The effects of an additional weight aquatic exercise program on balance and lower extremity strength in persons with stroke: randomized controlled study

  • Objective: The purpose of this study was to investigate the effects of an additional weight aquatic exercise program on the balance and lower extremity strength on aquatic environment in persons with stroke. Design: Randomized controlled trial. Methods: All subjects were randomly divided into three groups where thirteen subjects were in the additional weight aquatic exercise group, twelve subjects in the aquatic exercise group, and fifteen subjects in the control group. Subjects received a graded aquatic exercise program for 30 minutes, with 3 sessions per week for 6 weeks, and subjects in all groups received conventional physical therapy. All subjects were assessed with the Medical Research Council (MRC), the Berg Balance scale (BBS), Timed Up and Go test (TUG), and 10-meter walk test (10MWT) pre and post intervention. Results: The MRC, BBS, TUG, and 10MWT scores significantly improved post-intervention (p<0.05), and the control group also had significantly improved in all areas post-treatment (p<0.05). In addition, it has been confirmed that the additional weight aquatic exercise group had significantly improved in MRC, BBS, and TUG scores compared with the aquatic exercise and control group (p<0.05). Conclusions: The findings of this study suggested that the additional weight aquatic exercise program improves lower extremity and balance in persons with stroke.

Effects of Squat Exercise according to Weight Support on Balance and Gait in Patients after Total Hip Replacement: a Pilot Study

  • Purpose: The purpose of this pilot study is to identify the problems and stability of a study to investigate "Effects of Squat Exercise according to Weight Support on Balance and Gait in Patients after Total Hip Replacement." before proceeding with the study. Methods: Twenty-two rehabilitation patients after THR surgery who met the selection criteria participated. The study subjects were randomly assigned to a squat group using a slider or a squat group using a reformer. The interventions were applied for two weeks. The patients were assessed using Berg balance scale (BBS), Timed up and go test (TUG), and 10-meter walking test (10MW). Results: Although twenty-two study subjects participated in this study, eight study subjects participated dropouts occurred during the study period. There was a significant difference within the group in BBS and TUG in two groups (p<0.05). The difference between the two groups was not significant in all outcome measures (p>0.05). The largest effect size was 1.21 and the smallest effect size was 0.39, all from the BBS. Conclusion: This pilot study suggest that it is feasible with minor adjustment to conduct a larger scale, powered RCT to examine the efficacy of squat exercise according to weight support with patients after THR.

The Effect of Side-Step Tasks Based Circular Training Program on Balance and Gait in Stroke Patients

  • Objective: The purpose of this study was to investigate the effect of the side-step tasks based circular training program (STCT) on balance and gait characteristics in stroke patients. Design: A randomized controlled trial Methods: Twenty-four stroke patients were randomly divided into two groups of twelve patients each. One group was applied with the STCT whereas the other group was treated with conservative physiotherapy (CP). The ability of gait was measured in 10m walking test and stride length on both side using BTS G-WALK (BTS Bioengineering S.p.A, Italy) and the ability of balance was measured in Berg Balance Scale (BBS) and Timed Up and Go Test (TUG). Results: The STCT group was significant differences in the balance parameters of BBS and TUG (p<0.05) and showed significant differences in gait variables in 10m walking speed, stride length of affected and non-affected side after the experiment before and after the experiment (p<0.05). In addition, the STCT group showed a significant difference in BBS compared to the control group (p<0.05). Conclusions: The results of this study confirmed that the side-step tasks based circular training program (STCT) improves balance and walking ability in stroke patients. STCT is expected to be used as a useful intervention method for stroke rehabilitation.

라이프케어 증진을 위한 전신진동운동이 낙상위험 뇌졸중 환자의 균형 및 보행능력에 미치는 영향 (The Effect of Whole-Body Vibration Exercise for Life Care on Balance and Walking Ability in the Stroke Patient with Risk of Fall)

  • 본 연구는 라이프케어 증진을 위한 전신진동운동이 낙상위험 뇌졸중 환자의 균형 및 보행능력에 미치는 영향을 알아보았다. 본 연구의 대상자는 뇌졸중 환자 30명을 대상으로 전신진동운동군과 오타고운동군으로 15명씩 할당하여 균형능력은 버그균형척도(BBS)와 일어서서 걷기 검사(TUG), 보행능력은 10m 보행속도검사와 Tinetti 운동성 검사를 측정하여 운동 전과 운동 후, 군 간을 비교·분석하였다. 집단 내 운동 전·후의 차이를 비교하기 위해 대응표본 t-검정을 사용하였고, 집단 간의 차이를 비교하기 위하여 독립표본 t-검정을 실시하였다. 실험결과 두 군 모두 운동 전후 BBS와 Tinetti 운동성 검사는 유의하게 증가하였고, TUG와 10m 보행속도는 유의하게 감소하였다. 군 간 비교에서는 BBS, TUG, 10m 보행속도, Tinetti 운동성 검사 모두 유의한 차이가 있었다. 본 연구결과를 통해 라이프케어 증진을 위한 전신진동운동은 낙상위험 뇌졸중 환자의 균형능력과 보행능력을 개선하는데 영향을 미치는 것으로 사료된다. 따라서 낙상위험 뇌졸중 환자의 균형능력과 보행 능력을 향상을 통한 라이프케어 증진을 위해 전신진동운동 프로그램을 권장한다.

Correlations between Foot Sensory Impairment, Timed Up and Go Test and Berg Balance Scale, and Mental Activities for Prevention of Falling in Elderly People

  • PURPOSE: This research was designed to study the correlations between physical activities such as foot sensory impairment, balance and ambulatory abilities, as well as fall risk factors and mental activities such as cognitive performance and depression in order to provide fundamental data for the prevention of falling in the elderly. METHODS: A total of 36 elderly people over the age of 65 years who voluntarily joined the research were selected as subjects. Sensory perception in both feet was measured by using a monofilament in 10 parts with each part given one point. The points for each foot were integrated to analyze the correlations between physical and mental activities indices, and the results were analyzed by using Pearson's correlation coefficient. RESULTS: The results of this research show that the extent to which senses in feet were impaired was correlated to TUG, a functional activity index involved in tranfers, gait, and turning movements, which was correlated to BBS (balance index) as well as MMSE-K and K-GDS (mental activities index). BBS was interrelated to FES-K (physical activities index) and MMSE-K. The muscular strength of the right ankle dorsiflexor had mutual relations with FES-K. CONCLUSION: The results of this research indicate that TUG is widely correlated to foot sensory impairment as well as general physical and mental activities in elderly people. BBS was also shown to be correlated with TUG, FES-K, and MMSE-K. Thus, it might be necessary that TUG and BBS be included as two items in physical and mental check-ups for the elderly, and further studies on correlations using evaluation items for physical and mental activities should lead to the simplification of the evaluation criteria.

균형감각 및 보행 장애 뇌출혈 환자에 대한 한방치료 치험 3례 (The Effect of Korean Medical Treatment on Imbalance and Gait Difficulty of Cerebral Hemorrhage Patient: Report of Three Cases)

  • Objectives : The purpose of this study is to find the effectiveness of Korean medical treatment on sense of balance by cerebral hemorrhage. Gaitview analysis and Berg balance scale(BBS) research were used to evaluate it. Methods : We selected patients who were hospitalized at the Kangnam Korean Hospital, Kyung Hee University Hospital from May 2014 to August 2014 that satisfied inclusion/exclusion criteria. Before taking treatment, we took gaitview analysis and BBS first to analyze the ability of balance and gait of patients. In gaitview analysis, we checked Center of gravity(COG), pressure ratio, ave pressure, gait time ratio, statistic score. For about 10 days, patients were given acupuncture treatment, herbal medicine and physical therapy. Afterwards, we compared patients' progression between before-treatment and after-treatment with the same evaluation scale. Results : In the period of treatment, patients showed static and dynamic index change within the normal range or increase. And in statistic score, 2 cases score are increased. In BBS, all cases score are increased. Conclusions : Korean medical treatment improves imbalance symptom of cerebral hemorrhage patient. So the ability of gait and activity of daily living is improved. But, this study has only 3 cases, and has no comparable study. Prospective and Larger study will be necessary.

Clinical Criteria to Perform the Step through Step Gait with a Cane in Chronic Stroke Patients

  • PURPOSE: The purpose of this study was to propose clinical criteria to differentiate patients who are able to perform the step-through-step gait pattern in chronic stroke patients. METHODS: Sixty patients with chronic stroke patients participated this study. To differentiate patients who could perform the step-through-step gait pattern, age, gender, and causes of stroke were noted, a Chedoke-McMaster (CM) damage list, Fugl-Meyer (FM) assessment scales and the Berg Balance Scale (BBS) were determined. A 10 meter gait test and Timed Up and Go (TUG) test were conducted to determine the differences in gait speed and dynamic balance between patients walking with or without canes in the step-through-step gait pattern group. RESULTS: There was no significant statistical difference in age, gender, and stroke type between all subjects. There were significant differences in the CM scale for postural and lower extremities, and FM scale for lower extremities and BBS. The dynamic balance ability and gait speed showed significant differences between the subjects in the step-through-step gait pattern with or without a cane during gait. CONCLUSION: CM and FM scales for the lower extremities and postural control, as well as BBS scales, can be used as criteria to differentiate patients who are able to perform the step-through-step gait pattern. These results can also be used to provide beneficial information to patients that are walking with canes.

회전보행 훈련이 초기 뇌졸중 환자의 균형 자신감, 낙상 효능에 미치는 영향: 무작위 대조 예비 연구 (Effect of Rotation Curved Walking Training on Balance Confidence and Falls Efficacy in Early Stroke Patients: A Randomized Controlled Pilot Study)

  • Purpose: This study aimed to determine the effect of curved walking training on balance confidence and fall efficacy in early stroke patients. Methods: The study included 16 early stroke patients who were randomly allocated to a curved walking training group (experimental group, N=8) and a straight walking training group (control group, N=8). Both groups performed the exercise 5 times a week for 3 weeks. Outcomes were assessed using the Activities-specific Balance Confidence (ABC) Scale, Fall Efficacy Scale (FES), Berg Balance Scale (BBS), and Timed Up and Go (TUG) test. Results: After 3 weeks of training, both groups showed significantly improved ABC, FES, BBS, and TUG (p<.05 in both groups). However, the ABC, FES, BBS, and TUG scores in the experimental group were significantly better than those in the control group (p<.05). Conclusion: These findings indicate that curved walking training may be effective at improving balance confidence and decreasing fall risk in early stroke patients. Therefore, curved walking training can be used as a recommended walking method in early stroke patients.

관성센서를 이용한 버그균형검사 점수 분류 연구 (Berg Balance Scale Score Classification Study Using Inertial Sensor)

  • 본 논문에서는 균형평가도구 중 임상에서 가장 많이 사용되는 BBS(Berg Balance Scale)를 머신러닝 기법을 이용하여 점수 분류 정확도를 제시한다. 데이터취득은 Noraxon 시스템을 이용하여, 신체 8군데(왼쪽 오른쪽 발목, 왼쪽 오른쪽 엉덩이 위, 왼쪽 오른쪽 손목, 등(Back), 이마)에 관성센서를 부착하였다. 관성센서의 3축 가속도데이터를 기반으로 특징벡터 STFT(Short Time Fourier Transform), SAM(Signal Area Magnitude)를 추출하였다. 그 다음, BBS의 항목을 동작특성에 따라 정적인 동작(static movement)과 동적인 동작(dynamic movement)으로 나누었고, BBS의 각 항목에 대하여 점수에 영향이 있는 센서부착위치에 따라 특징벡터를 선별하였다. BBS의 항목마다 선별된 특징벡터는 GMM(Gaussian Mixture Model)을 이용하여 분류하였다. 실험대상자 40명에 대한 정확도 산출결과, 1번순으로 차례대로 55.5%, 72.2%, 87.5%, 50%, 35.1%, 62.5%, 43.3%, 58.6%, 60.7%, 33.3%, 44.8%, 89.2%, 51.8%, 85.1%의 분류 정확도를 확인하였다.

도시 거주 노인의 낙상 위험요인 분석 (Fall Risk Analysis of Elderly Living in the City)

  • 본 연구의 목적은 도시 거주 노인을 대상으로 낙상과 관련된 위험 요인들을 찾아 비교하는 것이다. D 광역시에 거주하는 65세 이상 노인 62명을 대상으로, 과거 1년 동안의 낙상 경험을 토대로 낙상군과 비낙상군으로 분류하였다. 일반적 특성, 낙상관련 생활습관 및 만성질환, 균형과 관련된 신체적(BBS), 심리적 검사도구(K-ABC)와 우울증 평가(SGDS)를 통해 그룹 간의 차이를 비교하고, 유의한 차이를 보인 변수들의 상관관계를 확인하였다. 연구 결과, BBS와 K-ABC, 그리고 SGDS 점수는 그룹 간에 통계학적으로 유의한 차이를 나타내었다(p<0.05). 또한, BBS와 K-ABC 점수는 양의 상관관계를 보였으며, K-ABC와 SGDS 점수는 음의 상관관계를 보였다(r=-0.472). 결과에 기초하여, SGDS와 함께 ABC와 BBS는 낙상 평가 도구로써 추천할 만한 도구임을 확인할 수 있었다.
