유방 미세석회화 조직검사에서 X선 유도 하 조직검사와 초음파 유도 하 조직검사의 유용성 (Usefulness of X-ray Guided Biopsy and Ultrasound Guided Biopsy in Breast Microcalcification Biopsy)

  • 유방암에 관한 사회적인 관심이 증가되고 있는 가운데 진단의 가장 기본이 되는 검사는 유방 X선 촬영과 유방 초음파 검사이다. 특히 유방 미세석회화는 조직학적 진단을 필요로 하며 유방 미세석회화 조직검사를 시행하고 있다. 이에 미세석회화 진단에 기본이 되는 X선 유도 하 조직검사(needle localized open biopsy)와 초음파 유도 하 조직검사(sono guided core needle biopsy)를 분석하여 평가하고자 한다. 유방 미세석회화를 주소로 내원한 환자에서 확대 촬영을 시행한 241예를 대상으로 X선 유도 하 조직검사(needle localized open biopsy)와 초음파 유도 하 조직검사(sono guided core needle biopsy)의 연령대 별 분포와 병소의 위치를 분석하고 확대촬영 시행 후 이루어진 검사를 분류해서 X선 유도 하 조직검사와 초음파 유도 하 조직검사의 빈도를 분석하고 악성과 양성의 결과를 확인 하였다. 그 결과 X선 유도 하 조직검사(needle localized open biopsy)가 64예(26.6%)로 초음파 유도 하 조직검사(sono guided core needle biopsy) 12예(4.9%)에 비해 5.4배 높은 것으로 나타났다. 초음파 장비의 발달과 입체정위 흡입법(stereotactic vacuum-assisted biopsy)등의 발달로 미세석회화의 조직학적 진단방법이 다양해지고 있지만, 각각 검사의 특성과 한계점을 고려해 볼 때 X선 유도 하 조직검사(needle localized open biopsy)가 유방 미세석회화 병변에서는 정확한 조직학적 진단을 제공한다고 사료된다.

Cope씨 침을 이용한 늑막 생검에 관한 임상적 고찰 (clinical evaluation of pleural biopsy by cope needle)

  • From June 1983 to September 1984, the pleural biopsies with Cope needle were performed at department of thoracic and cardiovascular surgery, Pusan Paik Hospital, Inje college, on 78 patients for exudative pleural effusion caused by various conditions. These results were analyzed clinically and summarized as follows: 2. The accuracy of pleural biopsy was 69.2% [54 of 78 patients]. The accuracy represented by ratio for the number of biopsy was 63.5% [54 of 85 biopsies]. 3. Among 61 patients of tuberculosis or malignancy, 37 [60.7%] were confirmed by pleural biopsy. 4. Tuberculosis was diagnosed in 48 patients, in 26[54.2%] out of these by pleural biopsy alone, in 3[6.2%] by pleural biopsy and isolation of AFB, in 2[4.2%] by pleural biopsy and operation, in 4[8.3%] by isolation of AFB, in 2[4.2%] by operation, and in 11[22.9%] clinically. 5. Among 13 patients of malignancy, 4[30.8%] were diagnosed by cytology alone, 4[30.8%] by pleural biopsy and cytology alone, 4[30.8%] by pleural biopsy and cytology, 1[7.7%] by pleural biopsy alone, 1[7.7%] by pleural biopsy and operation, and remained 3 by operation, lymph node biopsy, or bronchoscopy respectively. 6. False positive of clinical diagnosis was 12.5% for tuberculosis and 28.6% for malignancy. In pathological diagnosis there was no false positive. So specificity of pleural biopsy was very high. But false negative of pleural biopsy was 29.2% for tuberculosis and 46.2% for malignancy. 7. 4 cases[5.1%] of minimal pneumothorax were in the early series.

MRI-Targeted Prostate Biopsy: What Radiologists Should Know

  • Transrectal ultrasound (TRUS)-guided systematic biopsy, the current gold standard for the detection of prostate cancer, suffers from low sensitivity for clinically significant cancer. The use of diagnostic multiparametric MRI has increased the relevance of targeted biopsy techniques such as MRI-TRUS fusion biopsy and direct (in-bore) MRI-guided biopsy, which have higher detection rate for clinically significant cancer. Although primarily used in patients who remain at high clinical suspicion for prostate cancer despite a negative systematic biopsy, with the increasing use of upfront diagnostic MRI, these biopsies are expected to replace routine systematic biopsies. This pictorial essay aims to enhance our understanding of the concepts of these biopsy techniques so that they can be performed safely and provide maximum diagnostic yield.

Paradigm Shift in Prostate Cancer Diagnosis: Pre-Biopsy Prostate Magnetic Resonance Imaging and Targeted Biopsy

  • With regard to the indolent clinical characteristics of prostate cancer (PCa), the more selective detection of clinically significant PCa (CSC) has been emphasized in its diagnosis and management. Magnetic resonance imaging (MRI) has advanced technically, and recent international cooperation has provided a standardized imaging and reporting system for prostate MRI. Accordingly, prostate MRI has recently been investigated and utilized as a triage tool before biopsy to guide tissue sampling to increase the detection rate of CSC beyond the staging tool for patients in whom PCa was already confirmed on conventional systematic biopsy. Radiologists must understand the current paradigm shift for better PCa diagnosis and management. This article reviewed the recent literature, demonstrating the diagnostic value of pre-biopsy prostate MRI with targeted biopsy and discussed unsolved issues regarding the paradigm shift in the diagnosis of PCa.

미만성 폐질환에 대한 폐생검의 의의 (Open Lung Biopsy for Diffuse Infiltrative Lung Disease)

  • Retrospective review of 26 patients undergoing open lung biopsy at the Yonsei University during 10 years period was conducted to evaluate open lung biopsy for DILD. From January 1980 to August 1990, open lung biopsy was performed in 26 patients through a limited thoracotomy incision[a limited anterior or a posterolateral thoracotomy]. Open lung biopsy was indicated for diffuse interstitial pulmonary diseases undiagnosed by indirect clinical and radiological diagnostic methods. The types of incision were limited anterior[11] and limited posterolateral[15]. Preoperative evaluation of the lung disease included sputum culture[26], sputum cytology [19], bronchoscopy[9] and TBLB[7]. In 23 patients the histologic appearances after open lung biopsy were sufficiently specific histologic pictures to confirm diagnosis. The results of the biopsies changed usual therapeutic plan in 17 patients among them. The complications were resp. insufficiency[3], pulmonary ed6ma[3], sepsis[2], and others[3] in 6 patients. Diagnosis from the open lung biopsy was included respiratory pneumonia[7], fibrosis[7], infection[5], malignancy[2], others[5]. 4 patients died of respiratory insufficiency. The causes of the other three death were not due to direct result of the biopsy itself. Open lung biopsy in the patient with a diffuse infiltrative lung disease is an one of the accurate diagnostic method and frequently leads to change of the therapeutic plans. So we conclude that open lung biopsy remains our diagnostic method of choice in diffuse infiltrative lung disease undetermined etiology.

로봇을 이용한 경피 생체 검사 (Robotized Percutaneous Biopsy)

  • Biopsy is a type of histopathological examinations, in which a physician samples cells or tissues from a patient's suspicious lesion. Such a lesion frequently resides deep inside human body, and then a percutaneous biopsy is therefore performed using a thick needle with the assistance of medical imaging such as computed tomography(CT) and magnetic resonance imaging(MRI). Recently modern robotic technology is being introduced to percutaneous biopsy in order to reduce any possible human error and hazard on physicians caused by medical imaging. After medical imaging locates the exact location of lesion, an optimization algorithm plans the path for a biopsy needle. Subsequently, a robot system moves the biopsy needle to the lesion in accurate and safe way with the control of a practitioner or automatically. In this article, we try to look into the state-of-art of percutaneous biopsy using such robotic technology. We classified percutaneous biopsy robots by mechanical characteristics and by imaging technology. Then, advantage and disadvantage of each class type are described as well as the basic description, and a few representative designs for each type are introduced. Current research issues of robotized percutaneous biopsy are subjectively selected for the readers' convenience. We emphasize the basic technology of actuator and sensors compatible with imaging technology to conclude this review.

흉부 병터에 대한 경흉부 바늘생검 (Transthoracic Needle Biopsy of Thoracic Lesions)

  • In the diagnostic evaluation of thoracic lesions, the image-guided transthoracic needle biopsy was developed, and its role was expanded with the development of cross-sectional detection and characterization of thoracic lesions and advances in biopsy needle design and techniques. Particularly for diagnostic evaluation of solitary pulmonary nodules, transthoracic needle biopsy has emerged as the invasive procedure of choice. This article covers the indication, the pre-procedure preparation, various guidance-modalities and techniques, and complications.

미만성 침윤성 폐질환에 대한 폐 생검의 의의 (Open Lung Biopsy for Diffuse Infiltrative Disease of the Lung)

  • To evaluate the impact of open lung biopsy on diagnosis and treatment of diffuse infiltrative lung disease, we conducted a retrospective review of 28 patients who underwent this procedure at the Kyoungpook National University Hospital from 1986 to 1993. There were 19 men and 9 women; average age was 50.9 years. During open lung biopsy, The region of the lobe was radiographically and grossly identified and was examined by a biopsy. The biopsy yielded a specific diagnosis in 27 [96.4 % patients and changes in therapy in 24[85.7% patients. Complications developed in three[10.8% patients, directly related to the biopsy procedure in 2. One patient died[3.6% due to underlying disease. We conclude that open lung biopsy can be accomplished safely in the patient with diffuse infiltrative lung disease and it is an important tool in decision-making process and therapy.

폐종괴에 대한 경피적 세침흡인세포검사와 자동총부착 침생검의 비교 (Comparison between Transthoracic Fine Needle Aspiration Cytology and Gun Biopsy of Pulmonary Mass)

  • To compare the diagnostic yields and complication rates of transthoracic fine needle aspiration cytology(FNAC) and gun biopsy in the diagnosis of pulmonary mass, a retrospective review was performed in 125 cases. Under the fluoroscopic guide, FNAC was performed by 20G Chiba needle in 91 cases, core biopsy was done by 18.5 G vaccum needle attached with automated biopsy gun in 74 cases and both procedures were done together in 37 cases. Overall sensitivity was 88.4% in FNAC and 87.5% in gun biopsy. For malignant pulmonary tumors, correct type correlation with final diagnosis was obtained in 33(76.7%) out of 43 cases by FNAC and 30(75.0%) out of 40 cases by gun biopsy. For benign pulmonary lesions, there were correct type correlation in 14(35.0%) out of 40 cases by FNAC and 14(53.8%) out of 26 cases by gun biopsy. The complication was pneumothorax and hemoptysis. Pneumothorax occured in 11.1% of FNAC, 10.9% of gun biopsy and 10.9% of both technique, among which chest tube drainages were necessary in one patient by gun biopsy and in three patients by both technique. Although no significant difference of diagnositc accuracy and complication rate was found between FNAC and gun biopsy, gun biopsy was more helpful in the diagnosis of pulmonary benign lesions than FNAC.

미만성 폐침윤 질환에서 개흉폐생검 (Open Lung Biopsy Procedure for Diffuse Infiltrative Lung Disease -Collective Review of 50 Cases-)

  • Open lung biopsy still has important roles for the marking of diagnosis of diffuse infiltrative lung disease even though transbronchial bronchoscopic lung biopsy and percutaneous needle aspiration biopsy gain popularity nowadays. This is clinical retrospective review of the 56 patients with diffuse infiltrative lung disease undergoing open lung biopsy by minithoracotomy from 1984 to Dec. 1992 in the Department of Thoracic & Cardiovascular Surgery of Catholic University Medical College. 27 men and 29 women, aged 17 to 73 year [mean 49 year , were enrolled & divided into 2 groups;Group A consisted of patients with immunocompromised state [n=19 , Group B patients with non-immunocompromised state[n=38 . Pathologic diagnosis was made in 54 cases[96.4% of these two groups and as follows: infectious; 12 patients[21.4% , Neoplastic; 10 patients[17.9% , granulomatous; 4 patients[7.1% , interstitial pneumonia; 12 patients[21.4% , Pulmonary fibrosis; 8 patients[14.3% , others; 3 patients[5.4% , nonspecific; 5 patients[8.9% , and undetermined; 2 patients[3.6% . Therapeutic plans were changed in 39 patients[69.6% after taking of tissue diagnosis by open lung biopsy. Group B has higher incidence of infectious diseases and change of therapeutic plan than the Group A. The postoperative complications developed in 8 cases[14.3% ,and there is no difference of incidence between the 2 groups. 4 patients belongs to group A, died of respiratory distress syndrome [2 and sepsis [2 which were not related with open lung biopsy procedure. In conclusion, open lung biopsy is a reliable method to obtain a diagnosis in diffuse pulmonary infiltrates and can be performed safely, even in acutely ill, immunosuppressed patients.
