효과적인 병원감염관리 프로그램의 구성 요소에 대한 조사연구 (A Survey on Elements of an Effective Hospital Infection Control Program for Korea)

  • The main Purpose of this survey was to identify for elements of an effective hospital infection control program for Korea. Nurses and doctors who had participated in an education program for infection control or were re s ponsible for hospital infection control were selected as data informers. The data were collected from 51 subjects by employing a Delphi technique in a series of three rounds from september 1, 1987, to March 31, 1988. IN each round the responses to questionaires were analyzed and the results were communicated back to the individuals. Finally 32 elements of effective hospital infection control program were reduced to 10 elements. 10 elements are as follows ; 1) Hospital administrator's knowledge of importance and necessity for HIC(hospital infection control) 2) Supporting HIC administratively 3) Constituting infection control committee and role of ICC 4) Developing an eductional program and providing a work manual for the hospital infection control staff 5) Educating and informing medical staff about hospital infection 6) Surveillance for hospital infection 7) Developing patient care technique 8) Controlling the hospital environment 9) Executing regular health examination of all medical staff 10) Recruiting the medical staff sufficiently Three rankings of response rate about 32 elements are as follows ; 1) Hiring a full tim staff member for the HIC(66%) 2) Establishing a hospital policy and standards for the HIC(66%) 3) Activating the infection control committee and taking administrative action to support the ICC(63%) In addition the rankings of importance score by Likert 5 scale are as follows ; 1) Washing hands scrupulously(4.88) 2) Nurses participation as key members of the ICC(4.75) 3) Reviewing and evaluating all ongoing aseptic techniques (4.69) In conclusion, first of all, administrative support must be given to hiring a full time staff member and to organization of infection control committee for the HIC in Korea.

The combination of a histogram-based clustering algorithm and support vector machine for the diagnosis of osteoporosis

  • Purpose: To prevent low bone mineral density (BMD), that is, osteoporosis, in postmenopausal women, it is essential to diagnose osteoporosis more precisely. This study presented an automatic approach utilizing a histogram-based automatic clustering (HAC) algorithm with a support vector machine (SVM) to analyse dental panoramic radiographs (DPRs) and thus improve diagnostic accuracy by identifying postmenopausal women with low BMD or osteoporosis. Materials and Methods: We integrated our newly-proposed histogram-based automatic clustering (HAC) algorithm with our previously-designed computer-aided diagnosis system. The extracted moment-based features (mean, variance, skewness, and kurtosis) of the mandibular cortical width for the radial basis function (RBF) SVM classifier were employed. We also compared the diagnostic efficacy of the SVM model with the back propagation (BP) neural network model. In this study, DPRs and BMD measurements of 100 postmenopausal women patients (aged >50 years), with no previous record of osteoporosis, were randomly selected for inclusion. Results: The accuracy, sensitivity, and specificity of the BMD measurements using our HAC-SVM model to identify women with low BMD were 93.0% (88.0%-98.0%), 95.8% (91.9%-99.7%) and 86.6% (79.9%-93.3%), respectively, at the lumbar spine; and 89.0% (82.9%-95.1%), 96.0% (92.2%-99.8%) and 84.0% (76.8%-91.2%), respectively, at the femoral neck. Conclusion: Our experimental results predict that the proposed HAC-SVM model combination applied on DPRs could be useful to assist dentists in early diagnosis and help to reduce the morbidity and mortality associated with low BMD and osteoporosis.

비선형 반복 패턴과 스펙트럼 분석을 이용한 집중-비집중 분류기의 성능 평가 (Performance Evaluation of Attention-inattetion Classifiers using Non-linear Recurrence Pattern and Spectrum Analysis)

  • 집중은 관련된 사건을 선택적으로 주의하고, 관련 없는 사건을 무시하는 인간의 중요한 인지 기능중의 하나이다. 인간의 집중 능력을 관리 이용하는 컴퓨터 기반 장치에 있어서 집중과 비집중 상태를 구분하는 것은 필수적으로 요구되는 조건이다. 본 논문에서는, 뇌파신호로부터 분류기의 입력으로 사용되는 특징을 효율적으로 추출하기 위하여 비선형 반복 패턴 분석기법과 스펙트럼 분석 기법을 새로이 결합하였고(13개 특징 추출), 서포트벡터머신, 역전파 알고리즘, 선형분리, 로지스틱 회귀 분류 기반 분류기들을 포함하는 집중-비집중 분류기들의 성능을 분석하였다. 그중에서 81 %의 정확도를 보이는 서포트벡터머신 분류기가 가장 좋은 성능을 보였다. 또한 스펙트럼 분석으로 추출한 특징만을 사용하였을 경우(76 % 정확도)가 비선형 분석 방법으로 추출한 특징만을 사용했을 경우(67 % 정확도)보다 좀 더 우수한 성능을 보였다. 비선형-스펙트럼 분석법을 복합 적용한 서포트벡터머신 분류기가 추후 집중 관련 장비 설계에 있어서 효율적으로 적용될 수 있을 것이다.

무선 센서 망에서 싱크 그룹을 위한 에너지 효율 향상 이동성 지원 방안 (A Mobility Support Scheme Achieving High Energy-Efficiency for Sink Groups in Wireless Sensor Networks)

  • 무선 센서 네트워크에서 이동 싱크 그룹은 같은 임무를 수행하기 위해 지리적으로 공통적인 움직임을 가지며 동일한 데이터를 필요로 한다. 이들 그룹에 데이터를 전달하기 위해서는 이동 싱크 그룹의 현재 위치를 파악하고, 이를 소스에게 제공하는 기법이 중요해진다. 기존 연구에서는 영역 정보를 제공하기 위해 플러딩을 사용하여 모든 멤버 싱크들을 포함하는 영역을 계산하고 이 정보를 소스에게 전달하였다. 그러나 멤버 싱크들의 위치를 수집하기 위해 영역 안에서 매번 주기적으로 수행되는 플러딩과 소스로의 위치 업데이트는 에너지 소모 측면에서 상당한 오버헤드이다. 본 논문에서는 주기적인 플러딩과 위치 업데이트 없이 에너지 효율적으로 이동 싱크 그룹을 지원하는 방안을 제안한다. 제안 방안은 플러딩 없이 싱크 그룹의 고유한 성질을 이용하여 멤버 싱크 일부의 움직임으로 그룹 전체의 움직임을 근사하여 그룹의 위치를 계산할 수 있다. 또한, 제안 방안에서는 무선 통신 환경의 오버히어링(overhearing) 특성을 활용한 역전파 학습 방법으로 소스로의 지속적인 위치 업데이트 비용 없이 위치 정보를 전달한다. 시뮬레이션 결과는 이전 연구에 비해 에너지 소모 측면에서 상당히 개선되었음을 보여준다.

익수사고자에 대한 효과적인 응급처치 방법 (An effective emergency care of a person from water submersion)

  • This study was to exhibit the effective emergency care method for the drowning and non-drowning who are reached two-thousand peoples every year in our country. For investigate the effective emergency care, this study was discussed as follows ; Pathophysiology of the water submersion, Fresh-water & sea-water drowning, Factors affecting survival, and Prehospital management. The conclusions from this study were summarized as follows; 1. Remove the patient from the water. If you suspect neck or spinal injuries, Always support the head and neck level with the back and, begin rescue breathing. 2. Maintain the airway and support ventilation in the water use the jaw-thrust technique to avoid farther injury to the neck or spine. We might encounter more resistance to ventilations than you expect because of water in the airway. Once you have determined that there are no foreign objects in the airway, apply ventilations with more force; adjust ventilations until you see the patient's chest rise and fall but not until you see gastric distention. Do not attempt to remove water from the patient's lungs or stomach. 3. If there is no pulse, begin CPR. 4. Administer high-flow supplemental oxygen; suction as needed. 5. Once the patient is breathing and has a pulse, assess for hemorrhage; control any serious bleeding that you find. 6. Cover the patient to conserve body heat, Handle the patient very gently, and, Transport the patient as quickly as possible to Emergency Department, Continuing resuscitative measures during transport. If the patient have the hypothermia, follow hypothermia management.

뉴실버세대 여성을 위한 브래지어 개발 (A Study on Brassiere Development for the New Silver Generation Women)

  • This study develops a new brassiere for new silver women. A new brassiere design was developed for the silver generation based on apreliminary survey; subsequently, a wire and no-wire type of brassier were developed after a second fitting evaluation. The results were as follows. First, in terms of brassiere design, the cup shape was designed as mold type full cup, shoulder straps were designed as camisole type with cups, bias tape was designed for body correction in the side line and mash tape was designed for easy movement below the cups. On the other side ofthe cup shape was a pocketdesigned on a support pad forthe wire type and a support panel to serve as a wire in the no-wire type brassiere. Second, brassiere patterns were designed by referencing the educational H pattern. Thetotal length was elongated by 5cmto reduce pressure when wearing a brassiere and the height of the front center increased by 8cm for the stability of the front center of the brassiere. In addition, the side line was moved back by 3cm and the width of the brassiere wings was designed widely at 10.5cm. Third, a wearing test was conducted for the wire type and the no-wire type brassiere with comparison products, using a 7-point Likert scale. Appearance results and movement tests for the wire and no-wire type showed a higher score than comparison products.

Development 2.0: Principles and Warnings for Leveraging Advances in Information Communication Technologies for Improved Development Efforts

  • Advances in Information Communication Technologies (ICT) have demonstrated tremendous potential for solving development challenges and improving development processes, culminating in the new Development 2.0. Many development practitioners have embraced ICT (particularly on Web 2.0 and mobile phone technologies and applications), which have become hot topics in both the development community and the policy community as they engage in development practice and dialogue. Despite this excitement, there lacks among the policy community a robust understanding of the powers and pitfalls of ICT in development, executed actions to back the excited chatter, and dissemination of this understanding to practitioners and policymakers alike. We conducted a literature review, interviewed experts, and engaged in discussion with leaders in international development and science and technology policy to provide an operational framework base in which to view ICT in development. This framework regards ICT as tools that support more effective and efficient community development actions and appropriate consideration of general guidelines, which enable better engagement across and within sectors and individuals. Flexibility and accountability are critical requirements pervading throughout the various actions and guidelines, which promote transparent, partnership-based, and sustainable development. We highlight the strengths and weaknesses of ICT to focus on the cautions to keep ICT access and distribution in context, understand the various levels of technologies and services, and dig below the surface as excitement about ICT increases and threatens to become a short-term solution. We offer ideas for specific programs that policymakers can implement to contribute to a more efficient and effective development process to ultimately support global human development, but stress the endless possibilities that can be explored with creativity and flexibility beyond what is proposed here.

기본인명구조술 교육용 CD-Title 개발 연구 (A Study on Educational CD-Title develop to Basic Life Support)

  • The study is designed to develop an educational CD-Title for Basic Life Support. The study is conducted from July to December, 2002, Based on the course of program development suggested by Dick and Cray. the study followed the planning, development, education and evaluation of a program. The developed CD-Title consists of 8 parts. 1. Intro include Introduction, Adult CPR, Child CPR, Infant CPR, Adult Foreign-Body Airway Obstruction, Infant Foreign-Body Airway Obstruction, and Exercise in Menu at Right of screen. 2. Introduction consist of death process at cardiac arresst, chain of survival, introduction of CPR, respiratory & ciculatory anatomy and physiology. 3. Adult CPR consist of assessment responsiveness, activating EMS system, position victim, opening a airway, checking for breathing, rescue breathing, checking for sign of circulation, chest compression, 1 person & 2 persion adult CPR video. 4. Child & Infant CPR consist of, causes, assessment responsiveness, position victim, opening the airway, checking for breathing, rescue breathing, checking for sign of circulation, chest compression, activating EMS system, child & infant CPR video. 5. Adult Foreign-Body Airway Obstruction include assessment responsiveness, activating EMS system, position victim, opening a airway, checking for breathing, 2 rescue breathing, reopening the airway, 2 rescue breathing, finger sweep, 5 abdominal thrusts, unresponsiveness adult Foreign-Body Airway Obstruction video. 6. Infant Foreign-Body Airway Obstruction consist of, causes, assessment responsiveness, position victim, opening the airway, checking for breathing, 2 rescue breathing, reopening the airway, 2 rescue breathing, checking foreign-body object in oral cavity of victim, 5 back blow, 5 chest thsusts, activating EMS system, video. 7. 32 exercise consist of 8 Adult CPR, 12 Child & Infant CPR, 5 Adult Foreign-Body Airway Obstruction, 7 Infant Foreign-Body Airway Obstruction. Every part consist of animations to trigger students interests. This CD-Title will be useful education for first responders and lay rescuers.

쪼그려 들기와 허리 굽혀 들기 시 하지관절 움직임과 요추 전만에 관한 생체역학적 분석 (Biomechanical Analysis of Lower Limb Joint Motions and Lumbar Lordosis during Squat and Stoop Lifting)

  • In this study, lower extremity joint kinematics and kinetics and lumbar lordosis were investigated for two different symmetrical lifting techniques(squat and stoop) using the three-dimensional motion analysis. Twenty-six male volunteers lifted boxes weighing 5, 10 and 15kg by both squat and stoop lifting techniques. There were not significant differences in maximum lumbar joint moments between the two techniques. The hip and ankle contributed the most part of the support moments during squat lifting, and the knee flexion moment played an important role in stoop lifting. The hip, ankle and lumbar joints generated power and only the khee joint absorbed power in the squat lifting. The knee and ankle joints absorbed power, the hip and lumbar joints generated power in the stoop lifting. The bi-articular antagonist muscles' co-contraction around the knee joint during the squat lifting and the eccentric co-contraction of the gastrocnemius and semitendinosus were found to be important for straightening up during the stoop lifting. At the time of lordotic curvature appearance in the squat lifting, there were significant correlations in all three lower extremity joint moments with the lumbar joint. Differently, only the hip moment had significant correlation with the lumbar joint in the stoop lifting. In conclusion, the knee extension which is prominent kinematics during the squat tilling was produced by the contributions of the kinetic factors from the hip and ankle joints(extensor moment and power generation) and the lumbar extension which is prominent kinematics during the stoop lifting could be produced by the contributions of the knee joint kinetic factors(flexor moment, power absorption, bi-articular muscle function).

UK Urban Regeneration Policy for Comprtitiveness: A Government Perspective

  • The UK Government's approach to how to deliver regeneration in its towns and cities has changed considerably in recent years. Traditionally, urban regeneration policy focused on reversing physical, economic and social decline in an area where market forces would not do this without intervention. Since 2010 urban regeneration has become a vital part of the Government's approach to increasing local growth and competitiveness and building a strong and balanced economy. The current emphasis is on a place-based approach to regeneration that builds on the particular strengths of different places to drive growth and addresses the factors that hold them back. This paper outlines the key features of current UK urban regeneration policy and how interventions to support regeneration and growth are being pursued at different spatial scales to ensure all parts of the country benefit. They include pan regional initiatives like the Northern Powerhouse and Midlands Way, to groups of local authorities operating at a sub regional level (combined authorities/city regions), Local Enterprise Partnerships, and a variety of smaller scale programmes delivering regeneration in areas of economic and social decline. The paper explains some of the policy instruments and funding programmes available to support regeneration, and provides case studies of some major urban regeneration projects that illustrate the new approach including housing and infrastructure improvements like the planned High Speed Rail 2 line. These are supporting regeneration through the creation of strategic partnerships involving government, places and investors. The paper concludes with some lessons from past and future regeneration schemes to improve their effectiveness and impact on places and enhance local growth potential.
