봉침을 이용한 무작위배정 비교임상시험연구에서의 유해사례 보고에 대한 체계적 문헌고찰 (Reporting of Adverse Events in Randomized Controlled Trials of Bee Venom Acupuncture: A Systematic Review)

  • Objectives Bee venom acupuncture (BVA) is emerging as a potential therapeutic option for several conditions. However, evidence regarding accurate estimates of its adverse events are not available. Therefore, by combining occurrence rates of adverse events in relevant randomized controlled trials (RCTs), we have aimed to give an overall estimated rate of each adverse event following BVA intervention. Methods A systematic search for adverse events of BVA in Pubmed, Cochrane Central Register of Controlled Trials, Embase, NDSL, KoreaMed, KISS, RISS, KISTI, and KMbase was performed. For the quality assessment of the trials, we applied the CONSORT harms reporting recommendations. The overall occurrence rate of each adverse event was assessed as the primary outcome. The overall drop-out rate due to adverse events was assessed as the secondary outcome. Results Ten RCTs reported adverse events following BVA. Two RCTs were eligible of further synthesis. Adverse events of pruritus, localized edema, and erythema occurred significantly more frequently in the BVA group compared to the control group. Data of drop-outs were available in seven RCTs. However, no comparisons showed a significant difference. Conclusions Currently evidence of adverse events is far from sufficient for assessing a reliable, precise occurrence rate. Still, limited evidence shows a tendency that adverse events occur more frequently after the appliance of BVA compared to the control. Thus, we await well designed future studies of BVA with more proper and strict harms reporting methods.

Anti-nociceptive effect of bee venom treatment on chronic arthritic pain in rats

  • Bee venom (BV) has been traditionally applied to relieve pain and to cure inflammatory diseases such as rheumatoid arthritis (RA) and neuritis. While several investigators have evaluated the anti-inflammatory effect of BV treatment, the anti-nociceptive effect of BV treatment on inflammatory pain is not reported. Therefore, we decided to evaluate the analgesic effect of BV treatment using Freund's adjuvant induced chronic arthritis model. Freund's adjuvant-induced arthritis has been used as an experimental animal model for RA in humans to assess the efficacy of the anti-inflammatory/analgesic drugs. In this study, subcutaneous BV treatment (1mg/kg/day) produced significantly reductions of symptoms related to arthritic pain (i.e. mechanical hyperalgesia and thermal hyperalgesia). The anti-nociceptive effect of BV was observed from at least 12 days after BV treatment. Furthermore, BV treatment significantly suppressed adjuvant induced Fos expression in lumbar spinal cord. We also found that local injection of BV into near the inflammatory site (especially Zusanli-acupoint) showed more potent analgesic effect on arthritic pain rather than distant injection of BV from inflammatory site (arbitrary side of back). The present study demonstrates that BV treatment has anti-nociceptive effect on arthritis induced inflammatory pain. The analgesic effect of BV on RA is probably mediated by the effect of BV itself or possible other mechanism such as counter-irritation. Furthermore, it is possible that BV acupuncture is one of the promising candidates for long-term therapy of RA.

홍화 약침의 견비통에 대한 임상적 연구 (Clinical Study on Effect of Carthmi-Flos Herbal acupuncture theraphy on shoulder pain)

  • Objective : This study is designed to find out the effects of Carthmi-Flos Herbal acupuncture therapy on shoulder pain. Methods : Under the assumption that Bee-venom Therapy may be effective for treatment seborrheic dermatitis, Bee-venom acupuncture was injected subcutaneously into the lesion including Gv24(神庭), Gv23(上星), Gv22, Gv20(白會), B4(供差), B5(五處), B6(承光), S8(頭維), etc. Results : 1. Female patients were 18 cases and these were more 12 cases than male patients. Fifty aged patients were the most(11 cases, 36.7%). 2. Below 1 month duration of disease was the most(14 cases 46.7%). 3. The effects of treatment by condition of patients was follows : After treatment, Grade IV was decreased 26.7%, Grade III was decreased 6.6% and Grade II was increased 23.3%. 4. The effects of treatment by Duration was follow : Below 1 month & 1 month to 6 month were improved highly. 5. In evaluating of satisfaction, exellent was 6 cases, and good was 19 cases. The percentage of above good was 83.3%. Conclusions : According to the results, Carthmi-Flos Herbal acupuncture may be good effects on the decrease of shoulder pain.

흉곽출구증후군 환자에 대한 한의학적 통합치료 증례보고 1례 (A Case Report of Thoracic Outlet Syndrome Improved by Integrated Korean Medical Treatment)

  • Objectives : There are few reports on treatment of thoracic outlet syndrome in the field of traditional Korean medicine. The aim of this study is to report a case of a 29-year-old male patient diagnosed with thoracic outlet syndrome, whose symptoms were relieved after 3-week integrated Korean medical treatment. Methods : A patient diagnosed with thoracic outlet syndrome was admitted into the Korean medicine hospital. The patient had left shoulder pain and left arm numbness. The pain was worse if it had become cold. The patient was treated using Sa-am acupuncture(Daejangseunggyeok), sweet bee venom acupuncture, herbal medicine and other treatment including physical therapy from September 7th to 27th of 2012. Improvement of the patient's symptoms was evaluated by verbal numerical rating scale(VNRS), skin surface temperature difference between the left and right symmetric part of digital infrared thermographic image(DITI). Results : After 3-weeks of treatment, VNRS decreased and skin surface temperature difference between the left and right symmetric part of DITI showed moderate improvement. Conclusions : The results suggest that integrated Korean medical treatment may affect to reduce the symptoms of thoracic outlet syndrome. Further study is needed to evaluate the importance of this report.

MMA 증후군 모돈에서 생봉독의 치료효과 (Therapeutic Effects of Natural Bee Venom in Sows with Mastitis, Metritis and Agalactia Syndrome)

  • This study was designed to examine the clincotherapeutic effect of natural honeybee (Apis mellifera) venom in sows with mastitis, metritis and agalactia (MMA) syndrome. Sows with MMA syndrome after parturition were assigned to treated and nontreated control groups. In treated group, 22 sows were bee acupunctured once a day for 3 consecutive days. Acupuncture points of Jiao-chao (GV-1, at the indentation between the base of tail and the anus), Yang-ming (ST-18, outside at the base of teat) and Hai-men (ST-25, about 1 cm lateral to the umbilicus) were stung by the natural honeybees. In control group, 20 sows were intramuscularly injected with a standard dosage of penicillin G (400,000 IU/kg of body weight) once a day for 3 consecutive days. At post-treatment, 85.0% of control sows and 90.9% of sows in treated group recovered from MMA syndrome. Bee acupuncture therapy didn't show any side effects such as allergy, intoxication, hemorrhage, or infection. It might be concluded that apitherapy was effective in controlling of sows with MMA syndrome.

  • Objectives : This is a clinical report about neurogenic bladder patient with lumbar disc hernication. Neurogenic bladder is a dysfunction that results from interference with the normal nerve pathways associated with urination. Hypotonic (flaccid) neurogenic bladder can be caused by ruptured or herniated intervertebral disk. Methods : The patient was treated by Oriental medicine treatment with needle acupuncture, electropuncture, bee venom acua-acupuncture and Daeboonchungeum-gami etc. Voided volume and frequency, visual analogue scale and physical examination was used to estimate the efficacy of these treatment. Results : STZ As using these treatments, voided volume and frequency returns normal condition. Visual analogue scale dips as low as 2. Physical examination showed improvement as compared with the fist visit. And further, these treatment may influence on the recovery of neurogenic bladder patient with lumbar disc herniation. Conclusion : The results suggest that Oriental medicine treatment have an useful effect on neurogenic bladder patient's treatment and recovery.

Analysis of the Effects on the Level of Pain and Functional Improvement After Integrated Korean Medicine in Patients with Shoulder Impingement Syndrome: A Retrospective Chart Review

  • Background: This study investigated the clinical effectiveness of Korean medicine (KM) treatment for shoulder impingement syndrome (SIS). Methods: There were 61 patients who were diagnosed with SIS in the Jaseng hospital network of KM (7 hospitals located in Korea: Gangnam, Daejeon, Bucheon, Haeundae, Bundang, Ulsan, and Gwangju) between January 1st, 2015 and December 31st, 2020 who were retrospectively reviewed. The patients were grouped according to complications, intake of analgesics, duration of illness preadmission, and treatment. Treatments consisted of herbal medicine, acupuncture, cupping, Chuna, pharmacopuncture, bee venom pharmacopuncture, medicinal steaming, Daoyin exercises, and physical therapy. By comparing the Numeric Rating Scale (NRS), Shoulder Pain and Disability Index, and European Quality of Life 5-Dimensions questionnaire scores, the effectiveness of integrated KM treatment was evaluated. Results: There were 14 males and 47 females. For inpatients diagnosed with SIS, the mean NRS score decreased from 5.78 ± 1.33 to 3.40 ± 1.43 (p < 0.001). The mean Shoulder Pain and Disability Index score decreased from 53.87 ± 14.76 to 38.56 ± 18.87 (p < 0.001), and the mean European Quality of Life 5-Dimensions questionnaire increased from 0.67 ± 0.13 to 0.76 ± 0.09 (p < 0.001) after KM treatment. Medicinal steaming (0.398; p < 0.001), acupuncture (0.290), cupping (0.288), bee venom pharmacopuncture (0.282), and Daoyin exercises (0.262; p < 0.05) had a positive correlation with improved changes in the NRS score. Conclusion: Conclusion: Treatment with integrated KM treatment improved the pain, range of motion, shoulder function, and quality of life of patients with SIS.

Neuroimaging Assessment of the Therapeutic Mechanism of Acupuncture and Bee Venom Acupuncture in Patients with Idiopathic Parkinson's Disease: A Double-blind Randomized Controlled Trial

  • Objectives: The purpose of this study was to explore the therapeutic mechanism of acupuncture and bee venom acupuncture (BVA) in patients with idiopathic Parkinson's disease (IPD) using positron emission tomography (PET) and arterial spin labeling (ASL). Methods: Patients with IPD who received a stable dose of anti-parkinsonian medication for at least 4 weeks were recruited and randomly divided into one of two groups: treatment and control. The treatment group (11 subjects) received acupuncture and BVA at acupoints, and the control group (9 subjects) received sham acupuncture and normal saline injections at non-acupoints, twice per week for 12 weeks. The patients were examined using PET and ASL at baseline and after the 12-week treatment. In addition, age- and sex-matched healthy subjects without neurological symptoms and history were recruited to compare ASL data of patients with IPD. Results: PET results revealed that striatal dopamine transporter binding increased in each group after 12 weeks. Although the change was larger in the treatment group, the difference was not statistically significant. In ASL results, the treatment group exhibited hyperperfusion in specific regions compared with the healthy control group. After 12 weeks' intervention, hyperperfusion regions were recovered only in the treatment group. In contrast, significant changes were not found in hyperperfusion regions in the control group after 12 weeks. Conclusions: Our findings suggest that the therapeutic mechanisms of acupuncture and BVA in IPD are different from placebo and operate by altering dopamine availability and recovering hyperactivity in cerebral blood flow.

장시간 부동 자세로 야기된 급성 비골신경 마비 환자의 한방치료 증례보고 (Case Report of Korean Medical Treatment on Acute Peroneal Nerve Palsy Patient Caused by Prolonged Immobilization)

  • Objectives This study was carried out to investigate the clinical effects of Korean medical treatment on acute peroneal nerve palsy caused by prolonged immobilization. Methods One patient with acute peroneal nerve palsy was treated with various korean medical techniques such as acupuncture, moxibustion, cupping and herbal medicine, bee-venom acupuncture for 12 weeks. Manual Muscle Test, Range of motion, Numerical Rating Scale, Ankle-hindfoot scale, Digital infrared thermographing imaging system were used to evaluate treatment effect. Results After treatment, all the scales mentioned above were improved significantly. Conclusions This result showed that Korean medical treatment is effective on acute peroneal nerve palsy caused by prolonged immoblization.

Therapeutic Effect of Aquapuncture with Bee-Venom for Canine Pyoderma

  • The present study was performed to elucidate the therapeutic effect by aquapuncture with bee-venom (apitoxin) for treatment of canine pyoderma. The total 8 dogs with pyoderma were divided into 2 groups as control (antibiotics group: 4 dogs) and experimental groups (apitoxin group: 4 dogs). Antibiotics group was treated with bathing and antibiotics, while apitoxin group was treated with bathing and aquapuncture with apitoxin at Fei Shu (BL13), Wei Zhong (BL40), Da Zhui (GV14), Zu San Li (ST36), He Gu (LI04), Qu Chi (LI11), and San Yin Jiao (SP06). Based on the results of changes in clinical symptoms, total leukocyte counts, neutrophil/lymphocyte ratios and histopathological findings, antibiotics and apitoxin groups were all effective for treatment of canine pyoderma, and significant differences were not detected between the groups. It was suggested that apitoxin might be used as an alternative method to antibiotics for treatment of canine pyoderma.
