Short-term Clinical Observation of the Early Postpartum Low Back Pain in the Korean Medical Hospital (한방치료를 받은 출산 초기 산모의 요통 경과 고찰)

  • Objectives: The purpose of this study was to observe the early postpartum low back pain and pelvic girdle pain in the Korean medical hospital for 2-weeks. Methods: Fifty-one postpartum women who admitted for postpartum care in Korean hospital from December 3rd, 2011 to January 31st, 2012 had written out pain measurement questionnaires(SF-MPQ, VAS, ODI) three times for 2-weeks. SPSS 18.0 for Windows was used to analyse the data and the independent samples t-test, paired T-test were used to verify the results. Results: 1. Evaluation of low back pain within the first week after birth was as follows: SF-MPQ sensory area was $5.06{\pm}4.41$ points; SF-MPQ emotional areas was $0.86{\pm}1.34$ points; VAS mean was $3.79{\pm}2.02$ points; and ODI mean was $10.27{\pm}6.69$. 2. Early postpartum low back pain and pelvic girdle pain were significantly decreased in SF-MPQ, VAS, and ODI after 2-weeks(p < 0.01). 3. Age, birth type, parity, weight, and BMI increased during pregnancy does not affect the postpartum low back pain and pelvic girdle pain. Conclusions: The top 10% of postpartum low back pain scale was 10.8 points or more in the sensory area of SF-MPQ, and 3 points or more in the affective area of SF-MPQ. In the case of VAS, it was more than 7 points, and ODI was more than 21.8 points. Postpartum back pain with oriental medical treatment was significantly improved.

The length of hospital stay of the industrial workers with back injury (산업재해 요통근로자의 재원기간에 관한 연구)

  • Back injury is frequent in industry workers and is a common cause of productivity loss. It has been reported that the insured of industrial accident insurance tend to stay in hospital longer than that of other types of insurance. The purpose of this study was to identify factors affecting the length of hospital stay for the treatment of back injury in the workers under industrial accident insurance. The results of this study help insurers develop reasonable industrial accident insurance policy for back injury claims and prevention strategies of work-related back injury. A total of 2,949 patients whose industrial accident insurance claim has been approved for the treatment of work-related back injury from January to December 1999 were included in this study. Relationship between the length of hospital stay and characteristics of patient, work place, back injury, and hospital were assessed using ANOVA, t-test, simple linear regression and multiple resgression. The major findings of this study are as follows : 1. The average length of hospital stay(LOS) was 91.82 days, respectively. 2. Characteristics of Patient LOS of male patients was longer than that of female patients, there was positive correlation between age and LOS and between average wage and LOS. Working period was negatively correlated with LOS. Distance from resident to hospital was positively correlated with LOS and LOS was significantly different dependign on type of duty. 3. Characteristics of Work Place LOS was significantly different depending on types of industry and geographical region of work place. Size of work place was positively correlated with LOS. 4. Characteristics of Back Injury Occupational back pain required shorter LOS compared with back injury due to electric shock. Number of concomitant illnesses and severity of disability were positively correlated with LOS. 5. Characteristics of Hospital Patients treated in community hospitals required significantly longer LOS. Treatment in hospitals with rehabilitation program required decreased LOS. This was more prominent as number of physicians specialized in rehabilitation. 6. Multiple regression analysis revealed that distance form resident to hospital, geographical region of work place, size of work place, number of concomitant illnesses, severity of disability, and type of hospital were factors affecting LOS.

The Effects of Pelvic Floor Stabilization exercise on Pain, Function, Psychosocial, EMG Activity on the Lower Back Pain with Postpartum Women (골반 안정화운동이 산후 요통 여성에게 통증, 기능장애, 심리사회적수준, 근활성도에 미치는 영향)

  • Background: This study investigated the effects of pelvic floor stabilization exercise of pain, disfunction, psychosocial, electromyography (EMG) activity on the lower back pain with postpartum. Methods: The study included 20 postpartum women who were randomly assigned to a sling exercise group (SEG, n=10) or a general physical therapy group (GPTG, n=10). Outcomes were assessed using to lower back pain with postpartum the quadruple visual analogue scale (QVAS), the Korean version of the Oswestry disability index (KODI), the fear avoidance belief questionnaire (FABQ), the inventory of functional status after childbirth (IFSAC), the edinburgh postnatal depression scale-Korean (EPDS-K), and trunk muscle activity before and after a 4-week exercise intervention. Statistical analysis were performed using a mean, standard deviation, crosstab test, paired t-test, independent t-test. Kolmogorove-Smirnov test was used for test of normality. Results: Compared to the GPTG, the SEG showed significant improvement in the QVAS, KODI, FABQ, IFSAC, and EPDK-K scores (p<.05) after 4 weeks. Conclusion: Postpartum pelvic strengthening exercise proved to have a positive effect.

  • The Back Up Breaker and Making Switch are very important equipments for the short circuit test facility, and contacts are the most important parts of the above switches. There are very many kind of contacts according to switches characteristic and should be done the verification test before use, especially development contacts. This study describes the class of switches, arc chute, material of whole contacts and essential test for verification. The essential test for verification are dielectric test, mechanical operation test, short-time withstand current test, load current breaking test, and short-circuit making current test etc.

Effects of ultra-thin microcurrent patch application on pain and mobility in patients with chronic low back pain (초박형 미세전류패치 적용이 만성 허리통증 환자의 통증과 허리 가동성에 미치는 영향)

  • Background: The purpose of this study is to investigate the effect of ultra-thin microcurrent patch application on pain, tenderness, trunk flexion, and trunk extension in patients with back pain. Design: pretest-posttest design: single blind. Methods: Thirty men and women diagnosed with chronic back pain were classified into 15 microcurrent application groups and 15 placebo groups. Changes in pain were observed on a visual analog scale, tenderness was observed with a digital tenderness meter, and changes in trunk flexion and trunk extension angles were evaluated with a posture analyzer. The paired t-test was used to see the changes within each group before and one week after the experiment, and the independent t-test was used to see the change in the difference between the groups, and the significance level was 0.05. Results: In both the experimental group and the control group, there was a significant difference in pain within and between groups(p<0.05). In the experimental group, there were significant differences in the intra- and inter-group changes in the erector spinae muscle tenderness and low back pain disorder index(p<0.05). In the experimental group, there was a significant difference in the change in trunk flexion and extension within the group(p<0.05). Conclusion: In this study, it was confirmed that the application of ultra-thin microcurrent was effective for pain, tenderness, and movement of back extension in patients with low back pain. It is expected that it will be used as a basic data for microcurrent therapy and as a treatment method for improving the function of patients with back pain in the future.

Spin-up, Spring-back Load Analysis of KC-100 Nose Landing Gear using Explicit Finite Element Method (외연적 유한요소법을 이용한 KC-100 전방착륙장치 Spin-up, Spring-back 하중 해석)

  • The spin-up and the spring-back are most severe load cases in the aircraft landing gear design. These load cases are caused by reciprocal action of complex physical phenomenon such as the friction between a tire and ground, inertia of the rotation of a tire and the flexibility of a landing gear structure. Generally, the empirical formula or the theoretical formula is used to calculate the spin-up and spring-back load in the early stage of the development program of the aircraft landing gear. After the materialization of the design of a landing gear, spin-up and spring-back load are acquired by the free drop test. In this study, the spin-up and the spring-back load of the rubber shock absorber type KC-100 nose landing gear are calculated by the explicit finite element analysis. Through this analysis, more accurate and realistic spin-up and spring back loads could be applied to the early phase of the development of the aircraft landing gear.

Effects of Sling Exercise on Pain, Trunk Strength, and Balance in Patients with Chronic Low Back Pain

  • Purpose: The purpose of this study was to examine the effect of sling exercise on pain, trunk strength, and balance in patients with chronic low back pain in their 40s and 50s. Methods: Twenty patients with chronic low back pain were divided into two groups, 10 patients in the exercise group using a sling and 10 patients in the waist stabilization exercise group, applying a random assignment, draw-out method, and sling exercise was applied for 50 minutes a day, 3 times a week, for a total of 5 weeks. For data analysis, a corresponding t-test was performed for within-group changes and an independent t-test was performed for inter-group changes, and the significance level was α=0.05. Results: After 5 weeks, there were significant differences in all items of within-group changes, and the inter-group changes after 5 weeks demonstrated significant differences in pain, trunk flexion strength, and balance. Conclusion: The results of this study showed that exercise using a sling had positive effects on pain, trunk flexion strength, and balance changes in chronic low back pain patients. It is suggested that sling exercise can be used as an intervention method for pain reduction and functional improvement of patients with chronic low back pain in clinical practice.

A Comparative Study of Improvement of Neck Pain According to Hospitalization Period in Patients of Neck pain Caused by Traffic Accident (교통사고로 인한 경항통 환자의 입원 기간에 따른 통증 호전도 비교 연구)

  • Objectives : The aim of this study was to compare improvement according to the hospitalization period of patients with neck pain as a result of traffic accidents. Methods : Patients of 59 cases with neck pain induced by traffic accidents were investigated. Based on their hospitalization period, 59 subjects were divided into two groups: A group with a shorter hospitalization period and B group with a longer hospitalization period. Each group was treated by the same therapies including acupuncture, pharmacopuncture, and herbal medicine. To compare the treatment effects of the two groups, the Visual Analog Scale(VAS) was used. Statistical analysis between two groups was assessed using the chi-square test, Mann-Whitney U Test and paired t-test through the SPSS 12.0 Windows program. Results : After admission treatment, both A and B group showed a significant decrease in their VAS scores. In addition, B group, of which the hospitalization period was longer than A group, showed a significant improvement in comparison to A group in its VAS score. Conclusion : This study suggests that pain control is more effectively achieved in patients that received more than 8 days of admission treatment compared to those who received less than 7 days of admission treatment.

The Analysis of Research Trend about Management of Low Back Pain (요통관리에 관한 연구동향 분석)

  • The result of studying papers about management of lower back pain published in domestic and foreign nursing and medical magazines in these 10 years is as follows ; 1. General characteristic of lower back pain ; 1) In sex distribution, there were more men than women in 5 papers and more women than men in 4 papers among 9 papers surveryed. 2) In age distribution, thirties to forties of patients had more lower back pain as surveyed by general hospitals and fifties to sixties of patients had more lower back pain as surveyed by oriental medical hospitals. 3) In cause factor, there were 50 to 65% of sprain, 32 to 44% of herniated intervertebral disc and 13 to 29% of degenerative changes. 4) In symtom distribution, there were 26 to 57% of lower back pain, 42 to 65% of lower back pain with radiating pain and 34 to 99% of paravertevral muscle spasm. 5) In period of pain management distribution, 18 to 40% of patients experienced pain for less than 6 months and 59 to 82% of them experienced pain for more than 6 months in 3 papers among 4 papers. 6) In surveying the treatment, 66 to 88% of patients had conservative treatment and there were treatments of general hospital, oriental medicine, self remedy and traditional practice in conservative treatment. 7) In job distribution, 12 to 50% of them were housekeepers, 23 to 31% office workers, 4.6 to 36% blue color workers and 11 to 15% students. 2. As psychological character lower back pain paients had anxiety, depression, anger-hostility, phobic anxiety, neurasthenia, hypochondriasis, and interpersonal sensitivity. 3. To distinguish the cause of lower back pain, plain lumbar roentgenogram, straight leg rasing test, eletromyelogic findings, somatosensory evoked potentials CT and MRI were performed. 4. To relieve lower back pain. epidural adhesiolysis, epidulal injection of local anesthetic in mixture with steroid, lumbar spinal root block, low level laser therapy, acupuncture like transcutaneous nerve stimulation(AL TENS), topical capsaicin and lumbar orthotics were used in medical field, and relaxation technique was used in nursing field. 5. Mckenzie's extension exercise and William's flexsion exercise for lower back pain were used in medical field and Yoga exercise was applied in nursing field. 6. The more school education and self efficacy were high, the better they had active coping lower back pain positively and the less self efficacy was the more they had serious pain. As a result of studying the paper there have been very little research for lower back pain in nursing fields of Korea and foreign countries. Because 60 to 80% of population expeience lower back pain at least more than once, it is necessary to develop the study and clinical practice for management of lower back pain.

The Effects of 3-Dimensional Lumbar Stabilization Exercise have an effect on the improvement of pain and static or dynamic balance ability in 20's age group with Low Back Pain (3차원 요부안정화 운동이 20대 요통환자의 통증과 동적 및 정적 균형능력 향상에 미치는 효과)

  • Purpose: The aim of this study is to compare and assess the effects of lumbar stabilization exercise on the balance ability of young college studets with low back pain after having performed spinal stabilization exercise by using 3-dimensional air-balance system and gym ball. Methods: The subjects of this study were 34 low back patients in their early twenties. They were divided into two groups: 3-dimensional lumbar stabilization exercise group(N=17) and gym ball lumbar stabilization exercise group(N=17). The period of the intervention was for five weeks. VAS(Visual Analogue Scale) for pain test, ODI(Oswestry Disability Index) for ADL limitation test, Tetrax system for static balance test, and Air-balance system 3D for dynamic balance test were used as evaluation tools for this study. Results: Pain showed significant decrease in both groups after having performed the experiment, but ADL limitation of the groups did not show any remarkable difference between before and after the experiment. Dynamic balance ability in the 8-directional angle comparison test significantly increased in all directions except for the backward, left-backward, and right-backward directions. As for dynamic balance ability in the 8-directional postural test, 3D exercise group showed statistically significant reduction in every direction while gym ball exercise group did not(p<.05). However, when it comes to static balance ability in the weight distribution and stability test, there was not significantly change between pre and post test in both groups. Conclusion: This study shows 3-dimensional lumbar stabilization exercise is more effective in the lumbar stabilization of coordinated movement than gym ball exercise, which may imply that 3D air-balance system can be used for the therapeutic treatment of body imbalance for patients with low back pain.
