  • Background and Aims: Colorectal cancer is the second most frequent cancer in Malaysia. We aimed to assess the validity and reliability of the Malaysian Chinese version of European Organization for Research and Treatment of Cancer (EORTC) Quality of Life Questionnaire core (QLQ-C30) in patients with colorectal cancer. Materials and Methods: Translated versions of the QLQ-C30 were obtained from the EORTC. A cross sectional study design was used to obtain data from patients receiving treatment at two teaching hospitals in Kuala Lumpur, Malaysia. The Malaysian Chinese version of QLQ-C30 was self-administered in 96 patients while the Karnofsky Performance Scales (KPS) was generated by attending surgeons. Statistical analysis included reliability, convergent, discriminate validity, and known-groups comparisons. Statistical significance was based on p value ${\leq}0.05$. Results: The internal consistencies of the Malaysian Chinese version were acceptable [Cronbach's alpha (${\alpha}{\geq}0.70$)] in the global health status/overall quality of life (GHS/QOL), functioning scales except cognitive scale (${\alpha}{\leq}0.32$) in all levels of analysis, and social/family functioning scale (${\alpha}=0.63$) in patients without a stoma. All questionnaire items fulfilled the criteria for convergent and discriminant validity except question number 5, with correlation with role (r = 0.62) and social/family (r = 0.41) functioning higher than with physical functioning scales (r = 0.34). The test-retest coefficients in the GHS/QOL, functioning scales and in most of the symptoms scales were moderate to high (r = 0.58 to 1.00). Patients with a stoma reported statistically significant lower physical functioning (p=0.015), social/family functioning (p=0.013), and higher constipation (p=0.010) and financial difficulty (p=0.037) compared to patients without stoma. There was no significant difference between patients with high and low KPS scores. Conclusions: Malaysian Chinese version of the QLQ-C30 is a valid and reliable measure of HRQOL in patients with colorectal cancer.

  • Array observations of the vertical component of microtremors are frequently conducted to estimate a subsurface layered-earth structure on the assumption that microtremors consist predominantly of the fundamental mode Rayleigh waves. As a useful tool in the data collection, processing and analysis, the spatial autocorrelation (SPAC) method is widely used, which in practice requires a circle array consisting of M circumferential stations and one centre station (called "M-station circle array", where M is the number of stations). The present paper considers the minimum number of stations required for a circle array for efficient data collection in terms of analytical efficacy and field effort. This study first rearranges the theoretical background of the SPAC algorithm, in which the SPAC coefficient for a circle array with M infinite is solely expressed as the Bessel function, $J_0(rk)$ (r is the radius and k the wavenumber). Secondly, the SPAC coefficient including error terms independent of the microtremor energy field for an M-station circle array is analytically derived within a constraint for the wave direction across the array, and is numerically evaluated in respect of these error terms. The main results of the evaluation are: 1) that the 3-station circle array when compared with other 4-, 5-, and 9-station arrays is the most efficient and favourable for observation of microtremors if the SPAC coefficients are used up to a frequency at which the coefficient takes the first minimum value, and 2) that the Nyquist wavenumber is the most influential factor that determines the upper limit of the frequency range up to which the valid SPAC coefficient can be estimated.

  • Background: Inhaled corticosteroids (ICSs) are the most essential medication for asthma control. Many reports suggest that the usage of ICSs improves not only the control of asthma symptoms but also prevents exacerbation. We investigated whether increases in ICS prescriptions are associated with decreases in asthma exacerbation in the clinical practice setting. Methods: We retrospectively analyzed the database of adult asthma patients who had visited a tertiary referral hospital, the Asan Medical Center between January 2000 and December 2009. The number of emergency department (ED) visits, admissions, intensive care unit (ICU) care, deaths, and ICS prescriptions were analyzed to evaluate the time trend of asthma exacerbation as a function of the ICS prescription rate during the ten years. Results: The numbers of ED visits, admissions, and episodes of ICU care decreased during the ten years (p<0.001, p=0.033, p=0.001, respectively) while the number of ICS prescriptions increased (p<0.001). We found a correlation between the number of ICS prescriptions and the number of ED visits, admissions, or ICU care. For these outcomes, the correlation coefficients were r=-0.952, p<0.001; r=-0.673, p=0.033; r=-0.948, p<0.001, respectively. Conclusion: The number of ICS prescriptions increased during the past ten years while the number of asthma exacerbations decreased. Our results also showed a negative correlation between the ICS prescription rate and asthma exacerbation in the clinical practice setting. In other words, an increase in ICS prescription may be a major cause of a decrease in asthma exacerbations.

  • Background: With effective early treatments, many breast cancer patients suffer from psychological distress due to adverse effects and lifelong physical disfigurement. Our study aimed to evaluate the psychopathological profile of breast cancer patients in comparison with healthy women and explored demographic correlates. Method: We consecutively enrolled breast cancer patients who came to the hospital for follow-up or rehabilitation care after primary treatment, and healthy female relatives or friends of inpatients in the Cancer Institute of Chinese Academy of Medical Sciences between August 30, 2010 and January 1, 2012. Psychopathological profile was assessed based on the Symptom Checklist-90-R (SCL-90-R) for patients and controls. We compared demographics such as age, ethnicity, education, marriage, and occupation, and incorporated these data plus cancer status for the association with the general SCL-90-R index and scores for 9 major symptom dimensions in multiple regression analysis. Results: We surveyed a total of 291 female breast cancer patients and 531 healthy women. The average age was $55.1{\pm}6.40$ years for breast cancer patients and $43.1{\pm}12.8$ for healthy controls (P<0.01). The mean survival was 5.20 years for cancer patients (range, 0.60-9.90 years). There were statistically significant differences in education, marriage, and occupation between the two groups (P<0.01). General index ($1.45{\pm}0.45$ versus $1.32{\pm}0.37$) and 8 dimension scores (excluding anxiety) on SCL-90-R were significantly higher in patients (P<0.05). Multiple regression analysis showed that the breast cancer status was positively correlated with general SCL-90-R index and 6 dimension scores (excluding the anxiety, phobic anxiety and paranoid ideation dimensions) (P<0.05). Regression coefficients ranged from 0.10 (depression) to 0.19 (somatization). Higher interpersonal sensitivity was noticed in single women compared to married women. Conclusions: Chinese patients with breast cancer demonstrate greater psychopathology compared to healthy controls. The breast cancer status is an independent contributing factor to the general psychopathological profile. Breast cancer patients should be given particular counseling and care to alleviate their psychological distress.

  • BACKGROUND: An analytical method was developed using HPLC-UVD/MS to precisely determine the residue of flusulfamide, a benzenesulfonamide fungicide used to inhibit spore germination. METHODS AND RESULTS: Flusulfamide residue was extracted with acetone from representative samples of five raw products which comprised apple, green pepper, Kimchi cabbage, hulled rice, and soybean. The extract was diluted with large volume of saline water and directly partitioned into dichloromethane to remove polar co-extractives in the aqueous phase. For the hulled rice and soybean samples, n-hexane/acetonitrile partition was additionally employed to remove non-polar lipids. The extract was finally purified by optimized Florisil column chromatography. On an octadecylsilyl column in HPLC, flusulfamide was successfully separated from co-extractives of sample, and sensitively quantitated by ultraviolet absorption at 280 nm with no interference. Accuracy and precision of the proposed method was validated by the recovery experiment on every crop sample fortified with flusulfamide at 3 concentration levels per crop in each triplication. CONCLUSION: Mean recoveries ranged from 82.3 to 98.2% in five representative agricultural commodities. The coefficients of variation were all less than 10%, irrespective of sample types and fortification levels. Limit of quantitation (LOQ) of flusulfamide was 0.02 mg/kg as verified by the recovery experiment. A confirmatory method using LC/MS with selected-ion monitoring technique was also provided to clearly identify the suspected residue.

  • This study investigates the characteristics of cloud top brightness temperature (CTBT) of WV and IR1 from MTSAT-1R when lightning strikes in South Korea. For temporal and spatial collocations, lightnings, occurred only within ${\pm}5$ minutes from the six minutes added official satellite observation time (e.g., not 0600 UTC but 0606 UTC, considering the real scan time over South Korea), were selected. And the CTBTs corresponding to lightning spots were determined using the nearest pixel within 5 km. The brightness temperature difference (BTD, defined as WV - IR1) between two channels is negatively large when no lightning occurrs, whereas it increases up to positive values (sometimes, +5 K) and the largest frequency distributes around 225 K and 205 K in lightning cases. The probablistic approach for lightning frequency forecast, presented by Machado et al. (2008) in Southern America, was applied over South Korea and new exponential equations, with high coefficients of determination around 0.95 to 0.99, were developed using two channels' BTDs when lightning strikes. Moreover, a case study on 10th June, 2006, the largest number of lightning occurred between 2002 and 2006, was made. The major finding is that lightning activity is closely related to the dramatic decreases in BT and the increases in BTD (esp., equal to or larger than 0 K). Lightning frequency increases exponentially when BTD increases up to 0 K. Therefore, lightning forecast skill will be improved when the integrated strategy (synoptic background and satellite-based CTBT and BTD) is applied. It is believed that this study contributes to the application of the Korean first geostationary satellite (COMS), scheduled to launch at the end of this year, to severe weather detections.

  • Background: In radiation therapy, estimation of surface doses is clinically important. This study aimed to obtain an analytical relationship to determine the skin surface dose, kerma and the depth of maximum dose, with energies of 6 and 18 megavoltage (MV). Materials and Methods: To obtain the dose on the surface of skin, using the relationship between dose and kerma and solving differential equations governing the two quantities, a general relationship of dose changes relative to the depth was obtained. By dosimetry all the standard square fields of $5cm{\times}5cm$ to $40cm{\times}40cm$, an equation similar to response to differential equations of the dose and kerma were fitted on the measurements for any field size and energy. Applying two conditions: a) equality of the area under dose distribution and kerma changes in versus depth in 6 and 18 MV, b) equality of the kerma and dose at $x=d_{max}$ and using these results, coefficients of the obtained analytical relationship were determined. By putting the depth of zero in the relation, amount of PDD and kerma on the surface of the skin, could be obtained. Results: Using the MATLAB software, an exponential binomial function with R-Square >0.9953 was determined for any field size and depth in two energy modes 6 and 18MV, the surface PDD and kerma was obtained and both of them increase due to the increase of the field, but they reduce due to increased energy and from the obtained relation, depth of maximum dose can be determined. Conclusions: Using this analytical formula, one can find the skin surface dose, kerma and thickness of the buildup region.

  • Background: Bladder cancer is an international public health problem. It is the ninth most common cancer and the fourteenth leading cause of death due to cancer worldwide. Given aging populations, the incidence of this cancer is rising. Information on the incidence and mortality of the disease, and their relationship with level of economic development is essential for better planning. The aim of the study was to investigate bladder cancer incidence and mortality rates, and their relationship with the the Human Development Index (HDI) in the world. Materials and Methods: Data were obtained from incidence and mortality rates presented by GLOBOCAN in 2012. Data on HDI and its components were extracted from the global bank site. The number and standardized incidence and mortality rates were reported by regions and the distribution of the disease were drawn in the world. For data analysis, the relationship between incidence and death rates, and HDI and its components was measured using correlation coefficients and SPSS software. The level of significance was set at 0.05. Results: In 2012, 429,793 bladder cancer cases and 165,084 bladder death cases occurred in the world. Five countries that had the highest age-standardized incidence were Belgium 17.5 per 100,000, Lebanon 16.6/100,000, Malta 15.8/100,000, Turkey 15.2/100,000, and Denmark 14.4/100,000. Five countries that had the highest age-standardized death rates were Turkey 6.6 per 100,000, Egypt 6.5/100,000, Iraq 6.3/100,000, Lebanon 6.3/100,000, and Mali 5.2/100,000. There was a positive linear relationship between the standardized incidence rate and HDI (r=0.653, P<0.001), so that there was a positive correlation between the standardized incidence rate with life expectancy at birth, average years of schooling, and the level of income per person of population. A positive linear relationship was also noted between the standardized mortality rate and HDI (r=0.308, P<0.001). There was a positive correlation between the standardized mortality rate with life expectancy at birth, average years of schooling, and the level of income per person of population. Conclusions: The incidence of bladder cancer in developed countries and parts of Africa was higher, while the highest mortality rate was observed in the countries of North Africa and the Middle East. The program for better treatment in developing countries to reduce mortality from the cancer and more detaiuled studies on the etiology of are essential.

  • The vertical profiles and optical properties of Asian dust are investigated using ground-based measurements from 1998 to 2002. Vertical profiles of aerosol extinction coefficient are evaluated using MPL (Micro Pulse Lidar) data. Optical parameters such as aerosol optical thickness ($\tau$), ${\AA}ngstr\ddot{o}m$ exponent ($\alpha$), single scattering albedo ($\omega$), refractive index, and volume size distribution are analyzed with sun/sky radiometer data for the same period. We can separate aerosol vertical profiles into three categories. First category named as 'Asian dust case', which aerosol extinction coefficient is larger than $0.15km^{-1}$ and dust layer exists from surface up to 3-4km. Second category named as 'Elevated aerosol case', which aerosol layer exists between 2 and 6km with 1-2.5km thickness, and extinction coefficient is smaller than $0.15km^{-1}$. Third category named as 'Clear sky case', which aerosol extinction coefficient appears smaller than $0.15km^{-1}$. and shows that diurnal variation of background aerosol in urban area. While optical parameters for first category indicate that $\tau$ and $\alpha$ are $0.63{\pm}0.14$, $0.48{\pm}0.19$, respectively. Also, aerosol volume concentration is increased for range of 1 and $4{\mu}m$, in coarse mode. Optical parameters for second category can be separated into two different types. Optical properties of first type are very close to Asian dust cases. Also, dust reports of source region and backward trajectory analyses assure that these type is much related with Asian dust event. However, optical properties of the other type are similar to those of urban aerosol. For clear sky case, $\tau$ is relatively smaller and $\alpha$ is larger compare with other cases. Each case shows distinct characteristics in aerosol optical parameters.
