청소년기에서 발생한 슬개골 상단부 소매형 골절 (Sleeve Fracture of the Superior Pole of Patella in an Adolescent)

  • A patella sleeve fracture is a type of avulsion fracture in which a small osseous fragment gets, along with a sleeve of periosteum and cartilage, is pulled off from the patella. The avulsed sleeve of cartilage and periosteum is not apparent on plain radiographs, which can result in a missed diagnosis or an underestimate of the extent of injury. An avulsion or sleeve fracture of the patella in a child can occur at the superior or the inferior pole of the patella. However, most reported cases of sleeve fractures involve the inferior patellar pole, with fractures involving the superior patellar pole being very rare. The authors report a case of a sleeve fracture of the superior pole of the patella in an adolescent; the fracture was diagnosed with magnetic resonance imaging (MRI) and was treated with surgery after early diagnosis. The course was uneventful, and the outcome was excellent.

  • We report our experience of treating two patients with ankle amputation with different presentations. The first case was a clean-cut sharp amputation. The second case was an avulsion injury following a motor vehicle accident in a patient who arrived 8 hours after the injury. Replantation was successful in both cases. In avulsion injuries, a secondary operation for wound coverage is required at a later stage. With good strategy and a support team, encouraging limb survival outcomes are possible post-replantation.

  • Currently, the literature contains few studies that describe any potential complications following arthroscopic acromioplasty. Because part of the anterior deltoid originates from the anterior acromion, there is a risk for violation and subsequent iatrogenic rupture or avulsion during this procedure. This type of injury can be a devastating problem for patients that may lead to poor function and debilitating pain. We present a patient with deltoid insufficiency following arthroscopic acromioplasty who elected to proceed with operative management with a planned arthroscopic evaluation of the shoulder followed by an open deltoid repair. At the final follow-up visit 2.5 years postoperatively, the patient reported improved pain from baseline and no residual disability and was able to perform most activities of daily living without difficulty. This case serves as an example of a surgical repair for a deltoid avulsion following arthroscopic acromioplasty. As there is still a lack of standard guidelines, our suture repair technique can be considered one method of treatment for this type of injury.

  • Total scalp avulsion is rare but a devastating injury. This condition burdens the patient with disfigured cosmetic appearance and permanent psychosocial trauma. Throughout history, this condition has been favored in women working with mechanics since the long hair function as a vector appliance for oblique pull of the hair into a stationary torque. We present our experience of the replantation of the total avulsed scalp in male, first to be reported in Korea. Warm ischemic time was exceeding 16 hours along with severe crushed condition of the detached margin, a relatively satisfactory result was obtained.

  • The lateral hoof of right hind limb of a pregnant Masai giraffe was accidentally injured, and its coronet, hoof wall, and sole were detached in a mass. The bleeding was observed from the exposed dermis of the affected foot for several hours, with second scoreof supporting lameness. The bleeding from the digital artery at the exposed dermis was stopped by epinephrine spray and direct pressing. Nitrofurazone ointment was topocally applied for 5 days and sxytetracycline solusion was delivered by a fluid jet in the affected hoof one month. The calf was born 5 months after the avulsion. The horn of the hoof wall was grown from the coronet to sole, and was gradually filled from the heel to the toe. The growth rate and filling rate of the hoof wall were 0.32mm and 0.54mm a day, respectively.

  • The avulsion fracture of the calcaneal tuberosity is rare injury. Usually, it occurs from indirect trauma in old patients with osteoporosis or in patients with diabetic neuropathy. Especially, the bone and joint damage occurred in active patient with severe sensory loss or arthropathy related to nerve damage regardless of the cause is referred to neuropathic arthropathy. Generally, a patient with nondisplacement or minimally displacement is treated by conservative therapy and a patient with severe displacement is treated by open reduction and internal fixation. We experienced a 33 years-old woman with diabetes mellitus who had the displaced avulsion fracture of the calcaneal tuberosity without significant trauma and did not treat. We report upon this case at the 2 years follow-up.

  • Avulsion fracture of the calcaneal tuberosity is an uncommon injury. Usually it occurs from indirect trauma, and can be seen in old patients with osteoporosis or in patients with diabetic neuropathy. Follow-up studies showed healing of the fracture in most cases, but skeletal deformity may develop in some cases. Therefore we should take plain X-ray evaluations in diabetic patients with foot and ankle pain, even though there have been no definite trauma history. Four cases of calcaneus avulsion fracture were treated operatively in diabetic patients, and reported.

수지 첨부 재접합술 101예의 후향적 분석 (A Retrospective Analysis of 101 Cases of Distal Digital Replantation)

  • We retrospectively evaluated our results of replantations of distal digital amputations and analyzed the factors deterrent to the survival of replanted digits. From January 2004 to 2005 June, we performed 101 cases of replantations following complete amputations at or distal to interphalangeal joint level. The study included 98 patients with a mean age of 35.6 years (range 1 to 63 years). Amputation level correlated to zone I (distal to the lunula)in 47 cases and zone II (lunula to distal interphalangeal joint) in 54 cases according to Yamano's classification. According to the mechanism of amputation, 24 cases (22.9%) suffered from guillotine type injury, 27 cases (27.1 %) from avulsion type injury and 50 cases (50%) from crush type injury. In all cases, a single arterial anastomosis was performed. Venous anastomosis on either volar or dorsal side was performed in 12 cases of amputation in zone II. Salvage procedure for venous drainage was performed in 98 cases. The mean duration of salvage procedures was 5.9 days (ranging from 4 to 14 days). Successful replantation was achieved in 96 cases (95.1%), which included 93.7% cases in zone I amputations and 96.3% cases in zone II amputations. A single venous anastomosis was performed in 12 cases of amputation in zone II. All of them survived completely. Among the 5 cases that failed to survive, 3 cases were related with avulsion injury in zone I. Initial mechanism of injury determines the survival rate of amputated parts as it is directly related with the status of vessels and soft tissues. Meticulous precaution during the salvage procedure may affect the overall survival rate of distal digital replantations.

가로손가락손바닥활을 이용한 다발성 벗겨짐 손상 손가락 재접합술 - 증례보고 - (Case Report of Avulsion Amputation of Multiple Digits: Use of Rerouting the Transverse Digital Palmar Arch)

  • Purpose: Avulsion injuries of digits have been presented for a long time as complex management problems. Despite of microsurgical advances, it is difficult to achieve good functional results and their management remains somewhat controversial. However, in a finger there are three transverse digital palmar arches. The middle and distal transverse digital palmar arches are consistently large(almost 1 mm) and may be used for arterial vessel repairs either proximally or distally, depending on the length and direction needed. 39-year-old man presented with avulsion amputation of the ulnar three digits, was operated using only arterial anastomosis with rerouting the transverse digital palmar arches. Methods: Replantation was performed using the artery-only technique. Because the digital arteries had been damaged, we did that the transverse digital palmar arches were transposed in an inverted Y to I configuration and were lengthened with rerouting them for the purpose of direct anastomosis of the digital artery. Venous drainage was provided by an external bleeding method with partial nail excision and external heparin irrigation. Results: The authors conclude that complete avulsion amputations with only soft tissue at the distal to insertion of the flexor digitorum superficialis tendon were salvageable with acceptable functional results. All three fingers survived. Conclusion: With technical advancements, the transverse digital palmar arches play an important role for finger amputation. Three digital palmar arches give us additional treatment option for the finger amputation. In this case, replantation with only-arterial anastomosis was successful and we obtained good aesthetic and functional outcome.

  • 소아에서후방십자인대의경골부착부위의견열골절은발생빈도가매우낮으며그보고된경우도드물다. 이에저자들은슬관절동통과종창을주소로내원하였으며, 슬관절 후방 슬와부에압통과 종창, 혈관절증, 후방 불안정성, 경골의 후방 전위, 슬관절신전제한소견을보이고있으며 방사선학적검사상후방십자인대의경골과간후면 부착부위에견열골절을 보이는13세 소년에게서 슬관절후방슬와부를 통한 견인 봉합술(pull-out suture)로 고정하여 치료한1예의 결과를 문헌 고찰과 함께보고하고자한다.
