타액선 종양의 병리조직학적 분류 (Histopathologic Classification of Salivary Gland Neoplasm)

  • Background and Objectives : Salivary gland neoplasms are unique because of their infrequency, inconsistent classification, and highly variable biologic behavior. The aim of this study is to analysis the histopathologic classification of salivary glnad neoplasm and to suggest a guideline of management. Materials and Methods : The medical records of 310 patients with salivary gland neoplasm who treated at Asan medical center between 1992 and 2001 were analyzed retrospectively. Among the 310 patients, 138 patients were male and 172 patients were female. Mean age was 50.5 years. Results : Benign salivary neoplasms were 213 cases. They consisted of 153 cases (71.8%) of parotid tumor, 41 cases (19.2%) of submandibular gland tumor and 19 cases (8.9%) of minor salivary gland tumor. Pleomorphic adenoma was the most common benign neoplasm. Malignant salivary neoplasms were 97 cases. They consisted of 45 cases (46.4%) of parotid tumor, 26 cases(26.8%) of minor salivary gland tumor, 24 cases(24.7%) of submandibular gland tumor and 2 cases(2.1%) of sublingual gland tumor. Adenoid cystic carcinoma was the most common malignant neoplasm. Conclusions : The most commonly involved gland was parotid (64%) and the most frequent tumor was pleomorphic adenoma (52%). Although the majority of minor salivary gland neoplasms are malignant, three of parotid tumors are benign.

연조직고형종양의 악성과 양성 감별: 임상과 자기공명영상 복합소견 (Differentiation of Malignant from Benign Soft-Tissue Solid Tumors: Clinical and MR Finding Complex)

  • 목적: 최근 자기공명영상의 개발로 다양한 연조직종양의 영상의학적 진단이 가능해 졌다. 그러나 연조직종양의 다양한 조직구성이나 시간에 따라 구성분의 변화는 자기공명영상 만으로 악성과 양성 종양조차 감별을 어렵게 한다. 이에 본 저자들은 임상과 자기공명영상의 복합적인 소견으로 악성과 양성 연조직종양을 감별해 보고자 하였다. 대상 및 방법: 병리조직학적으로 확진된 82례(중간성종양을 악성종양으로 포함하여 37례와 염증성 종괴를 양성종양으로 포함하여 45례)를 후향적으로, 임상적 소견으로 연령, 크기, 위치, 그리고 자기공명영상 소견으로 종양경계, T2신호강도양상, 조영제T1신호강도양상, 그리고 조영증강 범위를 연관시켜 분석하였다. 자기공명영상으로 진단이 어렵지 않는 많은 전형적인 지방종과 결절종 그리고 농양같은 낭종은 양성종양 분류에서 제외하였다. 결과: 악성연조직종양은 양성에 비하여 연령으로 21~40세와 61~80세, 크기로 3.0 cm 이상, 발생위치로 몸체-골반-하지, 그리고 자기공명영상에서 불규칙한 경계, 50%이상의 조영증강범위 소견들의 빈도가 높았다. 결론: 발생위치로 몸체-골반-하지 와 상지-어깨-척추 로 나눈 임상소견이 악성과 양성 연조직종양을 감별하는데 통계학적으로 유의성있는 차이를 보였으며 나머지 다른 소견들은 특이적이지는 않았지만 부가적으로 악성과 양성을 감별하는데 도움이 되는 소견이었다.

기도 내 양성 종양의 굴곡형 내시경하 절제술 (Endoscopic Removal of Benign Endotracheal/Endobronchial Tumor)

  • 내시경을 이용한 기도 종양 절제술은 비침습적인 방법으로 개흉수술에 따른 각종 부작용을 감소할 수 있을 뿐만 아니라 폐실질을 보존할 수 있으며, 고령 등의 고위험군의 환자에서 안전하게 이용되고 있다. 기관내 종양(양성 지방종)과 기관지 내 종양(과오종)의 2예 환자에서 국소마취상태에서 굴곡형 기관지경직시하에 폴립제거용 기구와 전기소작술을 이용하여 완전절제를 시행하였다. 기관 내 지방 종환자는 술 후 6년째, 기관지내 과오종환자는 2년 6개월 째 재발없이 추적 중이다. 저자들의 방법이 환자에게 안전하며 좋은 시야에서 시술할 수 있는 장점이 있는 것으로 생각되었다.

  • Background and Objectives: Salivary gland tumors are relatively rare and constitute 3% to 4% of all head and neck neoplasms. Their relative infrequency, inconsistent classification, and highly variable biologic behavior make some difficulty, but some general features can be drawn regarding the incidence, pathology, and pattern of behavior of the various benign and malignant tumors of salivary glands. The present study aims to provide a clinical characteristics of salivary gland tumors. Materials and Methods: We analyzed retrospectively clinical features of the 138 patients who were treated surgically at Chonbuk National University Hospital from 1992 through 2002. Results: We found 107 benign and 31 malignant tumors. Among the patients, 58 were males and 80 were females. The most common age group was the fifties. The most common site of both benign and malignant was parotid gland in major salivary glands and palatal region in minor salivary glands. The most common presenting symptom was palpable mass in both benign and malignant tumor. Histopathologically, the most common type was pleomorphic adenoma in benign tumor and mucoepidermoid carcinoma in malignant tumor. All cases were treated surgically and the most common postoperative complications was transient facial nerve weakness. Conclusion: 138 cases of salivary gland tumors were presented with respect to their clinical features.

주타액선 종양의 임상적 고찰 (Clinical Investigations of Major Salivary Gland Tumors)

  • 저자들은 약 4년에 걸친 주타액선 종양에 대한 치료 경험을 통하여, 성공적인 치료를 위한 필요조건으로 다음과 같은 내용들을 고려하여야 할 것으로 생각하였다. 즉 타액선 종양은 다양한 조직학적 특징과 임상 경과를 보이는 질환군이므로 수술 전 세침흡인생검 등에 의한 정확한 병리조직학적 진단의 뒷받침이 필수적이라는 점이다. 또한 악성 종양의 경우, 질병분류 및 병기설정 방식이 완전히 통일되어 있지 아니한 상황에서 특정 질환의 특정 병기에 한 가지 치료방법을 고집하기보다는 다양한 종류의 치료를 복합적으로 사용하는 데에 적극적일 필요가 있다고 생각하였다. 특히 예후에 부정적인 영향을 미치는 요소를 가지고 있을 경우에는 적극적인 외과적 절제 및 수술 후 방사선 치료 등을 통하여 생존율을 높이는데 관심을 기울여야 할 것으로 생각되었다. 더 나아가 절개선 등 수술흔이 가지는 미용적 문제점 등도 고려하여 전체적인 치료의 만족도를 향상시키는데 기여해야 할 것으로 생각하였다.

타액선의 세침흡인 세포검사 - 221예 분석 - (Fine Needle Aspiration Cytology of the Salivary Gland - An analysis of 221 cases -)

  • Fine needle aspiration cytology of the salivary lesions was performed on 221 patients at Soonchunhyang University Hospital for 10 years. Of 221 aspirates, 6 aspirates(2.7%) were inadequate, 116 cases(52.5%) were non-neoplastic lesions, 76(34.4%) cases were benign neoplasms and 23 cases(10.4%) were malignant neoplasms. The cytologic diagnoses could be correlated with histologic findings in 58 cases. FNAC correctly discriminated between neoplastic and nonneoplastic lesions in fifty-seven lesions and failed in a case, and overall accuracy, sensitivity, and specificity were 98.3%, 98.0%, and 100.0%. FNAC correctly discriminated malignant neoplasms from benign neoplastic/nonneoplastic lesions in fifty-three cases and fatted in five cases, and overall accuracy, sensitivity, and specificity were 91.3%, 72.7%, and 95.7%. Among three false negative cases, two mucoepidermoid carcinomas were misdiagnosed as mucocele and benign neoplasm, and an acinic cell carcinoma were misdiagnosed as Warthin's tumor. Two false positive cases were a Warthin's tumor misdiagnosed as squamous cell carcinoma and a pleomorphic adenoma misinterpretated as suggestive of malignancy. In conclusion diagnostic accuracy of FNAC of salivary lesions is high, and the possibilities of low grade mucoepidermoid carcinoma and acinic cell carcinoma should be considered on hypocellular smears with mucoid or fluidy background.

Malignant Skin Tumor Misdiagnosed as a Benign Skin Lesion

  • Despite the fact that benign skin lesions can undergo malignant transformation, the necessity and timing of the surgical resection have yet to be established. In this study, we analyse three cases of benign-appearing skin lesions, which were found to be carcinomatous on histologic examination and review the literature regarding the importance of prophylactic removal of benign-appearing skin lesion. The first and second cases were female patients wishing for cosmetic surgery. The first patient had a benign-appearing lesion on dorsum nasi, and the second patient had an inconspicuous lesion right along the right nasolabial fold. The third patient was a middle-aged male with a pigmented lesion on the left cheek, who presented to the clinic only after having met the operating surgeon through an acquaintance outside the hospital setting. All of the lesions were suspected to be of benign nature and were excised for cosmesis only. However, histologic examination of these lesions showed that the first two tumors were basal cell carcinoma with the last tumor being squamouse cell carcinoma. Thus, it is considered that removal of benign like skin lesion will result in good prognosis of patients scheduled to undergo other surgery.

내측상악부분절제술의 임상적 고찰 (A Clinical Study of Medial Maxillectomy)

  • Background: Lateral rhinotomy and medial maxillectomy, an en bloc resection of the medial maxillary sinus, ethmoid sinus with the lamina papyracea, medial orbital floor, and lacrimal fossa-duct, have been advocated for lateral nasal wall neoplasms as a standard approach method. Objective: This report was conducted to investigate the clinical efficacy of lateral rhinotomy and medial maxillectomy for lateral nasal wall neoplasms. Materials and Methods: We retrospectively analyzed clinical data of 31 patients who were treated at department of otolaryngology-head and neck surgery, Catholic university of Korea, school of medicine between 1990 and 1996. Results: Twenty five patients had benign lesions(80.6%). By far, the largest percentage was inverted papillomas(80%, 20/25). Of the six malignant lesions(19.4%), 33.3%(2/6) was squamous cell carcinoma and other lesions were metastatic renal cell carcinoma, adecarcinoma, transitional cell cacinoma, and hemangiopericytoma. There were a 4% recurrence for benign tumors(1/25), 5% especially for inverted papilloma(1/20), and 50% for malignant neoplasms(3/6). The overall complication rate was 9.7%. Conclusion: Despite the various approach for treatment of lateral nasal wall neoplasms including inverted papilloma, we continue to advocate a lateral rhinotomy and medial maxillectomy as the treatment of choice.

폐에 발생한 경화성 혈관종 (Sclerosing Hemangioma of the Lung - A case report -)

  • The sclerosing hemangioma of the lung is rare, benign neoplasms, which are usually solitary, Althoughh their histologic apperances are distinct and well-defined, their histogenesis is uncertain. We experienced a typical lesion of pulmonary sclerosing hemangioma clinically and histologically, which was removed from the right lobe of 64-year-old female. We disccused histogenesis, microscopic feature and progress of the sclerosing hemangioma.

요로상피병변의 요세포학적 소견 (Urinary Cytologic Findings of Urothelial Lesions)

  • Urinary cytology is increasingly accepted as a diagnostic tool in the detection and follow-up of patients with bladder cancer. However, its value is reduced by several limitations, especially by the lack of cytologic criteria specifically reflecting the morphology of low-grade urothelial neoplasm. We reviewed histologically proven 50 cases of urine cytology with emphasis on cytologic findings of benign atypia and differential findings of urothelial neoplasm according to the grade. The diagnoses included 17 benign lesions(including 5 cases of urine calculi) and 33 malignant lesions (including 28 transitional cell carcinomas, 3 squamous cell carcinomas, 1 adenocarcinoma and 1 prostate adenocarcinoma), Diagnostic accuracy was 92%. Important cytodiagnostic criteria for benign atypia and low grade malignancy were cellularity, number of cell clusters, and morphology and arrangement of urothelial cells. The cytologic findings of urothelial neoplasms according to histologic grade were relatively well correlated with the histologic findings. However, the cytologic criteria were not sufficient to readily distinguish grade I from grade II. In view of this, we think that cytologic nomenclature "low-grade" and "high-grade" is a more reliable criterion. Recognition of subtle cellular morphologic features specific for urothelial lesions(including benign or malignancy) and proper fixation, processing and staining of specimen can expand the role of urinary cytology In detection and follow-up of patients.
