비디오 컨텐츠를 사용한 홈 트레이닝이 만성 허리 통증이 있는 대학생의 배근육 두께, 허리기능 및 통증에 미치는 영향 (The effect of home training using video contents on abdominal muscle thickness, Oswestry disability index, and pain in students with chronic low back pain)

  • PURPOSE: This study examined the effect of training using video content on abdominal muscle thickness, Oswestry disability index (ODI), and pain in college students with chronic back pain. METHODS: Twenty-nine college students with chronic back pain participated in this study. The subjects were assigned randomly to 15 experimental groups who trained using video content and 14 control groups who exercised voluntarily using back exercise leaflets. The video used for the intervention was obtained from YouTube, and the difficulty level of the video was classified into six levels. Both groups participated in the intervention for 40 minutes/day, three times a week for six weeks, and the variables of abdominal muscle thickness, ODI, and pain were compared before and after the intervention. RESULTS: In the experimental group, there were statistically significant changes in the thickness of the internal oblique and transverse abdominis muscles, ODI, and pain after the intervention, except for the external oblique muscle (p < .05). In the control group, there was no statistically significant difference in all variables after the intervention (p > .05). A statistically significant difference in all variables was observed between the experimental group and the control group after the intervention except for the external oblique muscle (p < .05). CONCLUSION: Treatment of chronic low back pain using video content is a possible alternative treatment if quality images are selected and the difficulty levels are adjusted.

Risk Factors Associated with Clinical Insomnia in Chronic Low Back Pain: A Retrospective Analysis in a University Hospital in Korea

  • Background: Insomnia is becoming increasingly recognized as a clinically important symptom in patients with chronic low back pain (CLBP). In this retrospective study, we have determined risk factors associated with clinical insomnia in CLBP patients in a university hospital in Korea. Methods: Data from four-hundred and eighty one CLBP patients was analyzed in this study. The Insomnia Severity Index (ISI) was used to determine the presence of clinical insomnia (ISI score ${\geq}15$). Patients' demographics and pain-related factors were evaluated by logistic regression analysis to identify risk factors of clinical insomnia in CLBP. Results: It was found that 43% of patients reported mild to severe insomnia after the development of back pain. In addition, 20% of patients met the criteria for clinically significant insomnia (ISI score ${\geq}15$). In a stepwise multivariate analysis, high pain intensity, the presence of comorbid musculoskeletal pain and neuropathic pain components, and high level of depression were strongly associated with clinical insomnia in CLBP. Among these factors, the presence of comorbid musculoskeletal pain other than back pain was the strongest determinant, with the highest odds ratio of 8.074 (95% CI 4.250 to 15.339) for predicting clinical insomnia. Conclusions: Insomnia should be addressed as an integral part of pain management in CLBP patients with these risk factors, especially in patients suffering from CLBP with comorbid musculoskeletal pain.

The Effects of Spinal Decompression Therapy on Pain and Disability in Patients with Chronic Low Back Pain

  • Purpose: The purpose of this study was to examine the effects of spinal decompression therapy on pain and disability in patients with chronic low back pain. Methods: Twenty patients with chronic low back pain were divided into an experimental group (spinal decompression therapy, n=10) and a control group (conservative physical therapy, n=10). Both groups were treated three times a week over a four-week period. Results: The comparison of between-group changes post-treatment revealed statistically significant lower levels of pain and disability in the experimental group than the control group. The comparison of within each group changes before and after the treatment showed statistically significant declines in pain and disability indexes of both groups. Conclusion: Spinal decompression therapy may be an effective intervention for improving pain and disability in patients with chronic low back pain.

추나치료를 병행한 임신초기 급성요통 한의 치험 1례 (A Clinical Study of Acute Low Back Pain treated by Chuna & General Oriental Therapy during Pregnancy)

  • Objectives: The object of this study is to report a clinical effect of oriental medical treatments with Chuna for acute low back pain during the early stages of pregnancy.Methods: The patient in this case, 31 years-old female was admitted for 1days (20th/June/2016-30th/June/2016) due to the acute low back pain on the 7 weeks of pregnancy. We diagnosed as 挫閃腰痛, 傷筋 (Sprain and strain of lumbar spine) and treated with Chuna therapy and other conservative treatments including acupuncture herbal mixture. And we measured Visual Analog Score (VAS), Oswestry disability index (ODI), McGill pain questionnaire-short form (SF-MPQ).Results: After treatments, Visual Analog Score (VAS), Oswestry disability index (ODI), McGill pain questionnaire-short form (SF-MPQ) were significantly improved in case.Conclusion: Oriental medical treatments with Chuna manual therapy were associated with improvement of acute low back pain during the early stages of pregnancy.

Effects of Sensorimotor Training on Postural Stability and Pain in Patients with Chronic Low Back Pain

  • Background: Back pain is associated with a high risk of recurrence. Various physical therapy techniques for back pain have been studied, including reprogramming the central nervous system by integrating sensation and motion with sensory exercise training. Objectives: To aimed verify the effectiveness of sensorimotor training in improving postural stability and pain levels. Design: A randomized controlled trial. Methods: The study population was randomized into a sensory exercise training group and trunk stabilization training group and treated three times a week for 4 weeks. Each group took part in sensorimotor training for 15 minutes or lumbar stabilization exercise for 15 minutes. Results: After the intervention both groups showed Improvements in the variables. There was a significant difference in the dynamic postural stability, limit of stability, and modified visual analog scale scores in the sensorimotor training group compared to the lumbar stabilization exercise group (P<.05). Conclusion: Sensorimotor training appears to be an effective physical therapy exercise program that can be applied in patients with low back pain to improve muscle control ability.

Relationship of trunk muscle atrophy and provocation position in patients with chronic low back pain

  • Objective: The purpose of this study is to compare chronic low back pain patients' pain provocation position so as to identify the relevance with lumbar stabilizing muscles atrophy and pain provocation position. Design: Cross-sectional study. Methods: Fifty five chronic low back pain patients were participated in this study. Subjects were eligible for study participation if they were 35-55 years old and had experienced low back pain for more than 3 months. Subjects were questioned about pain and pain provocation test were done. And then they were inspected their cross sectional area (CSA) of lumbar muscles (erector spinae, iliopsoas, and multifidus) by using computed tomography. Analyze the relevance through the result data with painful area, aspect of pain and pain provocation position. Results: CSA of erector spinae showed significant decrease on ipsilateral extension position (p<0.05). Iliopsoas muscle showed significant decrease on contralateral position (p<0.05). Multifidus showed significant decrease on the position of contralateral extension and contralateral flexion (p<0.05). Conclusions: Based on the results of our study, it may be possible to evaluate muscle atrophy by assessing causing position.

  • Background: Millions of patients with chronic sciatica are still treated with epidural corticosteroids. The efficacy of epidural corticosteroids remains questionable, especially in the failed back surgery syndrome (FBSS). We studied to evaluate outcome for 10 patients with failed back surgery syndrome treated with spinal nerve root block using corticosteroids, hyaluronidase, and local anesthetics. Methods: The affected nerve roots are localized with the help of fluoroscopy and contrast dye. Local anesthetic diluted in 1,500 U hyaluronidase and 40 mg methylprednisolone is injected. A small retrospective pilot group of 10 patients with FBSS was treated. The success rate is evaluated using a visual analogue scale at 1 week and 3 month interval after the last injection. Results: Initially, 7 patients experienced good pain relief; 5 patients suspained pain relief for 3 months. No complications were observed. Conclusions: This technique is worthwhile for patients with FBSS and where epidural fibrosis is suspected to be the pain origin.

승마 운동이 만성요통환자의 통증 및 체간 유연성에 미치는 영향 (The Effect of Horse-Riding Exercise on Pain and Body Flexibility for the Patient with Chronic Low Back Pain)

  • Purpose : The purpose of this study was to investigate the effect of horse riding exercise for pain and body flexibility in the patient with chronic low back pain. Method : 26 subjects in H-equestrian were randomly divided two group, ball exercise(BE) group and horse-riding exercise(HE) group. Each group carried out 40 minutes exercise three times a week for 8 weeks. VAS were measured for pain, and evaluation of body flexibility in position with forward and backward. Result : The results were as follows, the pain scales of VAS(visual analog scale) between ball exercise and horse-riding exercise groups in post-test, were significantly different in measures(p<.05). And there were significant in two group after exercise(p<.05). The flexibility scales of body between ball exercise and horse-riding exercise groups in post-test, were significantly different in measures(p<.05). And there were significant in two group after exercise(p<.05). Conclusion : These finding revealed that horse-riding exercise was effective on pain and body flexibility of patient with chronic low back pain so that these exercise can be new altematives exercise for pain and body flexibility in the patient with chronic low back pain.

요통의 통증과 기능장애 평가에 관한 고찰 (The study of pain and functional disability scales for low back pain)

  • Objective : The low back pain was the main reason of receiving acupuncture treatment. Despite its widespread prevalence, objective data assessing the pain and the results of various forms of treatment were difficult to find in korea. In order to compare the value of different types of treatment, it was necessary to use standard and meaningful forms of assessment. Methods : Pain and functional disability scales for low back pain were reviewed for contents, measurement properties and current methodological issues. Results and Conclusions : 1. The SF-36 Bodily Pain Scale and the Graded Chronic Pain Scale were commonly used for the standard pain scale. 2. The Oswestry Disability Questionnaire and the Roland-Morris Disability Questionnaire were commonly used for the standard function scale. 3. When evaluated, there was no definite answer to the results of the treatment. In the literature, the responsiveness of the Roland-Morris Disability Questionnaire ranged from 2 to 8 points but clinically, the ranges should be minimally changed from 2 to 3 points. 4. In the future, scale for low back pain should be standardized in multiple dimension so that the computerized adaptive testing by Item Response Theory could be widely used.

  • The purpose of this report is to investigate the clinical application of conservative treatment to a patient who has the low back pain and radiational pain. In this case, a female patient, 67 years old, who had low back pain and radiational pain on the right leg. We treated the patient with conservative treatment (acupuncture, herbal medicine, moxibustion) and chuna traction. In result, the in-patient's pain has decreased and walking distance has increased from 10m to 300m for 27 days. In conclusion, conservative treatment and chuna traction could be effective methods to treat the patient who has low back pain and radiational pain.
