요추유합술에서 응력방패 현상 감소를 위한 케이지의 유한요소해석 : CFRP 케이지와 티타늄 케이지 비교 연구 (Finite Element Analysis of Instrumented Posterior Lumbar Interbody Fusion Cages for Reducing Stress Shielding Effects: Comparison of the CFRP cage and Titanium cage)

  • 본 논문에서는 척추체 간 유합용 케이지의 응력방패현상을 감소시키기 위하여 강성 차이에 대한 연구를 수행하였다. 최근 의료 임플란트 분야에서는 탄소섬유강화 폴리머를 이용하여 좋은 결과를 보여 왔다. 그러나 생체 역학적으로 이 재료에 대하여 요추체의 안정성과 골 이식재가 받는 응력에 관련한 연구는 없었다. 따라서 이전에 유효화한 요추체 (L2-L5) 비선형 유한 요소 모델을 이용하여 L4-L5 분절의 케이지의 강성 차이에 따른 효과를 알아보기 위하여 탄소섬유강화 폴리머와 티타늄 케이지를 이용한 후방 요추체 유합 모델을 만들었다. 자가골 보다 강성이 작은 탄소 섬유강화폴리머 케이지는 인접 분절의 하종판에 응력이 적게 걸리며, 골 이식재에 응력은 증가시켰다. 위의 결과로 탄소섬유강화 폴리머 케이지는 응력 방패 현상을 감소시킬 수 있을 뿐만 아니라 골 유합률을 증가시킬 수 있다.

  • 운동 중에 있는 인체 무릎관절의 기능을 이해하기 위한 기구학적(Kinematic) 분석 과 동역학적(Kinetic) 분석을 행하는데 있어서 관절 표면의 기하학이 주요한 역할을 한다는 것은 잘 알려져 있다. 슬관절은 미끄러짐 (sliding)과 구름 (rolling) 운동을 고려하지 않고는 정확하게 모델 될 수 없다 본 연구에서는 미끄러짐 과 회전 운동 그리고 주요 인대 (ligament)를 고려한 3타원 인체 슬관절 모델을 제시한다 본 연구는. 슬관절의 전형적인측 평면 CT 영상의 확장으로부터 얻어진 두개의 캠 측면도를 이용하여 보다 실제에 가까운 관절의 기하학을 이용한 모델을 제시한다 개발된 모델은 보행 중 인체 슬관절을 통한 힘의 전달경로를 예측하는데 있어서 실험을 기초로 한 이전의 결과보고와 비교하여 볼 때 매우 잘 일치한다 대퇴골와 경골 사이의 접촉 점은 무릎의 굽힘이 진행되는 동안 전방에서 후방으로 이동하는데 이것은 무릎의 굽힘과 폄의 운동에 대한 전방/후방 운동의 커플링을 반영한 것이다. 본 연구에서 개발된 모델에 의하면 일회의 보행 사이클동안 경골 표면에 접촉 점의 전방/후방 이동변위는 바깥쪽 관절구 쪽이 약 16 mm 이고. 안쪽 관절구 쪽이 약 25 mm 이다 또한, 일회의 보행 사이클 동안 대퇴골 의 이동변위는 바깥쪽이 약 7 mm 그리고 안쪽이 약 10 mm 이다. 개발된 모델은, 관절의 퇴화를 진단 하는데 이용할 수 있는 가능성이 기대된다

슬관절 주위 글격의 기초과학 및 스포츠 손상 (KNEE: Basic Science and Injury of Bone)

  • 목적: 슬관절 주위 골격의 기초과학을 생체역학을 중심으로 설명하고 빈도가 높은 스포츠 손상들에 대해 기술하고자 한다. 해부 밀 운동학: 슬관절은 순수한 경첩 관절이 아니며 적합하지 않으므로 여섯 방향의 운동이 가능하다. 경골대퇴간 운동역학: 슬관절의 굴곡-신전 축은 시상면에 수직이 아닐 뿐 아니라, 관상면상 관절선과 평행하지 않으므로, 경골대퇴관절은 굴곡 시 내반과 내회전이 동반되고 신전 시에는 외반과 외회전이 동반된다. 슬개대퇴 관절: 슬개대퇴 관절 압박력은 슬관절의 굴곡 각도와 사두고근력에 비례한다 슬개골은 신전기전의 moment arm을 증가시켜 신전기전의 효율을 증대시키고 지렛대 역할을 한다. 슬개골 골절: 비전위성 골절이면서 하지 직거상 운동이 보존된 경우에 비수술적 치료의 적응증이 되며 수술적 치료 시 고정 방법의 선택은 골절 양상에 따라 결정되겠으나 어느 술식을 선택하여도 신전지대의 봉합은 필수적이며 중요하다 슬개골 불안정성: 선행 해부학적 이상을 조사해야 하며 급성 탈구에서도 골연골 골절편이 있거나 재발의 위험이 높은 운동선수에서 인대 봉합을 고려할 수 있다. 비수술적 치료 및 재활에도 불수하고 계속되는 재발성 아탈구나 탈구는 수술이 필요하다 학령기 스포츠 손상: Idiopathic Adolescent Anterior Knee Pain, Osgood-Schlatter Disease, Sinding-Larsen-Johansson Disease 등이 흔하다

한국인에 대한 새로운 관절주위 잠김금속판의 해부학적 적합성: 사체를 이용한 연구 (Anatomic Conformity of New Periarticular Locking Plates for Koreans: A Biomechanical Cadaveric Study)

  • Purpose: This study was conducted to confirm the anatomic conformity of the new periarticular locking plates designed by Zimmer on Korean adult bones and to identify the structures at risk during the application of these implants. Methods: The study was performed on the humerus, radius, and tibia of 10 adult cadavers(6 males and 4 females) procured from the cadaveric lab of our hospital. Anteroposterior (AP) and lateral X-rays were taken to confirm that the cadavers were free of any unusual lesions or anatomic variations. We used the 3.5-mm proximal humerus plate, 2.7-mm distal radius plate, 3.5- and 5.0-mm proximal tibia plates, and 3.5-mm distal tibia plate developed by Zimmer, Inc. (Zimmer periarticular locking plate). The longest plate from each group was used to confirm anatomical conformity. Standard approaches were used for each area, and soft tissue was retracted in order to pass the plate beneath the muscle. The position of the plate was confirmed using standard AP and lateral view X-rays. After this procedure had been completed, the region was dissected along the length of the implant to determine the conformity of the implant to bone and the penetrations of screws into the articular surface or violations of any vital structures, such as nerves, blood vessels, or tendons. Results: Excellent anatomical conformity was observed with Zimmer periarticular locking plates for Korean adults. The tibial nerve and the posterior tibial artery were found to be structures at risk when applying a distal tibial plate. Conclusion: Additional posterolateral fixation is recommended when dealing with cases of tibial plateau fracture when the fracture line extends to the posterolateral cortex. We recommend taking proper views using 10~15 degrees of internal rotation to ensure correct screw length and, thus, avoid penetration of vital structures and tendons.

앉았다 일어나는 동작동안 단축회전반경 무릎인공관절 수술자와 다축회전반경 무릎인공관절 수술자의 운동역학적 비교분석 (A Biomechanical Comparative Analysis between Single-Radius and Multi-Radius Total Knee Arthroplasty for Sit-to-Stand Movement)

  • 단축범위 무릎인공관절 수술자와 다축범위 무릎인공관절 수술자를 대상으로 앉았다 일어나는 동안 운동학적 및 운동역학적 요인들을 비교분석한 결과는 다음과 같다. 앉았다 일어나는 동작은 다축범위 수술자 집단이 단축범위 수술자 집단보다 0.19초(p= 0.033) 빠르게 나타났다. 최대상체 굴곡각도는 다축범위 수술자 집단이 단축범위 수술자집단 보다 $10^{\circ}(p=0.014)$정도 크게 나타났다. 상체굴곡 각속도는 다축범위 수술자 집단이 단축범위 수술자 집단보다 $7^{\Omega}{\cdot}S^{-1}$(p= 0.058)빠르게 나타났다. 단축범위 수술자 집단과 다축범위 수술자 집단의 ADD와 ABD의 차이는 거의 없었다. 대퇴사두근의 근전도분석은 내측광근은 무릎굴곡각 $60^{\Omega}-15^{\Omega}$(p<0.05)에서 단축범위 수술자 집단 근전도 값이 다축범위 수술자집단 근전도값 보다 작게 나타났다. 외측광근은 무릎굴곡각 $60^{\Omega}-45^{\Omega}$(p<0.05)에서 단축범위 수술자 집단 근전도 값이 다축범위 수술자집단 근전도값 보다 작게 나타났다. 대퇴직근의 값은 무릎굴곡각 $60^{\Omega}-30^{\Omega}$(p<0.05)에서 단축범위 수술자 집단 근전도 값이 다축범위 수술자집단 근전도값 보다 작게 나타났다. 대퇴이두근의 값은 무릎굴곡각 $75^{\Omega}-15^{\Omega}$(p<0.05)에서 단축범위 수술자 집단 근전도 값이 다축범위 수술자집단 근전도값 보다 작게 나타났다.

내로우 스쿼트 운동이 내반슬 성인의 무릎 사이 거리와 보행 시 무릎 관절의 생체역학에 미치는 즉각적인 영향 (The Immediately Effect of Narrow Squats on the Knee Joint Biomechanics During a Gait and Distance Between the Knees of Person With Genu-varum)

  • Background: Genu varum is also known as bow leg. It is a deformity wherein there is lateral bowing of the legs at the knee. it does give rise to pain, and persistent bowing can often give rise to discomfort in knees, hips and ankles. Objects: This study investigated the effect of narrow squats on the knee joint during a gait and distance between the knees of person with genu varum. Methods: This study analyzed 23 patient with genu varum that grade III, 12 narrow squat group and 11 genenal squat group in motion analysis laboratory. The subjects of experiment took gait before and after intervention, the range of joint motion, moment of knee joint adduction, power, distance of the knees were measured. And in order to make an analysis between groups, an paiered t-test and independent t-test was carried out. For statistical significance testing, it was decided that significance level ${\alpha}$ be .05. Results: It was shown that the group of narrow squat exercise significantly decreased in distance of knees (p<.05),In moment of adduction of knee joint, it was shown to significantly decrease in two groups (p<.05), was significantly decreased in adduction, abduction, and rotation (p<.05). In relation of peak-knee adduction moment and valgus angle, there was significant decrease in narrow squat group (p<.05). Conclusion: When the above result of study were examined, a narrow squat exercise given to the genu varum patients significantly decreased the distance between the knees, range of knee adduction and abduction, knee adduction moment, knee power. And stability gains through the decrease of excursion of knee medial part be effective for the correction of genu varum deformation.

Dislodgement resistance of modified resin-bonded fixed partial dentures utilizing tooth undercuts: an in vitro study

  • STATEMENT OF PROBLEM. Over the years, resin-bonded fixed partial dentures (RBFPDs) have gone through substantial development and refinement. Several studies examined the biomechanics of tooth preparation and framework design in relation to the success rate of RBFPDs and considered retention and resistance form essential for increase of clinical retention. However, these criteria required preparations to be more invasive, which violates not only the original intentions of the RBFPD, but may also have an adverse effect on retention due to loss of enamel, an important factor in bonding. PURPOSE. The object of this in vitro study was to compare the dislodgement resistance of the new types of RBFPDs, the conventional three-unit fixed partial denture, and conventional design of RBFPD (Maryland bridge). MATERIAL AND METHODS. Fifty resin mandibular left second premolars and second molars were prepared on dentiforms, according to the RBFPD design. After model fabrication (five group, n = 10), prostheses were fabricated and cemented with zinc phosphate cement. After cementation, the specimens were subjected to tensile loading at a cross head speed of 4 mm/min in a universal testing machine. The separation load was recorded and analyzed statistically using one-way analysis of variance followed by Duncan's multiple range test. RESULTS. Group V, the pin-retained RBFPDs, had the highest mean dislodgement resistance, whereas specimens of group II, the conventional RBFPDs, exhibited a significantly lower mean dislodgement resistance compared to the other 4 groups (P <.05). There were no significant differences between group I, III, and IV in terms of dislodgement resistance (P>.05). Group V had the highest mean MPa (N/$mm^2$) (P <.05). There was no significant difference between groups I, II, III and IV (P > .05). CONCLUSION. Within the limits of the design of this in vitro study, it was concluded that: 1. The modified RBFPDs which utilizes the original tooth undercuts and requires no tooth preparation, compared with the conventional design of RBFPDs, has significantly high dislodgement resistance (P < .05). 2. The modified RBFPDs which utilizes the original tooth undercuts and requires minimal tooth preparation, compared with the conventional FPDs, has significantly no difference in retention and dislodgement resistance)(P>.05). 3. The pin-retained FPDs showed a high dislodgement resistance compared to the conventional three-unit FPDs (P<.05).

교정용 와이어의 비틀림 모멘트 (Torsional moment of orthodontic wires)

  • 각형 호선이 edgewise 브라켓에 삽입되면 first, second order는 bending에 의해, third order는 torsion(비틀림)에 의해 3차원적인 force system이 발생한다. Bending에 관하여는 분석적 그리고 실험적인 많은 연구가 보고 되어 있는 반면 비틀림에 관해서는 상대적으로 많은 연구가 이루어 지지 않았다. 본 연구의 목적은 각형 와이어의 재료와 단면의 형태가 와이어의 비틀림 모멘트에 어떻게 영향을 주는지를 이론적, 실험적으로 밝혀서 임상적으로 적절한 모멘트를 가할 때 호선의 재료와 굵기를 합리적으로 선택할 수 있도록 하는데 있다. 실험재료로는 third order조절을 위해 가장 많이 사용하는 호선을 사용하였다. 크기별로 0.016x0.022, 0.017x0.025, 0.019x0.025인치 그리고 재료로는 stainless steel (Ormco), TMA(Ormco), NiTi(Ormco), 그리고 braided stainless steel(DentaFlex, Dentauum) 네 가지를 사용하여 총 12개의 조합을 사용하였다. Torsion formula를 이용하여 비틀림 강성 (torque/twist rate)을 계산하였고 torque gauge를 이용하여 비틀림 강성, 항복 비틀림 모멘트 (yield torsional moment), 그리고 최대 비틀림 모멘트 (ultimate torsional moment)를 측정하였다. Torsion formula에 의하면 비틀림 강성 (T/$\theta$)은 재료적인 특성(G)과 호선의 단면의 특성(J)에 비례하고 호선의 길이(L)에 반비례한다. 대부분의 실험치는 이론적인 값과 비슷하게 나타났다.

중량물 들기 작업시 물체 무게중심 및 발의 위치가 허리 근육의 최대 EMG 진폭에 미치는 영향 (Effects of Load Center of Gravity and Feet Positions on Peak EMG Amplitude at Low Back Muscles While Lifting Heavy Materials)

  • Objectives: This study's aims were to evaluate the effects of load center of gravity within an object lifted and feet placements on peak EMG amplitude acting on bilateral low back muscle groups, and to suggest adequate foot strategies with an aim to reducing low back pain incidence while lifting asymmetric load. Methods: The hypotheses that asymmetric load imposes more peak EMG amplitude on low back muscles contralateral to load center of gravity than symmetric load and maximum peak EMG amplitude out of bilateral ones can be relieved by locating one foot close to load center of gravity in front of the other were established based on biomechanics including safety margin model and previous researches. 11 male subjects were required to lift symmetrically a 15.8kg object during 2sec according to each conditions; symmetric load-parallel feet (SP), asymmetric load-parallel feet (AP), asymmetric load-one foot contralateral to load center of gravity in front of the other (AL), and asymmetric load-one foot ipsilateral to load center of gravity in front of the other (AR). Bilateral longissimus, iliocostalis, and multifidus on right and left low back area were selected as target muscles, and asymmetric load had load center of gravity 10cm deviated to the right from the center in the frontal plane. Results: Greater peak EMG amplitude in left muscle group than in right one was observed due to the effect of load center of gravity, and mean peak EMG amplitudes on both sides was not affected by load center of gravity because of EMG balancing effect. However, the difference of peak EMG amplitudes between both sides was significantly affected by it. Maximum peak EMG amplitude out of both sides and the difference of peak EMG amplitude between both sides could be reduced with keeping one foot ipsilateral to load center of gravity in front of the other while lifting asymmetric load. Conclusions: It was likely that asymmetric load lead to the elevated incidence of low back pain in comparison with symmetric load based on maximum peak EMG amplitude occurrence and greater imbalanced peak EMG amplitude between both sides. Changing feet positions according to the location of load center of gravity was suggested as one intervention able to reduce the low back pain incidence.

요추부 협착증 치료를 위한 극돌기 삽입술의 생체역학적 효과 분석 (A Biomechanical Analysis of an Interspinous Distraction Device for Treatment of Lumbar Spinal Stenosis)

  • Many types of interspinous distraction devices (IDDs) have been recently developed as an alternative surgical treatment to laminectomy and fusion with pedicle screws for the treatment of the lumbar spinal stenosis (LSS). They are intended to keep the lumbar spine in a slightly flexed posture to relieve pain caused by narrowing of the spinal canal and vertebral foramen. However, their biomechanical efficacies are not well known. In this study, we evaluated the kinematic behaviors and changes in intradiscal pressure (IDP) of the porcine lumbar spine implanted with IDD. For kinematics analysis, five porcine lumbar spines (L2-L6) were used and the IDD was inserted at L4-L5. Three markers (${\phi}{\le}0.8mm$) were attached on each vertebra to define a rigid body motion for stereophotogrammetric assessment of the spinal motion in 3-D. A moment of 7.5Nm in flexion-extension, lateral bending, and axial rotation were imparted with a compressive force of 700N. Then, IDD was implanted at L3-L4. IDPs were measured using pressure transducer under compression (700N) and additional extension moment (700N+7.5Nm). In kinematic behaviors, insertion of IDD resulted in statistically significant decrease 42.8% at the implanted level in extension. There were considerable changes in ROM at the adjacent levels, but statistically insignificant. In other motions, there were no significant changes in ROM as well regardless of levels. IDPs at the surgical level (L3-L4) under compression and extension moment decreased by 12.9% and 18.8% respectively after surgery (p<0.05). At the superiorly adjacent levels, IDPs increased by 19.4% and 12.9% under compression and extension, respectively (p<0.05). Corresponding changes at the inferiorly adjacent levels were 29.4% and 6.9%, but they were statistically insignificant (p>0.05). The magnitude of pressure changes due to IDD, both at the operated and adjacent levels, were far less than the previously reported values with conventional fusion techniques. Our experimental results demonstrated the IDDs can be very effective in limiting the extension motion that may cause narrowing of the spinal canal and vertebral foramens while maintaining kinematic behaviors and disc pressures at the adjacent levels.
