동공크기변이도를 통한 침자극과 스트레스의 상관관계 연구 (A Study on the Relationship with Acupuncture Stimulation and Stress -Using Pupil Size Variability(PSV)-)

  • Objectives : The purpose of this case-control research was to investigate the effects of acupuncture stimulation on autonomic nervous system for patients with PSV and to find out relationship with anti-stress effects. Methods : The study group consists of 24 patients with self-recognition of stress as the case group, and 20 normal person as the control group by similar age. We measured PSV of case and control groups before acupuncture stimulation, prick acupuncture in Hegu(LI4), Taichong(Liv3), Shenme(H7), Neiguan(P6), Zusanli(S36). After treating for 20 minutes, measurement values of PSV were compared for pre-acupuncture and post-acupuncture Results : 1. Pupil size of the case group were decreased significantly after acupuncture stimulation in the 1st 2nd experiments. Pupil size of the control group was decreased significantly after acupuncture stimulation in the 1st experiment. 2. Pupil size of the case group were decreased significantly between the 1st and 2nd experiments before acupuncture stimulation. Conclusion : The results suggest that acupuncture stimulation is associated with changed activity in the sympathetic and parasympathetic nervous system. Measurement values of PSV is suitable to estimate the activity of automatic nervous system.

LF norm, HF norm, and LF/HF were significant different between the 1st and 2nd experiment in HRV of patients whose symptoms improved. But HRV of patients whose symptoms unimproved didn't show significant difference. Conclusion : The results suggest that acupuncture stimulation is associated with changed activity in the sympathetic and parasympathetic nervous system. Measurement values of HRV is suitable to estimate the activity of automatic nervous system.

단중혈(膻中穴) 침자극이 정상 성인의 심박변이도에 미치는 영향 (Study on the Effect of Acupuncture at Jeonjung($CV_{17}$) on the Heart Rate Variability in Healthy Adults)

  • Objectives : This study was accomplished to investigate the effects of acupuncture at Jeonjung($CV_{17}$) on the heart rate variability(HRV) in healthy adults. And based on that, we tried to find out how acupuncture at Jeonjung($CV_{17}$) effects on the activity and balance of the autonomic nervous system. Methods : We investigated on 74 healthy volunteers consisted of 24 subjects in acupuncture group, 24 subjects in sham acupuncture group and 24 subjects in control group. On the other hand, two volunteers were ruled out by standard of elimination. The study was established by randomized trial. Acupuncture was applied at Jeonjung($CV_{17}$) during 10 minutes in acupuncture group and minimal acupuncture was applied at the same area in sham acupuncture group. No treatment was executed in control group. We measured HRV 3 times : baseline measurement and after treatment in acupuncture group and sham acupuncture group (after rest in control group) twise. The SPSS 15.0 for Windows was used to analyze the data by paired t-test, Wilcoxon signed ranks test (in group) and One-Way ANOVA, Kruskal-Wallis test (between the groups). Results: 1. In acupuncture group, Mean HRT, PSI, Normalized HF decreased significantly. SDNN, Ln (TP), Ln (LF), Ln (VLF), Normalized LF and LF/HF ratio increased significantly. RMSSD, Ln(HF) were not affected. 2. In sham acupuncture group, all indicators were not affected. 3. In control group, mean HRT, Ln(TP) decreased significantly. SDNN, RMSSD, PSI, Ln(LF), Ln(HF), Ln(VLF), normalized LF, normalized HF and LF/HF ratio were not affected. Conclusions : We suggest that acupuncture at Jeonjung($CV_{17}$) increases the activity of autonomic nervous system and makes effects on the balance of the autonomic nervous system.

  • In order to investigate the influence of four Sasang prescriptions, $So{\breve{u}}min$ P'almunjat'ang(SPG), $So{\breve{u}}min$ Sltibimikwanjungt'ang(SSK), $T^{\prime}ae{\breve{u}}min$ $Chowis{\breve{u}}ngch^{\prime}{\breve{o}}ngt^{\prime}ang$(TCS), and $T^{\prime}ae{\breve{u}}min$ $Ch^{\prime}{\breve{o}}ngp^{\prime}yesagant^{\prime}ang$(TCP), on the activity of the autonomic nervous system in the rats exposed to noise and vibration stress, the contents of catecholamines in the urine, body temperature, heart rate, blood pressure, and stomach mucosa change were checked in this experiment. The results obtained were as follows; 1. On the 7th and 12th days from the beginning, the experimental indexes of the controled group were increased more than those of the normal group, and it meant that the noise and vibration stress induced an autonomic nervous system disorder in rats. 2. The urine-epinephrine contents in all experimental groups were decreased more than in the controled group, and in the Tcs and the Tcp groups those were significantly decreased on the 7th and 12th days. 3. The urine-norepinephrine contents in the SPK, TCS, and the TCP groups were significantly decreased on the 7th day, and in the TCS and the SPG groupsthose were significantly decredsed on the 12th day. 4. The urine-dopamine contents in the TCS group were significantlly decreased on the 12th day. 5. The body temperaature was decreased significantly in the SPG and the SSK groups on the 7th day. 6. The heart rate was significantly decreased in the SPG group on the 7th day, and it was decreased significantly in the SPG and the SSK groups on the 12th day. 7. The blood pressure was decreased in ghe SPG group significantly on the 7th day. 8. The stomach mucosa remarkably recovered from gastric ulcers in the SPG group and significantly recorvered in the other groups on the 7th day. From the above findings, th could be assumed that Taeumin and Soumin prescriptions had some significant influence on the control of the autonomic nervous system in the rats that recieved stress.

  • Objectives : This study was accomplished to investigate the effects of venesection with cupping therapy at Jeonjung($CV_{17}$) on the heart rate variability(HRV) in healthy adults. And based on that, we tried to find out how venesection with cupping therapy at Jeonjung($CV_{17}$) affects the activity and the balance of the autonomic nervous system. Methods : We investigated on 60 healthy volunteers consisted of 30 subjects in venesection with cupping therapy group and 30 subjects in control group. The study was established by randomized trial. venesection with cupping therapy was applied at Jeonjung($CV_{17}$) for 10 minutes in venesection with cupping therapy group. No treatment was executed in control group. We measured HRV 3 times : First, baseline measurement and two more times after treatment in venesection with cupping therapy group(after rest in control group). The SPSS 12.0 for Windows was used to analyze the data by paired t-test(in both group), independent-test(between the groups) and Chi-square test(between the sexs). Results : 1. In venesection with cupping therapy group, SDNN, Ln(TP), Ln(LF), Normalized LF, and LF/HF ratio increased significantly. Normalized HF decreased significantly. Mean HRT, RMSSD, PSI, Ln(HF), and Ln(VLF) were not affected. 2. In control group, Ln(HF) decreased significantly. Mean HRT, SDNN, RMSSD, PSI, Ln(TP), Ln(LF), Ln(VLF), Normalized LF, Normalized HF, and LF/HF ratio were not affected. Conclusions : This study suggests that venesection with cupping therapy at Jeonjung($CV_{17}$) increases the activity of the autonomic nervous system and has effects on the balance of the autonomic nervous system.

  • Purpose: Heart rate variability(HRV) is the clinical consequence of various influences of the autonomic nervous system(ANS) on heart beat. HRV can estimate the potential physiologic rhythm from the interval between consecutive beats(RR interval or HRV data). It is known as RSA which represents respiration-related HR rhythmic oscillation. Previous studies demonstrated a specific breathing pattern(0.1Hz, 6breaths/min) to improve a physiological body condition related to the stress. In this paper, the level of stress would be evaluated in terms of three phases of the dental treatment, combined with 6breaths/min. Methods: These phases include before, during and after tooth extraction or anesthesia or something.36 patients' stresses were assessed using HRV stress analyzer in each phase in Kangdong Sacred Heart Hospital, and Chuncheon Sacred Heart Hospital, Hallym University Medical Center from Jun. to Sept. of 2007. HRV 5-min data collected were analyzed in time-domain and frequency-domain to evaluate the activity of autonomic nervous system(ANS) which represents the level of stress. Results: All HRV parameters including HF(high frequency), LF(low frequency) and LF/HF ratio showned a significant change affecting the ANS balance. There was a 6.4% difference between R(LF/HF)s on general breathing pattern for balance of Autonomic nervous system, but on controlled breathing pattern, 0.1Hz, was made narrow till 1.4%. The activity of ANS has increased by 1.4% on general breathing pattern, and by 2.9% on controlled breathing pattern, 0.1Hz. Conclusion: After analysis of preoperative stress changes and effect of breathing pattern of 0.1 Hz on the stress in 36 patients who have undergone third molar extraction, following was concluded. In the preoperative stage, the sympathetic change was the greatest?after the?anesthetic injection, and stress was relieved by controlling the breathing pattern to a frequency of 0.1Hz.

Effect of remimazolam and propofol anesthesia on autonomic nerve activities during Le Fort I osteotomy under general anesthesia: blinded randomized clinical trial

  • Background: This study evaluated the effect of remimazolam and propofol on changes in autonomic nerve activity caused by surgical stimulation during orthognathic surgery, using power spectrum analysis of blood pressure variability (BPV) and heart rate variability (HRV), and their respective associations with cardiovascular fluctuations. Methods: A total of 34 patients undergoing Le Fort I osteotomy were randomized to the remimazolam (Group R, 17 cases) or propofol (Group P, 17 cases) groups. Observables included the low-frequency component of BPV (BPV LF; index of vasomotor sympathetic nerve activity), high-frequency component of HRV (HRV HF; index of parasympathetic nerve activity), balance index of the low- and high-frequency components of HRV (HRV LF/HF; index of sympathetic nerve activity), heart rate (HR), and systolic blood pressure (SBP). Four observations were made: (1) baseline, (2) immediately before down-fracture, (3) down-fracture, and (4) 5 min after down-fracture. Data from each observation period were compared using a two-way analysis of variance with a mixed model. A Bonferroni multiple comparison test was performed in the absence of any interaction. One-way analysis of variance followed by Tukey's multiple comparisons test was performed when a significant interaction was observed between time and group, with P < 0.05 indicating statistical significance. Results: Evaluation of autonomic nerve activity in comparison with baseline during down-fracture showed a significant increase in BPV LF (P < 0.001), an increasing trend in HRV LF/HF in Group P, and an increasing trend in HRV HF in Group R. There were no significant differences in HR or SBP between the two groups. Conclusion: During down-fracture of Le Fort I osteotomy, sympathetic nerve activity was predominant with propofol anesthesia, and parasympathetic nerve activity was predominant with remimazolam anesthesia.

쑥뜸치료가 암환자의 심박변이도에 미치는 영향 (The Effects of Moxibustion on Heart Rate Variability in Cancer Patients)

  • Objective : The study aims to investigate the effect of moxibustion treatments on autonomic nervous system function of cancer patients through the evaluation of heart rate variability (HRV) biofeedback testing. Materials and Methods : Six cancer patients from inpatient care unit of Dunsan Oriental Hospital, Daejeon University were given three moxibustion treatment sessions every other day over one week period on five Oriental Medicine meridian points CV4, CV6, CV12, KD1, and PC8. HRV biofeedback was conducted before and after each treatment sessions. Three areas of analyses were done from the test conducted; Time Domain Analysis, Frequency Domain Analysis and Autonomic Nervous System (ANS) balance analysis. Results : Time Domain Analysis has shown increased Standard Deviation of all Normal R-R Intervals (SDNN), and decreased Mean Heart Rate and Physical Stress Index (PSI) levels, with statistical significance (P<0.05). In Frequency Domain Analysis, series of moxa treatments have increased Total Power (TP), Very Low Frequency Oscillation Power (VLF), High Frequency Oscillation Power (HF), normalized HF values while decreasing Low Frequency Oscillation Power (LF), normalized LF and LF/HF ratio with statistical significance (P<0.05). The values of ANS activity, ANS balance, Stress resistance, Stress index, have also shown significant changes. For cardiac stability stroke volume power (SP) and Blood Vessel Tension (BVT) were followed, which were both increased after treatment. All changes were statistically significant (P<0.05). Conclusion : The results have shown a positive correlation between the moxibustion treatments and autonomic nervous system responses on cancer patients through the HRV biofeedback testing. This study suggests possible application of moxibustion treatments for managing ANS functions of cancer patients, although additional studies with larger population are necessary to confirm the data.

폐경 전과 폐경 후 비만 여성의 심박변이도에 관한 연구 (Heart Rate Variability in Obese Climacteric Women in Korea;Relations between Autonomic Nervous System Activity and Obesity)

  • Objectives To investigate the relationship between Heart Rate Variability and menopause among obese middle-aged women in Korea. Subjects and Methods Forty middle-aged women (age 49.05 $\pm$ 2.64 years) with BMI > $23kg/m^2$ were recruited by local advertisement. Blood profiles of estrogen, FSH, LH, glucose, total cholesterol, HDL-cholesterol, and triglyceride were measured. Autonomic Nervous System (ANS) was estimated by short-term spectral analysis of heart rate variability (HRV). Visceral adipose tissue (VAT) and subcutaneous adipose tissue (SAT) were accessed using Computerized Tomography (CT), Bioimpedance Analysis (BIA), and anthropometry. To scrutinize the influence of ANS on obesity-related factors, we divided the subjects into pre- and post-menopausal women. Results The total cholesterol, FSH and LH were significantly higher in post-menopausal obese women group. The estradiol and standard deviation of all normal R-R intervals (SDNN), high-frequency components (HF) of the HRV were significantly lower in postmenopausal obese women group (P<0.05). Adipose tissue distribution and blood profiles were not significantly different between two groups. Conclusion Menopause induced a decrease in SDNN and HF values and increase in total cholesterol level in obese women although the VAT itself was not related with ANS in obese women.

미주신경 감각분지 분포영역의 자침이 자율신경 변화에 미치는 영향 (Effect of Acupuncture at the Field of the Auricular Branch of the Vagus Nerve on Autonomic Nervous System Change)

  • Objectives This study was designed to identify the changes of autonomic nervous system (ANS) which was induced by acupuncture at the field of the auricular branch of the vagus nerve. Methods 30 healthy adults were selected and classified into two groups; experimental group, control group. After providing mental stress, acupuncture was applied at external ear in experimental group and no treatment executed in control group. The evaluation of ANS function was measured by heart rate variability (HRV). We statically analyzed the difference of HRV parameters which include mean heart rate (MHRT), standard deviation of all N-N intervals (SDNN), square root of the mean of the sum of the squares of differences between adjacent N-N intervals (RMSSD), total power (TP), low frequency power (LF), high frequency power (HF), LF/HF ratio. Results All subjects showed significant increase in SDNN, LF after stress stimulation (p<0.05). Immediately after intervention, MHRT was significantly decreased (p<0.001) and RMSSD, HF were significantly increased in experimental group (p<0.05). After the end of intervention, SDNN, HF, RMSSD, TP, LF were significantly increased in experimental group (p<0.01, p<0.05). And when comparing baseline HRV, SDNN, LF were significantly increased in control group (p<0.01) and SDNN, RMSSD, TP, LF were significantly increased in experimental group (p<0.05). In the subgroup analysis, normal balance of ANS group showed significant increase in TP, LF, SDNN, HF (p<0.01, p<0.05). Conclusions We suggested that acupuncture at external ear, region of the vagus nerve distribution could increase parasympathetic activity and cause changes and reregulation of the ANS.
