김홍경의 기경팔맥관(奇經八脈觀)에 관한 연구(硏究) - 음양유교맥(陰陽維蹻脈)과 사상체질(四象體質)의 상관성을 중심으로 - (The reserch of Kim hong-kyung's view of extra meridians -With the connection yin yang wei qiao mai With four trigram constitution theory-)

  • Subject : Kim hong-kyung(金洪卿)'s view of eight extra meridians Object : Kim hong-kyung has not only deeply studied Saam acupuncture but also the function and characteristics of extra meridian at the same time. Especially, he has tried the study to connect yin yang wei qiao mai(陰陽維蹻脈) with Lee je-ma(李濟馬)'s four trigram constitution theory, therefore the author felt the need to make public the theory and started reserch on the theory. Method : I researched the physiological properties of extra meridians through the past eastern medical literature, I reported on the types of people to show well the properties of extra meridians gathering Kum-oh's verbal explanation when I had classes from him. Result : Kum-oh Kim hong-kyung defined meridians as passways of consciousness and feeling, he interpreted 12 meridians and extra meridians with the view of mind, which has very important meanings in the history of acupuncture in the point of first trial to interpret meridians from this view. He explained Ren mai(任脈) and Du mai(督脈) are meridians which show clearly properties of yin-yang to appear trusts of affirmation and distrusts of denial in the mind, as Ren(任) means entrusting and Du(督) means supervising in English. He explained Chong mai(衝脈) is the meridian representing impulsive man that has a meaning of impulse as being suggested the name of Chong mai, because Chong mai is borrowed from Foot shao yin kidney meridian(足少陰腎經). Dai mai(帶脈) is the meridian representing the characteristics of woman that has a meaning of binding as being suggested from the name of Dai mai which acts when people have something on their flank or they are pregnant, because Dai mai borrowed from Foot shao yang gall bladder meridian. He thought that four trigram divided from yin and yang is innate trigram, so Yang wei mai(陽維脈), Yin wei mai(陰維脈), Yang qiao mai(陽蹻脈), Yin qiao mai(陰蹻脈) among extra meridians have connections with Tai-yang ren(太陽人), Tai-yin ren(太陰人), Shao-yin ren(少陰人), Shao-yang ren(少陽人) of Four trigram constitution theory respectively which was created by Lee je-ma. He explained yang means opponents and wei(維) means binding. Yang wei mai is the meridian to be active when opponents bind or intervene in you, also the meridian is well active to the people who easily criticize others who are rude, to the people who well refuse restraints from organization. Because he explained yin means me and wei(維) means binding. Yin wei mai is the meridian to be active when I bind and intervene in opponents. He also state that the meridian is easily active to the people who have a tendency to gather people and make party apparatus by nature. He explained Yang qiao mai is the meridian to be active in the situation that people are waiting for others to bridge gap because yang means opponents and qiao(蹻) which has a meaning of standing on one's tiptoes means bridging gap between people. He also state the meridian is easily active to Shao yin ren who has a tendency to be passive, to easily settle down and not to move by nature. He explained Yin qiao mai is the meridian to be active in the situation that I bridge gap between me and others to make friends, because yin means me and qiao(蹻) which has a meaning of standing on one's tiptoes means bridging gap between me and others. He also state the meridian is easily active to Shao yang ren who is active in every business and who has remarkable ability to make friends. Conclusion : Kim hong-kyung stated the physiological properties of extra meridians as below. Ren mai is the meridian representing positive confidence which is easily active when people have a mind to trust others, Du mai is the meridian representing negative distrust which is easily active when people have doubts. Chong mai is the meridian representing the characteristics of man that has impulsive characteristics. Dai mai is the meridian representing the characteristics of woman to be easily active when she is pregnant. Yang wei mai has close connections with Tai-yang ren who has a tendency to refuse restraints from others and to criticize on opponent's remarks and behaviors that are wrong. Yin wei mai has close connections with Tai-yin ren, as the meridian is active in the situation that people enjoy restraining and intervening in others. Yang qiao mai has close connections with Shao yin ren, as the meridian is active to the people who are negative and passive by nature. Yin qiao mai has close connections with Shao yang yin who has a strong tendency to make friends easily by nature.

형장침법 연구 (Study on the Acupuncture in Hyungsang)

  • The following conclusions are induced from a study on the acupuncture therapy depending on hyungsang of the persons. The study is made on the basis of 'Internal classic (內經)& and &Clinical Lectures by Dr. Jeesan&. The acupuncture originated from the treatment of spasm with numbness in the southern area. The acupuncture is basically a remedy for the exterior disease of meridian but also it can be a cure for the interior disease of Jang and obstinate disease with accurate method. Three mechanisms of acupuncture are described in 'Internal classic'. The first is to make meridian circulate smoothly. The second is to regulate Ki and Hyul. The third is to regulate points through which the meridian-Ki goes in and out smoothly or adversely. There are two ways of acupuncture in 'Internal classic'. One is based on pulse and symptom and the other on the Hyungsang. The former is more generally used therapy, to which depletion method, Asi point method(阿是穴 療法), Inyoung-kigu pulse comparison method (人迎氣口脈法) and method depending on jang-bu disease belong. Acupuncture is done on Su points(輸穴) and back-su point(背兪穴) in case of jang-disease. In case of bu-disease, the treatment is done on Hap points(合穴) and Mo-points(募穴). The latter includes two methods; one according to invariable Hyungsang. And the other to variable Hyungsang. The method of acupuncture according to invariable Hyungsang usually selects Won-points(原穴). Different Hyungsang requires different method of acupuncture; In case of Dam type, the acupuncture is mainly practiced on four-Kwan points with reinforcing and reducing methods achieved by the direction of the needle tip pointing to. In case of Bangkwang type, the acupuncture is usually done on Jungwan(中脘) and Poongyung(豊隆) with reinforcing and reducing methods by means of respiration. In case of female, more effective are the acupoints on the right and lateral parts of the body selected on the basis of five su-points of the twelve meridians matching the heavenly stems and earthly branches. In case of male, more effective are the acupoints on the left, front and rear parts of the eight extra meridians. In case of acupuncture to the person with Hyungsang of five jang and six bu, each person's intrinsic Hyung, color, pulse, must be observed. Because symptoms of jang-bu disease also must be checked up. Acupuncture is done on the Won-points of the meridians related to the jang and bu where the disease starts. The disease of five jang is so obstinate that it requires both of medication and acupuncture for a long time. In case of acupuncture to the person with Hyungsang of animal types, diagnosis is made on the basis of shape, temper, function and color. And the treatment is given on the Won-points of corresponding exterior and interior meridians. For the fish type, the acupuncture is done on the kidney meridian of foot-soyin and the urinary bladder of foot-taiyang. For the bird type, on the heart meridian of hand-soyin, the pericardium meridian of hand-gualyin, and the small intestine meridian of hand-taiyang For the deer type, on the liver meridian of foot-gualyin and the gallbladder meridian of foot-soyang. For the turtle type, on the lung meridian of hand-taiyin and the large intestine meridian of hand-yangmyung.

수종(水腫)의 병인병기(病因病機) 및 침구치료(鍼灸治療)에 대한 문헌적(文獻的) 고찰(考察) (Literatual Study on Etiological Analysis, Pathogenesis and Acupuncture Treatment of Edema)

  • 수종(水腫)의 분류(分類), 병인(丙因), 병기(病機), 치법(治法), 변증시치(辨證施治)에 따른 치료혈(治療穴)의 관계(關係)에 대해 황제내경이후(黃帝內經以後) 34종의(種) 문헌(文獻)을 고찰한 결과(結果) 다음과 같은 결론(結論)를 얻었다. 1. 수종(水腫)은 육음외야(六淫外耶), 노권내상(勞倦內傷), 혹(或) 음식실조(飮食失調) 등으로 폐(肺) 비(脾) 비신(脾腎)과 방광(膀胱) 삼초(三焦)의 기능이 장애(障碍)되어 진액수포(津液輸布)를 실상(失常)함으로써 수액(水液)이 저유(貯留)하여 기부(肌膚)로 범일(泛溢)한 것으로 얼굴 팔다리 가슴 배, 심하면 온 몸에 머물러 붓는 병증(病症)이다. 2. 수종(水腫)의 분류(分類)는 병인(病因)과 맥증(脈證)에 따라서 오장수(五臟水) (간수(肝水) 심수(心水) 비수(脾水) 폐유(肺兪) 신수(腎水)), 오종수(五種水) 풍수(風水) 피수(皮水) 정수(正水) 석수(石水) 황한(黃汗)), 십수(十水)(청수(淸水) 적수(赤水) 황수(黃水) 백수(白水) 흑수(黑水) 원수(元水) 풍수(風水) 석수(石水) 이수(里水) 기수(氣水))로 구분(區分)되며, 이밖에 십이수(十二水)와 이십사수후(二十四水候), 양수(陽水)와 음수(陰水)로 대별(大別)되기도 한다. 3. 수종(水腫)의 병인(病因)은 풍사외습(風邪外襲) 폐기부선(肺氣不宣), 수습내침(水濕內侵) 비부건운(脾不健運), 노권태과(勞倦太過) 기포(飢飽) 생육부절(生育不節) 등(等)에서 벗어나지 않으며, 양수(陽水)의 병인(病因)으로 풍수범람(風水泛濫) 습열옹성(濕熱壅盛), 음수(陰水)의 병인(病因)으로 전양쇠허 신기쇠미(腎氣衰微) 등이 있다. 4. 수종(水腫)의 병기(病機)는 폐(肺) 비(脾) 신(腎) 삼경장기(三經臟氣)의 기능실조(機能失調)에 지나지 않으며, 그 병의(病) 근본(根本)은 모두 신에(腎) 있다. 5. 수종(水腫)의 침구치료(針灸治療)에 있어서, 침구치료(鍼灸治療)를 병용(竝用) 하거나 혹은 구법(灸法)만 사용하기도 한다. 문헌상(文獻上) 침자혈위(針刺穴位)는 '수구(水溝)' 혈이(穴) 최요혈(最要穴)이며 '수분(水分) 수구(水構)' 혈이(穴) 구법(灸法)의 최요혈(最要穴)로 기재(記載)되어 있다. 6. 수종(水腫)의 병인병기(病因病耭)에 따른 침구치료(針灸治療)에 있어서 주로 풍(風) 습(濕) 열에(熱) 해당하는 양수(陽水)나 실증(實證)엔 산풍(散風) 청열리습(淸熱利濕) 선폐리전하기 위해 '수구(水構) 족삼리(足三里) 비유(脾兪) 제릉천(除陵泉)(사)(瀉)' 등의 혈을(穴) 침랄사법(針剌瀉法) 하거나 혹구(或灸)하며, 비양허(脾陽虛) 신기허(腎氣虛)에 해당하는 음수(陰水)나 허증(虛證)엔 온운비양(溫運脾陽) 온신조양(溫腎助陽) 화기행수(化氣行水) 하기 위해 '수분(水分) 족삼리(足三里) 기해(氣海)(구)(灸) 비유(脾兪) 신유(腎兪) 삼초유(三焦兪) 태계(太溪)' 혈을(穴) 보(補) 평보평사(平補平瀉) (침자(針刺))하거나 구법(灸法)을 활용한다.

  • Objectives : Ten heavenly stems(10天干) and Twelve earthly branches(12地支) are symbols exposing change order in heaven and earth, and are a very important sign in studying oriental philosophy and oriental medicine. Especially, 10 heavenly stems(10天干) and 12 earthly branches(12地支) are indispensable for the study of Five Circuits And Six Qi(오운육기), and a deep study is needed. Methods : I have examined Yin Yang combination(음양배합), Five elements combination(오행배합), Six Qi 3Yin 3Yang combination(육기삼음삼양배합), viscera combination(장부배합), Mutual collision(상충), Six combination(육합), Three combination(삼합), etc. of 12 earthly branches(12지지) by referring to books such as "Yellow Emperor Internal Classic" ("黃帝內經") and "Principle of universe change" ("우주변화의 원리"). Results & Conclusions : Zi Yin Chen Wu Shen Xu(子 寅 辰 午 申 戌) become Yang(陽), Chou Mao Si Wei You Hai(丑 卯 巳 未 酉 亥) become Yin(陰), Zi Si Yin Mao Chen Si(子 丑 寅 卯 辰 巳) become Yang, and Wu Wei Shen You Xu Hai(午 未 申 酉 戌 亥) become Yin. Twelve earthly branches can be divided into five movements by its original meaning, where YinMao(인묘) is tree, SiWu(사오) is a fire, ShenYou(신유) is a gold, HaiZi(해자) is water, and ChenXuChouWei(진술축미) mediate in the middle of four movements So they become soil(土). SiHai(巳亥) is JueYin Wind Tree(궐음 풍목), ZiWu(子午) is ShaoYin Monarch Fire(소음 군화), ChouWei(丑未) is TaiYin Humid Soil(태음 습토), YinShen(寅申) is ShaoYang Ministerial Fire(소양 상화), MaoYou(卯酉) is YangMing Dry Gold(양명 조금), and ChenXu(辰戌) is TaiYang Cold Water(태양 한수). Viscera combination(장부배합) combines Zi(子) and Bile(膽), Chou(丑) and Liver(肝), Yin(寅) and Lung(肺), Mao(卯) and Large intestine(大腸), Chen(辰) and Stomach(胃), Si(巳) and Spleen(脾), Wu(午) and Heart(心), Wei(未) and Small intestine(小腸), Shen(申) and Bladder(膀胱), You(酉) and Kidney(腎), Xu(戌) and Pericardium(心包), Hai(亥) and Tri-energizer(三焦), Which means that the function of the viscera and channels is the most active at that time. Twelve earthly branches mutual collisions collide with Zi(子) and Wu(午), Chou(丑) and Wei(未), Yin(寅) and Shen(申), Mao(卯) and You(酉), Chen(辰) and Xu(戌), and Si(巳) and Hai(亥). The two colliding earthly branches are on opposite sides, facing each other and restricting each other by the relation of Yin-Yin and Yang-Yang it rejects each other so a collision occurs. Six Correspondences(六合) coincide with Zi(子) and Chou(丑), Yin(寅) and Hai (亥), Mao(卯) and Xu(戌), Chen(辰) and You(酉) and Si(巳) and Shen(申) Wu(午) and Wei(未). Three combination(三合) is composed of ShenZiChen(申子辰), SiYouChou(巳酉丑), YinWuXu(寅午戌), and HaiMaoWei(亥卯未). Three combination(三合) is composed of ShenZiChen(申子辰), SiYouChou(巳酉丑), YinWuXu(寅午戌), and HaiMaoWei(亥卯未). This is because the time Six Qi(六氣) shifts in these three years are the same.

최근 10년간(1988~1998) 경기지역 소아에서 A형 간염의 역학적 변화 및 임상에 관한 연구 (Epidemiological Changes and Clinical Features of Hepatitis A in Children, Living in Kyung-gi Province, Since 1988 to 1998)

  • 목 적 : 오염된 식수나 음식물에 의하여 전파되는 A형 간염은 전세계적으로 과거에 비해 현저한 발생 감소를 보이고 있고 국내에서도 1980년대 중반 이후부터는 돌발적 유행 양상은 소실되었고 매우 적은 수의 산발적 발생안 보고 되고 있다. 그러나 최근에 와서 성인과 소아에서 A형 간염이 점차 증가하는 추세를 보이고 있어 저자들은 최근 10년간 경기지역의 15세 이하의 소아에서 A형 간염의 역학적 변화와 이들 환아의 임상적 특성에 관하여 연구를 실시하였다. 대상 및 방법 : 1988년 7월부터 1998년 8월까지 경기지역에 있는 가톨릭대학교 의과대학 성모자애병원, 성가병원, 의정부성모병원, 성인센트병원에 임원된 환아중 혈청학적으로 A형 간염으로 진단된 환아들의 의무기록을 통하여 후향적으로 A형 간염의 연별, 월별, 연령별, 성별 발생상태와 발생과 연관된 역학적 특성, 그리고 임상적 특성, 경과 및 합병증 등에 관하여 분석하였다. 결 과 : 1) 연구 기간동안 46명이 A형 간염으로 진단되었고, 이들 환아중 28명(60.1%)이 1998년에 집중적으로 발생되었다. 2) 이들 환아는 연중 3월에서 7월 사이에 주로 발생되었고 또한 10세 이상의 연령(37명, 80.4%)에서 발생되었으며 남녀비는 1.2:1.0이었다. 3) 연구 환아들 부모의 사회경제 및 교육 정도는 중등도 이하가 각각 97.8%. 87.0%으로 낮았다. 그리고 이들 환아는 원인이 뚜렷하지 않고 집단적 발생은 없었으나 2례에서 가족내 발생이 확인되었다. 4) 임상적으로 내원시 이들 환아의 주소는 구토 및 구역, 황달, 식욕부진, 경미한 발열, 권태감, 복통, 상기도증상, 소화장애, 짙은 소변, 두통, 설사의 순으로 호소하였고, 진찰소견상에서는 황달, 간비대, 우상복부 압통, 비장종괸, 좌하복부 압통의 순으로 이상 소견을 보였다. 5) 간기능 검사상 전례에서 비정상적인 소견과 담즙 정체성 간염의 소견이 관찰되었으나 이들 소견은 2~4주 사이에 정상화되었다. 또한 복부 초음파 및 간동위원소 검사상 당낭벽 비후, 간비대, 비장비대, 복수 등의 소견이 관찰되었다. 6) 이들 연구 환아들의 평균 임원일 수는 11.1일이었고 전례에서 합병증 없이 좋은 경과를 보였다. 결 론 : 최근 10년간 경기지역에서 A형 간염의 발생의 양상은 사회경제 및 교육 정도가 낮은 가정의 10세 이상의 연령 소아에서 1998년도에 집중적으로 발생되고 있음을 알 수 있었고, 전례에서 비정상적인 간기능 소견이 관찰되었고 다수에서 담즙 정체성 간염의 양상을 보였으나 합병증 없이 좋은 임상 경과가 있었음을 확인하였다.
