Risk Factor's Affecting long-term Outcome of Alport syndrome (Alport 증후군의 예후와 관련된 위험요인 분석)

  • Purpose : Alport syndrome is a hereditary nephrotic disease characterized by progressive nephrotic symptom, sensorineural hearing loss, ophthalmic abnormality, typical microscopic findings, and familial occurrence. In this study, we tried to find the risk factors related with its prognosis by taking a close observation on clinical symptoms of children with Alport syndrome reviewing retrospectively. Materials & methods : We chose children diagnosed as Alport syndrome in renal biopsy during 20 years(from 1980, Jan. until 1999, Dec.) who could receive follow up studies in tile department of pediatrics. They were divided into two groups by comparing renal function at the time of diagnosis and at current status. We compared several clinical aspects in them, and applied nonparametric test for statistical analysis. Results : The sex ratio(male:female) of 24 children was 3:1. The most common clinical symptom presented at their first visit was gross hematuria. Among those 24 children, 11 cases($46\%$) of progressing into chronic renal failure(Group II) were observed. Hypertension, proteinuria and edema were seen much frequently in group II. The level of serum protein, albumin, and creatinine clearance were decreased while BUN, creatinine were relatively increased. All the results were statistically significant. Conclusion Clinically significant risk factors related to prognosis in Alport syndrome were the presence of hypertension, edema, and proteinuria at the time of diagnosis. Also, the level of serum protein, albumin, BUN, creatinine, and glomerular filtration rate were proved to be important factors in predicting prognosis. We believe that studies on these possible risk factors would be of great help in treating and predicting prognosis of children suffering with Alport syndrome. Clinical Study of Canine Pyometra (개 자궁축농증에 대한 임상학적 연구)

  • This study was conducted to examine the prognosis of canine pyometra after surgical treatment in terms of their clinical indications, hematology and serum chemistry. A total of 20 bitches diagnosed as pyometra after physical examination was hospitalized at the Veterinary Medical Teaching Hospital, Seoul National University, during the period of 1998 to 1999. After examining all profiles, ovariohysterectomy was performed to treat pyometra. All bitches had clinical symptoms of general depression, polyuria/polydipsia, anorexia and abdominal distension. In hematologic profiles, 65% of bitches had higher level of leukocyte count compared with normal level, but RBC and PCV values maintained within the normal ranges in most pyometric bitches. In serum chemistry profiler, ALT and ALP values were increased over the normal range, and BUN and creatine values were higher than normal ranges only in 10(50%) and 3 bitches (15%), respectively. Survival rate after ovariohisterectomy was 90% (18/20), but bitches that had higher ALT, ALP, BUN, creatine and potassium than normal values were died after surgical treatment. The results of this clinical investigation show that the recovery of pyometric bitches after ovariohysterectomy can be prognosed by hematologic and serum profiles including ALT, ALP, BUN, creatinine and potassium.

A Study on the Effects of Honey-fried Astragali Radix Extract on the Nephrotoxicity in Rats Induced by Cyclosporin A (밀적황기(蜜炙黃耆)가 Cyclosporin A로 유발(誘發)된 백서(白鼠)의 신독성(腎毒性)에 미치는 영향(影響))

  • The purpose of this study is to find out the effects of honey-fried Astragali Radix extract on the nephrotoxicity in rats induced by cyclosporin A(CsA). The experimental rats were divided into three groups of ten. The Normal group was given nothing. The Control group was given only saline water every day for 14 days after the subcutaneous IV injection of 50mg/kg in CsA every other day during a period of 14 days. The sample group was administered 4.8mg/200g of Astragali Radix extract daily for 14 days after the subcutaneous IV injection of 50mg/kg in CsA every other day for 14 days. These groups were observed for 14 days. This experimental research focused on measuring the levels of BUN, creatinine, total protein, sodium, potassium, chloride, AST and ALT in the serum and specific gravity and creatinine in the urine. The results were summarized as follows: 1. Changes in serum The levels of BUN, creatinine. AST and ALT in the serum were significantly decreased in the sample group as compared with those of the control group. Total Protein level in the serum was significantly increased in the sample group as compared with that of the control group. Sodium and Potassium levels in the serum in the sample group were a little lower than those of the control group but no significance was noted. The chloride level in the serum in the sample group significantly increased as compared with that of the control group on the 7th day but in the sample group was significantly decreased as compared with that of the control group on the 14th day 2. Changes in urine Urinary specific gravity in the sample group showed significant increase, compared to the control group on the 7th day but were a little higher than that of the control group on the 14th day. Creatinine level in the urine were a little higher than that of the control group but no significance was found. These results suggest that honey-fried Astragali Radix might be effective on the nephrotoxicity in rats caused by CsA.

Effects of Different Lengths of Treatment with Rhizoma Alismatis on Diabetic Mellitus of Streptozotocin-Induced Hyperglycemic Rats (택사(澤瀉) 시간별 투여(投與)가 Streptozotocin으로 유발된 고혈당 백서(白鼠)의 실험적(實驗的) 당뇨(糖尿)에 미치는 영향(影響))

  • Objectives : The objective of the study was to observe the effects of different treatment lengths of Rhizoma Alismatis on diabetes mellitus. Methods : Rats were divided into three groups: normal(non-treated group), control (group administered saline for 4, 11, and 18 days), and RA (group administered 2.45mg/200g Rhizoma Alismatis for 4, 11, and 18 days). The experimental results were derived from the measurement of the levels of glucose, ALP, AST, ALT, creatinine and BUN in the serum from the rats on days 4, 11, and 18. Results : The glucose level in the serum significantly decreased on the 18th day. The ALP level in the serum did not show statistical significance. The GOT and GPT levels in the serum slightly decreased on the 4th, 11th, and 18th days, but did not show statistical significance. The creatinine level in the serum was unchanged for all of them. The BUN level in the serum did not show statistical significance. Conclusions : According to the above results, it is concluded that Rhizoma Alismatis has a therapeutic effect on diabetes mellitus with a longer period ofintake. As to influence upon the liver and kidney, there was no damage orside effects.

Effects of Green Tea Polyphenol and Vitamin C on Type 2 Diabetic Rats Induced by Low Dose Streptozotocin Following High Fat Diet (고지방식이와 저용량 스트렙토조토신으로 유도된 2형 당뇨병 흰쥐에서 녹차 폴리페놀과 비타민 C 병합 투여 효과)

  • This study investigated the effects of green tea polyphenol and vitamin C on type 2 diabetes mellitus by administering polyphenon 60 (P60) and sodium ascorbate (SA) to diabetic rats induced by high fat diet/low-dose streptozotocin. The experimental group was divided into five different groups: non-diabetic control group (NC), diabetes control group (DC), diabetes+P60 group (DM+P60), diabetes+SA group (DM+SA), and diabetes+P60+SA group (DM+P60+SA). P60 and SA were dissolved in 0.1% drinking water. After P60 and SA were administered for 16 weeks, fasting blood glucose, plasma insulin, serum triglyceride, blood urea nitrogen (BUN), and creatinine levels as well as kidney alkaline phosphatase (AP) and ${\gamma}$-glutamyltranspeptidase (GGT) activities were measured. Fasting blood glucose level increased 5-fold in the DC group compared to the NC group. In the DM+P60 group, fasting blood glucose level decreased by 14%. In the DM+P60+SA group, fasting blood glucose level decreased by 28% compared to the DC group, whereas the DM+SA group did not show any significant difference. The homeostasis model assessment for insulin resistance index increased in the DC group and decreased in the DM+P60+SA group compared to the DC group. Serum creatinine level increased in the DC group, but decreased by 17% in the DM+P60 group and by 43% in the DM+P60+SA group compared to the DC group. The serum BUN level increased in the DC group, but decreased by 41% in the DM+P60+SA group compared to the DC group. Kidney GGT and AP activities decreased in the DC group compared to the NC group; however, they were reversed by DM+P60+SA group. These results show that combined administration of both green tea polyphenol and vitamin C had better effects on improving blood glucose level, insulin resistance, serum triglyceride level, and protecting kidneys than administration of either green tea polyphenol or vitamin C alone in the context of type 2 diabetes.

Exploratory Study of the Prevalence of Major Chronic Disease According to Daily Water Intake and Physiological Parameters related to Dehydration in Korean Elderly: Based on the Sixth Korea National Health and Nutrition Examination Survey (2015) (한국 노인의 일일 수분섭취 상태에 따른 주요 만성질환 유병률 및 탈수 예측 생리적 지표 탐색 연구: 2015년 국민건강영양조사 결과를 바탕으로)

  • Purpose: The aim of this study was to explore the prevalence of major chronic diseases related to daily oral water intake and to identify the physiological parameters related to dehydration in Korean elderly. Methods: The data were collected from the sixth Korea Health and Nutrition Examination Survey (KHANES), which was a nationwide and cross-sectional survey in 2015. We analyzed 1,392 participants using t-test and logistic regression. All participants were divided into the adequate water intake (AWI) group and the non-adequate water intake (NAWI) group based on the dietary reference intakes for Koreans. Results: There was a significant difference in the water intake between the AWI (6.8 cups in a day) and NAWI (2.8 cups) groups (p< .001). There was no statistically significant association between the level of water intake and any of the major chronic diseases. Blood urea nitrogen (BUN) and BUN/Creatinine (Cr) ratio were significantly higher in the NAWI group. Especially, BUN/Cr ratio shows that the NAWI group reached dehydration status. Older age (adjusted odd ratio, OR= 1.07, 95% confidence interval, CI [1.04-1.10]), female gender (adjusted OR= 1.56, 95% CI [1.05-2.33]), lower body mass index (BMI) (adjusted OR = 1.00, 95% CI [0.92-1.00]), higher BUN (adjusted OR = 1.04, 95% CI [1.01-1.08]), and higher urine specific gravity (USG) (adjusted OR= 1.56, 95% CI [1.19-2.05]) were factors associated with the NAWI group. Conclusion: The findings suggest that the level of water intake needs to be considered in relation to age, gender, BMI, BUN, and USG. These are sensitive physiological parameters used for predicting dehydration of the elderly according to their daily oral water intake. It would be helpful to develop strategies to prevent dehydration in elderly individuals and enhance their water intake.

Change of Blood Magnesium Level in Diabetes Patients Undergoing Off-pump Coronary Artery Bypass Grafting

  • We carried out this study to investigate differences of physiological variables between patients with (DM group) and without type II diabetes mellitus (Non-DM group) undergoing off-pump coronary artery bypass grafting (OPCABG). Postoperative $Mg^{++}$ and $Ca^{++}$ levels were lower, whereas $Na^+$ level was higher in DM group than those in Non-DM group. ICU (intensive care unit) stay time in DM group was longer than that of Non-DM group. Postoperative platelet counts tended to decrease, whereas C-reactive protein (CRP) and cardiac troponin-I (cTNI) levels tended to increase in DM group compared with Non-DM group. Postoperative albumin level was lower, while blood urea nitrogen (BUN) and creatinine levels were greater in DM group than those in Non-DM group. DM group had higher incidence of post-operative arrhythmias than Non-DM group. These results reveal that type II DM patients undergoing OPCAB may have higher incidences of postoperative hypomagnesemia, hypocalcemia and arrhythmias, and increases of CRP, cTNI, BUN, and creatinine levels than in Non-DM patients undergoing OPCAB. The perioperative check and control (supplement) of $Mg^{++}$ levels should be considered in cardiovascular surgery combined with DM.

Protective Effect of White-Skinned Sweet Potato (Ipomoea batatas L.) against Renal Damage in Streptozotocin-Induced Diabetic Rats (Streptozotocin으로 유발된 당뇨쥐의 신장 손상에 대한 white-skinned sweet potato (Ipomoea batatas L.) 추출물의 보호효과)

  • White-skinned sweet potato (Ipomoea batatas L.) has been traditionally used for diabetes treatment and management in many countries. In this experiment, methanol extract of white-skinned sweet potato (WSPMe) at a dose of 100 or 200 mg/kg body weight was tested to evaluate its effect on renal damage in streptozotocin (STZ)-induced diabetic rats. Its efficacy was compared with that of insulin secretogogue, glimepiride ($50\;{\mu}g/kg$ body weight). Experimental diabetes was induced by a single dose of STZ (45 mg/kg, i.p.) injection. The WSPMe and glimepiride were administered orally for 14 days and the effects on glucose, renal markers including blood urea nitrogen (BUN), creatinine and lactate dehydrogenase (LDH), lipid peroxide (LPO) level, antioxidant enzymes superoxide dismutase (SOD), catalase (CAT), glutathione peroxidase (GPx) and glutathion-S-transferase (GST) activities in kidney were studied. An increase in BUN, creatinine, LDH, glucose, LPO levels and decrease in SOD, CAT, GPx and GST features were observed in diabetic control rats. Administration of WSPMe at a dose of 200 mg/kg body weight caused a significant improvement in blood glucose, LPO level, renal markers, lipid peroxidation markers and increased antioxidant levels in diabetic kidney. In conclusion, the WSPMe was found to be effective in reducing oxidative stress, thus confirming the ethnopharmacological use of I. batatas L. in protecting diabetes and its complications.

The Hematologic Study as risk factors on premature examination of stroke (PES) (중풍위험인자로서의 혈액학적 소견 연구 - 중풍조기검진 326명에 대한 분석 -)

  • 1.Purpose Stroke is the most frequent cause of mortality and morbidity rate in Korea with ischemic heart disease and cancer. Recent recurrence rate of stroke is high, but little attention had been given to the features of its' cause. The purpose of this studies is to examine the hematologic states in Korean normal adults through premature-examination of stroke(PES). 2.Method The study group consisted of 326 cases that they had no significant results by CT through premature examination of stroke(PES) in Cheonan oriental hospital of Daejeon university. We tested RBC, hemoglobin, hematocrit, WBC, PT, PTT, PLT, total protein, albumin, total bilirubin, direct bilirubin, GOT, GPT, $\gamma$-GTP, total cholesterol, triglyceride, HDL-C, glucose, BUN, creatinine, Na, K and evaluated by cross sectional study. 3.Result In the PES, there were many patients with low RBC(11%), shorten PT(26.7%), delayed PTT(19.3%), high TP, GPT, ${\gamma}$ -GTP(respective1y 16.9%, 14.4%, 13.5%), high cholesterol level and triglyceride level(respectively 18.7%, 28.2%). We didn't found significant cases in hemoglobin, hematocrit, WBC, PLT, albumin, total bilirubin, direct bilirubin, GOT, HDL-C, glucose, BUN, creatinine, Na, K. 4.Conclusion In this study, we demonstrated that short PT, high triglyceride level are special views in PES. Also prospective studies are needed continuously to search the preventing methods of the CVA.

The Effect of Smilacis glabrae Rhizoma aqua-acupuncture on injured kidneys Induced by $HgCl_2$ Intoxication in Rats (토복령약침자극(土茯笭藥鍼刺戟)이 $HgCl_2$중독(中毒) 흰쥐의 손상신(損傷腎)에 미치는 영향(影響))

  • Purpose: In order to study the therapeutic effect of TBR(Smilacis glabrae Rhizoma) aqua-acupuncture on the $HgCI_2$(mercury bichloride) intoxicated kidneys of rats. Methods: 10% TBR extract solution was aqua-punctured into the bilateral loci of Shin-su(BL23), every other day for 18 days, and the amount of leucocytes, erythrocytes, thrombocytes, hematocrit, hemoglobin, serum BUN and creatinine levels were measured and renal mercury level and histopathological findings were studied as well. The experimental groups were into nonmaI group, control group, sample I group, sample II group and sample III group. Results: 1. There were not any significant changes of leucocytes, erythrocytes and serum creatinine level in the TBR aqua-acupuncture group, saline aqua-acupuncture group and TBR aqua-acupuncture group respectively as compared with control group. 2. It showed a significant increase of hematocrit and hemoglobin level in the TBR aqua-acupuncture group on the 8th experimental day as compared with the control group. 3. It showed a significant increase of platelet in the TBR aqua-acupuncture group on the 8th experimental day, whereas significant decreases on the 18th experimental day as compared with the control group. 4. It showed a significant decrease of serum BUN level in the TBR aqua-acupuncture group on the 8th and 18th experimental day as compared with the control group. 5. It showed a significant decrease of renal mercury level in the TBR aqua-acupuncture group on compared with the control group. 6. In the histopathological findings, TBR aqua-acupuncture group showed slight atrophy of the renal tubules, mitosis and regeneration of tubular lining cells with visible nuclei, whereas the control group showed acute tubular necrosis, especially severe necrosis of tubular epithelial cells. Conclusions: Smilacis glabrae Rhizoma aqua-acupuncture on injured kidneys Induced by HgCl2 Intoxication in Rats increases hematocrit, hemoglobin and platelet level, and decreases BUN, renal mercury level. This suggests that Smilacis glabrae Rhizoma aqua-acupuncture helps to recover kidney from HgCl2 intoxication.
