구개열의 유형에 따른 발음 비교 (Speech Evaluation Based on the Type of Cleft Palate)

  • Purpose: Authors evaluated results of palatoplasty by speech analysis in bilateral, unilateral complete, and unilateral incomplete and submucous cleft palate patients. Methods: The speech outcomes were studied in 15 bilateral, 28 unilateral complete, and 46 unilateral incomplete and submucous cleft palate patients who underwent push-back palatoplasties from January 1998 to July 2004. The patients were divided into 2 groups as 3 to 6, 7 to 10-year-old and compared with 20 normal children(control groups were divided into 10 children on each side). Nasal emission test, hypernasality test, and articulation test were done by speech evaluation table which was composed of 39 different words. Results: In all speech evaluation tests, the group of bilateral cleft palate patients got the worst score. And 7 to 10-year-old groups got better score when compared to the same type cleft palate. Conclusion: Bilateral cleft palate patients have many more speech problems than other patients. In cleft palate patients, the speech problem was improved with ages, postoperatively. And the speech therapy can improve the operative outcomes.

양측 후이개 종물로 발병한 Kimura씨 병 1예 (A Case of Bilateral Retro-Auricular Kimura's Disease)

  • Kimura's disease is a chronic inflammatory disease with unknown etiology. The most common clinical presentation of Kimura's disease is non-tender unilateral mass in the head and neck area, but bilateral involvement has rarely been reported. Histopathological confirmation should be taken for the diagnosis. Various management options are available, including surgical excision, steroid therapy, and radiotherapy. We report a male patient diagnosed as Kimura's disease involving bilateral retro-auricular region. Simultaneous bilateral neck involvement is an extremely rare finding. For cosmetic reasons, the patient was treated surgically, with satisfactory postoperative results. When evaluating patients with mass lesion in bilateral parotid/retro-auricular area, Kimura's disease should be considered as differential diagnosis.

  • Bronchiectasis is bilateral in approximately 30% of patients. Although the presence of bilateral bronchiectasis was frequently considered a contraindication to surgical resection due to excessive loss of functional pulmonary parenchyma, it is a correct view that the involved broncho-pulmonary segments are functionless and risks to the as yet uninvolved segments and should be removed if the patient`s pulmonary function is tolerable. We report a case of multisegmental bilateral bronchiectasis treated by bilateral simultaneous pulmonary resection through a median sternotomy. Five bronchiectatic segments were resected, which were right middle lobe, anterobasal segment of the right lower lobe, and lingula of the left upper lobe.

양측성 기관지 환장증의 수술적 치료에 관한 검토 (Surgical Treatment of Bilateral Bronchiectasis)

  • We reviewed medical records of 179 patients with bronchiectasis admitted in our department in our department in Pusan Paik Hospital, Inje University from Dec. 1979 to Jun. 1989. Surgical resections were performed in 167 patients [bilateral disease 29, unilateral 138]. To evaluate the results of surgical treatment of bilateral bronchiectasis, it was compared with that of unilateral one. The results were as follows: 1.Bilateral disease was 20.1% of all patients with bronchiectasis. The average age of patients with bilateral disease treated surgically was 28.8 2. There was no significant difference in age, clinical symptoms and disease distribution between bilateral and unilateral bronchiectasis. 3. The numbers of diseased segments in bronchography were 6.86$\pm$2.00 and 4.33$\pm$2.01 in bilateral and unilateral diseases respectively. 4. Six patients underwent bilateral resections, and 25 patients had some of diseased segment [average 2.24$\pm$1.51] after resections, 5. After resections, 48.3% of patients with bilateral disease were free of symptoms and it was less than that in the unilateral one [87%]. But the remaining 51.7% of bilateral disease presented much improved clinical symptoms without aggravation.

횡복직근 유리피판술에 의한 양측 유방 재건 (Bilateral Breast Reconstruction with Free TRAM Flaps)

  • Free TRAM flap is now increasingly suggested to patients requiring breast reconstruction after the mastectomy. This study is to introduce the experiences of bilateral free TRAM flaps for reconstruction of bilateral breasts and to suggest the way of getting the more satisfactory results. A total of 6 breasts were reconstructed in 3 patients using bilateral free TRAM flaps immediately following the mastectomy. Average operative time for bilateral breast reconstruction was 8 hours comparing to 6.5 hours for unilateral breast reconstruction. Partial or total flap loss did not occur in 6 flaps. Abdomen was repaired directly with muscle and fascia sparing technique without necessity of mesh graft. There was no complication in donor site like abdominal hernia. Bilateral breast reconstruction can achieve exceptionally good aesthetic result with low complication if it is performed with skillful technique and experience. The reason for this is that fairly good symmetry usually is obtained in the initial surgery and in most cases only minimal additional surgery is required to achieve a satisfactory aesthetic result. The one disadvantage of bilateral reconstruction with autologous tissue is the length of the surgical procedure. Although the initial bilateral breast reconstruction can be a long, tedious procedure if free flaps are used, it must be a valuable treatment option for bilaterally mastectomized patients.

Bilateral distal femoral epiphyseal detachment in a young adult: a case report

  • Epiphyseal dislocations and fractures frequently occur in children and adolescents. However, these injuries seldom involve the distal femur, and bilateral cases are even rarer. We present an unusual case of bilateral epiphyseal detachment of the femur in a 21-year-old man who was involved in a traffic accident. Open reduction and fixation surgery were performed 5 days after the incident. The patient experienced favorable progress, with consolidation occurring at 3 months. No lower limb length discrepancies were observed at the 20-month follow-up. In summary, bilateral distal femoral epiphyseal detachment is an uncommon condition that can be effectively treated through surgical intervention.

A Rare Case of Bilateral Pulmonary Sequestration Managed with Embolization and Surgical Resection in a Patient

  • In this article, we report a rare case of a 22-year-old male with bilateral pulmonary sequestration, treated with embolization and surgical resection. The initial plan involved staged bilateral lobectomy for both lungs and prophylactic embolization of feeding vessels for preventing unexpected hemorrhage during operation. Symptomatic right lower lobe was resected with video-assisted thoracic surgery after embolization, and the patient refused surgery of left lower lobe upon symptomatic relief. The two-year follow-up examination revealed that the patient was healthy and had no relevant discomfort.

  • LAS (Laparoscopy Assisted Surgery) has been substituted alternatively for traditional open surgery. However, when using a commercialized robot assisted laparoscopic such as Da Vinci, surgeons have encountered some problems due to having to depend only on information by visual feedback. To solve this problem, a haptic function is required. In order to realize the haptic teleoperation system, a force feedback and bilateral control system are needed. Previous research showed that the perturbation value estimated by a SPO (Sliding Perturbation Observer) followed a reaction force that loaded on the surgical robot instrument. Thus, in this paper, the force feedback problem of surgical robots is solved through the reaction force estimation method. This paper then introduces the possibility of the haptic function realization of a laparoscopic surgery robot using a bilateral control system. For bilateral control, the master uses an impedance control and the slave uses a SMC (Sliding Mode Control). The experiment results show that a torque and force sensorless teleoperation system can be implemented using a bilateral control structure.
