Evaluation of Nitritation of High Strength Ammonia with Variation of SRT and Temperature using Piggery Wastewater (SRT와 온도 변화를 통한 돈사폐수 내 고농도 암모니아의 아질산화 평가)

  • The purpose of this study is to investigate the high strength ammonia oxidation of piggery wastewater. Laboratory scale reactors was operated using influent of piggery wastewater and effluent of anaerobic digester from piggery wastewater at $35^{\circ}C$ and $20^{\circ}C$. Results of various operating conditions were compared and analyzed. After analyzing the results, effluent of anaerobic digester from piggery wastewater required shorter Solid retention time (SRT) than influent of piggery wastewater. In terms of the temperature, stable ammonia removal and denitrification was achieved on the both of the condition. At the temperature of $35^{\circ}C$, nitrite conversion rate was better than $20^{\circ}C$. It can be concluded that treating the piggery wastewater using anaerobic digester on the condition of the temperature at $35^{\circ}C$ is more efficient on the nitritation of the piggery wastewater.

  • From an information security perspective, protecting sensitive data requires utilizing algorithms which resist theoretical attacks. However, treating an algorithm in a purely mathematical fashion or in other words abstracting away from its physical (hardware or software) implementation opens the door to various real-world security threats. In the modern age of electronics, cryptanalysis attempts to reveal secret information based on cryptosystem physical properties, rather than exploiting the theoretical weaknesses in the implemented cryptographic algorithm. The correlation power attack (CPA) is a Side-Channel Analysis attack used to reveal sensitive information based on the power leakages of a device. In this paper, we present a power Hacking technique to demonstrate how a power analysis can be exploited to reveal the secret information in AES crypto-core. In the proposed case study, we explain the main techniques that can break the security of the considered crypto-core by using CPA attack. Using two cryptographic devices, FPGA and 8051 microcontrollers, the experimental attack procedure shows that the AES hardware implementation has better resistance against power attack compared to the software one. On the other hand, we remark that the efficiency of CPA attack depends statistically on the implementation and the power model used for the power prediction.

Stem cell-derived extracellular vesicle therapy for acute brain insults and neurodegenerative diseases

  • Stem cell-based therapy is a promising approach for treating a variety of disorders, including acute brain insults and neurodegenerative diseases. Stem cells such as mesenchymal stem cells (MSCs) secrete extracellular vesicles (EVs), circular membrane fragments (30 nm-1 ㎛) that are shed from the cell surface, carrying several therapeutic molecules such as proteins and microRNAs. Because EV-based therapy is superior to cell therapy in terms of scalable production, biodistribution, and safety profiles, it can be used to treat brain diseases as an alternative to stem cell therapy. This review presents evidences evaluating the role of stem cell-derived EVs in stroke, traumatic brain injury, and degenerative brain diseases, such as Alzheimer's disease and Parkinson' disease. In addition, stem cell-derived EVs have better profiles in biocompatibility, immunogenicity, and safety than those of small chemical and macromolecules. The advantages and disadvantages of EVs compared with other strategies are discussed. Even though EVs obtained from native stem cells have potential in the treatment of brain diseases, the successful clinical application is limited by the short half-life, limited targeting, rapid clearance after application, and insufficient payload. We discuss the strategies to enhance the efficacy of EV therapeutics. Finally, EV therapies have yet to be approved by the regulatory authorities. Major issues are discussed together with relevant advances in the clinical application of EV therapeutics.

Ionized Cal/MagTM Complex as an Alternative Supplement for Enhancing Bone Mineral Density - Preliminary Results from Primary Care Centers - (골밀도 향상을 위한 대체 요법으로 칼슘/마그네슘 복합체의 효능)

  • The medicines for treating osteoporosis currently in use have minor to severe side effects, and can be financially burdensome. Thus, there is a need for prevention and alternative supplement that is relatively inexpensive, and can be easily consumed daily as an alternative dietary therapy. In this study, bone marrow density of the spine and femur of osteoporosis patients were checked before and after consuming complex composed of calcium and magnesium, considered to be the core of bone mineral content. November 2017-November 2021, patients with T-score of less than -2.5 or -1.0 < T-score < -2.5 with history of fractures or recent fractures were enrolled. The data of 60 patients who orally administered Ionized Cal/MagTM Complex were reviewed retrospectively, and it was significantly confirmed that the average value of T-score was up-regulated by 0.5. Additionally, the cumulative dose was observed to have a positive effect, on the improvement of BMD in the 2nd Lumbar and Femur neck. It is expected that better results will be achieved if use of the supplement is continued.

Intravaginal Administration of Oriental Medicine on Vaginal Atrophy in Menopause : A Systemic Review (폐경기 질 위축에 대한 한약제제 질 내 투여의 효과 : 체계적 문헌 고찰)

  • Objectives: The purpose of this study is to review clinical studies and evaluate effectiveness of intravaginal administration of oriental medicine on vaginal atrophy in menopausal women. Methods: 'postmenopause', 'menopause', 'vaginal atrophy', 'atrophic vaginitis', 'vaginal cream', 'vaginal gel', 'pessary', 'suppository' were searched on 3 online databases (Pubmed, Cochrane Library, OASIS). The risk of bias was assessed by using Cochrane risk of bias (RoB) 2.0 tool. Results: 9 randominized controlled trials (RCTs) were selected among 489 articles. The types of intervention were intravaginal administration of oriental medicine. 6 studies showed that treatment group was better than placebo control group or both treatment group and western medicine control group improved similarly. 2 studies were not significant effective in some results and 1 study was not statistically significant. Conclusions: Intravaginal administration of oriental medicine can be an effective option in treating menopausal women with vaginal atrophy. but considering the small number and low quality of studies, further well-designed studies are needed to confirm the effectiveness of this treatment.

A Study on the Medicinal Application in the Zangfuxushibiaobenyongyaoshi - Based on the Zangfuyaoshibuzheng of Zhang Shanlei - (『장부허실표본용약식(臟腑虛實標本用藥式)』에 나타난 약물(藥物) 운용(運用)에 관한 고찰(考察) - 장산뢰(張山雷)의 『장부약식보정(臟腑藥式補正)』을 중심으로 -)

  • Objectives : Based on the Zangfuyaoshibuzheng of Zhang Shanlei, this paper examines the tip/root division of the Zangfu, features of medicinal application according to deficiency/excess or cold/heat, and erroneous cases in the Zangfuxushibiaobenyongyaoshi of Zhang Jiegu. Methods : Categorization according to tip/root, cold/heat, deficiency/excess and respective medicinal applications in the Zangfuxushibiaobenyongyaoshi were organized, followed by thorough analysis based on the annotations in the Zangfuyaoshibuzheng. Results : First, in the treatment principle of the Zangfu diseases, each of their physiological function and pathological mechanism were analyzed so that the disease patterns are understood accurately and treated accordingly. Second, in categorizing treatment methods, terminology application was modified in overlapping or ambiguous cases so that they could be better distinguished. Third, medicinal categories that were designated to treat each Zangfu disease were explained well based on unique features. In cases where medicinals were falsely included, they were identified and corrected based on sound reasoning. Conclusions : The pattern diagnosis in the Zangfuyaoshibuzheng is clear, and medicinal application analysis is intelligible. As it thoroughly corrects and revises errors in Zhang Jiegu's theories, it could provide valuable assistance in selecting each medicinals when treating disease patterns of the Five Zang.

A Study on the Gagamjeong-gisan in the Onbyeongjobyeon - In Comparison with the Gwakhyangjeong-gisan - (『온병조변(溫病條辨)』 에 나타난 가감정기산(加減正氣散)에 대한 고찰 - 곽향정기산(藿香正氣散)과의 비교를 중심으로 -)

  • Objectives : The purpose and application method of the five kinds of Gagamjeong-gisan in the Onbyeongjobyeon were analyzed and their significance was examined. Methods : First, contents within medical texts on the Gwakhyangjeong-gisan, the original formula for the Gagamjeong-gisan were analyzed. Next, application, symptoms and pathogenic mechanism, treatment principle, composition of the Gagamjeong-gisan as written in the Onbyeongjobyeon were analyzed. Characteristics of each formula as deduced from this process were compared and examined. Results : The Gwakhyangjeong-gisan covers both external contraction and internal damage, applied regardless of external or internal symptoms. It restores the Yang qi of the Spleen and Stomach, balancing the Zheng qi to respond to the external qi. The Gagamjeong-gisan takes after the Gwakhyangjeong-gisan, reflecting such properties. However, the latter differs in that it excludes ingredients unfit for treating dampness-warmth, and uses different application methods according to the level of discharge of dampness pathogen. Conclusions : The Gagamjeong-gisan is modified from the Gwakhyangjeong-gisan, which is applied to cold damage and internal damage to better treat warm disease. It can be said to have acted as a bridge for warm disease scholars in composing formulas that deal with dampness-heat disease.

Palliative and Neoadjuvant Chemotherapy for Advanced Gastric Cancer Patients (진행성 위암에서의 항암요법에 대하여)

  • Gastric cancer is the second most common cancer and the third leading cause of cancer-related deaths in Korea. Many cases of gastric cancer are detected in the early stages on standard medical examinations; complete surgical and endoscopic resection is the most recommended treatment for early-stage gastric cancer. Nevertheless, many patients have already progressed to advanced gastric cancer (AGC) upon diagnosis, and the prognosis of such patients is very poor. Combination chemotherapy has been shown to produce a better quality of life (QOL) and to increase overall survival in AGC patients. However, approximately 50% of patients do not respond to the current first-line chemotherapy, while most patients who do respond eventually show disease progression. Accordingly, various second-line regimens have been investigated, and active salvage chemotherapy has been shown to improve the QOL and clinical outcomes in select AGS patients who can tolerate it. There is also an increasing need for neoadjuvant therapy for treating gastric cancer; therefore, various clinical trials have been set up to investigate different regimens. Neoadjuvant therapy is currently established as the standard treatment for locally AGC in Europe; it has contributed to lowering the nodal stages and has reduced overall mortality rates. Despite these benefits, many uncertainties remain. Therefore, further prospective, high quality randomized controlled trials for neoadjuvant therapies are needed to clarify their clinical benefits and to establish the most effective treatment strategies for AGC.

Indoor air quality analysis based on genetic algorithm for childhood facilities (유전 알고리즘을 이용한 어린이 시설의 실내 공기질 분석)

  • Children are vulnerable to bad indoor air quality, and many researches on indoor air quality have been done with various methodologies. Herein, we used the genetic algorithm, one of the optimization methods, for the analysis based on better estimation values that are not easy to measure. A children playroom and a Taekwondo gym were chosen for the different degree of physical activity. After estimation of the number of occupants, the generation degree of CO2 and PM2.5 were determined from the data of the indoor air quality monitors. Relative errors were below 1% for all cases. Due to many air-treating electronics, the PM2.5 in the children playroom was well-managed compared to that in the Taekwondo gym. The PM2.5-generating activities were calculated and that of the Taekwondo gym was higher than that of the children playroom. The PM2.5 generating values were on the positive relationship with CO2 generating values. This means that we can obtain meaningful information from limited measurement data. For the numerous children facilities, indoor air quality can be easily analyzed and this might contribute to enhancing the children health.

The "Swing-Door" Regrafting of Donor Site: An Alternative Method for Split-Thickness Skin Graft in the Hand

  • Background Skin defects in the hands are common injuries, and autologous skin grafting is the ideal treatment. However, complications can occur at the donor and recipient sites. This study compares the "Swing-door" technique with conventional skin grafting. Methods From August 2019 to February 2023, 19 patients with skin defects of hand underwent the "Swing-door" split-thickness skin graft (STSG) technique. The thin epithelial layer was elevated with proximal part attached. Skin graft was harvested beneath. Donor site was then closed with epithelial flap like a "Swing-door". The outcomes were evaluated in terms of healing time, scar formation, and pain at the donor and recipient sites. The data were compared with the conventional STSG. Results The "Swing-door" group had lower graft take percentages, but complications did not significantly differ between the two groups. The "Swing-door" technique resulted in better cosmetic outcomes, as evidenced by lower Vancouver Scar Scale scores, faster donor site epithelialization, and reduced pain and discomfort during the early postoperative period, as measured by Visual Analog Scale. Conclusion The "Swing-door" STSG is a useful alternative for treating hand skin defects.
