Comparison of fracture strength after thermomechanical aging between provisional crowns made with CAD/CAM and conventional method

  • PURPOSE. The objectives of this study were to evaluate the fracture strength and fracture patterns of provisional crowns fabricated from different materials and techniques after receiving stress from a simulated oral condition. MATERIALS AND METHODS. A monomethacrylate-based resin (Unifast Trad) and a bis-acryl-based (Protemp 4) resin were used to fabricate provisional crowns using conventional direct technique. A milled monomethacrylate resin (Brylic Solid) and a 3D-printed bis-acrylate resin (Freeprint Temp) were chosen to fabricate provisional crowns using the CAD/CAM process. All cemented provisional crowns (n=10/group) were subjected to thermal cycling (5,000 cycles at 5°-55℃) and cyclic occlusal load (100 N at 4 Hz for 100,000 cycles). Maximum force at fracture was tested using a universal testing machine. RESULTS. Maximum force at fracture (mean ± SD, N) of each group was 657.87 ± 82.84 for Unifast Trad, 1125.94 ± 168.07 for Protemp4, 953.60 ± 58.88 for Brylic Solid, and 1004.19 ± 122.18 for Freeprint Temp. One-way ANOVA with Tamhane post hoc test showed that the fracture strength of Unifast Trad was statistically significantly lower than others (P<.01). No statistically significant difference was noted among other groups. For failure pattern analysis, Unifast Trad and Brylic Solid showed less damage than Protemp 4 and Freeprint Temp groups. CONCLUSION. Provisional crowns fabricated using the CAD/CAM process and the conventionally fabricated bis-acryl resins exhibited significant higher fracture strength compared to conventionally fabricated monomethacrylate resins after the aging regimen. Therefore, CAD/CAM milling and 3D printing of provisional restorations may be good alternatives for long term provisionalization.

주조 금속 구조물을 이용하여 강화한 임시보철물 수복 증례 (Reinforcement of provisional restoration with cast metal framework: A case report)

  • 임시보철물은 고정성 보철물로의 수복 과정에서 지대치 보호 및 기능과 심미성 회복의 측면에서 중요한 역할을 수행한다. 특히 이를 장착한 상태로 최종 수복 전에 장기간의 관찰이 필요한 환자에서 그 중요성이 더욱 강조되나, 상대적으로 낮은 강도로 인하여 잦은 파절이 발생할 수 있으며 이를 방지하기 위해 강화된 임시보철물의 제작을 고려할 수 있다. 이 문헌에서는 교합 수직 고경의 증가를 필요로 하는 환자의 증례를 소개하고자 하였으며, 수직 고경 증가의 영향으로 임시보철물 사용 시 더 큰 부하의 작용 및 장기간의 사용이 예상되었다. 이에 주조 금속 하부구조물을 첨가하여 bis-acryl 레진으로 강화된 임시보철물을 제작하였으며, 약 4개월의 관찰 기간 동안 만족스러운 결과를 보였다.

Wear resistance of indirect composite resins used for provisional restorations supported by implants

  • PURPOSE. The aim of this study was to investigate simulated localized and generalized wear of indirect composite resins used for implant supported provisional restorations. MATERIALS AND METHODS. The study investigated ten indirect composite resins. Two kinds of wear were simulated by 400,000 cycles in a Leinfelder-Suzuki (Alabama) machine. Localized wear was simulated with a stainless-steel ball bearing antagonist and generalized with a flat-ended stainless-steel cylinder antagonist. The tests were carried out in water slurry of polymethyl methacrylate beads. Wear was measured using a Proscan 2100 noncontact profilometer in conjunction with Proscan and AnSur 3D software. RESULTS. Both localized and generalized wear were significantly different (P<.05) among the indirect composite resins. SR Nexco and Gradia Plus showed significantly less wear than the other indirect composite resins. The rank order of wear was same in both types of wear simulation. CONCLUSION. Indirect composite resins are recommended when a provisional implant-supported restoration is required to function in place over a long period. Although only some indirect composite resins showed similar wear resistance to CAD/CAM composite resins, the wear resistance of all the indirect composite resins was higher than that of bis-acryl base provisional and polymethyl methacrylate resins.
