Prevalence of binocular anomalies in adult Koreans (한국인의 양안 시기능 이상에 관한 고찰)

  • To describe the prevalence of binocular anomalies in adult Koreans. Patients included were 19 to 40 years of age, 41 males and 60 females and living in Kwang-Ju Korea. Refractive correction was estimated objectively with an autorefractometer and subjectively refined without cycloplegia. Myopia was defined as a refractive error less than -0.50 diopters hyperopia was defined as a refractive error greater than +0.50 diopters. 101 no strabismic patients who had a refractive error and a near lateral phoria(46%) and an AC/A(accommodative convergence/accommodation) anomaly(50.4%). The prevalence of a near vergence anomaly(52.5%) was higher than a near divergence anomaly(55.5%). The prevalence of a positive relative accommodation(PRA) anomaly(61.4%) was higher than a negative relative accommodation(NRA) anomaly(54.5%). According to Morgan's analysis method, patients with vergence anomaly(21.7%) were seen slightly less frequently than those with accommodative interaction anomaly(29.7%). 34.6% of patients had both vergence anomaly and accommodative interaction anomaly. These results indicate that full prescription for a refractive corrections should be considered as these can improve binocular visual function for ametropia.

  • Binocular vision had a short history in Korea. As there were many near works in these days, the needs about comparative study have been increased. There was related to both refractive error and binocular anomalies, but it is difficult to applying for binocular vision expected findings in itself due to the fact that Korean differ from foreigner. Objects were 100 adults in 18-36 years old ages, The test was Von Gaefe method and used aparatus was phoropter(Shinnippon VT10)and visual chart(Shinnippon CT30). According to interview results was that symptom in near works were headaches 28.0%, blinking 27.3%, red eye 25.1%, eyepain 15.6%, watering 15.3%, itch 12.2%, photophobia 8.5% and eye strain 7.4%. A people who have above ${\pm}0.50$ D refractive error in total objectives (100-male 45/female 55) were classified into ametropia. There was a results such as emmetropia (12.0%), ametropia(88.0%), exophoria(32.0%), esophoria(12.0%). Far negative relative convergence were that in case of high 43.0%, in case of low 7.0%. Far positive relative convergence were that in case of high 15.0%, in case of low 38.0%. Near phoria was exophoria(32.0%), esophoria(12.0%). Near negative relative convergence were that in case of high 23.0%, in case of low 38.0%. Far positive relative convergence were that in case of high 29.0%, in case of low 23.0%. Near negative relative accommodation were that in case of high 10.0%, in case of low 14,0%, Far positive relative convergence were that in case of high 69.0%, in case of low 12.0%. Results were different from expected findings, and especially positive relative accommodation was very high, However, We suggest that the expected findings in Korea for several subjects must study in binocular function.

  • The purpose of this study is to identify the effect of Teacher Participation-oriented Education Program (TPEP) for Elementary School Teachers to Improve Class Expertise in Science Classes with a focus on visual attention. The participants were two elementary school teachers in Seoul and taught science subjects. The lesson topic applied to this study were 'Structure and Function of Our Body' in the second semester of fifth grade and 'Volcano and Earthquake' in the second semester of fourth grade. The mobile eye tracker SMI's ETG 2w, which is a binocular tracking system was used in this study. In this study, the actual practice time, participant's visual attention, visual intake time average, and visual intake time average were analyzed by class phase. The results of the study are as follows. First, as a result of analyzing the actual class execution time, the actual class execution time was almost in line with the lesson plan after the TPEP application. Second, visual attention in the areas related to teaching and learning activities was high after applying TPEP. Factors affecting the progress of the class and cognitive burdens were identified quantitatively and objectively through visual attention. Third, as a result of analyzing the visual intake time average of participants, there was a statistically significant difference in all classes. Fourth, as a result of analyzing the visual intake time average of participants, the results were statistically significant in the introduction(video), activity 1, activity 2, and activity 3 stages in the lecture type class. The Teacher Participation-oriented Education Program (TPEP) for Elementary School Teachers to Improve Class Expertise in Science Classes can extend elementary science class expertise such as self-class analysis, eye tracking, linguistic, gesture, and class design beyond traditional class analysis and consulting.

Change of Binocular Vision Induced by Longitudinal Chromatic Aberration during Near Work (근거리 작업 시 종색수차에 따른 양안시의 변화)

  • Purpose: The current study was performed to compare the difference in binocular visual function depending on variable background colors at near work. Method: Fifty four adults (18 males, 34 females) who consented to the present study and had no ocular disease, ocular surgery history, strabismus and amblyopia with normal binocular vision were participated into this study. The subjects were asked to read the novels with black letter printed on white, red, green and blue background for 15 min. Then, their heterophoria, AC/A ratio, near point of convergence, accommodation facility, relative accommodation and vergence were measured before and after reading. The difference of measurements were compared. Result: Overall heterophoria was tended to decrease with regardless of background color. AC/A ratio showed a tendency of increase after reading the novels with all backgrounds except white background. Near point of convergence was significantly increased compared to before reading at all background color. Accommodative facility of dominant and non-dominant eyes were also significantly increased after reading however, binocular accommodative facility showed a tendency of decrease. Negative relative accommodation also decreased at all background colors however, the change of positive relative accommodation was not significantly different. In case of vergence, there was significant difference in break point of far BO and recovery point of far BI by the wavelength of background color. Conclusions: From the results, it was known there is convergence change depending on the wavelength of light even though same amount of accommodation and convergence is required when doing near work for certain period. Thus, it can be suggested that the adjustment of the near working environment which perception of various color was required, should be conducted according to the main wavelength.

A Case of Prismatic Correction for Cyclovertical Heterophoria (회선수직사위의 프리즘 교정 증례)

  • Purpose: The purpose of this case study is to prescribe a prismatic correction in uncommon case of cyclovertical heterophoria. Methods: The prescribed prism was used to balance the vertical vergence break values by the binocular vision evaluation such as phoria, vergence, accommodative function and the others. Results: Although asymptomatic lateral phoria was changed, the vertical prismatic correction improved the symptoms such as eyestrain, headaches, diplopia and others. And cyclophoria was disappeared by prismatic correction. Conclusions: The prismatic correction for this case was useful for alleviating symptoms of cyclovertical deviations.

The Accommodative Lag and Refractive Error in Early Adults (초년 성인의 굴절이상과 조절래그 분포의 연관성)

  • Purpose: The purpose of this study was to determine the distribution and correlation of accommodative lag with refractive error. Method: We had tested the clinical refraction and the accommodative lag in clinically normal 49 young adults (total 98 eyes) aged 18 to 25 years without abnormal binocular function. Monocular and binocular accommodative lag were tested with 0.50 D cross-cylinder lens and near vision test chart which had cross-hairs after full correction of LogMAR visual acuity over 0.05. Results: There was no statistical differences in monocular accommodative lag between right ($0.64{\pm}0.64$ D) and left eye ($0.63{\pm}0.64$)(p=0.858). The accommodative lag of male was higher than female and the range of the value was broader than female in binocular accommodative lag (p=0.015). The wider the inter-pupillary distance was, the higher the accommodative lag was (p=0.003). However, there were no differences with age (p=0.800) and dominant eye (p=0.402). The ranges of accommodative lag of low, middle, and high myopia were 0.75 ~ -0.25 D, 1.25 ~ -0.50 D, and 1.50 ~ -0.75 D, respectively, and the regression was 'y = -0.03953x+0.09205'. Conclusions: These data suggest that clinically normal young adults with high amounts of refractive error have more variable accommodative lag and increased spherical equivalent refraction.

Screen Disparity and Size Perception Function of Various 3D Stimuli (양안시차에 따른 다양한 3D 자극의 크기지각 예측함수 개발)

  • Although there has been much advance in the development of 3D displays of various purpose, 3D contents are not yet so used as expected in those displays. One well-known obstacle in the enjoyment of 3D contents is visual fatigue, but another major issue is image distortion of 3D contents. In the previous research, Shin, Li, & Kim (2012) reported systematic linear relationship between screen disparity and size perception of a simple object whose retinal size was constant across different disparities. In this research, we intended to generalize the previous finding by using various 3D stimuli in the test of the relationship between screen disparity and size perception of those stimuli. Consistent with previous findings, our data indicated that size perception linearly changes as a function of screen disparity and the linearity was observed in all stimuli types we used in this research. We described the empirical relationship between screen disparity and size perception in the form of prediction function for size perception in which visual angle is the predictor. This function will be very useful in the creation of 3D contents as one can make reasonable predictions on the to-be-perceived size of an object being filmed using screen disparity of their camera setting.

The Functional Change of Accommodation and Convergence in the Mid-Forties by Using Smartphone (스마트폰 사용에 의한 40대 중년층의 조절 및 폭주기능 변화)

  • Purpose: The present study was aimed to investigate the effect of excessive near work by using a smartphone on subjective symptoms and accommodative and convergent function in their 40s. Methods: A total of 40 subjects(male, 10; female, 30; age, $43{\pm}7.2year$) in their 40s who have monocular and binocular visual acuities of 0.8 and 1.0, respectively, were divided into presbyopia group and non-presbyopia group. The subjects were asked to watch a movie on the screen of smartphone for 30 minutes. Their accommodative amplitude and facility, and relative accommodation were measured and compared before and after the use of smartphone. Changes in fusional vergence and near heterophoria by using smartphone were also evaluated. Furthermore, the change of subjective symptoms was surveyed using a questionnaire. Results: The presbyopia in mid-40s reported discomfort in an order of asthenopia, blur and dryness after the use of smartphone. Accommodative function and non-strabismic binocular function were generally decreased. Accommodative functions such as monocular accommodative amplitude, and relative accommodation were significantly decreased after smartphone use, and the change of phoria was observed as a result of decreased convergence and divergence. Negative fusional vergence was also significantly reduced. On the other hand, non-presbyopia in mid-40s reported discomfort in an order of asthenopia, dryness and blur, and only accommodative amplitude among the accommodative functions was significantly reduced. Significant reduction of negative fusional vergence was also observed. Conclusion: From the results, it was confirmed that the subjective discomfort of mid-40s after smarphone use might be related to whether presbyopia or not. It was due to not only the reduction of accommodative function but also the overall deterioration of visual function including heterophoria and fusional vergence. Therefore, it suggests that the accurate determination of the cause based on the overall visual functional tests such as heterophoria, fusional vergence as well as the decrease of accommodation due to the aging may be necessary when the mid-40s feels discomfortable symptoms by near work.

Evaluation of Applicability of Webtoon Content for Visual Function Education and Consultation for the General Public (일반인을 위한 시각기능 교육 및 상담용 웹툰 콘텐츠의 적용 가능성 평가)

  • The author thought about how to explain visual functions in an easy-to-understand way to customers visiting the optical shops and provide easy customer consultation, and attempted to evaluate the usability and value of the educational webtoon for patient consultation by applying it to the optical shops's visitors. Survey study was conducted targeting 152 customers who visited optical shops A in Incheon from January to December 2023. In this study, consultation was conducted using educational webtoons to help the general public understand concepts that are difficult to understand, such as binocular vision function, the concept of heterophoria, prism lenses, and binocular balance. The areas of 'understanding', 'interest', 'usability', 'necessity', and 'expression of expertise' felt by the visitors who received counseling were scored and evaluated, and differences according to counseling method were evaluated using an independent sample t-test. Statistical verification was performed. When the optometrist only explained verbally, the score distribution was relatively low: 1.96 points for understanding, 2.06 points for interest, 2.96 points for usability, 4.20 points for necessity, and 3.19 points for expertise. When the optometrist provided explanations along with the book, understanding was 4.14 points, interest was 3.28 points, usability was 3.60 points, necessity was 4.17 points, and expertise was 3.55 points. When the optometrist explained with a webtoon, understanding was 4.57 points, interest was 4.96 points, usability was 4.18 points, necessity was 4.65 points, and expertise was 3.15 points. When explanations were given only verbally, all items showed the lowest scores, whereas consultations using webtoons showed 4 points or more (excellent) in all items except the 'expression of expertise' item. It is evaluated that actively using webtoons in various consultation situations that occur in optical shops will help increase customer interest and concentration.

The Comparative Research of Dominant Eye and Non-dominant Eye by Ages (연령대별 우세안과 비우세안의 비교 연구)

  • Purpose: When we look at the object, we used the dominant eye mainly. For this reason, a prescription of the dominant eye is an important factor for glasses and contact lenses. This study evaluated visual acuity differences between dominant and nondominant eyes through analyzing refractive power changes in both eyes by the ages. Methods: This study was performed to investigate the relationship between refractive error and dominant eye which had the superiority in the function of binocular. 186 subjects without ocular disease were examined on the dominant eye. The dominant eye was examined by the Hole-in-the-card test. For the consistency of the measurements, we tested refractive power in three times by the same person. Results: Using SPSS, the relationship between vision and the dominant eye was analyzed. 135 people of the whole subjects have the dominant eye on right. The Number of the non-dominant eye is 51. We were divided into 3 types, the group under the age of 10 that begins to expose environment factor affect on vision (the average age $8.8{\pm}1.18$) and the age group of 10 to 20 that begins to change refractive power in earnest (the average age $14.1{\pm}2.58$) and the group after the age 20 that began to stabilize vision (the average age $51.8{\pm}17.51$). The visual acuity of dominant eye was higher than non-dominant eye in all age groups. Nevertheless, these results were not statistically significant. Mean astigmatism of dominant eye was smaller than the non-dominant eye, and this is significant, statistically (p=0.017<0.05). Conclusions: It is expected that the balanced eye with a lower level of astigmatism has a more possibility become a dominant eye.
