Relationship between Upper Airway and Sleep-Disordered Breathing in Children with Mouth Breathing (구호흡 어린이에서 수면호흡장애와 상기도와의 관계)

  • The most common cause of mouth breathing is obstacles caused by mechanical factors in upper airway. Mouth breathing could be consequently pathological cause of sleep-disordered breathing. Sleep-disordered breathing in children can cause growth disorders and behavioral disorders. The purpose of this study was to investigate relationship between upper airway and sleep-disordered breathing in children with mouth breathing. Twenty boys between 7 - 9 years old who reported to have mouth breathing in questionnaire were evaluated with clinical examination, questionnaires, lateral cephalometric radiographs, and portable sleep testing. This study assessed apnea-hypopnea index (AHI) and oxygen desaturation index (ODI) for the evaluation of sleep-disordered breathing and was done to investigate the correlation between these values and the upper airway width measured by lateral cephalometric radiographs. There was no significant correlation with the size of the tonsils (p = 0.921), but the adenoid hypertrophy was higher in the abnormal group than in the normal group (p = 0.008). In the classification according to AHI and ODI, retropalatal and retroglossal distance showed a statistically significant decrease in the abnormal group compared to the normal group (p = 0.002, p = 0.001). As AHI and ODI increased, upper airway width tended to be narrower. This indicates that mouth breathing could affect the upper airway, which is related to sleep quality.

Information Cascade and Share Market Volatility: A Chinese Perspective

  • The purpose of this paper is to understand the underlying dynamics for the share market bubbles in China during the most recent decade. By using the behavioral finance theory and the Shanghai Composite index prices during the periods from 2005 to 2008 and from 2014 to 2015 as the study samples, we find that the large volatilities in the Chinese share market are closely related to information blockage, which impedes share prices to timely respond to economic conditions as well as external shocks and increases (decreases) the demand of shares when the supply is difficult to adjust. Although the Chinese government has introduced a series of programs designed to increase more reliable information to the public, the share market still tends to confront issues of information asymmetry. The potential reason is that the reforms did not change the long-stand situation in China, where individuals or groups related to government bureaucracy who play a dominant role in the society are given priority to gain access and obtain information that benefits. By identifying the main reasons for the large volatilities in the market, policy makers are given advice as to which areas they may need to focus on to improve future market performance.

Factors Influencing Parenting Stress in Mothers of Preschoolers Born Prematurely (미숙아로 출생한 학령전기 아동 어머니의 양육 스트레스의 영향요인)

  • Purpose: This study was conducted to describe parenting stress in mothers of preschoolers who were born prematurely and to determine factors affecting parenting stress in child's problem behavior, mother-child interaction and parenting alliance. Methods: An exploratory survey study was conducted with 66 mothers of preschool children (5~6 years) with preterm births (PTB). Data were collected using Parenting Stress Index (PSI), Child Behavior Checklist (CBCL), Mother-Preschool Child Interaction Scale (MPIS), and Parenting Alliance Inventory (PAI). Results: Of the 66 mothers, 8(12.1%) showed high scores that were more than borderline for the PSI total score. In the multiple linear regression analysis, lower scores on the PAI (${\beta}=-.41$, p<.001), higher scores on the externalizing problem behavior of the CBCL (${\beta}=.40$, p=.001), and lower scores on the MPIS (${\beta}=-.21$, p=.043) were statistically significant contributors to maternal parenting stress. Conclusion: Findings indicate that mothers of PTB preschool children are at risk for parenting stress. Child's externalizing problem behavior, poor maternal-child interaction and parenting alliance were independent factors raising maternal parenting stress. More attention is needed on paternal parenting support, child's behavioral development, interaction with children for effective prevention and management of maternal parenting stress of PTB young children.

Effects of the Integrative Weight Control Program Including East Asian Traditional Medicine on the Degree of Obesity and Body Composition (한방요법을 포함한 통합비만관리 프로그램이 체성분과 비만도 변화에 미치는 영향)

  • Purpose: The purpose of this study was to evaluate the effects of an integrative weight control program including East Asian traditional medicine on the degree of obesity and body compositions. Method: Using one group pre-post test design, 63 subjects aged 19 years or older were conveniently recruited from D University hospital. The weight control program consisted of acupuncture, aerobic exercise, moderate caloric restriction, and behavioral modification for weight loss. Paired t-tests were conducted using the SPSS V18.0. Results: Body weight (t=10.44, p<.001), body fat mass (t=11.47, p<.001), percentage of body fat (t=11.49, p<.001), the degree of obesity (t=10.48, p<.001), body mass index (t=10.66, p<.001), waist circumference (t=10.25, p<.001), hip circumference (t=6.10, p<.001), and WHR (t=2.21, p=.040) decreased after administering the integrative weight control program. Conclusion: This integrative weight control program effectively reduced the obesity degree and percentage of body fat. Further study is needed to replicate our program in a larger sample with control group to validate the findings.

Changes on function and morphology of liver in carcinogen-induced hepatoma rats (I. Changes on function of liver) (발암제에 의해 간종양이 유발된 쥐에서 간기능과 형태학적 변화에 관한 연구 (I. 간기능 변화에 관하여))

  • This study is concerned with assessment of diethylnitrosamine(DEN) induced liver cell carcinogenesis by measurement of changes preceding the development of neoplasms. Therefore, it was undertaken to investigate the changes of liver-specific enzyme activities in rats (Sprague-Dawley) by ad libitum feeding of DEN. And also, as another objective index in urine, the level of urinary biopterin was measured by high performance liquid chromatography(HPLC) method. The results were as follows ; 1. Minor behavioral change, brittleness of hair and decreased amount of water and diet intake were observed in rats 7 weeks after DEN administration. 2. The body and liver weights were significantly(p<0.05) decreased in rats 11 weeks after DEN administration. 3. The ratio of liver weight to body weight was rather stable and not significantly decreased in the all treatment groups. 4. The liver specific enzyme activities(AST, ALT,$\gamma$-GTP) were significantly(p<0.05) increased in all treatment groups compared to control group. 5. Compared to normal level, urine biopterin level was significantly (p<0.05) increased in all treatment groups(p<0.05). In conclusion, this result confirmed that the DEN was one of the potent hepatocarcinogens. In experimental model of rats exposed to DEN, the results indicated that values of liver specific enzyme activities(AST, ALT, $\gamma$-GTP) and urine biopterin level could be useful complementary tumor indices.

A Study on the Failed Rest After Work in Association with Cardiovascular and Other Diseases as Well as Physical disorders

  • This survey provides, at a participation rate of 70%, 4,790 examinees. The purpose of this study is to study the association of the failed rest after work with 34 diseases including cardiovascular diseases. The index of the failed rest after work was composed of 4 questions about "thinking of work for several hours", "feeling exhausted", feeling unsatisfied or depressed", and "needing to go to bed early for next day′s work". Estimation of correlation among 4 variables, factor analysis, and ANCOVA adjusted for sex, age and job were carried out. A self-rating questionnaire of one′s own disease history and the "London School of Hygiene Cardiovascular Questionnaire" were used in order to discriminate each morbid group from the opposite group. Brief explanations of the result are as follows: 1) Every variable of failed rest after work shows significant difference between the morbid group and the no morbid group for possible infarction; for angina pectoris in the total, and men. 2) Among 4 variables ′exhaustion′ best discriminates the infarction group from the no infarction group, and the angina group from the no angina group. 3) The factor of failed rest after work is a significant factor that distinguishes the infarction group from the no infarction group, and the angina pectoris group from the no angina group. Therefore, stress management through health education and promotion such as behavioral modification can be used to reduce cardiovascular diseases and stress as perceived by an individual.

Development of a Nursing Competency Scale according to a Clinical Ladder System for Intensive Care Nurses (중환자실 간호사의 임상등급 (clinical ladder)별 간호역량 측정도구 개발)

  • Purpose: This study was done to develop a nursing competency scale according to a clinical ladder system for intensive care nurses. Methods: Index of content validation was done by 20 clinical experts and 80 nurses in Intensive Care Units (ICU). Results: The process and results of study are as follows. First, 12 nursing competencies were used in the establishment of the clinical ladder system (Jang, 2000). Second, the first draft of the competency lists was developed. It was based on the clinical nurses' behavioral indicators of nursing competency by Jang (2000), and was modified and supplemented through various literature reviews including competency standards for specialist intensive care nurses in Australia and consultation with 2 clinical nurses with over 10 years experience in the ICU. Third, the draft was examined by 20 clinical experts for content validity. Finally, the final draft was analysed using clinical validity where 20 nurses in each ladder participated. The final number of items was fixed at 309. Conclusion: The tool represents expected nursing competency of nurses working in ICU. Intensive care nurses can recognize their strengths and weaknesses, and identify directions for their professional growth by analysing results of their competency evaluation using this tool.

PTSD Symptoms in Elementary School Children After Typhoon Rusa

  • Purpose. A natural disaster negatively affects children's emotional and behavioral adjustment. The purpose of this paper was to examine the prevalence, symptoms, and correlates of PTSD after the occurrence of Typhoon Rusa. Method. 261 elementary school children living in Kimcheon, which was a devastated rural area in South Korea by Typhoon Rusa, were selected. Data were collected 4 months after the disaster using the PTSD Reaction Index categories recommended by Frederick, severity of PTSD. Results. 12.3% of the children had either moderate or severe PTSD symptoms; 22.7% reported mild symptoms; and the remaining 65% had sub-clinical symptoms of PTSD. The most frequent symptom was recurrenct fear(67.0%). 13% to 17.2% of children exhibited difficulty in concentration, sleep disturbance, and guilt feeling. The regression model of severity of PTSD was composed of the level of exposure to traumatic experiences, grade in school, gender, negative coping style, and social support, and explained 34.3% for PTSD symptoms. Exposure to traumatic experiences was the strongest factor of all predictors. Conclusion. Emotional support from friends and coping style were correlated with PTSD severity. School-based interventions that emphasizes coping with disaster related problems and problem-solving may prove to be useful, and may aid in building close and supportive ties with teachers, classmates, and friends.

A Case Report for an Eating Disorders with Banhabaeckchulchunma-tang and Acupuncture Treatment (반하백출천마탕(半夏白朮天麻湯)과 침 치료에 호전을 보인 섭식장애 환자 치험 1예)

  • Eating disorders is a significant failure on the dietary behavior, can be distinguished by anorexia nervosa and bulimia nervosa. For the treatment of eating disorders, screening for medical conditions and hospitalizations are needed. Also, medications such as antidepressants may be helpful, and it should be considered a dynamic psychotherapy, cognitive behavioral therapy, and family therapy together. Recently we experienced a 24-year-old woman who was accompanied with binge eating at night, habitual vomiting, dizziness, anorexia, and general weakness. We applied Banhabaeckchulchunma-tang and acupunture treatment during 2 weeks. Through this program, patient's condition was improved on body mass index and Eating Attitude Test-26.

Effects of Weight Control Program on Nutrient Intakes and Physical Fitness in Obese Korean Adult Women (체중조절 프로그램이 성인 비만 여성의 영양소 섭취량 및 운동 능력에 미치는 효과)

  • This study was conducted to investigate the effects of weight control program on nutrient intakes and physical fitness of Korean obese adult women. The subjects of this study were 33 obese adult women aged 30-65 years residing in Seoul. The weight control program for obese women included nutrition education, cognitive behavioral therapy and exercise for 12 weeks. There were significant decrease (p < 0.001) of blood pressure and atherogenic index of obese women after the weight control program. Energy intake was significantly decreased (p < 0.001) from 1556.9kcal to 1044.9kcal after 12 weeks of the weight control program. Also protein, fat and carbohydrate intakes were significantly decreased. The ratio of energy intakes per day for the subjects was carbohydrate 64% : protein 16% : fat 20%. The frequency of vegetables intakes was significantly increased (p < 0.05) and there were no significant differences in other food groups. After the weight control program, muscle endurance, flexibility, balance and explosive muscular strength of subject's physical fitness were significantly improved. These results suggest that the weight control program for obese women may contribute to nutrient intakes reduction and improvement of physical fitness.
