근전도 신호를 이용한 운동 처방 장치 개발 (Development of a Excercise Prescription Device using EMG Signal)

  • 본 논문은 인체 근육의 생체 전기 분석을 통한 근육 운동 처방 시스템을 개발하고 그 처방법을 제시한 것이다. 이 시스템은 근전도(EMG)를 통한 개인별 몸 상태를 우선 진단한 후 이를 근거로 운동형태, 운동 강도, 운동시간, 운동 빈도 등을 포함하는 정확한 운동처방이 이루어지도록 설계되었다. 이에 따라 사용자가 최적의 운동 상태를 유지할 수 있도록 하여 과사용 증후군과 같은 부작용을 없애 각종 의료사고 예방하고, 틀어진 자세 측정, 근육의 근력, 근육의 경직상태, 근육의 피로한 상태, 근육의 밸런스 측정 등 여러 방법으로 활용이 가능하도록 한다. 또한, 편리한 사용방법으로 전문 의료기관이 아닌 헬스장, 스포츠센터, 복지관, 보건소, 학교, 유치원 등에서도 누구나 손쉽게 활용할 수 있도록 한 것이다.

Effect of Deep Lumbar Muscle Stabilization Exercise on the Spatiotemporal Walking Ability of Stroke Patients

  • Background: Walking is a complex activity. The main components of walking include balance, coordination, and symmetrical posture. The characteristics of walking patterns of stroke patients include slow walking, measured by gait cycle and walking speed. This is an important factor that reflects post-stroke quality of life and walking ability. Objective: This study aimed to examine the effect of deep lumbar muscle stabilization exercise on the spatiotemporal walking ability of stroke patients. Design: Quasi-experial study Methods: The experiment was conducted 5 times per week for 4 weeks, with 30 minutes per session, on 10 subjects in the experimental group who performed the deep lumbar muscle stabilization exercise and 10 subjects in the control group who performed a regular exercise. Variables that represent the spatiotemporal walking ability (step length, stride length, step rate, and walking speed) were measured using GAITRrite before and after the experiment and were analyzed. Results: There was a significant difference in the pre- and post-exercise spatiotemporal walking ability between the two groups (p<.05). Furthermore, there was a significant difference in the step rate and walking speed between the two groups (p<.05). Conclusions: Deep lumbar muscle stabilization exercise is effective in improving the walking ability of stroke patients. Therefore, its application will help improve the spatiotemporal walking ability of stroke patients.

Effect of Intensive Ankle Training Using Biofeedback on Normal Adults' Ability to Control Weight Distribution

  • Objective: The purpose of the present study is to investigate whether intensive ankle training using biofeedback to increase proprioceptive senses and ankle-joint muscle strength effectively improves the balance of normal adults. Method: This study included 20 voluntary participants with normal adults. The subjects were randomly divided into 2 groups: the biofeedback intensive ankle training group (n=10) and the ankle training group (n=10). The biofeedback group used a visual biofeedback program. Subjects of the group that underwent ankle training only tried to keep their bodies balanced as well as possible on an Aero-Step. Both groups performed the exercise equally for three 30-minute sessions weekly for six weeks. The differences between the measurements before and after the experiment were analyzed using Wilcoxon signed-rank tests. Mann-Whitney U tests were used to analyze the differences in variations between the groups. Results: The biofeedback group showed significant differences in the weight distributions A, B, C, D, LEFT, and HEEL and on the weight-distribution index. In contrast, the ankle training group showed significant differences in the weight distributions A, C, and LEFT. A comparison of the differences between the two groups found significant differences between them in the weight distributions A, B, C, D, LEFT, and HEEL. Conclusion: The biofeedback group showed a greater ability to control weight distribution. A longer experimental period involving more subjects would aid in developing exercise programs that are more diverse.

Effects of Combined Functional Electrical Stimulation and Joint Mobilization on Muscle Activation and Mobility of Ankle Joints and Modified Functional Reach Test in Stroke Patient

  • PURPOSE: This study was conducted to investigate the effects of combined Joint Mobilization and Functional Electrical Stimulation on Muscle Activation and Mobility of ankle joints in stroke patients and their Modified Functional Reach Test (MFRT) results. METHODS: A total of 26 patients with stroke were randomly selected for enrollment in this study. (1) Functional Electrical Stimulation (FES) (2) combined Joint Mobilization and FES. An EMG system was used to measure tibialis anterior and gastrocnemius activities. Range Of Motion (ROM) of Ankle Joint and MFRT for Dynamic Balance. Pre and post intervention results were compared by paired-t-tests and differences in changes after intervention between groups were identified by the independent t-test. RESULTS: The muscle activation, ROM, and MFRT differed significantly in the experimental group (p<.05). The ROM was significantly different for the active dorsiflexion pre and post intervention in the group that received FES alone (p<.05). CONCLUSION: The results of this study suggest use of a systematic program of proactive posture control to prevent dysfunction when planning interventions for ankle joints can help stroke patients walk efficiently.

상시 근골격 모니터링과 재활을 위한 온스킨 센서 디바이스 기술 (Imperceptible On-Skin Sensor Devices for Musculoskeletal Monitoring and Rehabilitation)

  • As the society is superaging, the number of patients with movement disabilities due to musculoskeletal or nervous system illness is rapidly increasing. To improve public health and reduce medical expenses, it is essential to develop rehabilitation systems that allow patients to resume their daily-life activities. However, the existing musculoskeletal illness diagnosis and rehabilitation method is limited in terms of precision and efficiency because it is based on an empirical diagnosis and prescription without regard for individual characteristics. To overcome these limits, it is critical to design a novel concept of routine rehabilitation therapy device that is capable of inducing musculoskeletal balance by the precise analysis of musculoskeletal usage patterns via the motion and the muscle activity tracking of linked muscles. This study introduces the trend of on-skin sensor device technology for routine musculoskeletal monitoring and therapy. For on-skin rehabilitation systems, skin-adhesive and stretchable motion/posture, electromyography, pressure sensors, small-size and low-power wireless sensor interfaces, and user-friendly rehabilitation contents based on new algorithms are combined.

Correlation among Functional Leg Length Discrepancy, Muscle Activity, Muscle Contraction Onset Time and Vertical Ground Reaction Force during Simple Lifting Task

  • Purpose: Leg length discrepancy causes the posture deformation, gait asymmetry, and lower back pain. The purpose of this study is to investigate the correlation among functional leg length discrepancy (FLLD), muscle activity, muscle contraction onset time and vertical ground reaction force (vGRF) during simple lifting task. Methods: Thirty-nine subjects participated in this study. FLLD was measured from the umbilicus to medial malleolus of left and right leg using a tape. The subjects performed to lift a 10 kg box from the floor to chest. The muscle activity and muscle contraction onset time of rectus abdominis, erector spinae and rectus femoris was measured using EMG system and vGRF was measured by two force plate. Pearson correlation was used to fine out the correlation among FDDL, muscle activity, muscle contraction onset time and vGRF during simple lifting task. Results: Correlation between FLLD and difference of muscle activity of short-long side was very high (r>0.9) during simple lifting task. Correlation between FLLD and difference of muscle contraction onset time of short-long side was very high (r>0.9) during simple lifting task. And correlation between FLLD and difference of vGRF of short-long side was high (r>0.7) during simple lifting task. Conclusion: This study suggests that there is high correlation between FLLD and muscle activity, muscle contraction onset time, and ground reaction force during simple lifting task. Therefore, FLLD could negatively affect the postural balance.

Comparison of the Changes in the Activation of the Quadriceps Muscle based on the Plantar Flexion Degree of the Ankle Joint in Healthy Young Females during the Stand-to-Sit movement

  • Purpose: The purpose of this study was to compare the changes in the muscle activation of the quadriceps muscle (rectus femoris, vastus lateralis, vastus medialis) during the stand-to-sit (StandTS) movement according to the plantar flexion angle of the ankle joint. Methods: A total of 22 healthy young females participated in this study. During the StandTS under the three conditions (plantarflexion angle 0°, 20°, and 45° of the ankle), electromyography (EMG) data (% maximum voluntary iso¬metric contraction) of the rectus femoris, vastus lateralis, and vastus medialis were recorded using a wireless surface EMG system. Results: There was a significant difference in the muscle activation of rectus femoris, vastus lateralis, and vastus medialis according to the plantar flexion angle (0°, 20°, and 45°) of the ankle. The muscle activation of the quadriceps was the highest at a 45° angle of plantarflexion and the lowest at 0°. One-way repeated ANOVA was used to analyze the muscle activation data of the lower extremity muscles according to the angle of the ankle joint. Conclusion: Based on the results of our study, it was confirmed that the muscle activity of the quadriceps can be increased even in the StandTS movement, which involves the eccentric contraction of the quadriceps muscle. This suggests that maintaining a plantar flexion posture for a long time, say by wearing high-heeled shoes, can quickly cause muscle fatigue in the lower-limb muscles, which can cause a decrease in balance ability leading to falls.

'Lumbar Degenerative Kyphosis' Is Not Byword for Degenerative Sagittal Imbalance : Time to Replace a Misconception

  • Lumbar degenerative kyphosis (LDK) is a subgroup of the flat-back syndrome and is most commonly caused by unique life styles, such as a prolonged crouched posture during agricultural work and performing activities of daily living on the floor. Unfortunately, LDK has been used as a byword for degenerative sagittal imbalance, and this sometimes causes confusion. The aim of this review was to evaluate the exact territory of LDK, and to introduce another appropriate term for degenerative sagittal deformity. Unlike what its name suggests, LDK does not only include sagittal balance disorder of the lumbar spine and kyphosis, but also sagittal balance disorder of the whole spine and little lordosis of the lumbar spine. Moreover, this disease is closely related to the occupation of female farmers and an outdated Asian life style. These reasons necessitate a change in the nomenclature of this disorder to prevent misunderstanding. We suggest the name "primary degenerative sagittal imbalance" (PDSI), which encompasses degenerative sagittal misalignments of unknown origin in the whole spine in older-age patients, and is associated with back muscle wasting. LDK may be regarded as a subgroup of PDSI related to an occupation in agriculture. Conservative treatments such as exercise and physiotherapy are recommended as first-line treatments for patients with PDSI, and surgical treatment is considered only if conservative treatments failed. The measurement of spinopelvic parameters for sagittal balance is important prior to deformity corrective surgery. LDK can be considered a subtype of PDSI that is more likely to occur in female farmers, and hence the use of LDK as a global term for all degenerative sagittal imbalance disorders is better avoided. To avoid confusion, we recommend PDSI as a newer, more accurate diagnostic term instead of LDK.

PID 알고리즘을 이용한 역 진자 시스템의 자세 제어에 관한 연구 (A Study on Pose Control for Inverted Pendulum System using PID Algorithm)

  • 현재 도립진자는 많은 분야에서 연구 중이며 미사일, 로켓, 등의 자세 제어와 2족 보행 로봇 등에 응용되고 있다. 본 연구에서는 256펄스의 로터리 엔코더와 DC 모터를 이용한 회전형 도립진자(Rotary Inverted Pendulum)를 구성하여 회전형 역 진자의 수직 자세 제어를 연구하였다. 비선형 시스템의 경우 복잡한 알고리즘과 제어기가 필요하지만 고전적이며 비교적 간단한 PID(Proportional Integral Derivation)알고리즘을 이용한 제어 방법을 회전형 도립진자 시스템에 적용하였으며 간단하지만 원하는 성능을 높이는 방안을 연구하였다. 본 연구에서 사용된 회전형 도립진자 시스템은 비선형적이고 불안정한 시스템으로 선형화된 모델링에서 마이크로칩 사의 dsPIC30F4013 임베디드 프로세서를 이용한 PID 제어기를 설계 및 구현하였다. 보통 PID 제어기는 하나 혹은 두 가지 이상을 조합하여 설계하며 우수한 제어 성능에 비해 구조가 간단하며 제어 이득 조정이 다른 제어기들에 비해 비교적 쉽다는 장점이 있다. 본 연구에서는 시스템의 물리적 구조를 수학적 방법으로 분석하고 모델링을 통한 회전형 도립진자의 수직 균형을 위한 제어를 실현하였다. 또한 회전형 역 진자를 이용하여 PID 제어기로 제어가 가능한지 시뮬레이션과 실험을 통하여 그 타당성을 검증하였다.

Muscle Activities in the Lower Limbs for the Different Movement Patterns on an Unstable Platform

  • We performed experimental studies on the muscle activities in the lower limbs for the different movement patterns on an unstable platform. A training system for postural control using an unstable platform that we previously developed was applied for the experiments. This unstable platform provides 360 degrees of movement allowing for training of posture in various directions and provides simultaneous excitations to visual sensory, somatic sensation and vestibular organs. Compare with the stable platform, keeping body balance on the unstable platform requests more effective sensation from vision, vestibular sense and somatic sense. Especially, the somatosensory inputs from the muscle proprioceptors and muscle force are crucial. To study the muscle activities for the different movement patterns and find the best training method for improving the ability of postural control through training and improving the lower extremity muscular strength, fifteen young healthy participants went through trainings and experiments. The participants were instructed to move the center of pressure following the appointed movement pattern while standing on the unstable platform. The electromyographies of the muscles in the lower limbs were recorded and analyzed in the time and the frequency domain. Our experimental results showed the significant differences in muscle activities for the different movement patterns. Especially, the spectral energy of electromyography signals in muscle for the movement pattern in anterior-posterior direction was significantly higher than those occurred in the other patterns. The muscles in the lower leg, especially tibialis anterior and gastrocnemius were more activated compared to the others for controlling the balance of body on the unstable platform. The experimental results suggest that, through the choice of different movement pattern, the training for lower extremity strength could be performed on specific muscles in different intensity. And, the ability of postural control could be improved by the training for lower extremity strength.
