Radioisotope Treatment for Benign Strictures of Non-vascular Luminal Organs (비혈관성 관강 장기의 양성 협착 질환의 방사성동위원소 치료)

  • Tissue hyperplasia is one of the most frequently encountered complications when self-expanding metallic stents are placed in benign non-vascular luminal organ strictures, thus causing restenosis of the lumen. The investigators postulated that ionizing irradiation could be applied to prevent restenosis caused by tissue hyperplasia in non-vascular luminal organs as it reduced coronary or peripheral arterial narrowing successfully. The authors combined $\beta$-irradiation using $^{188}Re-MAG_3$ solution with balloon dilation for animal and clinical studies because this new treatment approach had the advantages such as low penetration depth of $\beta$-ray, self-centering irradiation, and mechanical effect of balloon dilation over using $\gamma$-irradiation with afterloading devices in this article, the concept and mechanism of radioisotope balloon dilation, and animal and clinical studies using radioisotope balloon dilation are reviewed.

The Effect of Postural Correction and Subsequent Balloon Inflation in Deformity Correction of Acute Osteoporotic Vertebral Fractures

  • Objective : To determine deformity correction by postural correction and subsequent balloon inflation in acute vertebral compression fractures (OVCFs) and to examine the effect of bone mineral density on deformity correction. Methods : A totol of 50 acute OVCFs received balloon kyphoplasty. Lateral radiographs were taken and analyzed at five different time points : 1) preoperative, 2) after placing the patient in prone hyperextended position, 3) after balloon inflation, 4) after deposition of the cement, and 5) postoperative. All fractures were analyzed for height restoration of anterior (Ha), middle (Hm) and posterior (Hp) vertebra as well as Cobb angle and Kyphotic angle. The bone mineral density (BMD) of lumbar spine was measured by dual-energy X-ray absorptiometry. According to the T-score, the patients were divided into two groups which were osteoporosis group and osteopenia group. Results : Postoperative measurements of Ha, Hm and the Cobb angle demonstrated significant reduction of 4.62 mm, 3.66 mm and $5.34^{\circ}$ compared with the preoperative measurements, respectively (each p<0.05). Postural correction significantly increased Ha by 5.51 mm, Hm by 4.35 mm and improved the Cobb angle by $8.32^{\circ}$ (each p<0.05). Balloon inflation did not demonstrate a significant improvement of Ha, Hm or the Cobb angle compared with baseline prone hyperextended. Postural correction led to greater improvements of Ha, Hm and Cobb angle in osteoporosis group than osteopenia group (each p<0.05). Conclusion : In acute OVCFs, the height restoration was mainly attributed to postural correction rather than deformity correction by balloon inflation. BMD affected deformity correction in the process of postural correction.

An Experimental Investigation of Yarn Tension in Simulated Ring Spinning

  • Yarn tension is a key factor that affects the efficiency of a ring spinning system. In this paper, a specially constructed rig, which can rotate a yam at a high speed without inserting any real twist into the yarn, was used to simulate a ring spinning process. Yarn tension was measured at the guide-eye during the simulated spinning of different yarns at various balloon heights and with varying yarn length in the balloon. The effect of balloon shape, yarn hairiness and thickness, and yam rotating speed, on the measured yarn tension, was examined. The results indicate that the collapse of balloon shape from single loop to double loop, or from double loop to triple etc, lead to sudden reduction in yarn tension. Under otherwise identical conditions, a longer length of yarn in the balloon gives a lower yarn tension at the guide-eye. In addition, thicker yarns and/or more hairy yarns generate a higher tension in the yarn, due to the increased air drag acting on the thicker or more hairy yarns.

Relation between Ablation Execution Time and Radiation Exposure Effect in the Treatment of Atrial-fibrillation using Cryo-balloon and 3D Radio-frequency Ablation (냉각 풍선 절제술과 3D 고주파 절제술을 이용한 심방세동 치료 시 절제술 시행 시간과 방사선 피폭 영향과의 연관성)

  • Atrial fibrillation treatment includes 3D RFCA and Cryo-balloon ablation. Both procedures have in common that they enter after understanding the structure of the heart using angiography equipment. Therefore, there is a disadvantage that the effect of exposure according to the procedure time can be a threat to both the patient and the operator, so this study aims to confirm the relationship between the total ablation time and the effect of radiation exposure. We used follow-up data (retrospective) from 41 patients who underwent coronary angiography and arrhythmia at the same time from March 2019 to July 2022. The range for total ablation time was based on the recorded data from the start to the end of the total ablation. The end point of 3D RFCA was when the ablation was completed for 4 pulmonary veins, and in the case of Cryo-balloon ablation, the data that succeeded in electrical insulation were included. As a result of analyzing the total ablation time, the time taken for Cryo-balloon ablation was 1037.29±103.66 s, which was 2448.61 s faster than 3D RFCA using 3485.9±405.71 s, and was statistically significant. (p<0.05) As a result of analyzing the total fluoroscopy time, the exposure time for 3D RFCA was 2573.75±239.08 s, which was less by 1717.15 s than the exposure time for Cryo-balloon ablation, 4290.9±420.42 s, and was statistically significant. In the case of total area dose product, 3D RFCA was 59.04±13.1 uGy/m2, which was lower than Cryo-balloon ablation 980.6±658.07 uGy/m2 by 921.56 uGy/m2, which was statistically significant. As the insulation time of the Cryo-balloon ablation is shorter than that of the 3D RFCA, the method using the Cryo-balloon ablation is considered to be effective when the patient's condition is not good and a quick procedure is required. However, in patients with permanent Atrial fibrillation, there is a high probability of structural changes in the heart, so it is considered that 3D RFCA is better than Cryo-balloon ablation, which is difficult to manipulate.

Clinical Analysis Comparing Efficacy between a Distal Filter Protection Device and Proximal Balloon Occlusion Device during Carotid Artery Stenting

  • Objective : The main concern during transfemoral carotid artery stenting (CAS) is preventing cerebral embolus dislodgement. We compared clinical outcomes and intraprocedural embolization rates of CAS using a distal filter protection device or proximal balloon occlusion device. Methods : From January 2011 to March 2015, a series of 58 patients with symptomatic or asymptomatic internal carotid artery stenosis ${\geq}70%$ were treated with CAS with embolic protection device in single center. All patients underwent post-CAS diffusion-weighted magnetic resonance imaging (DW-MRI) to detect new ischemic lesions. We compared clinical outcomes and postprocedural embolization rates. Results : CAS was performed in all 61 patients. Distal filter protection success rate was 96.6% (28/29), whose mean age was 70.9 years, and mean stenosis was 81%. Their preprocedural infarction rate was 39% (11/28). Subsequent DW-MRI revealed 96 new ischemic lesions in 71% (20/28) patients. In contrast, the proximal balloon occlusion device success rate was 93.8% (30/32), whose mean age was 68.8 years and mean stenosis was 86%. Preprocedure infarction rate was 47% (14/30). DW-MRI revealed 45 new ischemic lesions in 57% (17/30) patients. Compared with distal filter protection device, proximal balloon occlusion device resulted in fewer ischemic lesions per patient (p=0.028). In each group, type of stent during CAS had no significant effect on number of periprocedural embolisms. Only 2 neurologic events occurred in the successfully treated patients (one from each group). Conclusion : Transfemoral CAS with proximal balloon occlusion device achieves good results. Compared with distal filter protection, proximal balloon occlusion might be more effective in reducing cerebral embolism during CAS.

Effects of Posture Correction Training on Pulmonary Function with Turtle Neck Syndrome (자세교정훈련이 거북목증후군의 폐기능에 미치는 영향)

  • Purpose : The purpose of this study was to investigate the impact of postural correction training on pulmonary function on 28 college students suspected of turtle neck syndrome, and the following conclusions were obtained. Methods : Turtle neck syndrome suspicion 28 person were randomly divided into a posture training group (n = 14) and group that does not perform posture training (n = 14). Respiratory function was measured by SPIROVIT SP-1 and respiratory gas analyzer. The posture training group performed balloon blowing and stair climbing after 20 minutes of posture training, and the group without posture training carried out balloon blowing training and stair climbing. Five times a week and for two weeks. Results : 1. The comparison of the FVC before and after experiments caused by balloon blowing showed a higher level of effortful pulmonary function in the control group than in the experimental group. 2. Comparison of PEFs before and after the experiment by balloon blowing showed that the experimental group's peak flow rate was higher than that of the control group. 3. Comparison of the FIVC before and after experiments with balloon blowing showed that the comparison of the FIVC showed a higher level of effortless intake pulmonary function in the control group than in the experimental group. 4. The comparison of the maximum ventilation volume(VE) before and after the experimental gas measurement showed that the maximum ventilation rate of the experimental group was higher than that of the control group. 5. The comparison of pre-test and post-test heart rate(HR) by breath gas measurement showed that the heart rate of the control group was higher than that of the experimental group. Conclusion : the results of this study showed that postural correction training, balloon blowing training, and stair climbing could have a positive impact on improving pulmonary function. However, the two-week experiment conducted five times a week showed an increase in pulmonary function, but it was difficult to see the effect due to the short study period. Therefore, it is hoped that later studies will be conducted more systematically on the effects of breathing exercises on improving pulmonary function after post-postural correction training for patients with pulmonary function problems.

Preventive Effect of Traditional Korean Formulations on Intimal Thickening of Rat Carotid Artery Injured by Balloon Catheter

  • The objective of this study was to examine the effect of 7 traditional Korean formulations (7TKFs) on intimal thickening of rat carotid artery injured by balloon catheter in vivo and on the proliferation of human smooth muscle cells (HASMCs) and secretion of matrix metalloproteinase-2 (MMP-2) in vitro. 7TKFs (400 mg/kg) were administered orally for 4 weeks from the day of balloon injury in the rats. HASMC proliferation was assessed by 3-(4,5-dimethylthiazol-2)-2,5-diphenyltetrazolium bromide (MTT) assay while enzymatic action of MMP-2 was carried out by gelatin zymography. Among 7TKFs, Samhwang-sasim-tang (SST), Banha-sasim-tang (BST) and Kegi-honghwa-tang (KHT) significantly reduced the intimal thickening by suppressing HASMC proliferation and MMP-2 expression in both extracellular and intracellular levels. Thus, the results suggest that SST, BST and KHT can be considered as a therapeutic value in the prevention of atherosclerosis because restenosis after PTCA (percutaneous transluminal coronary angioplasty) is supposed to be 'accelerated atherosclerosis'.

  • The purpose of Demand Response is to reduce the cost of excessive resources and equipment by spontaneous load reductions at peak loads. Having enough power consumers participating in these schemes is key to achieving the goal. Demand Response Aggregator (DRA) is responsible for recruiting demand resources and managing them to participate in reducing the load. DRAs change the price elasticity of demand functions by providing incentives to demand response, thereby affecting price formation in the electricity market. In this paper, this process is modeled to analyze the relationship between DRA's strategic bidding and market outcomes and load reductions. It analyzes the results by applying to competition between DRAs, competition between DR and Gencos, and coexistence of DR load and non-DR load. It is noteworthy that we have found a phenomenon called the Balloon Effect.

A Comparison of Acute Effect of Air Stacking Using Resuscitator Bag versus Balloon on Pulmonary Functions (도수 소생기와 풍선을 이용한 공기 누적이 폐 기능에 미치는 즉각적인 효과 비교)

  • Purpose : To compare the immediate effects of air stacking maneuver using resuscitator bags and balloons. Methods : Twenty healthy young adults participated in this study. Forced vital capacity (FVC) and peak cough flow (PCF) tests were performed at pre-intervention, and then, the maximum insufflation capacity (MIC), forced expiratory volume in 1 s (FEV1), FEV1/FVC, peak expiratory flow (PEF), and peak cough flow (PCF) were measured using the air stacking maneuver via resuscitator bags and balloons. Interventions were randomly performed, and a 40-min break was provided between interventions. The evaluation process in this study was conducted in accordance with the guidelines of the American Thoracic Society (ATS) 2019. To compare the three outcomes measured at pre-and post-interventions, repeated measures analysis of variance was performed. Results : A significant difference was found in the MIC, FEV1, PEF, and PCF after the air stacking maneuver using resuscitator bags and balloons, whereas no significant difference was observed between resuscitator bags and balloons. Conclusion : No significant difference was found in the immediate effect of the air stacking maneuver using resuscitator bags and balloons in this study. Air stacking maneuver using balloons can increase the success rate of the techniques by providing visual feedback on the amount of air insufflation when performed with balloon blowing exercise. Balloons are cheaper and easier to buy compared to manual resuscitator bags; therefore, education on the air stacking maneuver using balloons will have a positive effect on pulmonary rehabilitation.

Effects of Alachlor and Pendimethalin on Germination and Growth of Balloon Flower (Platycodon grandiflorum) (알라클로르와 펜디메탈린이 도라지의 발아 및 생육에 미치는 영향)

  • Balloon flower (Platycodon grandiflorum) is one of minor crops in Korea. Only four ACCase inhibiting herbicides are registered for P. grandiflorum and the farmers usually use general but unregistered herbicides which can cause critical phytotoxicity. So, this study was conducted to investigate effects of alachlor and pendimethalin on germination and early growth of P. grandiflorum. To evaluate the effect of alachlor and pendimethalin, two herbicides were treated to the seeds in petri-dish and soil. In the petri-dish, alachlor completely inhibited the germination as well as pendimethalin inhibited the germination slightly but not significantly. In case of soil application, alachlor inhibited germination and plant height of P. grandiflorum significantly as well as pendimethalin inhibited plant height and root length significantly. We showed the possibility of phytotoxicity of alachlor and pendimethalin to P. grandiflorum and strongly suggest that two herbicides should not be used to the cultivation of P. grandiflorum. Unregistered herbicides should not be used for weed management and additional researches for screening of safe herbicide to minor crops should be conducted as soon as possible.
