공공도서관 건립의 경제적 타당성 분석에 관한 연구 - 금샘도서관 건립 사례를 중심으로 - (A Study on the Economic Feasibility Analysis of Public Library Construction - Focused on the Case of Geumsaem Library Construction -)

  • 이 연구는 부산시 금정구 금샘도서관(가칭) 건립 타당성 조사 연구의 일부로서, 도서관 건립의 경제적 타당성 분석에 대한 결과를 검토하는 데 그 목적이 있다. 이러한 목적을 위하여, 이 연구에서는 도서관 건립에 대한 정책적 타당성 분석, 수요조사, 경제적 타당성 분석의 결과를 살펴보았다, 경제성 분석 도구로 비용편익비율, 순현재가치, 내부수익률을 활용하여 경제적 타당성을 분석하였으며, 조건부가치측정법을 활용하여 편익을 추정하였다. 연구의 결과, 금샘도서관 건립의 비용편익비율은 1.28로 나타났으며, 도서관 건립은 경제적 타당성을 확보한 것으로 분석되었다.

  • Purpose: The purpose of this study was to use cost-benefit analysis of activity to clarify the economic effect of prepared nurses versus atmospheric environment managing engineers as healthcare managers. Methods: For the study 111 workplaces were surveyed, workplaces in which nurses or atmospheric environment managing engineers were employed as healthcare managers. The survey content included annual gross salaries, participation in external job training, costs in joining association covered by the company, location and year of construction of the healthcare office, various kinds of healthcare expenditures, costs in operating healthcare office, health education, and activity performance in the work of environment management. Results: In the case of the healthcare manager being a nurse, benefit was larger than input costs at a ratio of 2.31. On the other hand, in the case of healthcare manager being an atmospheric environment managing engineer, input costs were larger than benefits (benefit-cost ratio 0.88). Conclusion: Results indicate that nurses are an effective healthcare human resource and can offer good quality healthcare service. Therefore companies should hire nurses and actively promote the economic efficiency of nurses in workplace.

방위력개선사업의 비용편익 분석을 통한 획득대안 분석 연구 (A Study on Cost-benefit analysis of Defense Improvement Project for Acquisition Alternative Analysis)

  • 무기체계 획득사업은 막대한 예산을 투입하여 국가에 미치는 파급효과가 큰 특성이 있어 무기체계 획득사업을 추진하기 위한 기획연구 단계에서 경제적 타당성을 확보한 획득대안 분석이 요구되고 있다. 이러한 요구에 따라 획득연구 단계에서 획득대안에 대한 경제성 분석을 수행하고 있으나, 현재는 초기 단계로 경제적 분석에 따른 획득대안 분석에 대한 연구는 미비한 실정이다. 본 연구에서는 현재 획득대안에 대한 의사결정 방법인 효과분석의 제한점을 식별하고 추가로 비용편익분석 방법을 제시하여 획득대안에 대한 경제성 분석 방법을 제안한다. 비용편익 분석의 결과를 비용/편익비율(Benefit/Cost Ratio)로 제시하여 복수 대안뿐만 아니라 단일 대안에 대한 경제성 분석이 가능하고, 경제적 타당성을 확보한 보편적인 결과 제시가 가능하다. 제시 방법을 OO 유도로켓 사업 획득연구에 적용하여 각 대안에 대한 경제성 분석결과를 제시한다.

계란 자동세척기의 경제적 타당성 평가 (Economic Feasibility of Automatic Egg Washer)

  • 계란 세척기 평가 금액 추정 결과는 다음과 같다. 계란 세척기 총수입(인건비 절감으로부터의 조수입 증대)에서 산정하였으며, 계란 세척기 총비용은 구입가격, 고정비용 및 유동비용의 합계로 나타내었다. 계란 세척기 평가액은 할인율 수준에 따라 최소 44,778천원(할인율 10%)에서 최대 45,778천원(할인율 5%), 평균 45,278천원으로 추정된다(Table 7). 계란세척기의 경제적 타당성을 살펴보면, 내부수익율 IRR의 값은 가정했던 할인율(5%~10%)보다 훨씬 높고, 순현재가치(NPV)는 0보다 훨씬 크며, B/C 비율도 1.0 이상으로 나타나 계란 세척기 개발사업의 경제적 타당성은 있는 것으로 판단된다(Table 8).

건설업 보건관리자 선임 관련 비용편익분석 (Analysis of Cost Benefit Related to Appointing a Health Care Manager in the Construction Industry)

  • Purpose: The construction worker has diverse harmful factors such as noise, dust, and dealing with chemicals. Therefore this research aimed to examine the necessity of appointing a health manager in the construction industry by examining the cost-benefit analysis when the construction industry appoints a health manager. Methods: In order to calculate the healthcare staff employment cost and the benefits from their activities in 1,425 construction companies with the staff of 300 or more people during 2011, this study analyzed existing data and existing research data, as well as national data. Results: Total annual costs were 99,920,070,900 won and total annual benefits were 324,807,182,625 won. Benefits were found to be 224,887,111,725 won exceeding costs. Benefit/cost ratio resulting from appointing a health manager in the construction industry workplaces was 3.25 times. Conclusion: The findings of this research can be used as the base data to make rational decision to positively encourage the employment of healthcare staff in construction companies pursuant to relevant laws.

의약분업의 비용-편익 분석 (Cost-benefit Analysis of Mandatory Prescription in Korea)

  • Objective : To evaluate the relative benefits and the costs associated with the introduction of the new pharmaceutical provision called 'Mandatory Prescription System' which separates the role of physicians from that of pharmacists with respect to the prescription and dispensation of from the perspective of consumers (i.e., patients). Methods : The costs of the system were measured by considering both direct and indirect costs. Direct costs included additional payments for ambulatory care and dispensing fees that occurred under the new system. indirect costs consisted of transportation expenses and costs related to time spent for physician consultation, waiting for the prescriptions to be filled, and extra traveling. Benefits identified in this study were the reduction of drug misuse and overuse, and the overall decrease in drug consumption among the Korean population. Sensitivity analysis was peformed for the inclusion of benefits for outpatients of hospitals, price elasticity, and increased fees for established patients. Results : The net benefit was estimated to be about minus 1,862 billion won and the benefit-cost ratio was 0.478. This indicates that the costs of 'Mandatory Prescription' outweigh its benefits, relative to the previous system. The sensitivity analysis results for all the variables considered in this study consistently showed the benefit-cost ratio to be less than 1. Conclusion : The results of this study suggest that implementing Mandatory Prescription System in Korea might be inefficient from the consumer's perspective. The results of this study do not coincide with the results of previous studies, presumably because of the differences in study design and in which items of costs and benefits were considered.

모자보건 증진사업에 대한 비용편익 분석 - 라오스 국가를 중심으로 - (Cost-Benefit Analysis of Maternal and Child Health Program - Focusing on Lao People's Democratic Republic -)

  • Objectives: The purpose of this analysis was to assess the maternal and child health program in Lao PDR, which was implemented to reduce maternal and child mortality in Laos. Target areas for the project included 2 provinces (Xiengkhuang, Houaphan) with all 18 districts within them. The beneficiaries of this project included 121,000 childbearing women and 62,000 children under 5 years old (LSIS, 2012). Methods: In this study, it was done for efficiency evaluation of the assistance projects of the health sector of maternal and child health promotion program in Laos that took place from 2010 to 2013. We conducted a cost-benefit analysis for the evaluation of the effectiveness of that program was being carried out effectively. Results: This evaluation adopted the Cost-Benefit analysis approach. Key findings of the Cost-Benefit analysis are the following: The Benefit-Cost Ratio of 1.012, Net Present Value of 84,250,000 Korean Won, an estimated Internal Rate of Return of 10.12%. These findings suggest that project activities were efficiently implemented. Conclusion: As a result, maternal and child health project in Laos has been analyzed that there was economic efficiency. Therefore, It is considered necessary and continued support expansion of program in the future. The direction of maternal and child health project in Laos, approach and community policy support must be included. Integrated approach between the program approach and overall health should be performed for healthy life habit.

노인환자에게 제공하는 개국약국 약료서비스의 경제적 가치 (Economic Value of Pharmaceutical Care for the Elderly Patients in Community Pharmacies)

  • This study was to evaluate economic impact of a comprehensive pharmaceutical care intervention provided by community pharmacists on drug-related morbidity and mortality in the elderly population, in a societal perspective. Clinical outcomes of pharmaceutical care included compliance increase, inappropriate medication discontinuation, and subsequent drug-related morbidity and mortality reduction. Economic outcomes included cost savings from direct medical costs reduction such as medication and healthcare resource utilization. Input costs for pharmaceutical care included pharmacist time and computerized prescription review supporting program costs. Model parameters of outcomes were derived from published literatures, and costs were from literatures and health insurance statistical data in Korea. Annual costs and benefits were estimated in the year 2005. Current usual care and standardized pharmaceutical care required 0.3 and 2.0 hours per year respectively, for elderly outpatient using average 4.4 prescription drugs per visit and average annual frequency of 17.8 pharmacy visits. Comprehensive pharmaceutical care provided to overall elderly outpatients at community pharmacies would have cost of \74,994 mil. and benefit of \357,002 mil. per year. Benefit:cost ratio was 4.8:1 and net benefit was \282,008 mil/year. It was corresponded to net benefit of \73,816/year for individual elderly patient. In addition, pharmaceutical care was estimated to reduce 1,531 drug-related deaths/year. Conclusively this study, a first attempt in Korea to evaluate an economic value of pharmaceutical care at community pharmacies, proved that it was a cost-effective intervention having significant economic benefit.

IoT기반 철도 화차 안전운송 통합 품질관리시스템에 관한 경제성 평가지표 분석 (Economic Evaluation Analysis of Effect of Train Freight Car Safety Transport Integrated Quality Management System Based on Internet of Things(IoT))

  • Purpose: The objective of this study is to verify the economic validation of quality management integrated train freight car by analyzing economic evaluation indicators such as benefit and cost, net present value, and inter rate of return. Methods: First, we itemize benefit and cost field by reviewing literatures; Benefit consists of 1)Safety, 2)Operation, and 3)Maintenance; Cost consists of 1)Set-up fee, 2)Wireless internet fee, and 3)Cloud storage using fee. Second, based on these estimated values, we conduct an economic evaluation analysis. Among them, benefit and cost, net present value, and internal rate of return are selected. Results: As a result, all estimated values are highly over criterion of economic validity($$B/C{\geq}_-1$$, $$NPV{\geq}_-0$$, $$IRR{\geq}_-R$$); 1)benefit over cost ratio is 28.22, 2)Net present value is 8,121.66million KRW, and 3)Internal rate of return value is 2272%. Conclusion: The findings of this study will help making a decision when train industry adopts IoT technology for improving the effectiveness.

A Systematic Review of the Economic Evaluation of Telemedicine in Japan

  • Objectives: There is no systematic review on economic evaluations of telemedicine in Japan, despite over 1000 trials implemented. Our systematic review aims to examine whether Japan's telemedicine is cost-saving or cost-effective, examine the methodological rigorousness of the economic evaluations, and discuss future studies needed to improve telemedicine's financial sustainability. Methods: We searched five databases, including two Japanese databases, to find peer-reviewed articles published between January 1, 2000 and December 31, 2014 in English and Japanese that performed economic evaluations of Japan's telemedicine programs. The methodological rigorousness of the economic analyses was assessed with a well-established checklist. We calculated the benefit-to-cost ratio (BCR) when a reviewed study reported related data but did not report the BCR. All cost values were adjusted to 2014 US dollars. Results: Among the 17 articles identified, six studies reported on settings connecting physicians for specialist consultations, and eleven studies on settings connecting healthcare providers and patients at home. There are three cost-benefit analyses and three cost-minimization analyses. The remaining studies measured the benefit of telemedicine only, using medical expenditure saved or users' willingness-to-pay. There was substantial diversity in the methodological rigorousness. Studies on teledermatology and teleradiology indicated a favorable level of economic efficiency. Studies on telehomecare gave mixed results. One cost-benefit analysis on telehomecare indicated a low economic efficiency, partly due to public subsidy rules, e.g., a too short budget period. Conclusions: Overall, telemedicine programs in Japan were indicated to have a favorable level of economic efficiency. However, the scarcity of the economic literature indicates the need for further rigorous economic evaluation studies.
