하악지치 발치 중 극심한 출혈 치험례 (ACTIVE BLEEDING CARE DURING SURGICAL EXTRACTION OF MANDIBULAR THIRD MOLAR: REPORT OF TWO CASES)

  • The experienced surgeon can be surprised & challenged by the hazards of active bleeding during oral & maxillofacial surgical procedure, because of alterations in the surgical anatomy, bleeding disorders and surgical intervention of infected tissues. This is a report of two cases of active bleeding during surgical extraction of mandibular third molar, that had the pericoronitis, osteitis and adjacent neurovascular bundle in its apex. When the abrupt active bleeding was occurred during surgical extraction of mandibular third molar, pressure packing by hemostatie agent(bone wax) & wet gauze biting were applied into the extraction socket during 30 minutes. After 30 minutes, the wound was explored about the bleeding and active bleeding was then continued. In spite of repeated bleeding control method of the pressure dressing, the marked hemorrhage was generated continuously. Therefore, the author decised the bleeding as immediately uncontrollable hemorrhage and the pressure dressing was again applied for the more longer duration without wound closure. After 3 days, the pressure dressing was removed and iodoform gauze drainge was then established without the bleeding. The drain was changed as the interval of 3~5 days for prevention of infection & secondary hemorrhage and relatively good wound healing was then resulted in 6 weeks.

Bleeding After Gastric Endoscopic Submucosal Dissection Focused on Management of Xa Inhibitors

  • Purpose: The use of direct oral Xa inhibitors (DXaIs) to prevent venothrombotic events is increasing. However, gastrointestinal bleeding, including that related to endoscopic resection, is a concern. In this study, we evaluated bleeding and coagulation times during the perioperative period of gastric endoscopic submucosal dissection (ESD). Materials and Methods: Patients who consecutively underwent gastric ESD from August 2016 to December 2018 were analyzed. Bleeding rates were compared among the 3 groups (antiplatelet, DXaIs, and control). DXaI administration was discontinued on the day of the procedure. Prothrombin time (PT), activated partial thromboplastin time, and the ratio of inhibited thrombin generation (RITG), which was based on dilute PT, were determined before and after ESD. Results: During the study period, 265 gastric ESDs were performed in 239 patients, where 23 and 50 patients received DXaIs and antiplatelets, respectively. Delayed bleeding occurred in 17 patients (7.4%) and 21 lesions (7.1%). The bleeding rate in the DXaI group was significantly higher than that in the other groups (30.4%, P<0.01), and the adjusted odds ratio of bleeding was 5.7 (95% confidence interval, 1.4-23.7; P=0.016). In patients using DXaIs, there was a significant (P=0.046) difference in the median RITG between bleeding cases (18.6%) and non-bleeding cases (3.8%). Conclusions: A one-day cessation of DXaIs was related to a high incidence of bleeding after gastric ESD, and monitoring of residual coagulation activity at trough levels might enable the predicted risk of delayed bleeding in patients using DXaIs.

Cervical Spinal Epidural Hematoma Following Cervical Posterior Laminoforaminotomy

  • A 65-year-old man who had lateral cervical disc herniation underwent cervical posterior laminoforaminotomy at C5-6 and C6-7 level right side. During the operation, there was no serious surgical bleeding event. After operation, he complained persistent right shoulder pain and neck pain. Repeated magnetic resonance image (MRI) showed diffuse cervical epidural hematoma (EDH) extending from C5 to T1 level right side and spinal cord compression at C5-6-7 level. He underwent exploration. There was active bleeding at muscular layer. Muscular active bleeding was controlled and intramuscular hematoma was removed. The patient's symptom was reduced after second operation. Symptomatic postoperative spinal EDH requiring reoperation is rare. Meticulous bleeding control is important before wound closure. In addition, if patient presents persistent or aggravated pain after operation, rapid evaluation using MRI and second look operation is needed as soon as possible.

Recent Trends in the Endoscopic Management of Variceal Bleeding in Children

  • Variceal bleeding results in significant morbidity and mortality in both children and adults. The guidelines for the management of variceal bleeding are well established in adults but not in children as there have been insufficient pediatric studies of this disorder. In addition, the adult guidelines for treatment of variceal bleeding cannot be applied directly to children as the etiology and natural course of this disease differs between children and adults. Examples of recommended treatments in children include endoscopic variceal ligation as secondary prophylaxis for biliary atresia whereas a meso-Rex shunt operation for extrahepatic portal vein obstruction. In this review, we discuss prophylaxis options and some technical aspects of endoscopic management for variceal bleeding in children.

항혈소판제를 투여받는 환자의 치과치료 (Dental treatment of the patient with antiplatelet agent)

  • Antiplatelet agent is administered to the patients who have ischemic heart disease, transient cerebral infarction, as well as hypertension, etc. Antiplatelet agent prevents thromboembolism by inhibition of platelet aggregation by various mechanism. Due to that reason, patient who administered antiplatelet agent has bleeding tendency. Surgeon does not want to make a complication by bleeding during and after operation, and want to stop taking antiplatelet agent. However, It is very dangerous for the patient to stop antiplatelet agent. Local bleeding as a complication after operation is considered minor one, whereas thromboembolism is life threatening serious complication. Most dental intervention can be performed without withdrawal of antiplatelet agent. Dental intervention should be limited area, and surgeon should do active bleeding control.

Effects of an electric toothbrush combined with 3-color light-emitting diodes on antiplaque and bleeding control: a randomized controlled study

  • Purpose: This randomized controlled study aimed to evaluate the effects of an electric toothbrush with 3 colors of light-emitting diodes (LEDs) on antiplaque and bleeding control. Methods: This randomized, placebo-controlled, double-blinded, parallel-group clinical trial included 50 healthy adults with gingivitis, who were randomly assigned to 2 groups. The experimental group used electric toothbrushes with 3 colors of LEDs and the control group used the same electric toothbrush as the experimental group, but with LED sources with one-hundredth of the strength. The subjects used the electric toothbrush 3 times a day for 4 minutes each time. As clinical indices, bleeding on marginal probing (BOMP), the Löe-Silness gingival index (GI), and the Turesky-Quigley-Hein plaque index (QHI) were assessed at baseline, at 3 weeks, and at 6 weeks. Results: There were significant decreases in all clinical indices (BOMP, GI, QHI) in both the experimental and control groups compared to baseline at 3 weeks and at 6 weeks. In a comparison between the experimental and control groups, no statistically significant differences were observed for any clinical indices at 3 weeks (P>0.05). However, at 6 weeks, statistically significant differences were observed between the experimental and control groups in BOMP and GI, which are indicators of gingival inflammation (P<0.05). Conclusions: This study demonstrated that an electric toothbrush combined with 3-color LEDs reduced gingival bleeding and inflammation after 6 weeks.

글란즈만 혈소판무력증 환자의 치과적 관리 (Dental Management in a Patient with Glanzmann's Thrombasthenia : A Case Report)

  • 글란즈만 혈소판무력증(Glanzmann Thrombasthenia, GT)은 상염색체 열성 유전으로 나타나는 희귀한 선천성 질환으로 혈소판 기능에 이상이 있어 혈액 응고 장애를 보이며 대표적인 증상은 지혈이 잘 되지 않는 출혈이다. 1918년 소아과의사 글란즈만에 의해 처음보고 되었으며 글란즈만씨병(Glanzmann's disease)이라고도 불리 운다. GT는 남성과 여성에서 동등하게 나타나고, 중동, 인도 프랑스 등 근친결혼을 하는 곳에서 많이 발생하며, 대략 1,000,000명당 1명에서 발견된다. 우리나라에서는 2018년 질병관리본부 희귀질환과에 따르면 200여명의 환자가 보고되고 있다. 임상증상은 점상출혈, 반상출혈, 비 출혈, 치은 출혈 등이 관찰되고, 유치의 자발적 탈락에서도 수혈을 고려해야 할 정도의 출혈이 발생할 수 있다. 치료의 일반적인 목표는 출혈 예방과 지혈이다. 여러 가지 지혈을 위한 처치에도 출혈이 지속되면 혈소판 수혈을 해야 한다. 소아치과의사는 구강을 청결히 하여 치은염과 치아 우식증을 예방함으로써 치은 출혈을 최소화하고, 구강 위생과 규칙적인 치과 검진의 중요성을 주지시켜야 하며, 치과치료시 출혈에 대비한 치료계획의 수립 및 세심한 진료가 필요하다. GT를 동반한 만6세 여아로 치아우식증을 주소로 내원하여 아산화질소-산소를 이용한 흡입진정하에 레진수복을 시행하였으며 술 후 치료부위의 압박지혈이 필요하였다.

치은염 환자에게 적용한 오일풀링과 전문가 칫솔질법의 효과 (Effects of coconut oil pulling and professional toothbrushing on gingivitis)

  • Objectives: The purpose of this study was to provide basic data for gingivitis management programs by dental hygienists by evaluating the effect of oral health education, oil pulling, and professional toothbrushing on gingivitis. Methods: A total of 38 subjects were divided into three groups: control group (12 subjects), experimental group 1 (13 subjects), and experimental group 2 (13 subjects). The control and experimental groups were instructed to brush using the rotating method. Distilled water was provided to the control group after training. Coconut oil was provided every morning for about 10 minutes. In the experimental group 2, a professional brushing method was used at each visit. Results: There were no significant differences in oral health among the three groups, and there was homology between patient hygiene performance (PHP) index (p=0.144) and bleeding rate (p=0.213). The PHP index showed a significant interaction between the group and measurement time. The control and experimental groups showed changes in the PHP index with time (F=3.711; p=0.013). The bleeding rate showed a significant interaction between the group and measurement period. The control and experimental groups showed changes in the bleeding rate with time (F=6.707; p<0.001). Conclusions: Oral health education, professional toothbrushing, and oil pulling specialists in oral care of gingivitis were effective in managing gingivitis. It is necessary to educate people on self-management methods for oral health promotion using gingivitis management programs by dental hygienists.

Do Chamomile effect on duration, amount of bleeding, and interval of menstrual cycles?

  • Objectives: Heavy menstrual bleeding has a significant effect on women's daily life and health consequences for theirs. Attention to women's and girls' menstrual health is critical for their health. This study presents the results of a novel research on the effect of chamomile capsule on the amount of bleeding, and the duration and interval of menstrual cycles. Methods: The current study was a randomized, double-blind, clinical trial, which was carried out on 118 female students living in dorms of Guilan University of Medical Sciences. The participants were divided into two groups of 59, each receiving either chamomile capsule 250 mg or placebo three times a day, for an interval beginning from 7 days before the starting menstruation till the next onset. Higham chart is the pictorial blood loss assessment to determine the amount and duration of menstrual bleeding. Data were analyzed using SPSS version 20. Alpha was set at 0.05 for all analyses. Results: The average amount of bleeding in the chamomile group decreased after taking the capsule (p = 0.001). However, statistical tests did not show any significant difference in terms of duration and interval between two bleeding in both the experimental and control groups (p > 0.05). Conclusion: The present study showed that the chamomile capsule decreases the amount of menstrual bleeding and can be used as a therapeutic method. The incidence of postoperative hemorrhage after tooth extraction in patients with thrombocytopenia

  • Objectives: The risk of bleeding after tooth extraction in thrombocytopenia patients remains unclear. Therefore, the present study aimed to assess the risk of bleeding after tooth extraction in patients with thrombocytopenia. Materials and Methods: The study included 220 patients who had a medical history of thrombocytopenia and underwent tooth extraction (330 teeth). The patients were divided into those who had thrombocytopenia (platelet count <150k) immediately before the tooth extraction, and those who had platelet counts that were between 150k and 250k before the extraction. Bleeding complications were recorded and compared between the patient groups. Results: Of the 220 patients, 130 underwent tooth extractions while having thrombocytopenia (platelet count <150k), and 90 had platelet counts that were between 150k and 250k before tooth extractions. Bleeding complications occurred in 11 patients (5.0%) of the 220 patients. Among those 11 patients with bleeding complications, 10 patients (7.7%) had thrombocytopenia (platelet count <150k) of the 130 patients, and 1 patient (1.1%) had a normal platelet count of the 90 patients. There was a significant difference between the patient groups regarding bleeding after extractions (P<0.001). No significant difference in the incidence of post-extraction bleeding was found between the subgroups by platelet count within the thrombocytopenia group. Conclusion: Thrombocytopenia (platelet count <150k) increases the risk of post-tooth extraction bleeding. Therefore, bleeding control under the proper evaluation of hemostasis and performing delicate tooth extraction procedures using hemostatic plugs is necessary during the tooth extraction of patients with thrombocytopenia.
