Statement of problem. Conventional denture treatment for totally edentulous patients is associated with a variety of functional and psychosocial problems. The placement of implants in the anterior region of the maxilla and mandible and the fabrication of an implant-retained overdenture might solve these problems. Purpose. This study compared the marginal bone loss around the implant and evaluated the implant survival rate and complications in patients treated with overdentures retained by implants for 2 years. Material and methods. Patients who had received implant-retained overdentures using a Dolder bar at Samsung Medical Center from January 1999 to June 2005 and had participated in the annual recall programs for two years were selected for this study. A total of 18 patients and 56 $Br{\aa}ne-mark\;system^{(R)}$ implants were used, and their data were reviewed. Evaluations of the survival rate, bone quality, marginal bone loss, and complications were performed. The data on the Dolder bar length and clip length were measured. The change in marginal bone loss and the correlation between the marginal bone loss and bar length, clip length, or bone quality were investigated. Results. Implants placed in this study showed a 100% survival rate. The average annual bone loss was 1.12mm in the first year and 0.27mm in the second year in the maxilla, and 0.58mm in the first year and 0.22mm in the second year in the mandible. The marginal bone loss in the maxilla showed no significant association with those in the mandible. (P>.05). There was no significant difference in marginal bone loss around implants between the first and second year. (P>.05) There was no statistically significant relationship (P>.05) between the marginal bone loss and bone quality, clip length, or Dolder bar length. The Dolder bar length showed a high correlation with the clip length. (P<.05) Various complications were noted. Conclusion. These results confirmed the favorable outcome for patients treated with implant-retained overdentures.
PURPOSE. The purpose of this study was to determine the effect of anchorage systems and palatal coverage of denture base on load transfer in maxillary implant-retained overdenture. MATERIALS AND METHODS. Maxillary implant-retained overdentures with 4 implants placed in the anterior region of edentulous maxilla were converted into a 3-D numerical model, and stress distribution patterns in implant supporting bone in the case of unilateral vertical loading on maxillary right first molar were compared with each other depending on various types of anchorage system and palatal coverage extent of denture base using three-dimensional finite element analysis. RESULTS. In all experimental models, the highest stress was concentrated on the most distal implant and implant supporting bone on loaded side. The stress at the most distal implant-supporting bone was concentrated on the cortical bone. In all anchorage system without palatal coverage of denture base, higher stresses were concentrated on the most distal implant and implant supporting bone on loaded side. CONCLUSION. It could be suggested that when making maxillary implant retained overdenture, using Hader bar instead of milled bar and full palatal coverage rather than partial palatal coverage are more beneficial in distributing the stress that is applied on implant supporting bone.
PURPOSE. This retrospective study evaluated the outcome of implant-retained overdentures (IODs) after 5-19 years of clinical function. MATERIALS AND METHODS. A retrospective analysis of patient files was performed referring to 27 patients who received 36 IODs with 3 different bar designs (group A=prefabricated round bars, n=7; group B=one-piece anterior milled bars, n=20; and group C=two bilaterally placed milled bars, n=9) in the mandible (n=24) and/or in the maxilla (n=12). The analysis focused on the survival and success rates (according to Kaplan-Meier) of the implants and prostheses. Technical complication rates for each type of restoration were analyzed and compared via one-way ANOVA and the Chi-squared test. The prevalence of peri-implantitis (radiographic bone loss ${\geq}3.5mm$) was evaluated by digital analysis of panoramic radiographs taken postoperative (baseline) and after 5-19 years of clinical function (follow-up). RESULTS. The mean observational time was 7.3 years. The survival rates of the prostheses and implants were 100% and 97.7%, respectively. Technical complications occurred more frequently in group A (mean: 3.5 during observational time) than in the other two groups (B: 0.8; C: 1.0). However, this difference was not statistically significant (P=0.58). Peri-implantitis was diagnosed for 12.4% of the implants in 37% of the patients. CONCLUSION. Bar-retained IODs are an adequate treatment option for edentulous jaws. These restorations may exhibit high implant/prosthesis survival rates (>97%), and a limited incidence of technical complications after a mean observational period of >7 years. Nevertheless, peri-implantitis was identified as a frequent and serious biological complication for this type of reconstruction.
치아 상실에 이어 심각한 치조골 소실을 보이는 무치악 환자들은 전통적인 방식의 총의치를 사용하는데 어려움이 많다. 이러한 문제를 보완할 수 있도록 임플란트를 이용한 피개의치가 사용되고 있다. SFI-bar$^{(R)}$는 진료실에서 직접 조정이 가능한 기성품으로 복잡한 기공과정을 필요로 하는 기존의 바 시스템보다 간결하여 시간과 비용을 절감할 수 있다. 첫 번째 증례는 55세 남자 환자로 기존 하악 임플란트-지지 피개의치가 파절되었다는 주소로 내원하였고 내구성이 있으며 통증이 없는 의치를 원하였다. 파절된 Locator$^{(R)}$ 어태치먼트를 제거 후 바를 이용한 피개의치로 치료하였다. 두 번째 증례는 파킨슨병을 앓고 있는 77세 여자 환자로 하악 치조골이 심각하게 흡수되어 의치가 자주 탈락한다는 주소로 내원하였고 통증이 없고 유지력이 있는 의치를 원하였다. 하악 전치부에 두개의 임플란트를 식립한 후 임플란트에 연결된 어댑터에 바를 연결하였다. 그 후 바에 맞는 여성부를 새로운 의치에 장착하여 치료하였다. 2년의 추적 관찰 결과 상기 환자 모두 심미적, 기능적으로 만족할만한 결과를 보였으므로 증례를 보고하고자 한다.
PURPOSE. This study was accomplished to assess the biomechanical state of different retaining methods of bar implant-overdenture. MATERIALS AND METHODS. Two 3D finite element models were designed. The first model included implant overdenture retained by Hader-clip attachment, while the second model included two extracoronal resilient attachment (ERA) studs added distally to Hader splint bar. A non-linear frictional contact type was assumed between overdentures and mucosa to represent sliding and rotational movements among different attachment components. A 200 N was applied at the molar region unilaterally and perpendicular to the occlusal plane. Additionally, the mandible was restrained at their ramus ends. The maximum equivalent stress and strain (von Mises) were recorded and analyzed at the bone-implant interface level. RESULTS. The values of von Mises stress and strain of the first model at bone-implant interface were higher than their counterparts of the second model. Stress concentration and high value of strain were recognized surrounding implant of the unloaded side in both models. CONCLUSION. There were different patterns of stress-strain distribution at bone-implant interface between the studied attachment designs. Hader bar-clip attachment showed better biomechanical behavior than adding ERA studs distal to hader bar.

Objectives: In mandibular edentulism, the treatment option with a two-interforaminal implant-retained bridge and a removable partial denture is rarely evaluated in literature. The aim of this in vitro study was to evaluate the stress distribution of this treatment option by comparing it with traditional treatment options with interforaminal implants in the edentulous mandible. Materials and Methods: Two interforaminal implants were placed in a formalin-fixed cadaver mandible, and overdentures with three different types of attachments were fabricated: (1) two ball attachments and an overdenture, (2) a Dolder bar and an overdenture, and (3) screw-retained two-implant inter-canine porcelain fused to a metal bridge and an implant-assisted removable denture (IARD) with precision attachments. Three biting conditions were generated for each denture type, and the strains were documented under vertical loading of 100 N. Results: The calculated strain values from measured strains in all measurement sites and loading conditions for the screw-retained two-implant intercanine porcelain fused to a metal bridge and a cast framework partial denture with precision attachments situation were lower than in the other scenarios (P<0.05). Conclusion: Within the limitations of the present study, it can be concluded that an IARD may be a reasonable and valuable alternative to ball attachments or a bar in two interforaminal implants, especially when the patients prefer to be able to show their teeth even when they take out their removable dentures.
상악 완전 무치악에 6개 이상의 임플란트가 식립된 경우 임플란트-지지형 피개의치 제작이 가능하다. 임플란트-지지 피개의치에 사용되는 다양한 부착장치 중에 milled-bar의 경우 의치의 회전을 허용하지 않고, 안정성이 뛰어나며, 여러 개의 임플란트를 연결 고정하는 효과를 가진다는 장점이 있다. 본 증례에서는 전치부에 심한 골소실을 동반한 상악 완전 무치악 환자에서 세 가지 다른 시스템으로 구성된 7개의 임플란트 상부에 CAD/CAM 맞춤형 지대주와 시멘트 접착형의 milled bar를 이용한 피개의치를 제작하여 상실된 심미와 기능을 효과적으로 회복하였다.
최근에, 의치의 유지, 안정성, 지지를 개선시키기 위해 골유착 임플란트가 사용되고 있다. 여러 연구에서 하악에 두 개의 임플란트를 식립한 후 제작된 피개의치가 환자 보철에서 기능과 만족의 유의한 증가를 나타내고 있다. stud, magnet, 그리고 bar attachment를 포함하여 다양한 attachment 시스템을 사용해 임상적으로 예상 가능하고 효과적인 것이 증명되었다. 본 증례에서 2개의 임플란트를 이용한 피개의치가 무치하악의 치료를 위한 고려의 기본이 될 수 있다는 것을 나타낸다.
전통적인 바와 클립 유지형 피개의치의 경우, 인상채득 및 기공과정에서 오차가 생길 수 있어 바가 nonpassive fit하게 제작될 수 있다. 최근에 나온 기성품 바 어태치먼트 시스템인 SFI-Bar (Stress Free on Implant Bar) system (Cendres+Metaux, Biel/Bienne, Switzerland)은 튜브바로 연결하여 조립하는 방식이므로 밀링방식이나 캐스팅방식에서 겪을 수 있는 작업상의 많은 어려움과 긴 작업 시간의 단점을 보완 할 수 있고, 임플란트 주위골에 가해지는 스트레스도 줄여줄 수 있다. 본 증례에서는 골흡수가 심한 상악 무치악 환자에게 최근에 나온 stress free implant bar인 SFI-Bar system을 사용한 피개 의치로 수복하였다. SFI Bar를 이용한 피개의치 제작시 수동적 적합을 얻을 수 있어 임플란트에 스트레스를 덜 가하는 장점을 얻을 수 있을 것으로 생각한다. 추후 정기적인 검사를 통해 장기적인 결과에 대한 평가가 필요하리라 사료된다.
연구 목적: 이 연구의 목적은 텔레스코픽 크라운 하악 임플란트 지지 피개의치와 치아 지지 피개의치에서 지대치의 수와 위치에 따른 응력 분산을 비교하기 위함이다. 연구 재료 및 방법: 본 연구에서는 임플란트를 4개 식립하고 임플란트 지지 피개의치로 설계하였고, 식립 위치를 견치와 소구치 부위에 위치 별로 3개 또는 2개존재시를 실험군으로 설정하였다. 자연치아를 갖는 경우도 견치와 제2소구치 4개를 가진 경우를 대조군으로 설정하고 부위별로 3개 또는 2개를 가지는 경우를 실험군으로 설정하였다. ANSYS Version 10.1(Swanson, Inc., USA)로 분석하였다. 결과: 악골내 응력의 경우, 전반적으로 임플란트(IM)로만 구성된 경우가 치아(TH)로만 구성된 경우에 비해 응력이 크게 발생하였다. 상부구조의 경우, 치아군(TH)과 임플란트군(IM) 사이의 차이는 크게 없었으며 편측 견치와 제2소구치에 지대치 또는 임플란트가 위치하는 경우 가장 큰 응력이 나타났고 바(bar)에서 발생된 응력이 임플란트와 치아에서 발생되는 응력에 비해 상대적으로 훨씬 크게 발생하였다. 지대치와 임플란트의 경우, 치아군(TH)이 임플란트군(IM)보다 응력이 작게 발생하였다. 결론: 본 연구의 결과로부터 지대치(임플란트 또는 치아)를 설정할 때는 하중작용점과 지대치 사이의 거리가 너무 길어지지 않도록 지대치의 수와 위치를 확보해야 하며 소구치 자리에 지대치를 확보하는 것이 유리하다. 앞으로, 실제 임상에 적용하였을 경우, 임플란트 및 자연치아와 악골에 미치는 결과에 대한 연구가 더 필요할 것으로 생각된다.
