수행에 대한 지식의 상대적 빈도가 편마비 환자의 균형 재훈련에 미치는 영향 (Effects of Relative Frequency of Knowledge of Performance on Balance Retraining in Patients With Hemiplegia)

  • During therapy sessions, feedback is often provided concurrently by the physical therapist as the patient attempts to perform a movement and after the movement attempt. This feedback is provided to enhance the patient's balance abilities. However, recent studies in nondisabled populations have suggested that frequent feedback may be detrimental to retention or learning of motor skills. This study compared the effects of 100% relative frequency of knowledge of performance (KP) with 66% relative frequency of KP for motor learning on balance retraining in patients with hemiplegia. Twenty patients with hemiplegic were randomly assigned to one of two experimental groups. The acquisition phase consisted of 16 blocks of 5 trials for 2 days (80 total practice trials). The retention phase consisted of 2 blocks of a short-term retention test, one day after the end of the acquisition phase and a long-term retention test, one week after the end of the short-term retention test. In the 100% feedback condition, participants received feedback after every practice trial. A faded KP schedule was used in the 66% condition. No significant differences were found between the two groups during all experimental phases (acquisition and retention phases), (p>.05). However, there were significant decreases in balance index for both groups of acquisition phase (p<.05). These results suggest that 66% relative frequency of KP is not more effective than 100% relative frequency of KP with respect to retention over time when hemiparetic patients attempt to learn balance.

트레드밀운동이 뇌졸중 환자의 걷기기능향상에 관한 문헌연구 (Effects of Treadmill Training on Gait of Stroke Patients : A Systematic Review)

  • Background : Treadmill training has been proposed as a useful adjunct to conventional physical therapy to restore ability to walk after stroke. The purpose of this study to inform clinical practise by evaluating the research evidence for the effectiveness of treadmill training after stroke. Methods : We searched to the effectiveness of any form of intervention for effect of treadmill training by Dankook University electronic library databases of Medline, Embase, Cinahl, Amed and PEDro combined with a hand search of papers published in relevant peer-reviewed journals. Any type of study relevant to the topic published in English during time period from 1980 to 2007 was included. Results : The literature search identified 35 studies. The included studies enrolled a total of 374 subjects. 1. Treadmill retraining without partial body weight support might be more effective than no treatment at improving gait velocity, get up and go time, gait endurance and step length symmetry. 2. Treadmill retraining with partial body weight support might be more effective than no treatment in improving step length symmetry, gait velocity, gait endurance and balance. 3. Treadmill retraining might be more effective with partial body weight support than without it at improving gait velocity and motor improvement as measured by The Stroke Rehabilitation Assessment of Movement(STREAM). 4. Treadmill retraining without partial body weight support may be no different from physiotherapy and increase gait velocity to conventional gait therapy. 5. Treadmill retraining with partial body weight support may be no different from physiotherapy for gait velocity, motor recovery and balance. Conclusion The review suggests that although treadmill training of gait, especially with partial body weight support, might Improve gait parameters and functional mobility, unless treadmill training is directed at Improving gait speed it might be no more effective than conventional physical therapy at improving gait parameters.

고유수용성 조절이 만성 편마비 환자의 균형에 미치는 영향 (Effects of Proprioceptive Control on the Balance in Patients With Chronic Hemiplegia)

  • Hemiplegic patients usually present with difficulty maintaining balance. Balance retraining is the major component of rehabilitation program for patients with neurological impairment. The purpose of this study was to investigate the effects of prorpioceptive exercise program on the improvement of balance in the patients with chronic hemiplegia. Thirty subjects (mean age $57.0{\pm}9.8$) were recruited and the subjects were divided into a proprioceptive group and a visual group. The subjects for the proprioceptive group were participated in the proprioceptive exercise program for 4 weeks, and the visual group were treated visual feedback training using Balance Master. At 4 week follow-up test, Berg Balance Scale significantly improved 1.1 points (p<.01), Timed Up & Go test improved 4.2 second (p<.01), and weight distribution during sit to stand also improved 5.0% (p<.01). As a result of this study, the proprioceptive control approach improved dynamic balance in the patients with chronic hemiplegia. It is suggested that there was no benefit of visual feedback training like as Balance Master when administrated in combination with other physical therapy interventions, compared with physical therapy alone using proprioceptive control approach to hemiplegia.

However, improvement was only observed in the forward direction in the visual feedback group. Therefore, the proprioceptive control approach improves the LOS in chronic hemiplegia patients. Conclusion: These results suggest that compared with physical therapy alone using a proprioceptive control approach to hemiplegia, there was no additional benefit of visual feedback training, such as Balance Master, when administrated in combination with other physical therapy interventions.

뇌졸중 환자의 균형능력 회복을 위한 가정운동프로그램 효과 (The Effects of Home Based Exercise Program on Balance Recovery in a Post-Stroke Population)

  • 본 연구는 가정운동프로그램이 뇌졸중 환자의 균형능력회복에 미치는 효과를 알아보기 위하여 실시하였다. 본 연구는 2013년 9월에서 2013년 12월까지 실시하였으며, 대상자는 총 20명으로 일반적인 통원 물리치료를 실시한 조군 10명, 일반적인 통원 물리치료를 실시하고 추가적으로 가정운동프로그램을 실시한 운동군 10명으로 나누어 배정하였다. 가정운동프로그램은 신경계 물리치료사가 대상자 집을 방문하여 8주동안 주 2회씩 매회 30분간 시행하였으며 체중이동훈련, 지구력훈련, 운동재훈련, 감각재훈련, 하지운동으로 구성되었다. 균형능력을 평가하기 위하여 Biorescu를 이용하여 족저압, 안정성한계, 동요속도를 측정하였고 임상적인 측정방법 중 하나인 기능적 팔뻗기 검사를 실시하였다. 연구결과 대조군과 운동군은 안정성한계, 동요속도, 기능적 팔뻗기 검사에서 유의한 차이가 나타났다. 본 연구는 가정운동프로그램이 뇌졸중 환자의 균형회복을 회복하는데 효과적인 치료전략 중 하나라는 것을 나타낸다.
