본태성고혈압(本態性高血壓)에서의 혈장(血漿) Renin 활성(活性)에 관(關)한 연구(硏究) (A Study on Plasma Renin Activity in Essential Hypertension)

  • Radioimmunoassay for the measurement of plasma renin activity (PRA) was performed in 43 normal Koreans and 45 patients with essential hypertension. Plasma samples were drawn in supine position in the morning and after upright posture for 4 hours. Urinary sodium excretion rates were measured in the concurrent 24 hour urine samples, as an index of their sodium balance. The results were as follows: 1. There was an inverse correlation between 24hr sodium excretion and PRA. The normal values of PRA in supine position ranged from 1.0 to 7.0 ng/ml/hr. when 24 hour sodium excretion were between 50 to 150 mEq. PRA in elderly tended to be low. 2. When stimulated by 4 hour upright posture, PRA increased by 2.6 times from the baseline value. 3. Of the 45 patients with essential hypertension, PRA was low in 10 cases (22.2%), normal in 28 cases (62.2%), and high in 7 cases (15.6%). 4. In the normal and high renin groups, who tended to be younger in ages, mean diastolic blood pressure and BUN were higher than in low renin group. Though hypertensive retinopathy and left ventricular hypertrophy in ECG were more prevalent in the former, no significant differences were noted as in the case of serum cholesterol. 5. There were 8 cases of cardiovascular complications (7 with cerebral vascular accident, 1 with myocardial infarction); 3 in low renin group (30%), 2 in normal renin (7.1%) and 3 in high renin group (42.9%). This figure indicated higher rate of cardiovascular complications in high renin groups, and lower rate in normal renin group. But the incidence of the complication was not significantly low in low renin group.

Relationship between anticipatory postural adjustment of the trunk, dual tasks and physical performance with chronic stroke survivors: a pilot test

  • Objective: The purpose of this study was to investigate the relationship between anticipatory postural adjustment (APA), single task, dual tasks and physical performances. The trunk muscles of APA consist of bilateral erector spinae (ES) and bilateral internal oblique (IO) adnominal muscles, during rapid stepping with the affected or unaffected leg in a sitting posture. Design: Cross-sectional study. Methods: In patients with chronic stroke, electrodes of surface electromyography (EMG) were attached on the bilateral erector spinae (ES), bilateral internal oblique adnominal (IO), and bilateral rectus femoris (RF) muscles. RF acts as the prime mover. The stroke patients performed hip flexion until $20^{\circ}$ as fast as possible at each leg in a sitting posture according to a visual cue. The visual cue unexpectedly appeared on monitor in front of the stroke patient. The single task was the Timed Up and Go (TUG) test. The dual tasks were the TUGconitive, which increased cognitive capacity, and the TUGmanual task, which had an external focus. Results: All EMG data showed earlier onset latency before the prime mover. In affected leg raising, the onset time of unaffected ES muscle of the stroke patients was correlated with the single and dual tasks (p<0.05). In unaffected leg raising, the onset time of the affected IO muscle was related to all the tasks (p<0.05). Gait speed showed a relationship with the unaffected ES muscle only. Conclusions: The trunk muscles of the bilateral ES and bilateral IO play an important role in APA. The single and dual tasks using TUG test were correlated with the APA s of ES and IO muscles. Dual task by the TUG test is a good measuring tool for reflecting the real life in patients with chronic stroke.

Human Postural Dynamics in Response to the Horizontal Vibration

  • The dynamic responses of human standing postural control were investigated when subjects were exposed to long-term horizontal vibration. It was hypothesized that the motion of standing posture complexity mainly occurs in the mid-sagittal plane. The motor-driven support platform was designed as a source of vibration. The AC Servo-controlled motors produced anterior/posterior (AP) motion. The platform acceleration and the trunk angular velocity were used as the input and the output of the system, respectively. A method was proposed to identify the complexity of the standing posture dynamics. That is, during AP platform motion, the subject's knee, hip and neck were tightly constrained by fixing assembly, so the lower extremity, trunk and head of the subject's body were individually immovable. Through this method, it was assumed that the ankle joint rotation mainly contributed to maintaining their body balance. Four subjects took part in this study. During the experiment, the random vibration was generated at a magnitude of $0.44m/s^2$, and the duration of each trial was 40 seconds. Measured data were estimated by the coherence function and the frequency response function for analyzing the dynamic behavior of standing control over a frequency range from 0.2 to 3 Hz. Significant coherence values were found above 0.5 Hz. The estimation of frequency response function revealed the dominant resonance frequencies between 0.60 Hz and 0.68 Hz. On the basis of our results illustrated here, the linear model of standing postural control was further concluded.

  • The final goal for an orthognathic surgery is a functional and esthetic achievement based on occlusion theory. All the dental treatment should be done with the occlusion in mind, though, they tend to be ignored with no good reason. We cannot think of occlusion without temporomandibular joint because it is the first clue to define an occlusion. As normal occlusion comes from the central tendency of distribution of population, we can get it by examining the population that closely meet the criteria of ideal occlusion. To perform proper occlusal function and to maintain the stability after treatment, the case must be finished in normal occlusion closer to ideal one. Our aim is to achieve the ideal occlusal scheme like the mutually protected occlusion with the best masticatory efficiency and the stability. The facial esthetics are influened by culture, race and the time in which human live. While the occlusal function rarely changes as time goes by, esthetics tend to do from one country to another. Orthodontists and maxillofacial surgeons should have solid sets of treatment goals to achieve the best facial esthetics and the ideal occlusion dictated by the joint. Doing orthognathic surgery, two factors aforementioned should be taken into account to establish the Surgical Treatment Objectives(STO). The doctors who are planning orthognathic surgery need to have a very logical and systematic thought process to make STO. The author examined 28 selected beautiful Korean female adults with normal skeleton with normal occlusion and analyzed the hard and the soft tissue relationship into five parts : dentomaxillary relationship, intermaxillary relationship, posture to hard tissue relationship, facial balance, and posture to soft tissue relationship. This study presents a sequential flow of diagnosis and treatment planning especially for surgical patients and it also can be applied to the nonsurgical patients.

Comparative Immediate Effects of Isometric Chin-tuck and Dynamic Neuromuscular Stabilization on Neck Flexor Muscle Thickness and Upright Sitting Height Posture

  • Background: Cervical dysfunction is a common pathomechanical marker in individuals with forward head posture (FHP). To overcome the limitations of the isometric chin-tuck (ICT) exercise, dynamic neuromuscular stabilization (DNS), which emphasizes an entire spinal chain exercise, has recently shown promising clinical results. Objects: Purpose of this study was to compare the immediate effects between ICT and DNS techniques. Methods: 43 young subjects (mean age, $24.0{\pm}5.0$ years) were recruited. Group of subjects with FHP were measured under baseline, ICT, and DNS conditions. Outcome measures included sitting height, longus colli (LC) and sternocleidomastoid (SCM) muscle thickness and LC/SCM thickness ratio. One-way repeated measures ANOVA was used to compare the continuous dependent variables among FHP, ICT, and DNS conditions at p<.016. Results: Both ICT and DNS exercise conditions yielded significantly increased LC muscle thickness, LC/SCM thickness ratio and sitting height than did FHP condition (p<.0001, respectively). Sitting height was significantly greater in DNS exercise than in the ICT exercise (p<.0001). Conclusion: The present results demonstrated that sitting height was greater in the DNS exercise than in the ICT exercise, as well as both corrective postural training exercises were effective on LC/SCM muscle balance ratio when compared with the baseline FHP condition. Therefore, it is considered that DNS exercise can be the recommended exercise for people with FHP.

유도 허벅다리걸기 기술발휘 시 받기의 자세와 저항수준에 따른 중심변인 분석 사례연구[II] (A Case Study on Center of Gravity Analysis when Performing Uchimata by Posture and Voluntary Resistance Levels of Uke in Judo[ll])

  • It was to study as a following-research of "A Case Study on Center of Gravity(COG) Analysis when Performing Uchimata(inner thigh reaping throw) by Posture and Voluntary Resistance Levels(VRL) of Uke in Judo[I]". The purpose of this study was to analyze the COG variables when performing uchimata(inner thigh reaping throw) by two postures and voluntary resistance levels(VRL) of uke(reciver) in Judo. The subjects, who were one male judoka(YH) for 1992 Barcelona Olympic Games Olympian(silver medalist), and one male trainee; Y.I.University representative member (SDK), and were filmed on two S-VHS 16mm video cameras(60fields/sec.) through 3-dimensional motion analysis methods, that postures of uke were shizenhontai (straight natural posture) and jigohontai(straight defensive posture), VRL of uke were 0% and 100%, respectively. The kinematical variable was COG variable, distance of COG, and distance of resultant COG between uke and tori(the thrower), velocity and acceleration of COG. The data of this study collection were digitized by SIMI Motion Program computed the mean values and the standard deviation calculated for each variables. When performing uchinmata according to each posture and VRL of uke and classifying. From the data analysis and discussion, the conclusions were as follows : 1. Displacement of COG Subject YH, COG was the highest in kuzushi(balance -breaking), vertical COG was low when following in tsukuri(positioning; set-up), kake(application; execution), and COG was pattern of same character each postures and resistance, respectively. Subject SDK, COG was low from kumikata(engagement positioning) to kake, and COG was that each postures and resistance were same patterns, respectively. Subject YH, SDK, each individual, postures and resistance, vertical COG was the lowest in kake phase, when performing. 2. Distance of COG between uke and tori The distance of COG between uke and tori when performing, subject YH was $0.64{\sim}0.70cm$ in kumikata, $0.19{\sim}0.28cm$ in kake, and SDK was $0.68{\sim}0.72cm$ in kumikata, $0.30{\sim}0.42\;cm$ in kake. SDK was wider than YH. 3. Distance of resultant COG between uke and tori The distance of resultant COG between uke and tori when performing, subject YH was $0.27{\sim}0.73cm$ from kumikata to kake. and SDK was $0.14{\sim}0.34cm$ in kumikata, $0.28{\sim}0.65cm$ in kake. Jigohontai(YH:$0.43{\sim}0.73cm$,SDK:$0.59{\sim}0.65cm$) was more moved than shizenhontai(YH:$0.27{\sim}0.53cm$, SDK: $0.28{\sim}\;0.34cm$). 4. Velocity of COG The velocity of COG when performing uchimata, subject YH was fast anterior-posterior direction in kuzushi, ant.-post. and vertical direction fast in tsukuri and kake. SDK was lateral, ant.-post. and vertical direction in kuzushi, ant.-post. and vertical direction in tsukuri and ant.-post. direction in take, respectively. 5. Acceleration of COG The acceleration of COG when performing uchimata, The trend of subject YH was showed fast vertical direction in kuzushi and tsukuri, ant.-post. and vertical direction fast in kake. The trends of SDK showed lateral direction in kuzushi, lateral and ant.-post. direction in tsukuri and ant.-post. direction in kake, respectively.

구두 굽의 높이가 한다리 서기 자세에서 배곧은근, 척추세움근, 넙다리곧은근, 뒤넙다리근의 근활성도에 미치는 영향 (Effects of Height of the Shoe Heel in a Static Posture on Muscle Activity of the Rectus Abdominis, Erector Spinae, Rectus Femoris and Hamstring)

  • 본 연구의 목적은 젊은 여성을 대상으로 구두 굽 높이에 따라 한다리 서기 자세를 수행하는 동안 배곧은근, 척추세움근, 넙다리곧은근, 뒤넙다리근의 근활성도를 비교하기 위해 연구를 실시하였다. 본 연구는 건강한 20대 여성 20명을 대상으로 0cm, 3cm, 7cm 의 각각 다른 높이의 굽이 있는 구두를 착용하고 안정한 지지면 위에서 정적 균형검사 중 하나인 오른쪽 다리로 한다리 서기를 하는 동안 표면 근전도 장비를 이용하여 오른쪽 배곧은근, 척추세움근, 넙다리곧은근, 뒤넙다리근의 근활성도를 측정하여 비교 분석하였으며 연구 결과 척추세움근과 넙다리곧은근의 근활성도는 굽높이가 증가함에 따라 유의한 증가가 있었고(p<0.05), 배곧은근, 뒤넙다리근의 근활성도는 유의한 차이가 없었다(p>0.05). 구두 굽 높이의 증가는 척추세움근 및 넙다리곧은근의 근활성도를 증가시키며 이로 인한 허리 앞굽음과 골반의 앞쪽 기울임 증가는 신체분절의 위치나 무게 중심의 변화를 일으켜 허리 골반 부위 및 엉덩관절, 무릎관절에 근골격계 통증과 같은 문제를 일으킬 수 있을 것이다.

평균대 옆공중돌기 동작의 운동역학적 분석 (Kinetic Analysis of the Salto Side-Ward Tucked on the Balance Beam)

  • 본 연구는 평균대에서 필수요구조건(EGR)에 해당되는 동작인 옆공중돌기 동작의 성공과 실패를 운동역학적으로 비교 분석하였다. 연구대상은 국가대표 선수들로 하였으며, 연구의 목적은 기술의 실수 요인을 규명하여 지도자 및 선수들에게 과학적으로 유용한 정보를 제공하는데 있다. 성공시 옆공중돌기 동작의 신체중심은 평균대 중앙 중심으로 좌-우축으로 벗어나는 결과가 나타나고 동작의 특성상 회전하는 방향으로 신체중심이 이동한다는 것을 나타내고 있다. 도약구간인 event2와 3에서 성공시 상 하 변화가 크게 나타나 체공시간을 높이는 것으로 투사변인이 실패시 보다 높게 나타나는 결과를 뒷받침 해 주고 있다. 또한 공중 비약을 위해 다리를 차는 동작을 하는 오른쪽 고관절 각도와 속도, 각속도가 실패시 보다 성공시 동작에서 모두 크게 나타나 도약의 최적조건을 수행하고 있다. 어깨를 중심으로 상지분절의 속도와 견관절 각속도를 크게 함으로써 운동량을 증가시켰으며 그중 오른쪽 견관절 각속도가 크게 나타난 이유는 오른쪽 다리를 잡기 위한 결과이다. 옆 공중돌기 동작에서 실패시 보다 성공시 동작에서 x축을 중심으로 각운동량이 크게 나타나 공중 비행 동작은 x축을 중심으로 각운동량을 크게 하는 것이 중요한 요인으로 나타났으며 옆공중돌기 특성상 y축과 z축으로 각운동량도 적정한 비율로 동작이 이루어져야 성공적인 동작을 만들수 있다.

Traditional Korean Medical Treatment for Dizziness and Gait Instability due to Cerebellar Infarction: A Case Report

  • This case study reports the effect of Korean medicine treatments on a 73 year-old female who had a cerebellar infarction. She was hospitalized for 120 days (without visiting Western medicine hospital) where she was treated with acupuncture, herbal decoction, pharmacopuncture, chuna, moxibustion and physiotherapy. Following treatment, her symptoms of dizziness were evaluated using the numeric rating scale and showed pain had reduced (3 to 0). The K-Modified Barthel, showed that life performance had improved (15 to 74), and the Berg balance scale showed an improved balance (2 to 32). Steps per minute and gait posture at stance phase for ataxia also showed improvement. This case report shows that Korean medicine treatment is effective in alleviating dizziness and improved gait instability caused by cerebellar infarction.

드롭랜딩 시 착지형태에 따른 충격흡수구간의 운동역학적 특성 (The Biomechanical Properties of the Shock Absorption Phase during Drop Landing According to Landing Types)

  • Objective : The purpose of this study was to investigate the biomechanical properties of shock absorption strategy and postural stability during the drop landing for each types. Methods : The motions were captured with Vicon Motion Capture System, with the fourteen infra-red cameras (100Hz) and synchronized with GRF(ground reaction force) data(1000Hz). Ten male soccer players performed a drop landing with single-leg and bi-legs on the 30cm height box. Dependent variables were the CoM trajectory and the Joint Moment. Statistical computations were performed using the paired t-test and ANOVA with Turkey HSD as post-hoc. Results : The dominant leg was confirmed to show a significant difference between the left leg and right leg as the inverted pendulum model during Drop Landing(Phase 1 & Phase 2). One-leg drop landing type had the higher CoM displacement, the peak of joint moment with the shock absorption than Bi-leg landing type. As a lower extremity joint kinetics analysis, the knee joint showed a function of shock absorption in the anterior-posterior, and the hip joint showed a function of the stability and shock absorption in the medial-lateral directions. Conclusion : These findings indicate that the instant equilibrium of posture balance(phase 1) was assessed by the passive phase as Class 1 leverage on the effect of the stability of shock absorption(phase 2) assessed by the active phase on the effect of Class 2 leverage. Application : This study shows that the cause of musculo-skeletal injuries estimated to be focused on the passive phase of landing and this findings could help the prevention of lower damage from loads involving landing related to the game of sports.
