Case-Control Study of Diet in Patients with Cervical Cancer or Precancerosis in Wufeng, a High Incidence Region in China

  • Purpose: To investigate the diet of patients with cervical cancer and precancerosis in the Wufeng area, a high-incidence region in China. Methods: In the case group, 104 patients diagnosed with cervical cancer or cervical intraepithelial neoplasias (CINII/III) were recruited from the Wufeng area. Nine hundred thirty-six healthy women were selected from the same area as the matched controls. A questionnaire, which included questions about general lifestyle conditions, smoking and alcohol status, source of drinking water, green tea intake, and diet in the past year, was presented to all participants. Results: Green tea intake (P=0.022, OR=0.551, 95% CI=0.330-0.919) and vegetable intake (P=0.035, OR=0.896, 95% CI=0.809-0.993) were identified as protective factors against cervical cancer or CINII/III. There was no indication of any associations of other lifestyle factors (smoking status, alcohol status, source of drinking water) or diet (intake of fruit, meat/egg/milk, soybean food, onion/garlic, staple food and pickled food) with cervical cancer. Conclusions: The results suggest that eating more fresh vegetables and drinking more green tea may help to reduce the risk of cervical cancer or CINII/III in people of the Wufeng area.

Japan's Policy and Implications for Expansion of Doctoral Manpower (일본의 의사인력 확충 정책과 시사점)

  • Under "Emergency Doctor Security Act of 2007", Japanese government focuses on relocating doctors in medically underserved areas and matching supply and demand of manpower in order to resolve the ubiquity of specific treatment departments. Particularly, there are two alternatives to secure the number of local physicians and avoid the regional shortage of doctors in the short term. Firstly, the government attracts existing physicians to the region. Secondly, the government increases the capability of regional clinical training system to prevent the influx of doctors to the larger cities. Also, it seeks solutions from various perspectives, such as cultivating manpower to work in vulnerable areas through the regional frame system and autonomous medical college. This study introduces the case of Japan as a benchmark and suggests that policymakers should focus on the recent conflict between the government and the medical consultation. More specifically, this study provides policy implications on the alternative measures for securing the manpower of regional doctors.

  • The Patient Safety Act was enacted on July 26, 2016. Patient safety law is a method to prevent harm by collecting and accumulating various errors through the reporting system. Therefore, in order for this law to be successfully implemented, it is necessary to vitalize 'the autonomous reporting and reporting learning system of patient safety accidents'. And In order for this system to be activated, a large amount of reporting data accumulation is a prerequisite. Nevertheless, there were only two reports in about 17 months. In this paper, I will criticize the validity of the current autonomous reporting system and the two proposed amendments, I would like to propose the introduction of a partial obligation reporting system.

Modified PSO Based Reactive Routing for Improved Network Lifetime in WBAN

  • Technological advancements taken the health care industry by a storm by embedding sensors in human body to measure their vitals. These smart solutions provide better and flexible health care to patients, and also easy monitoring for the medical practitioners. However, these innovative solutions provide their own set of challenges. The major challenge faced by embedding sensors in body is the issue of lack of infinite energy source. This work presents a meta-heuristic based routing model using modified PSO, and adopts an energy harvesting scheme to improve the network lifetime. The routing process is governed by modifying the fitness function of PSO to include charge, temperature and other vital factors required for node selection. A reactive routing model is adopted to ensure reliable packet delivery. Experiments have been performed and comparisons indicate that the proposed Energy Harvesting and Modified PSO (EHMP) model demonstrates low overhead, higher network lifetime and better network stability.

Fishborne Trematode Metacercariae in Freshwater Fish from Guangxi Zhuang Autonomous Region, China

  • A survey was performed to investigate the infection status of fishborne trematode (FBT) metacercariae in freshwater fish from Guangxi Zhuang Autonomous Region, China. A total of 307 freshwater fish of 31 species were collected from 5 administrative regions of Guangxi Zhuang Autonomous Region. They were examined by artificial digestion method from July 2003 to August 2004. No metacercariae were detected in fish from Fusui-xian. In fish from Mashan-xian and a market in Nanning, 3 species of metacercariae, Haplorchis taichui, Haplorchis pumilio, and Centrocestus formosanus, were mainly detected. Metacercariae (8 in number) of Clonorchis sinensis were found in 1 Chanodichthys dabryi purchased from a market in Nanning. In fish from Yangshuo, Metagonimus yokogawai metacercariae were detected from all 18 fish species examined. Total 13 C. sinensis metacercariae were found in 3 out of 10 Hemibarbus maculatus from Yangshuo. All7 Zacco platypus from Yangshuo were infected with 8-112 Echinochasmus perfoliatus metacercariae. In fish from Binyang-xian, H. pumilo metacercariae were mainly detected in all 5 fish species examined, and only 1 metacercaria of C. sinensis was found in a Hemiculter leucisculus. From the above results, it was confirmed that some species of freshwater fish play a role of second intermediate hosts for FBT in Guangxi Zhuang Autonomous Region, China. In particular, 4 species of intestinal flukes, M. yokogawai, H. taichui, H. pumilio, and C. formosanus, were prevalent in fish hosts, whereas C.sinensis metacercariae were detected only in 3 fish species.

  • TIM-1 (also known as KIM-1 and HAVcr-1) is a type I transmembrane glycoprotein member of the TIM family that may play important roles in innate and adaptive immune responses. The overexpression of proteins associated with membrane proteins is a major obstacle to overcome in studies of membrane protein structures and functions. In this study, we successfully coupled the overexpression of the TIM-1 protein with a C-terminal enhanced green fluorescent protein (GFP) tag in Escherichia coli. To the best of our knowledge, this report is the first to describe the overexpression of human TIM-1 in E. coli. The purified TIM-1-EGFP fusion protein recognized and bound directly to apoptotic cells and did not to bind to viable cells. Furthermore, we confirmed that the interactions of TIM-1-EGFP with apoptotic cells were blocked by TIM-1-Fc fusion proteins. This fusion protein represents a readily obtainable source of biologically active TIM-1 that may prove useful in future studies of human TIM-1.

Genotype Distribution and Behavioral Risk Factor Analysis of Human Papillomavirus Infection in Uyghur Women

  • We investigated the distribution of HPV genotypes in Uyghur women in Xinjiang region of China, and behavioral factors which could predispose them to HPV infection. In this cross-sectional study, women aged 15-59 years were recruited by cluster sampling method in Yutian region in 2009. Liquid-based cytology samples were analyzed centrally for HPV genotype with a linear array detector. Univariate and multivariate logistic regression analyses were performed to identify behavioral risk factors for HPV infection. A total of 883 Uyghur women were recruited successfully. The prevalence of high-risk HPV and low-risk HPV were 7.25% and 1.58%, respectively; the most common HPVs were HPV16, 51, 31, 39 and 58. We found that age of first sexual intercourse was a strong predictor for HPV infection (odds ratio of 4.01 for ${\leq}15$ years versus ${\geq}25$). Having sexual partners ${\geq}3$ was the second predictor (OR 3.69, 95% CI 2.24-7.16). Cleaning the vagina after sex showed an increased risk of HPV infection (OR 2.72; 95% CI 1.98-5.13); Using the condom showed protective factors for HPV infection (OR 0.36; 95%CI0.12-0.53). HPV16, 51, 31, 39 and 58 were the priority types; the age of first sexual intercourse was identified as a major risk factor for HPV infection. Other notable risks were number of sexual partners and cleaning the vagina after sex. Changing these behavioral risk factors could help to reduce the occurrence of cervical cancer in this population.

Long Non-coding RNA GAS5 Functions as a Tumor Suppressor in Renal Cell Carcinoma

  • Background: Renal cell carcinoma (RCC) is a malignancy with a poor prognosis. We aimed to explore whether the expression of Long Non-Coding RNA (LncRNA) growth arrest-specific transcript 5 (GAS5) is associated with RCC genesis. Methods: We selected twelve clinical samples diagnosed for renal clear cell carcinoma and found that the LncRNA GAS5 transcript levels were significantly reduced relative to those in adjacent unaffected normal renal tissues. Results: In addition, expression of GAS5 was lower in the RCC cell line A498 than that in normal renal cell line HK-2. Furthermore, using functional expression cloning, we found that overexpression of GAS5 in A498 cells inhibited cell proliferation, induced cell apoptosis and arrested cell cycling. At the same time, the migration and invasion potential of A498 cells were inhibited compared to control groups. Conclusion: Our study provided the first evidence that a decrease in GAS5 expression is associated with RCC genesis and progression and overexpression of GAS5 can act as a tumor suppressor for RCC, providing a potential attractive therapeutic approach for this malignancy.

Design and Implementation of a Career Planning Program at Chonnam National University Medical School (전남대학교 의과대학 졸업 후 진로지도 프로그램 설계와 운영)

  • Chonnam National University Medical School has designed and implemented two career planning programs: a three-phase curriculum-based program and a longitudinal non-curricular program over the course of 6 academic years. The three phases of the curriculum-based career planning program are self-assessment, career exploration, and field experience. The non-curricular career counseling program operates through a faculty advisor system, with each faculty member guiding a group of students from each academic year, and students in each year forming a mentor-mentee relationship. The non-curricular career exploration program consists of a student research support program, an international practice program in basic and clinical medicine, and a specialty exploration fair. A survey conducted among 38 graduates (54.3%) working as interns at Chonnam National University Hospital revealed that graduates preferred autonomous elective subjects within the curriculum-based program. They also responded positively to the faculty advisor system, through which they maintained close relationships. A focus group interview with three interns indicated that subjects providing direct experience in fields of interest and courses that students could choose freely were helpful in career decisions. Through follow-up research, it is necessary to design and operate a systematic career planning program based on an analysis of the needs of graduates taking part in a residency training program after selecting a medical specialty.

A Study on the classification of materia medica in medicinal part of Treasured Mirror of Eastern Medicine("東醫寶鑑" "湯液篇") ("동의보감(東醫寶鑑)" "탕액편(湯液篇)"의 본초(本草) 분류에 대한 연구)

  • The medicinal part of Treasured Mirror of Eastern Medicine("東醫寶鑑" "湯液編") is based on Classified Emergency Materia Medica("證類本草"), Compendium of Prescriptions from the Countryside("鄕藥集成方"). But it distinguished materia medica of countryside(鄕藥) from materia medica of China(唐藥) and properly selected the sentences of them by actual medical circumstances in Korean peninsula. Especially upon assortment of medical herbs, the medicinal part of Treasured Mirror of Eastern Medicine added the part of Water, Earth, Metal, and divided the part of Fish & Bug, Jade & Stone into Fish, Bug, Jade, Stone part. Moreover, it abolished the three grade classification adopted in Classified Emergency Materia Medica, Compendium of Prescriptions from the Countryside and attempted a new approach on itemization; 'representative herb' and 'secondary herb'. Hence, medicinal part of Treasured Mirror of Eastern Medicine should be evaluated as the 'settling in of Chinese medicine with autonomous interpretation, other than 'the fusion of prescriptions from countryside and Chinese medicine.
