청각적 생체 되먹임 훈련과 공차기 훈련이 뇌졸중 환자의 체중지지율에 미치는 영향 (Effect of Auditory Biofeedback Training and Kicking Training on Weight-bearing Ratio in Patients with Hemiplegia)

  • PURPOSE: The purpose of this study was of scale using auditory biofeedback training and kinging training on walking speed and weight bearing ratio in patients hemiplegia with stroke to determine of the effects of such training would be maintained even after stopping the intervention. METHODS: The 30 subject were classified into three groups : 12 times, 3 times a week receiving the control, the experimental group scale using auditory and kicking training for 4 weeks. In addition, all subjects in the control group and experimental groups received the same general exercise treatment 12 times, 3 times a week for 4 weeks, and underwent follow-up tests. The significance of differences between the control group and the experimental groups was analysis by repeated-ANOVA, Interaction time and groups was analysis by repeated-ANOVA. In case where there were differences, post-hot tests were conducted using repeated measure ANOVA. RESULTS: There were significant differences in 10 m walking speed and weight bearing ratio between the control group and experimental group after the performance of the scale using auditory training and kicking training. Scale using auditory biofeedback training 4 weeks was more effective than kicking training. After 8 weeks weight bearing ratio maintained on scale using auditory training. CONCLUSION: These finding suggest that the scale using auditory biofeedback training and kicking training has positive effects on hemiplegia with stroke.

The Effects of Auditory Biofeedback Training and Kicking Training on Walking Speeds in Patients with Hemiplegia

  • The objective of this study was to examine the effects of auditory biofeedback training and kicking training on walking times in patients with hemiplegia to determine if the effects of such training would be maintained even after stopping the intervention. Thirty hemiplegia patients were selected and randomly assigned to a control group of 10 patients to receive general exercise treatment; an auditory biofeedback training group of 10 patients to receive auditory biofeedback training, along with general exercise; and a kicking training group to receive kicking training, along with general exercise. All the patients received their respective training 12 times in total, three times per week for four weeks. In addition, all those in the control and experimental groups received the same general exercise treatment 12 times in total, three times per week for four weeks, and underwent follow-up tests thereafter. The patients' 10m walking times were measured using a stopwatch. The significance was analyzed using repeated-ANOVAs. In cases where there were interactions between measuring times and groups, in each group were examined using repeated-ANOVAs. In cases where there were differences, post-hoc tests were conducted using repeated of contrast test. The 10m walking times of the control and experimental groups were significant differences in 10m walking times were shown between measurement times(p<.05), and significant differences in the interactions between measuring times and groups were shown between the groups(p <.05). However, no significant differences in 10m walking times were shown between the groups(p>.05). The auditory biofeedback training group showed significant decreases in walking times four weeks after the beginning of the intervention(p<.05) and significant increases eight weeks after the beginning of the intervention(p<.05). The kicking training group showed significant decreases in walking time four weeks after the beginning of the intervention(p<.05) and maintained the walking times without showing any significant differences eight weeks after the beginning of the intervention(p>.05). The walking speeds of only the kicking training group were maintained until eight weeks after the beginning of the intervention.

  • Biofeedback devices have been used successfully to improve head control and symmetrical walking in cerebral palsied children. Biofeedback of postural sway was investigated as a therapeutic technique to reduce postural abnormality in 24 selected children with cerebral palsy. Subjects were evaluated their weight distribution of both sides during their standing before and after the visual and auditory feedback training. The effectiveness of biofeedback was compared to conventional physical therapy practices in reestabilishing symmetrical standing in cerebral palsied children. Our study found biofeedback was effective in training symmetrical standing posture.

만성 뇌졸중 환자의 일어서기 훈련 시 시각적 피드백과 청각적 피드백이 균형과 수행력에 미치는 효과 비교 (Comparison of Visual and Auditory Biofeedback during Sit-to-stand Training for Performance and Balance in Chronic Stroke Patients)

  • PURPOSE: The purpose of this study was to determine the comparison of visual and auditory biofeedback during sit-to-stand training in patients with stroke. METHODS: Thirty-five subjects with chronic stroke were divided into three groups: a visual feedback group (12 subjects), an auditory feedback group (12 subjects) and a control group (11 subjects). All Groups received neurodevelopmental treatment, and sit-to-stand training for 30minutes three times a week for four weeks. During the sit to stand training, the experimental groups received visual feedback and auditory feedback, whereas the control group performed sit-to-stand training without feedback. Five times sit-to-stand test (FTSST), motion analysis and postural sway during sit-to-stand were used to evaluate sit to stand performance ability. In addition, Berg balance scale (BBS) was performed for evaluation of balance function in participants. RESULTS: All groups showed significant increase on FTSST and BBS between pre- and post-intervention. The BBS scores in visual feedback group was significant increase than control group. The motion analysis and postural sway, more improvement was observed in the visual and auditory feedback groups compared with the control group. The only visual feedback group was a better performance of midline excursion during sit-to-stand than control group. CONCLUSION: These findings suggest that sit-to-stand training using a biofeedback may help to improve sit to stand performance and balance ability of stroke patients.

근전도 바이오피드백을 이용한 훈련이 안면신경마비 환자의 운동학습에 미치는 영향 (Effect of Electromyographic Biofeedback Training on the Muscle Activities of Unilateral Facial Palsy)

  • The purpose of this study was to determine EMG biofeedback training effect on the muscle activities in 3 unilateral facial palsy patients along with multiple baseline design across subjects. The auditory feedback about facial muscles (orbicularis oris, orbicularis oculi, frontalis) was provided with each patient during facial exercise training. Electromyographic (EMG) activity during maximal voluntary contraction and maximal compound muscle action potential (CMAP) amplitude elected by supramaximal electrical stimulation on facial nerve of facial muscles were measured pre- and post- EMG biofeedback training to evaluate motor learning. EMG activity during maximal voluntary contraction was increased after EMG biofeedback training and CMAP amplitude elected by supramaximal electrical stimulation was not changed in all subjects. The results indicate that EMG biofeedback training is useful method to improve motor learning of facial excercise training in unilateral facial palsy patients.

  • Weight bearing training on the involved leg is impotant for ambulation and activities of daily living in ambulatory hemiplegic patients. Traditionally, physical therapists have relied on exercise therapy and subjective evaluation. The goal for this study was to measure lower extremity weight distribution in standing with ten hemiplegic pations(M:8, F:2) and to determine the traing effect on symmetrical standing posture using a "Limloader". The Limloader is a machine designed for training symmetrical weight bearing posture. The results showed that the ability to keep the center of gravity within the limits for balance was improved significantly but the ability to adjust weight bearing on the involved leg was not (p<0.05). This study demostrated that hemiplegics can improved their symmetrical weight bearing ability using sensory biofeedback.

  • We developed a biofeedback gait training system; a 12 m measuring walkway with a training walker which moves at prescribed velocity. The walkway measures a.11 temporal and distance factors of gait. This system provides visual feedback for distance factors and auditory one for temporal at the prescribed walking velocity. Experiments were performed on normal and degenerative knee joint subjects, and this system was verified to be very useful.

  • PURPOSE: This study aimed to investigate the effect of emphasized initial contact gait training on balance and gait ability in hemiplegia patients. METHODS: Twenty-four hemiplegic patients were randomly allocated to an experimental group or control group. All participants received 30-min neurodevelopmental treatment. Furthermore, the experimental group received initial contact-emphasized auditory feedback gait training, whereas the control group received gait training without auditory feedback. The intervention was performed 3 times per week, 20 min per each time, for a total of 6 weeks. Balance was assessed using the center of pressure path length, center of pressure velocity, and limitation of stability path length, whereas gait ability was assessed using the 10-m walking test and functional gait assessment. RESULTS: In both groups, center of pressure path length and center of pressure velocity significantly decreased after training. Compared to the control group, the experimental group showed a 10% significant improvement (p<.05). In the limitation of stability path length of both sides, the experimental group showed a significant increase compared to that before intervention. Compared to the control group, the experimental group showed a 7% significant improvement in results of the 10-m walking test and functional gait assessment (p<.05). CONCLUSION: Emphasized Initial contact gait training is considered an effective treatment for improving gait ability and balance ability in stroke patients.

편마비 환자의 대칭적 체중부하 유도를 위한 청각적 피드백 보행훈련 시스템 개발 및 평가 (Development and Evaluation of the Auditory Feedback Gait Training System Induced Symmetrical Weight-Bearing in Hemiplegic Patients)

  • 본 연구에서는 로드셀 센서를 활용하여 편바미 환자의 환측에 부하되는 체중부하에 따른 청각적 피드백을 적용한 대칭 보행유도와 보행패턴 분석이 가능한 무선 보행훈련 시스템을 개발하였다. 또한 개발된 시스템의 유효성을 검증하기 위해 정상인군 및 편마비군 대상자에게 수행한 임상실험을 진행하였으며, 각 대상군마다 피드백 적용 후 보행 속도와 체중 부하량, 그리고 보행 시 입각기비율과 분속수가 증가하였음을 확인하였다. 연구 결과를 토대로 편마비 환자 뿐만 아니라 대칭적 체중부하가 요구되는 다양한 하지 질환자들에 대한 임상실험을 확대하고 장기간 훈련의 효과를 관찰한다면, 본 연구에서 개발된 청각적 대칭 체중부하 피드백 보행 시스템은 다양한 패턴 분석과 함께 보행 향상을 위한 기기 적용이 가능할 것으로 기대된다.
