  • The complications of a tracheotomy are caused by inappropriate surgical techniques, unsuitable cannula selections, and improper wound care. Among these, the solutions to problems of surgical technique and wound care have been reported in many articles. Detailed methods for preventing complications by the cannula are rare. The authors tried to find a way of preventing complications by the cannula Materials and Methods : The authors analized complications in 70 patients who had a temporary tracheotomy and were wearing a cannula. And the complications were compared between 4 commercial cannulas used in our institute. The examination methods used were a simple neck lateral radiogram and flexible endoscopy. Results: The order of most commonly found complications were as followed; at the suprastoma, end of cannula, level of tracheotomy, and infrastoma. Among 4 cannulas, a particular product had so many complications compared to the other 3 cannulas. The most common cause of complications was unsuitable cannula. All complications were cured with no sequelae. Flexible endoscopy is far superior to radiologic exam for detecting tracheal complications. Conclusion: Flexible endoscopy through the tracheostoma is very helpful for detecting complications early and determining if a proper cannula is used, which can prevent further complications such as stenosis or innominate artery rupture. The authors, therfore, recommend using the flexible endoscopy to all patients wearing tracheotomy tubes. Some complications can simply be prevented by replacing the one to another cannula properly fit for the individual patients. Various cannulas should be prepared at the hospital because the tracheal curvature and distance of skin to trachea are individualized.

  • The family as a social institution has survived most diverse political periods and appears resilient or at least able to reconstitute itself even in the aftermath of destructive events such as wars. Age at first marriage is one possibility to systematize the strategies that families follow in times of internal conflicts (e.g., civil wars), external interventions or peaceful times. The authors found that age at first marriage correlates with socio-political events whereas perceptions of insecurity lead to a decline in marital age. This paper is based on three case studies that the authors have conducted through ethnographic methods among Tajiks in the cities Kulob, Khujand, and Mazar-e Sharif in Tajikistan and Afghanistan. Combining Grounded Theory with the genealogical methods from social anthropology in order to generate demographic data, the authors introduce the method of grounded demography as a way to generate demographic data through ethnographic methods. Grounded demography offers a way to produce statistical data grounded in ethnographic research.

  • Scholars are paying increasingly close attention to brain research and the creation of biological neural networks, artificial neural networks, artificial intelligence, neurochips, brain-computer interfaces, prostheses, new research instruments and methods, methods of treatment, as well as the prevention of neurodegenerative diseases based on these data. The authors of the study propose their hypothesis on the understanding of the phenomenon of consciousness that answers questions concerning the criteria of consciousness, its localization, and principles of operation. In the study of the hard problem of consciousness, the philosophical and scientific categories of consciousness, and prominent hypotheses and theories of consciousness, the authors distinguish "the area of the conscious mind", which encompasses several states of consciousness united by the phenomenon of integrated consciousness. According to the authors, consciousness is a kind of executor of the phenomenological idea of the "chalice", so the search for it should be conducted deeper than the processes in the power of thought consciousness and transconsciousness, to which integrated consciousness can act as a lever. However, integrated consciousness may have the capacity to transcend into lower states of consciousness, which requires further study.

  • Purpose - The purpose of this paper is to analyze the reformed duty of fair presentation provisions and related caselaw of the Insurance Act 2015 to gain a clearer understanding of the differences between the Act and the preceding legislation. Design/methodology - The authors analyzed caselaw from South Korea and China that involved breaches of the duty of disclosure. Cases highlighting differences between the duties of disclosure and fair presentation were selected. Findings - Changes in the practice of marine insurance laws are expected from the application of the reformed duty of presentation provisions. In particular, the rights of the insured are expected to increase, resulting in the fairer conduct of insurance contracts. Due to the fact that the Insurance Act 2015 has only recently taken effect, the provisions of existing caselaw have not yet been applied. This has limited the authors' scope of analysis. Originality/value - This paper describes the implications of the duty of fair presentation by analyzing caselaw from South Korea and China that involves the duty of disclosure. To the best of the authors' knowledge, this is the first paper that investigates the reformed duty of fair presentation provisions of the Insurance Act 2015 in the context of the legislation's implications for trade practices.
