협부에 발생한 점액양 지방육종의 치험례 (A Case of Myxoid Liposarcoma of the Cheek)

  • Purpose: Liposarcoma is the most common soft tissue sarcoma, and usually occurs on the thigh or in the retroperitoneal space, but rarely in the oral region. This report presents a case of liposarcoma of the cheek and includes a review of the literatures. Methods: A 21-year-old woman was admitted with a palpable mass in her cheek of about two years duration, which increased in size gradually initially, but had increased rapidly over the three months. There was no particular pain or tenderness. MRI showed a well-enhanced, well-defined mass, which suspected to be hemangioma. Results: The spherical, well-encapsulated mass was surgically excised. Biopsy results revealed myxoid liposarcoma. FDG PET-CT on the seventh postoperative day, revealed a minimal to mild FDG-uptake soft tissue lesion around the mass defect area without evidence of distant metastasis. The patient is being observed and undergoing radiation therapy. Conclusion: Liposarcoma in the head and neck region is a rare disease, and can be overlooked as a benign tumor without a pathologic diagnosis. Therefore, proper treatment and follow-up are required based on an understanding of this disease.

경부에 발생한 지방모세포종 1예 (A case of neck lipoblastoma in a pediatric patient)

  • Lipoblastoma is a rare benign tumor with 80-90% occurring in children less than 3 years of age and 40% occurring in children less than 1 year of age. The most common site of incidence is limb, and then trunk. Neck is the rare site of incidence. The main symptom that the patient complains about is a rapidly growing neck mass without pain. When the size of mass increases, it can cause dyspnea, Horner's syndrome. Lipoblastoma is usually diagnosed as a lipoma in the fine needle aspiration. Since it is not differentiated from lipoma, liposarcoma, and hibernating adenoma in CT and MRI, the definitive diagnosis is histologic diagnosis through surgical resection. The treatment is complete surgical resection. And recurrence rate is 9-25% due to incomplete resection. Authors report this case with a review of literatures since we experienced a case of lipoblastoma diagnosed histopathologically after surgical treatment of neck mass.

Case report of a solitary fibrofolliculoma on the alar rim

  • Fibrofolliculoma is a benign tumor characterized by a smooth, dome-shaped papule of size 2-4 mm. Most fibrofolliculomas occur as multiple lesions, and very rarely, they are solitary. Herein, we report a case of solitary fibrofolliculoma found in the alar rim, without the typical characteristics of a fibrofolliculoma. A 42-year-old man visited the hospital with a protruding lesion that had occurred 1 year previously. A mass of size 5×7 mm was observed on the left alar rim. The tumor was dome-shaped and palpable. The patient did not have any similar lesions elsewhere. No family member was known to have such a lesion. An incisional biopsy was performed before surgery, and pathological examination revealed hyperkeratosis and dyskeratosis; however, an accurate diagnosis was not made. Complete resection was planned for the mass on the alar rim. The resected mass was subjected to permanent biopsy, and the pathological examination results led to the diagnosis of fibrofolliculoma. Therefore, when diagnosing a dome-shaped mass in the alar rim, despite the suspicion of a very rare disease, it is necessary to suspect fibrofolliculoma and consider the process from diagnostic examination to treatment.

An unusual exophytic pleomorphic adenoma on the upper lip skin without mucosa involvement: a case report

  • Pleomorphic adenoma is a benign tumor that can occur in the salivary glands, most commonly in the parotid gland. While it primarily occurs in the major salivary glands, it can sometimes be found in the minor salivary glands. Within the minor salivary glands, it most often originates in the hard palate and soft palate, and less frequently in the upper lips. Due to its location in the minor salivary glands, most pleomorphic adenoma involve and protrude on the mucosa. A 61-year-old man presented with 1.5 cm exophytic mass on the skin of his upper lip. This mass was exophytic on the skin and did not involve or protrude into the inner lip mucosa. The mass was entirely excised, and a subsequent permanent biopsy diagnosed it as a pleomorphic adenoma. In such situations, it can be challenging to suspect pleomorphic adenoma during a physical examination, leading to potential diagnostic confusion. It might also be mistaken for an inclusion cyst or another type of mass, making it tempting to treat without verifying the pathological results.

경부 종물로 발견된 정맥 내 화농성 육아종 1례 (A Case of Intravenous Pyogenic Granuloma Found as a Neck Mass)

  • Intravenous pyogenic granuloma, commonly known as intravenous lobular capillary hemangioma, is a rare benign tumor of the vein. It rarely occurs in the neck, and its character is not enough to diagnosis clinically. It could be diagnosed with preoperative radiologic examinations such as ultrasound and computed tomography and typical pathologic findings that demonstrate lobules of multiple capillaries lined with flattened endothelial cells admixed with fibromyxoid stroma. The authors report a case of a 32-year-old male who presented with a palpable neck mass for one month with a review of the literature. He was successfully treated with resection, including the tumor and normal external jugular vein, without any complications.

Surgical Treatment of Extensive Tumoral Calcinosis Associated with Systemic Sclerosis

  • Extensive tumoral calcinosis affecting a large joint is uncommon in patients with systemic sclerosis. We report the case of a 52-year-old female patient referred for a growing calcified mass in the shoulder. She was diagnosed with interstitial lung disease and progressive systemic sclerosis. Although the pain and disability associated with the affected joint was not severe, the patient underwent surgical excision because the mass continued to grow and was likely to produce shoulder dysfunction and skin ulceration. The patient appeared well 10 months after surgery with no signs of recurrence. This report highlights the timing and indication of surgical excision in similar cases.

A Case of Calvarial Hemangioma in Cranioplasty Site

  • It is not uncommon for hemangiomas to occur in the calvarium, accounting for about 10% of the benign skull tumors. A 46-year-old man was presented with a palpable scalp mass on the left parietal region. Past medical history indicated that he had undergone cranioplasty 25 years prior due to a depressed skull fracture suffered from a traffic accident. Magnetic resonance imaging revealed mixed signal intensity mass on T1 -and T2- weighted images pushing a linear signal void lesion outward in the left parietal region. After total surgical removal along with cranioplasty, he was discharged without neurological deficits. Histological examination of the surgical specimen revealed a cavernous hemangioma. A skull hemangioma occurring at the site of a cranioplasty has not yet been reported. Therefore, authors report this case in combination with a pertinent literature review.

흉막에 발생한 다발성 지방종의 치험 1례 (A Case Report of Lipomatosis in the Pleura)

  • Lipomatosis is a condition containing multiple lipomatous masses.Lipoma is a benign neoplasm composed of adult adipose tissue, and occur most often in the fifth or sixth decade and rarely in the pleura. Pleural lipomas are usually asymptomatic and revealed as an incidental roentgenographic findings. The patient was 59 year-old male and admitted because of dyspnea-on-exertion for 30 years. Chest CT revealed right pleural mass abutting on the chest wall, measuring minus 80 hounsfield units. The mass was resected with calcified pleural plaque and confirmed to be lipomatosis with collagenous fibrosis arising from viserai pleura.

Desmoplastic Fibroma of the Skull

  • Desmoplastic fibromas are rare intraosseous bone tumors. They are benign but locally aggressive and frequently found in the long bones and mandible. We report radiographic and histopathologic finding of a case desmoplastic fibroma involving right temporal skull bone. A 53-year-old woman presented at our hospital complaining of continuous right side headaches for a year. Simple skull X-ray film showed $3{\times}2.5cm$ lytic lesion with mild sclerotic margin on right temporal area. A large craniectomy 1cm lateral to margin was fashioned. The resected mass showed encapsulated mass colored white gray. Histologic diagnosis was compatible with that of a the desmoplastic fibroma. There was no evidence of recurrence during the 15months of follow-up period.

  • 초음파의 다양한 장점에도 불구하고 초음파의 질감(echotexture)과 균질성(homogenecity)의 문제로 악성 종양과 양성종양을 감별하는데 한계가 있을 수 있으므로, 초음파를 이용한 고형의 연부조직 종괴를 진단할 때는 주의가 필요하다.
