Three-dimensional evaluation of alveolar changes induced by nasoalveolar molding in infants with unilateral cleft lip and palate: A case-control study

  • Objective: The objectives of this study were to evaluate linear and volumetric alveolar changes induced by nasoalveolar molding (NAM) in infants with complete unilateral cleft lip and palate (UCLP) and compare the maxillary dimensions after NAM with the normal dimensions in infants without clefts. Methods: A total of 26 infants with UCLP treated by NAM (mean age before and after NAM: $14.20{\pm}8.09days$ and $118.16{\pm}10.06days$, respectively) comprised the treatment group, while 26 infants without clefts (mean age: $115.81{\pm}8.71days$) comprised the control group. Changes in the maxillary dimensions following NAM were measured on three-dimensional models using Mimics software, version 17.0. Results: During NAM, there was a decrease in the cleft widths, maxillary arch depths, and rotation of the greater segment. While the anterior alveolar arch width exhibited a significant decrease, the posterior arch width was mostly maintained. There were no changes in the anterior vertical deviations of the alveolar segments. The alveolar crest lengths, arch circumference, and bilateral posterior volumetric measures exhibited an increase. After NAM, the anterior arch width was comparable between the treatment and control groups, whereas the posterior arch width and anterior vertical deviations were greater in the treatment group than in the control group. The maxillary arch depths, alveolar crest lengths, and maxillary volumes were smaller in the NAM group than in the control group. Conclusions: During NAM in infants with UCLP, the cleft width and anteroposterior and transverse alveolar dimensions exhibited a decrease while the vertical dimensions were maintained. Compared with infants without clefts, those with UCLP treated by NAM exhibited sagittal and vertical alveolar growth deficiencies and tissue insufficiency.

Report of 45 Cases of Cleft Lips and Cleft Palates in Kang Won Do People (강원도민 토순환자 45명의 무료진료보고)

  • The authors had treated 45 cases of cleft lip and cleft palate patients including 30 male patients and 15 female patients. There were 41 cases of unilateral cleft lip patients and 4 cases of bilateral cleft lip patients, and 4 cases with together cleft lips and cleft palates among the total 45 cases were showed. We had accomplished that surgical plastic closures of cleft lips and cleft palates had been performed by using of Millara's, Hagedorn's, Meyer's, LeMesurier's and Veau's methods with Z-plasty.

Effect of Preoperative Nasal Retainer on Nasal Growth in Patients with Bilateral Incomplete Cleft Lip: A 3-Year Follow-Up Study

  • Background The purpose of this study was to evaluate changes in nasal growth after the implementation of a preoperative nasal retainer in patients with bilateral incomplete cleft lip. Methods Twenty-six infants with bilateral incomplete cleft lip and cleft palate were included in the study. A preoperative nasal retainer was applied in 5 patients from the time of birth to 2.6-3.5 months before primary cheiloplasty. Twenty-one patients who were treated without a preoperative nasal retainer were placed in the control group. Standard frontal, basal, and lateral view photographs were taken 3 weeks before cheiloplasty, immediately after cheiloplasty, and at the 1- and 3-year postoperative follow-up visits. The columella and nasal growth ratio and nasolabial angle were indirectly measured using photographic anthropometry. Results The ratio of columella length to nasal tip protrusion significantly increased after the implementation of a preoperative nasal retainer compared to the control group for up to 3 years postoperatively (P<0.01 for all time points). The ratios of nasal width to facial width, nasal width to intercanthal distance, columellar width to nasal width, and the nasolabial angle, for the two groups were not significantly different at any time point. Conclusions Implementation of a preoperative nasal retainer provided significant advantages for achieving columellar elongation for up to 3 years postoperatively. It is a simple, reasonable option for correcting nostril shape, preventing deformities, and guiding development of facial structures.

Distraction osteogenesis in patients with complete cleft lip and palate (완전 구순구개열을 가진 환자에서의 골신장술)

  • Patients with cleft lip and palate usually show up maxillary hypoplasia. In these cases, a large amount of maxillary advancement is often needed to correct the severe deformity, but local soft-tissue scars around the maxilla restrict maxillary advancement and increase the relapse rate. Maxillary distraction osteogenesis is an effective method for facial and occlusal improvement in these patients. By gradually lengthening both the bones and the soft tissues, distracted midface can greatly increase postoperative stability and decrease the relapse rate. However, the maxillary extraoral appliances of the early days used were esthetically unappealing as well as difficult for the patient to manage. Recently, more inconspicuous intraoral distraction appliances have been developed and used with success. We acquired favorable result in two patients(bilateral 1 patient and unilateral 1 patient) with severe maxillary hypoplasia secondary to complete cleft lip and palate were treated with midface distraction using internal distractor (Zurich Pediatric Maxillary Distractor, KLS Martin, Tuttlingen, Germany). So, we report our experience with literatures.

  • 대한악안면성형재건외과학회는 2004년도부터 매년 인도적 차원에서 의료 환경이 낙후된 Vietnam의 cleft lip and palate 환자들을 치료하기 위해 무료로 해외 진료 봉사 활동을 해왔다. 초창기 해외 진료 봉사 활동 당시 베트남 의료 기술 발달이 미비하여 준비해야 할 것들이 많았으나 이 후 베트남의 경제 발전과 함께 의료 시설 및 의료 기술이 발달함에 따라 준비 품목 등이 간단명료해지고 현지 지원 및 수술 준비도 원활히 진행되었다. 이러한 무료 의료 봉사 활동을 계기로 개발도상국의 낙후된 의료 시설 및 의료 기술 발전, 의료 혜택을 받지 못한 구순 구개열 환자의 정상적 안모 및 기능 회복, 조건 없는 의료 봉사 활동을 통한 민간외교 효과 및 양국간의 우호 증진 등의 성과가 있었다. 대한악안면성형재건외과학회에서는 2009년도 역시 베트남 National Institute of Odonto-stomatology 에서 11월17일부터 26일까지 cleft lip and palate 무료수술진료단(단장 신효근)을 보내 수술봉사 활동을 펼치고 돌아 왔다. 일반적인 구순구개열 뿐만 아니라 양측성 안면열(Bilateral Facial Cleft), 사경(Torticollis) 등의 희귀 기형을 포함하여 수술을 진행하였으며 총 31례의 수술을 성공적으로 마쳤으며 지난 6년 동안의 성과와 함께 이에 대한 보고를 하고자 한다.

Unusual Sj${\ddot{o}}$gren's Syndrome with Bilateral Parotid Cysts

  • Sj${\ddot{o}}$gren's syndrome is a chronic autoimmune exocrinopathy that destroys salivary and lacrimal gland tissue. We report an unusual case of this disease in a 54-year-old woman who presented with multiple and bilateral parotid cystic masses. The multiple, small, bead-like cysts were clearly evident in the computed tomography sections in this patient, a visible reminder that this may be the initial presentation in a patient with Sj${\ddot{o}}$gren's syndrome. As the case illustrates, Sj${\ddot{o}}$gren's syndrome should be included in the differential diagnosis of multiple and bilateral cystic parotid lesions.

A Case of Bilateral Warthin's Tumor (이하선에 발생한 양측성 Warthin씨 종양 치험례)

  • Warthin's tumor is a benign neoplasm that most frequently occurs in the parotid gland. Warthin's tumor shows a tendency of bilateral occurrence, but most of bilateral tumors occur at different time. It usually affects older man and grows slowly. Surgical excision is appropriate treatment for patients with Warthin's tumor and rarely recurs after removal. We report a rare case of bilateral Warthin's tumor which detected at the same time. A 54-year-old man had painless, slowly growing masses on the both mandibular angles. The masses were detected ten years ago. CT scan, ultrasonographic evaluation and fine needle biopsy was performed for preoperative diagnostic study. Superficial parotidectomy was done one after the other at ten-day intervals. After excision, histological examination revealed a mixture of ductal epithelium and a lymphoid stroma. The overlying surface of lymphoid tissue was covered with epithelial cells that was abundant of eosinophilic cytoplasm. Both masses were diagnosed as Warthin's tumor. Postoperatively, the patient did well and had no evidence of recurrence of tumor for 2 years follow-up period. Authors experienced a rare case of bilateral Warthin's tumor which detected at the same time. We recommend stepwise superficial parotidectomy at one or two-week intervals as treatment of bilateral Warthin's tumor.

CORRECTION OF MICROSTOMIA BY BILATERAL COMMISSUROPLASTY USING "OVER AND OUT" BUCCAL MUCOSA FLAPS: REPORT OF A CASE (협점막 외전 피판을 이용한 양측성 구각성형술에 의한 소구증의 교정 1예)

  • Microstomia can be occurred as a result of direct injury to tissues such as chemical, thermal and electrical burns, and animal bites. It also may be secondary to contracture of burned perioral skin, or may result from scarring after reconstructive lip surgery. Narrowing of the oral aperture is not only disfiguring, but also limiting the oral access needed for introduction of food, insertion of dentures, oral hygiene, and dental treatment. Limited mouth opening may also interfere with mastication and speech. Few reports exist regarding correction of microstomia and reconstruction of the corners of the mouth. A 16-year-old girl with a bilateral cleft lip and palate presented with the limited mouth opening (approximately 20 mm), the esthetic problem due to the small lip, and the cleft lip-nasal deformity. The microstomia was corrected by bilateral commissuroplasty using "over and out" buccal mucosa flaps proposed by Converse. The intercommissure distance was increased from the preoperative 40 mm to the postoperative 60 mm. The one-year postoperative intercommissure distance was 54 mm, because the 6 mm relapse was occurred. The bilateral commissuroplasty using "over and out" buccal mucosa flap could increase the width and general size of the oral aperture and improve the lip appearance.
