d of MEO relative to 1.6 mL/d and control diet. Ruminal protozoa and hyper ammonia producing (HAP) bacteria counts were not affected by addition of MEO in the diet. Relative to the control, no changes were observed in the red and white blood cells, hemoglobin, hematocrit, glucose, beta-hydroxybutyric acid, cholesterol, total protein, albumin, blood urea nitrogen and aspartate aminotransferase and alanine aminotransferase concentration. Apparent total tract digestibility of dry matter, crude proten, organic matter, and neutral detergent fiber were not influenced by MEO supplementation. Conclusion:The results of the present study suggested that supplementation of MEO may have limited effects on apparent nutrient digestibility, ruminal fermentation and protozoa and HAP bacteria count, blood cells and metabolites.

Effect of corn grain particle size on ruminal fermentation and blood metabolites of Holstein steers fed total mixed ration

  • Objective: This study was conducted to investigate the effect of corn grain particle size on ruminant fermentation and blood metabolites in Holstein steers fed total mixed ration (TMR) as a basal diet to explain fundamental data of corn grain for cattle in Korea. Methods: Four ruminally cannulated Holstein steers (body weight $592{\pm}29.9kg$) fed TMR as a basal diet were housed individually in an auto temperature and humidity modulated chamber ($24^{\circ}C$ and 60% for 22 h/d). Treatments in a $4{\times}4$ Latin square design were TMR only (control), TMR with whole corn grain (WC), coarsely ground corn grain (CC), and finely ground corn grain (FC), respectively. The corn feeds substituted for 20% energy intake of TMR intake. To measure the ruminal pH, ammonia N, and volatile fatty acids (VFA), ruminal digesta was sampled through ruminal cannula at 1 h intervals after the morning feeding to determine ruminal fermentation characteristics. Blood was sampled via the jugular vein after the ruminal digesta sampling. Results: There was no difference in dry matter (DM) intake between different corn particle size because the DM intake was restricted to 1.66% of body weight. Different corn particle size did not change mean ammonia N and total VFA concentrations whereas lower (p<0.05) ruminal pH and a ratio of acetate to propionate, and higher (p<0.05) propionate concentration were noted when the steers consumed CC compared with WC and FC. Concentration of blood metabolites were not affected by different particle size of corn grain except for blood triglyceride concentration, which was significantly (p<0.05) increased by FC. Conclusion: Results indicate that feeding CC may increase feed digestion in the rumen, whereas the FC group seemed to obtain inadequate corn retention time for microbial degradation in the rumen.

A Study of the Difference of Vital Sign by Stress Reaction (스트레스 반응으로 나타나는 활력증상 변화에 대한 실증적연구)

  • The main purpose of this study was to examine the validity of the vital sign as an instrument of stress reaction measurement. From July to August 1986, stress reaction was evaluated by the difference of endoscopic vital sign on 93 G-I troubled out-patients who underwent endoscopy for the first time and did not have any evidence of cardiovascular disease. The data were analysed by x$^2$-test, Paired . t-test, ANCOVA and Multiple Comparison Test. The result of study were as follows: 1. The frequency of gastric disease was differed by the family type, and the mobility of gastritis and gastric cancer were more increased in nuclear family than in large family (p=0.019). 2. In a comparison of before with after 5 minutes endoscopic vital sign, and a Pulse rate (P=0.0001), respiration rate (p=0.0001), systolic blood pressure (p=0.0002) and diastolic blood Pressure (P=0.006) were significantly increased after 5minutes by endoscopy in contrast with before 5minutes. 3. The control of before 5 minutes of endoscopic vital sign, after 5 minutes of endoscopic systolic (p=0.024) and diastolic blood pressure (p=0.0146) were more elevated in biopsyed group than in non-biopsyed, group. And after 5minutes of endoscopic respiration rate was more increased in gastric cancer than in gastritis (p=0.0406) or gastric ulcer (p=0.0073). And after 5 minutes of endoscopic systolic blood pressure was elevated over 50years old men (P=0.0238). In short, the increase of a pulse rate af ter 5 minutes of endoscopy was not influenced by general characteristics of samples in this experiment. And systolic blood pressure over 50years old men must be considered of physiological hypertension.

  • The purpose of this study was to investigate the effects of strengthening exercise program on the improvement of balance in the elderly Subjects were forty members living in Daegu (20 males, 20 females), between 65 and 81 years of age. The subjects were divided into two groups; an experimental group and a control group, and each group included 10 males and 10 females. The subjects for the experimental group were participated in the strengthening exercise program for 8 weeks, between April 2001 and June 2001 The results of this study were as follows: 1. General characteristics statistically significantly affecting for CTSIB were mental status; weight, height, vision, systolic blood pressure, diastolic blood pressure, pulse rate, mental status, obesity and thigh BMD were BBT; age, weight, height, vision, blood pressure systolic, blood pressure diastolic, blood pulse rate, mental status, obesity, spine BMD and thigh BMD were for OLST. 2. After the exercise program, there was no significant difference in the balance ability between the experimental group and the control group in CTSIB. However, there was a significant difference in OLSTR on the hard (p<0.05) and soft (p<0.05) surface with open eyes condition between the experimental group and the control group. In the case of OLSTL, there was a significant difference on the hard surface with open eyes condition (p<0.05), and on the soft surface with open eyes condition (p<0.05) and closed eyes condition (p<0.05) between the experimental group and the control group. There was no significant difference in the balance ability between the experimental group and the control group in BBT.

Effects of feed form and feed particle size with dietary L-threonine supplementation on performance, carcass characteristics and blood biochemical parameters of broiler chickens

  • An experiment was conducted to evaluate the effect of form and particle size of feed supplemented with L-threonine on growth performance, carcass characteristic and blood biochemical parameters of broiler chickens. The experimental design was a $2{\times}2{\times}2$ factorial arrangement of treatments evaluating two feed forms (pellet or mash), two feed particle sizes (fine or course), and two inclusion rates of dietary L-threonine (with or without) which adopted from 7 to 42 days of age. In this experiment, 360 a day old chicks in two sexes were assigned in each treatment and each experimental unit was included 15 chicks. Feed consumption and weight gain were measured weekly. At 35 days of age, blood samples were taken to analysis blood biochemical parameters. At the end of the experimental period, two birds were slaughtered in each treatment and carcass analysis was carried out. The results showed that the effect of feed form on body weight gain and feed intake in whole of experimental period was significant (P < 0.05). Broilers fed pelleted diets had more weight gain than the mash group. Growth performance parameters were not affected by feed particle size and dietary L-threonine supplementation in whole of experimental period (P > 0.05). The results of carcass analysis showed that liver and gizzard relative weights were influenced by feed form (P < 0.05). However, pancreas and liver relative weights were affected by feed particle size and dietary L-threonine supplementation, respectively (P < 0.05). Triglyceride and VLDL levels were affected by feed form and dietary L-threonine supplementation (P < 0.05). The effect of feed particle size on blood biochemical parameters was not significant (P > 0.05). In conclusion, the experimental results indicated that feed form increased feed consumption and weight gain in whole of experimental period (1 to 42 days of age) while feed particle size and dietary L-threonine had no effect on broiler performance.

The Clinical Feature and Prognostic Factor of Glyphosate Intoxication Patients (글리포세이트 중독 환자의 임상 양상 및 사망 관련 인자 분석)

  • Purpose: Glyphosate is widely used and its toxic exposures are not rare. Occasionally, glyphosate intoxication can lead to death. The aim of this study is to analyze clinical findings and fatality in glyphosate intoxication. Methods: Clinical data on acute glyphosate intoxication were prospectively collected at 28 hospitals nationwide between August 2005 and July 2006. The patients' clinical symptoms and characteristics of fatalities were investigated and statistical analysis was performed. Results: Among 105 patients who were finally included, gastrointestinal symptoms(59%) were the most common. A significant difference in the amount ingested was observed between patients with higher systolic blood pressure and those with systolic blood pressure less than or equal to 80 mmHg (p<0.001). The more the patients ingested, the more aggravated their mental status became (p=0.004). Seven patients(6.7%) died, and all of them had ingested greater than or equal to 200 ml. Patients who died had ingested greater amounts than the survivors (p<0.001), and their mental status was worse (p<0.001), and systolic blood pressure was lower (p<0.001). According to the result of logistic regression analysis, relative risk was 24.1-fold higher in the 'poor' mental status group compared with 'good'. Conclusion: Patients who ingested large amounts of glyphosate showed poor mental status and lower blood pressure. Statistical difference in amount ingested, mental status, and systolic blood pressure was observed between survivors and patients who died. Ingested amounts and mental status were the most important factor of the prognosis of glyphosate intoxication.

A Study on Characteristics of Acid-Base Balance in Goats (염소의 산·염기 균형 특성에 관한 연구)

  • The changes of acid-base status in vitro of the venous blood for 24 hours in ten Korean native goat were investigated. The acid-base parameters were measured within ten minutes after collection of the blood, and every hour during the first six hours and finally after twenty four hours of storage. Blood samples were stored at two different temperatures ($0-4^{\circ}C$ and $21-24^{\circ}C$). Twelve goats were induced acute acid-base disturbances by intravenous infusion of either hydrochloric acid or sodium bicarbonate and inhalated with $CO_2$ gas mixture (20% $CO_2$, 80% $O_2$) or hyperventilation were performed by means of respirator. The results were as follows; 1. Blood samples could be stored during the first two hours in ice water ($0-4^{\circ}C$) and one hour at room temperature without significant changes in pH. The magnitudes of changes were similar to those of cow, and lower than those of men and dogs. 2. The mean values of acid-base parameters in normal goat were arterial pH, 7.40; $P_{CO_2}$, 35.4mmHg; $HCO_3{^-}$, 21.8mEq/L. 3. Both the base excess and the bicarbonate showed high correlation (r=0.99) during the metabolic disturbance and were represented as $B.E.=1.38\;HCO^-{_3}-29.7$. 4. The slope of blood buffer curve obtained from the in vivo experiment was 16.3mEq/L/pH. 5. The magnitudes of changes in hydrogen ion concentration per unit change of $P_{CO_2}$ were 0.8nM/mmHg in hypercapnia and 1.0nM/mmHg in hypocapnia. 6. The ranges of acid-base parameters in normal goat urine were pH, 6.0-8.1; $P_{CO_2}$, 42-61mmHg; $HCO_3{^-}$, 2-110mEq/L. The concentration of potassium was higher (60-200mEq/L), and that of sodium was lower (8-70mEq/L) than those of human urine.
