앉은 자세에서 능동적 체간 훈련이 초기 뇌졸중 환자의 균형과 에너지 소모에 미치는 효과 (The Effect of Active Trunk Training in Sitting Position on Balance and Energy Consumption in Early Stroke Patients)

  • PURPOSE: This study aimed to determine the effect of active trunk training in the sitting position on balance and energy consumption in early stroke patients. METHODS: The subjects of this study were twenty four early stroke patients were recruited and randomly divided into two groups; experimental group (n=12) and control group (n=12). The subjects in the experimental group participated in active trunk training (20 min) and conventional physical therapy (20 min) five times a week for four weeks. The control group conducted general balance exercise (20 min) and conventional physical therapy (20 min) five times a week for four weeks. The outcomes were assessed using the Berg balance scale, Timed up and go test and energy cost and physiological cost index. RESULTS: Both groups showed a significant increase in their Berg balance scale and significant decrease in their Timed up and go test score, and energy cost and physiological cost index (p<.05). Compared to the control group, the experimental group experienced a greater increase in the Berg balance scale (p<.05) and a larger decrease in the timed up and go test score, and energy cost and physiological cost index (p<.05). CONCLUSION: We suggest that active trunk training in the sitting position may be effective in improving balance and energy consumption in early stroke patients. Thus, active trunk training is important for such patients. Further studies are needed to better understand the effectiveness of trunk training in early stroke patients.

브로거 운동을 병행한 산전운동프로그램이 임산부의 허리 통증 및 균형에 미치는 영향 단일 사례 연구 (Effect of prenatal exercise program combined with Brugger's exercise on low back pain and balance in pregnant women: A single-subject study)

  • Background: The purpose of this study was to effect of prenatal exercise program combined with Brugger's exercise on low back pain and balance in pregnant women. Design: A Single-Subject Study. Methods: This study was applied to 28 weeks of pregnant women who complained of low back pain for a total of 4 weeks. A total of 10 prenatal exercise programs in this study consists of sessions, and each session was applied with a Bruger exercise for 1 minute before the start. For the fatigue and safety of pregnant women, a break for 1 minute was provided for each session and was conducted under the supervision of the therapist The visual analogue scale (VAS), Oswestry disability index (ODI), and static balance (BT-4) were measured to evaluate the pregnant women's low back pain and balance ability before and after the intervention Results: The change in back pain after the intervention was VAS 5 before the intervention, but the VAS gradually decreased at 4 weeks after the intervention. Reduced to VAS 2 after 4 weeks of intervention. The Oswestry Disability Index (ODI) was 54%, which was at the level of severe disability, but the dysfunction index gradually decreased 4 weeks after intervention, reaching 19% in 4 weeks, and there were no problems with daily life. As for the static balance ability, the sway area and the sway distance values gradually decreased after the intervention in both the eyes open and closed state, showing an improvement in the static balance ability of pregnant women after the intervention than before the intervention. Conclusion: The prenatal exercise program combined with Brugger's exercise was effective to improve low back pain and static balance in pregnant women. The result of this study may helpful as basic data for obstetric physical therapy.

Effects of Progressive Core and Ankle Muscle Strengthening Exercises Using Thera-Band on Body Balance

  • Purpose: This study aims to compare the effect on balance during core and ankle muscle strengthening exercises using the Thera-Band. Methods: 21 healthy college students were recruited. The participants were divided into a core strengthening group (CSG), an ankle strengthening group (ASG), and a non-exercise group (NEG). CSG and ASG were performed twice a week for a total of 4 weeks, and static and dynamic balance were measured before and after the intervention. The static balance were measured as stability index and weight distribution index using Tetrax®. The dynamic balance was measured in each direction by the Y balance test. The Thera-Band intensity was increased after 2 weeks of exercise, and the exercise was subsequently performed. Results: Participants showed static balance with Tetrax®, a significant difference was noted between normal eye closes and pillow with eye closes in ASG (p<0.05). In the case of dynamic balance with the Y balance test, a significant difference was observed in posterolateral direction (PL) and composite score (CS) between each group for the pre- and post-intervention differences (p<0.05). A significant difference was observed between PL and CS in CSG (p<0.05). Conclusion: These findings show that the progressive Thera-Band exercise had a positive effect on balance abilities. It was confirmed that core strengthening was effective for dynamic balance, and ankle strengthening was effective for static balance.

Static and Dynamic Balance Comparison Between the Involved and Uninvolved Sides in Patients Who had Anterior Cruciate Ligament Reconstruction: One-year Follow-up Study

  • Background: Anterior cruciate ligament reconstruction (ACLR) causes a reduction in the balance of the lower extremities. Static and dynamic balance were evaluated separately to confirm the decrease in balance in patients underwent ACLR. The commonly used methods include the Biodex Balance System (BBS) for static balance and the Y balance test (YBT) for dynamic balance. No study has evaluated whether the static and dynamic balance of the involved side recovers as much as the uninvolved side one year after ACLR. Objects: The purpose of this study was to investigate the recovery of static and dynamic balance between the involved and the uninvolved sides. Methods: The BBS (overall, anteroposterior index, and mediolateral index) and YBT (anterior, posterolateral, and posteromedial) of 58 patients underwent ACLR were measured one year postoperation. Both sides of the BBS and the YBT were compared using the paired t-test. Results: All the index of the BBS showed no difference between the involved and the uninvolved sides, while all the scores of the YBT showed a significant difference in both sides. The YBT anterior result was 54.64 ± 5.62 cm in the involved side and 56.90 ± 5.41 cm in the uninvolved side (p = 0.001). The YBT posterolateral results were 90.12 ± 10.51 cm and 92.34 ± 9.85 cm (p = 0.013). The YBT posteromedial results were 93.72 ± 8.84 cm and 96.14 ± 9.37 cm (p = 0.002). Conclusion: A year after ACLR, the static balance showed no difference, while the dynamic balance showed a significant difference in the involved and the uninvolved sides. The static balance of the involved side recovered as much as the uninvolved side, but the dynamic balance did not. Therefore, dynamic balance training should be considered in the rehabilitation program for patients underwent ACLR.

뒤로 걷기 운동에 따른 뇌졸중 편마비 환자의 균형능력 및 보행능력의 변화 (Changes in Balance and Gait Following Backward Walking Exercise in Hemiplegic Stroke Patients)

  • PURPOSE: The purpose of this study was to identify whether backward walking exercise was more effective than conventional physical therapy for balance and gait in hemiplegic stroke patients. METHODS: Eighteen patients with chronic stroke were randomly assigned to the experimental (n=9) or control (n=9) group. The experimental and control group performed backward walking exercise and conventional physical therapy, respectively, for 8 weeks. Stability Index (SI) and Weight Distribution Index (WDI) during standing were assessed using the Tetrax Balance System. The Timed Up and Go (TUG) test and Korean version of the Berg Balance Scale (K-BBS) were used to evaluate balance and fall risk. Walking speed, stride length, and step length on the affected side were measured using the 10-Meter Walk and ink foot printing tests. Wilcoxon signed-rank and Mann-Whitney U tests were used for within- and between- group comparisons, respectively. RESULTS: The experimental group showed significantly higher changes in SI (p<.01), WDI (p<.01), TUG (p<.001), and BBS score (p<.001) following intervention compared with the control group. The experimental group also showed significantly greater improvements in walking speed (p<.01), stride length (p<.001), and step length on the affected side (p<.001) after intervention compared with the control group. CONCLUSION: Backward walking exercise is an effective intervention to improve balance and gait in hemiplegic stroke patients.

Effects of Knee Malalignment on Static and Dynamic Postural Stability

  • Purpose: This study was conducted in order to determine the effects of knee malalignment including genu varum, valgum, and recurvatum on static and dynamic postural stability. Methods: A total of 80 subjects were enrolled in this study. Subjects who showed over 3 cm in the distance between the knees were classified as the genu varum group, and subjects who showed over 3 cm in the distance between the ankles were classified as the genu valgum group. Subjects who showed over 1 cm in the distance between the patella and a table in prone position were classified as the genu recurvatum group. Static and dynamic stability were measured as overall, anterioposterior, and mediolateral balance index using a Biodex Balance System. Results: This study showed that knee alignment affected static and dynamic postural stability. In particular, there were significant differences in the mediolateral stability index among genu varum, valgum group, and the other groups, but no differences in overall and anteriolateral stability index. Significant differences in the anterioposterior stability index were observed between genu recurvatum and the other groups, however, there were no differences in overall and mediolateral stability index. Conclusion: The findings were that knee malalignment affects postural stability toward a specific direction. Treatment to improve postural stability for treatment of knee malalignment or to prevent falling or injuries is needed and postural stability toward a specific direction according to the knee alignment conditions should be considered.

바로 선 자세에서 발목과 무릎관절의 고정이 자세안정성에 미치는 영향 (Effects of Immobilization of the Ankle and Knee Joints on Postural Stability in Standing)

  • This study was designed to examine the effects of temporary immobilization of the ankle and knee joints on standing in healthy young adults with the use of a postural control mechanism. The subjects were twenty-four college students (12 males and 12 females, aged between 20 and 28). A Biodex balance system SD 950-302 and its software were used to measure indirect balance parameters in standing. Each subject underwent postural stability tests in 4-different joint conditions: free joints, ankle immobilization only, knee immobilization only, and ankle and knee immobilization. In addition, the postural stability test was conducted once with the subject's eyes open and once with the eyes closed conditions. For data analysis of the postural stability tests, the overall stability index, antero-posterior stability index, and medio-lateral stability index were recorded. The overall stability index (p=.000) and medial-lateral index (p=.003) were significantly greater different conditions with eyes closed in postural stability. Therefore, the eyes closed condition is expected to be used as an effective postural stability training for treatment planning in patients with unstable postures. In addition, training based on the dynamic multi-segment model can improve postural stability and is available to therapeutic programs, helping people with unstable balance to reduce their risk of falling.

  • This paper presents an algorithm to obtain an approximate optimal solution for the service restoration and load balancing of large scale radial distribution system in a real-time operation environment. Since the problem is formulated as a combinatorial optimization problem, it is difficult to solve a large-scale combinatorial optimization problem accurately within the reasonable computation time. Therefore, in order to find an approximate optimal solution quickly, the authors proposed an algorithm which combines optimization technique called cyclic best-first search with heuristic based feeder loadings balance index for computational efficiency and robust performance. To demonstrate the validity of the proposed algorithm, numerical calculations are carried out the KEPCO's 108 bus distribution system.

금강유역의 습윤/건조 상태에 대한 경향성 분석 (Trend Analysis of Wetness/Dryness in Geum River Basin)

  • In this study the wetness/dryness in Geum river basin are classified by dint of cumulative probability density function of monthly moisture index and the long term changes of them are analyzed based on climatic water balance concept. The drought events in Geum river basin are selected through evaluation of monthly moisture index and the various hydrological properties of them are investigated in detail. Also the trends of time-series of climatic water balance components are examined by Seasonal Kendall test and the variability of hydrological cycle in Geum river basin during the recent decade is inquired. It is judged that the results of this study can be contributed to establishment of the counter plan against the future drought events as the fundamental information.

부하균등화를 고려한 배전계통의 정전복구 (Service Restoration Considering Load Balancing In Distribution Networks)

  • Service restoration is an emergency control in distribution constrol centers to restore out-of-service area as soon as possible when a fault occurs in distribution networks. therefore, it requires fast computation time and high quality solutions for load balancing. In this paper. a load balance index and heuristic guided best-first search are proposed for these problem. The proposed algorithm consists of two parts. One is to set up a decision tree to represent the various switching operations available. Another is to identify the most effective the set of switches using proposed search technique and a load balance index. Test results on the KEPCO's 108 bus distribution system show that the performance is efficient and robust.
