창원지역 일부 비만아의 체격지수와 생화학적 지표에 관한 연구 (Analysis of Anthropometric and Clinical Data in Obese Children in Changwon)

  • In this study, we investigated the effects of obesity index on the risk factors of chronic diseases in obese children. The subjects were male and female children aged 9 to 12. The average obesity index was 147.7$\pm$13.8% in boys and 147.6$\pm$16.5% in girls. The average percentages of body fat was significantly higher in boys(33.9$\pm$5.7%) than in girls(30.7$\pm$4.4%). It was found that 26% of subjects had abnormal serum GOT and GPT values. Percentages of subjects at risk of cardiovascular disease(CVD) based on corresponding criteria of TG, T chol, HDL chol, and LDL chol were 28.6%, 28.6%, 45.5% and 42.7%, respectively. The risk of CVD was increased with the degree of obesity, which was not significant. The AI(atherogenic index) was higher in boys(4.6$\pm$2.3) than in girls(3.6$\pm$1.2). The serum T chol and LDL chol levels of subjects whose mother are in thirties were significantly higher than those of subjects whose mother are in fourties. The T chol levels of subjects who had mother with job were significantly higher compaired to those of subjects who had housewife mother. The results indicated the urgent need of nu tritional management for the obese children to prevent further devel opment of childhood obesity and hyperlipidemia. Based upon this study, devising method and media for the mother nutritional education as well as for the children nutritional education is required for better growth and health promotion of primary school children.

Nonfatal injuries in Korean children and adolescents, 2007-2018

  • Background: Injury is the leading cause of death or disability in children and adolescents. Rates of deaths from injuries have recently declined, but studies of the occurrence of nonfatal injuries are lacking. Purpose: This study aimed to investigate nonfatal injuries in children and adolescents younger than 20 years based on data from the Korean National Health and Nutrition Survey, 2007-2018. Methods: A questionnaire survey was conducted to determine whether children and adolescents had experienced an injury requiring a hospital visit in the previous year. We investigated each injury's risk factors and characteristics. Results: Of a total of 21,598 children and adolescents, 1,748 (weighted percentage, 8.1%) experienced one or more injuries in the previous year. There was no yearly difference in the proportion of injuries experienced. Among the male subjects, 10.0% had an injury experience; among the female participants, 6.1% had an injury experience (P<0.001). The highest rate was 9.0% in children aged 1-4 years. In multivariate logistic regression analysis, male sex; having an urban residence; having restricted activity due to visual, hearing, or developmental impairment; and attention deficit/hyperactivity disorder were significant risk factors for injury experience. The characteristics of up to 3 injuries per patient were investigated, and 1,951 injuries were analyzed. Falls and slips accounted for 34.9%, collisions for 34.1%, and motor vehicle accidents for 11.3% of the total injuries. Ninety-six percent of injuries were unintentional, 20% caused school absences, and 10% required hospitalization. Conclusion: Among Korean children and adolescents, 8.1% experienced injuries at least once a year with no significant differences in incidence over the past 12 years. Greater attention and effort to prevent injuries are needed.

성남시 소재 초등학교 저학년에서 아토피 피부염 환자군과 대조군의 식생활 비교 (Comparison of Dietary Behavior between Atopic Dermatitis Patients and Controls in Elementary School Students Living in the Sung-nam Area)

  • The prevalence of atopic dermatitis (AD) in children has increased dramatically in recent years. Although AD has genetic determinants, this rapid increase is most likely due to changes in environmental influences--for example, dietary changes. The purpose of this case-control study was to assess the relationship between the risk of developing AD and dietary factors, including eating habits, food intake, and the consumption of various functional foods in children at ages of 7 or 8 years. 143 AD patients and 335 healthy children participated in this study. A mini-dietary assessment was utilized to evaluate the food intake and dietary patterns of the children, and other information, including demographic and socioeconomic characteristics, eating habits, and the frequency of functional food use was collected using a questionnaire. The results demonstrated that, among the demographic and socioeconomic factors assessed in this study, female gender, mother's employment, and the family history of AD significantly increased the risk of AD. However, no differences in dietary habits and specific food intake between AD patients and healthy controls were identified. On the other hand, the frequencies of taking multivitamin supplements, Spirulina, or gamma-linoleic acid were significantly higher in AD patients than in the controls. These data indicate that alterations in eating habits and the intake of certain foods may not be a critical cause associated with the risk of AD in school-age children, and caution should be taken in recommending food elimination diets for the purpose of preventing AD. Further studies are required in order to determine whether the intake of specific nutrients could contribute to the increase or prevention of the development of AD in school-age children.

Waist-to-height ratio as a screening tool for obesity and cardiometabolic risk

  • The waist-to-height ratio (WHtR), calculated by dividing the waist circumference (WC) by height, has recently gained attention as an anthropometric index for central adiposity. It is an easy-to-use and less age-dependent index to identify individuals with increased cardiometabolic risk. A WHtR cutoff of 0.5 can be used in different sex and ethnic groups and is generally accepted as a universal cutoff for central obesity in children (aged ${\geq}6$ years) and adults. However, the WHtR has not been validated in preschool children, and the routine use of WHtR in children under age 6 is not recommended. Prospective studies and meta-analysis in adults revealed that the WHtR is equivalent to or slightly better than WC and superior to body mass index (BMI) in predicting higher cardiometabolic risk. In children and adolescents, studies have shown that the WHtR is similar to both BMI and WC in identifying those at an increased cardiometabolic risk. Additional use of WHtR with BMI or WC may be helpful because WHtR considers both height and central obesity. WHtR may be preferred because of its simplicity and because it does not require sex- and age-dependent cutoffs; additionally, the simple message 'keep your WC to less than half your height' may be particularly useful. This review article summarizes recent publications on the usefulness of using WHtR especially when compared to BMI and WC as a screening tool for obesity and related cardiometabolic risks, and recommends the use of WHtR in clinical practice for obesity screening in children and adolescents.

Helicobacter pylori Infection and Risk Factors in Relation to Allergy in Children

  • Purpose: To analyze presence of Helicobacter pylori infection and environmental risk factors among children with and without allergy. Methods: Parents of children at primary health care centres/kindergartens and allergologist consultation were asked to answer a questionnaire and to bring a faecal sample. H. pylori infection was detected by monoclonal stool antigen test. Prevalence of H. pylori infection and risk factors were compared between individuals with and without allergy using ${\chi}^2$ test, ANOVA test and logistic regression. Results: Among 220 children (mean age, 4.7 years; ${\pm}standard$ deviation 2.3 years) H. pylori positivity was non-significantly lower among patients with allergy (n=122) compared to individuals without allergy (n=98): 13.9% (17/122) vs. 22.4% (22/98); p=0.106. In logistic regression analysis presence of allergy was significantly associated with family history of allergy (odds ratio [OR], 8.038; 95% confidence interval [CI], 4.067-15.886; p<0.0001), delivery by Caesarean section (OR, 2.980; 95% CI, 1.300-6.831; p=0.009), exclusive breast feeding for five months (OR, 2.601; 95% CI, 1.316-5.142; p=0.006), antibacterial treatment during the previous year (OR, 2.381; 95% CI, 1.186-4.782; p=0.015). Conclusion: Prevalence of H. pylori infection did not differ significantly between children with and without allergy. Significant association of allergy with delivery by Caesarean section and antibacterial therapy possibly suggests the role of gastrointestinal flora in the development of allergy, while association with family history of allergy indicates the importance of genetic factors in the arise of allergy.

바닥분수에서 물놀이를 하는 아동들의 Legionella균 감염 위해도 분석 (Quantitative Microbial Risk Assessment on Legionella Infection in Children Playing in Interactive Fountains)

  • 최근 국내의 각종 시설들에서 경관과 여름철 물놀이를 위한 바닥분수 설치가 선호되어, 그 수가 연간 50% 이상 급증하고 있다. 본 연구는, 정량적 미생물학적 위해성 평가 방법으로, 아동이 바닥분수에서 물놀이를 하는 동안 Legionella spp.에 의한 감염 위해도가 어느 정도인지 정량 하였다. 용수 중 Legionella균의 농도를 $10^{-1}-10^6$ CFU/L로 상정하고, 공기 중으로 에어로졸과 함께 비산하게 되는 Legionella균의 농도와 노출시간, 호흡률의 분포, 폐까지 도달하는 에어로졸의 비율을 곱하여 아동들의 노출량을 산정하고, Legionella pneumophila의 위해도를 도출하는 것으로 알려진 지수함수 모델에 따라, 감염 위해도로 변환하였다. 최고 위험군은 뛰어다니면서 물놀이를 하는 신체가 큰, 혹은 연령이 높은 아동이었으며, 이들이 $10^4$ CFU/L 이상의 Legionella균이 존재하는 분수 용수를 1회 이상 이용 시 0.05 이상의 위해도가 발생하였다. 이 결과에 따르면, $10^3$ CFU/L 미만의 Legionella균 농도를 공중이용시설의 모든 배관용수에 대하여 허용하는 현행 질병관리본부의 레지오넬라증 관리지침은 타당한 것으로 판정된다. 그러나, 이 기준은 건강한 성인 대상으로는 적합하나 아동의 면역력이 낮은 점을 감안하지 않았으므로, 아동에게 적합한 감염위해도 모델을 연구하여, 보다 철저한 관리 기준을 적용할 필요성이 있다.

Association between assisted reproductive technology and autism spectrum disorders in Iran: a case-control study

  • Background: Autism spectrum disorder (ASD) is a neurodevelopmental disorder defined by impairments in social interaction and verbal and nonverbal communication. Purpose: Determine the association between use of assisted reproduction technology (ART) and the risk of ASD among children. Methods: This case-control study included 300 participants (100 cases, 200 controls). The control group included women with a child aged 2-10 years without ASD, while the cases were women with a child aged 2-10 years with ASD. We used a researcher-made questionnaire. Data were analyzed using Stata ver. 14 at the 0.05 significance level. Results: In the univariate analysis, there was significant association between child sex, delivery mode, history of preterm delivery, history of using ART, and maternal age at child's birth and the risk of ASD. After the adjustment for other variables, this association was significant for male sex (2.66; 95% confidence interval [CI], 1.11-4.31; P=0.001) and history of using ART (4.03; 95% CI, 1.76-9.21; P=0.001). Therefore, after the adjustment for confounder variables, there was no significant association between ART and the risk of ASD among children (4.98; 95% CI, 0.91-27.30; P=0.065). Conclusion: After the adjustment for other variables, risk factors for ASD were male sex and history preterm delivery. Thus, there was no significant association between ART and the risk of ASD among children.

Investigation of the Factors Affecting Bone Mineral Density in Children with Celiac Disease

  • Purpose: Children with celiac disease (CD) are at an increased risk of low bone mineral density (BMD) owing to malabsorption of fat-soluble vitamins, inflammation, and malnutrition. This study aimed to determine the prevalence and risk factors for low BMD in Iranian children with CD. Methods: This prospective cohort study examined 149 Iranian children with CD between 2011 and 2018 at Zabol University of Medical Sciences. BMD was measured using dual-energy X-ray absorptiometry. Demographic, clinical, and laboratory data were collected from patients' medical records. Logistic regression analysis was performed to identify the factors associated with low areal BMD (BMD-Z <-2) in the lumbar spine and femoral neck. Descriptive data were analyzed using the mean, standard deviation, and relative frequency. Data were analyzed using the chi-square test, t-test, and analysis of variance. Results: Of the 149 children with CD, 27.5% had osteoporosis. The mean body mass index (BMI) Z score was -1.28±1.2. Lower BMI was associated with a higher likelihood of BMD-Z (odds ratio 2.17; p≤0.05). Conclusion: Overall, the findings of this study showed that there was no correlation among Marsh classification, presence of specific human leukocyte antigens, and low BMD in Iranian children with CD. BMI can be a predictor of bone density in children with CD and may be applied clinically in early screenings to evaluate the bone health status in these children.

Risk Factors Associated with Emotional and Conduct Behavior Problems of UK Female Adolescents: Comparative Exploration between Non-Stepfamilies and Stepfamilies

  • Adolescents from stepfamilies have been considered as at-risk group of mental and behavioral problems. This study aims to determine whether there is a group difference of emotional or behavior disturbance between young people in step and non-stepfamilies and whether risk factors linked to emotional and behavior symptoms of young people in stepfamilies are different from those of girls in non-stepfamilies. The study used 'the Mental Health of Children and Young People in Great Britain, 2004 (MHCYP 2004)'. In the MHCYP 2004, the Rutter Problem Behavior Questionnaire, the Strengths and Difficulties Questionnaire (SDQ), and Child Behavior Checklist (CBCL) were administered to parents, teachers, and children drawn from young people aged 5-17 living in private households in England, Scotland (including the Highlands and Islands) and Wales. The total of 2,471 samples of aged 13 through 17 from them were selected for this study. The selected data was analyzed using SPSS. The findings showed group differences in emotional and behavior symptoms between young people in step and non-stepfamilies: Young people from stepfamilies were more emotional and have behavioral disturbances than those from nonstepfamilies. The result also confirmed the different risk factors associated with emotional behavior problems. The study supports that professionals need to develop effective treatment and preventive approaches designed for young people in stepfamilies who may have different risk factors different from those in non-stepfamilies.

성동구내 어린이집 어린이의 알레르기 질환과 허약아의 실태 및 관련성에 대한 보고 (The Relationships between Allergy and Weakness in Children in Seong-Dong District)

  • Objectives The purpose of this study is to find the relationships between the allergy and weakness in children in Seong-Dong district. Methods 802 day care centers children were participated for survey on allergic disease, risk factors, and weakness. From the result, we analyzed the relationships between the aforementioned factors. Results 1. 802 children were participated for the survey. We eliminated survey answers with unclear answers, and selected 792 survey sheets as our final subject of study. The age of majority of subjects(720 children) were between one to six years old, and the others were under the age of one and over seven. 2. The prevalence rate of asthma in a lifetime was 6.69%, allergic rhinitis was 14.87%, atopic dermatitis was 28.82%, allergic conjunctivitis was 6.29%, food allergies was 9.58%, and drug allergies was 1.4%. In 2009, the prevalence rate of allergies during treatments is 6.69% for asthma, 13.41% for allergic rhinitis, 13.64% for atopic dermatitis, 4.13% for allergic conjunctivitis, 5.23% for food allergies, 0.96% for drug allergies. 3. The significant risk factors were types of dwelling, pet with fur, dirty environments without vacuum cleaners, the numbers of times bed covers were washed, frequency of air ventilation, changes in body after moving, family history. 4. The distribution of the number of children with weakness within the five visceras were respiratory weakness, digestive weakness, psychoneurologic weakness, neuromotor weakness, and urogenital weakness in the order of most to the least. More specifically, children who only has respiratory weakness were the majority, while children who have respiratory and digestive weaknesses, children who have respiratory, digestive, and psychoneurologic weaknesses, children who have respiratory and psychoneurologic were followed. 5. With the exception of children with urogenital weakness, Most of the children with respiratory weakness, digestive weakness, psychoneurologic weakness, and neuromotor weakness had been treated for allergies in 2009, while children who were diagnosed with allergic diseases at least once in their lifetime most likely had weak respiratory, digestive or psychoneurologic systems. Conclusions The results of this study showed that there was a correlation between children with weak viscera and children with allergic diseases. However, further research would be necessary.
