의료급여수급권자의 사례관리 요구측정도구의 타당도와 신뢰도 검증 (Validation of a Needs Assessment Tool for Case Management in Korean Medical Aid Beneficiaries)

  • Purpose: The purpose of this study was to validate the Needs Assessment Tool for Case Management (NATCM) for use with Korean medical aid beneficiaries. Methods: Psychometric testing was performed with a sample of 645 Korean medical aid beneficiaries, which included 41 beneficiaries who were selected using proportional sampling method, to examine intraclass correlation coefficients (ICC). Data were evaluated using item analyses, exploratory factor analysis (EFA), confirmatory factor analysis (CFA), Cronbach's alpha, and ICC. Results: Through psychometric testing the final version of NATCM was found to consist of two subscales: 1) Appropriateness of Health Care Utilization (5 items) and 2) Self Care Ability (6 items). The two subscale model was validated by CFA (RMSEA=.08, GFI=.97, and CFI=.93). Internal consistency measured by Cronbach's alpha was .82, and subscale reliability ranged from .79 to .84. The ICC of the NATCM between case managers was .73 and between case managers and health care professionals. .82. Conclusion: This study suggests that the final version of NATCM is a brief, reliable, and valid instrument to measure needs of Korean medical aid beneficiaries. Therefore, the NATCM can be effectively utilized as an important needs assessment as well as outcome evaluation tool for case management programs in Korea.

성당연계 가정간호사의 가정간호 서비스 필요도와 유헬스케어 인식정도 (The Need for Home Care Services and Awareness of U-healthcare in Nurses of the Catholic Parish Home Care Center)

  • Purpose: We analyzed the need for home care services and awareness of U-healthcare in nurses of the Catholic Parish Home Care Center to link the ubiquitous health care system and the home care. Methods: We recruited 46 nurses from a home care center in the catholic parish of the C medical center from April 4th to June 8th, 2007. Results: The highest needs were 'assessment and diagnosis of the problem', 'management of hypertension & diabetes patients', 'counseling of the patient', and 'counseling of major caregiver and family problems'. Therapeutic nursing showed the highest needs in bedsore care. Nurses want hospital medical records available through the ubiquitous health care system. Conclusion: Home care services are supported from the agency, with high needs in assessing and diagnosing the problem, counseling, and managing of hypertension & diabetes patients. Education and public relations efforts on the U-healthcare system should improve system awareness.

중년여성의 보건교육 요구도에 관한 연구 (A Study on the Health Education Needs-Assessment of Middle Aged Women)

  • This study has been attempted to measure health education needs-assessment of middle-aged women and to analyze the factors affecting health education needs-assessment. There were 618 subjects(middle-aged women, 40 to 59 years of age). The major findings of the study were as follows: 1. Seventy six point six percent of the respondents wanted health education. The remainder did not want health education because of the lack of the time. 2. For those desiring to receive health education, there was statistically significant difference in education(p〈0.001), income(p〈0.05), employment(p〈0.05), recreation(p〈0.001), knowledge on health(p〈0.001), routine medical check-ups(p〈0.01) and health education experience(p〈0.001). 3. Forty nine percent of the respondents wanted formal education as a way of education and 41.7% wanted life-long education center for the place of education. Sixty two point five percent of responded that ‘once a month’ is adequate for the health education, and 62.7% felt that ‘around one hour’ duration is appropriate. 4. The area the respondents were most interested in was disease control especially in the order of cancer and osteoporosis. 5. There was statistically significant difference between the age group of 40's and 50's in personal health care and environmental health area(p〈0.01), weight control area(p〈0.01), mental health and exercise area(p〈0.05), drinking and smoking area(p〈0.01). 6. The more knowledge on health, the higher health education needs-assessment. Middleaged women who received routine medical check-ups had more health education needs-assessments than those who didn't.

Educational Status and Needs of Premature Birth Prevention and Its Association with Preconception Health Behavior among Women of Childbearing Age in Korea

  • Purpose: The aim of this study was to investigate the educational status and needs of premature birth prevention, and to identify factors associated with preconception health behaviors. Methods: The study design was a crosssectional descriptive study. Data were collected through an online questionnaire survey, and the subjects were 192 women of childbearing age in Korea. Data were analyzed using the Mann-Whitney U test, Kruskal-Wallis test, Spearman's correlation coefficients, and multiple regression. Results: The proportion of subjects who received education on premature birth prevention was 8.9%, and 75.5% of subjects answered that they needed education on premature birth prevention. They demanded education through online media, small groups, cases, cartoons (webtoon) with stories, pictures, and videos. A related factor of preconception health behavior was self-efficacy for high-risk pregnancy health care (β=.20, p=.012), which accounted for 8.2% of the total variance related to preconception health behavior. Conclusion: There was a need for more development of education programs to prevent premature birth for women of childbearing age. Its education programs should be applied with online, small group activities using various educational media. It is also required to promote preconception health behavior through self-efficacy for high-risk pregnancy health care.

동티모르 에르메라 지역의 모성보건사업 요구 분석 (Needs assessment for maternal health care in Ermera, Timor-Leste)

  • Objectives: The purpose of this study is to obtain the basic data for the development of maternal health care by analyzing the status and needs of service target persons in Timor-Leste. Methods: The subjects were selected through the non-probability sampling method applying the FGI. Researchers interviewed 3 maternal health service managers, 6 midwives at Gleno and Railaco Health Centers and 2 women between 15 and 45 years of age. Results: In the results of on-site visit of the delivery facility and the FGI, we found poor sanitation in delivery room, lack of medical equipment related to antenatal consultation and delivery. In the case of the health center manager, the public health center provides various maternity health services, but the lack of the staff has difficulty in providing the service and managing the subjects. Midwives asked for regular maintenance education. Women in child bearing age living in mountainous areas had poor access to delivery facilities and lack of awareness of delivery services. Conclusions: It is necessary to increase the maternity management rate through regular maternity and maternal health check service and application to maternal management database, to improve the sanitation of the maternity clinic in the public health center, to strengthen the midwife competency program.

뇌손상 환자의 장애정도와 간병가족의 교육요구도 (Brain Injury Patients's Disability Degree and Educational Needs of Family Caregivers)

  • Purpose: This study was conducted in order to identify the brain injury patients's disability degree and educational needs of family caregivers. Methods: A convenience sample of 94 families with brain injury patients, who have been receiving treatment at the neurological intensive care unit and neurosurgery ward, were used. Data was collected with a self-report questionnaire from September 5 to November 28, 2011, and was analyzed using SAS program. Results: 'Defecation/urination' disability was the highest score of patient's physical disability and the next ranking was 'paralysis'. 'Memory impairment' disability was the highest score of patient's cognitive disability, and the next ranking was 'personality changes'. Overall, educational needs of family caregivers scored 4.15 out of the perfect score of 5. The factor, which scored highest, was 'information related with disease'. In addition, educational needs of family caregivers were positively related with patient's degree of. Conclusion: Educational needs of family caregivers are distinct, according to the disability degree of brain injury patient. Therefore, the study suggests the development of individualized educational program for family with brain injury patient.

장기입원 의료급여 환자의 재원일수에 미치는 영향요인: 요양병원 입원유형 중심으로 (Factors Affecting the Length of Stay of Long-Stay Medical Aid Inpatients in Korea: Focused on Hospitalization Types in Long-Term Care Hospitals)

  • Background: In Korea, the length of stay and medical expenses incurred by medical aid patients are increasing at a rate faster than the national health insurance. Therefore, there is a need to create a management strategy for each type of hospitalization to manage the length of stay of medical aid patients. Methods: The study used data from the 2019 National Health Insurance Claims. We analyzed the factors that affect the length of stay for 186,576 medical aid patients who were hospitalized for more than 31 days, with a focus on the type of hospitalization in long-term care hospitals. Results: The study found a significant correlation between gender, age, medical aid type, chronic disease ratio, long-term care hospital patient classification, and hospitalization type variables as factors that affect the length of hospital stay. The analysis of the differences in the length of stay for each type of hospitalization showed that the average length of stay is 291.4 days for type 1, 192.9 days for type 2, and 157.0 days for type 3, and that the difference is significant (p<0.0001). When type 3 was 0, type 1 significantly increased by 99.4 days, and type 2 by 36.6 days (p<0.0001). Conclusion: A model that can comprehensively view factors, such as provider factors and institutional factors, needs to be designed. In addition, to reduce long stays for medical aid patients, a mechanism to establish an early discharge plan should be prepared and concerns about underutilization should be simultaneously addressed.

시뮬레이션 기반 허약노인 방문간호 교육 요구도 (Educational needs for the development of a simulation module of home visiting care for the frail elderly)

  • Purpose: This study aimed to identify the educational needs of nurses and nursing students for the development of a simulation module of home visiting care for frail, elderly people. Methods: Focus group interviews were conducted with 15 home visiting nurses working in public health centers and 14 nursing students who experienced home visiting from September 10 to October 10, 2018. Results: Bloom's taxonomy of learning objectives, namely, cognitive, affective, and psychomotor domains was used as a framework for data analysis. The defined educational needs for each domain were as follows: "understanding frail, elderly people" for the cognitive domain; "intervention for mental health" and "building a therapeutic relationship" for the affective domain; and "nursing skills", "health education for healthy lifestyles", "referral to the community resource connection", "protection for visiting nurses" for the psychomotor domain. Conclusion: Based on the findings of this study, a simulation module of home visiting care for frail, elderly people can be developed and used for nursing students and nurses to strengthen the capacity for home visiting care.

보건교사의 수업연수에 대한 요구도 조사 (Needs Assessment of In-service Education on Teaching for School Health Teachers)

  • 이 연구의 목적은 보건교사들의 수업연수에 대한 요구도를 조사하기 위한 것이다. 연구대상자는 G시와 J도에 근무하고 있는 504명의 보건교사였다. 보건교사들의 수업에 대한 질문지가 2013년 11월 9일부터 23일까지 배부되었으며 수집된 자료는 빈도분석과 카이자승 검증을 통해 분석되었다. 연구 결과는 다음과 같다. 첫째, 보건교사 업무의 우선순위 1위는 건강 문제 관리였으며, 2위가 보건교육이었으며, 이 결과는 학교급에 따른 차이를 보였다(p<.01). 두 번째, 보건수업 시간에 주로 사용하는 수업방법은 강의, 토의 그리고 실습의 순서로 나타났다. 세 번째, 보건수업 시간에 주로 사용하는 수업자료는 PPT, 교과서, 인터넷동영상의 순으로 나타났다. 네 번째, 대부분의 보건교사는 창의성 교육의 중요성에 동의하나 실제 사용하고 있는 비율은 낮은 것으로 나타났다. 다섯 번째, 대부분의 보건교사들은 수업연수를 원하고 있으며 그 내용으로 창의성 교육 또는 새로운 수업을 위한 스토리텔링 등을 희망하는 것으로 나타났다.

사용자 중심설계를 이용한 임산모 건강관리 교육프로그램 개발 연구 (Development of a Health Educational Program for Maternal Health)

  • Purpose: The purpose of this paper is to describe the development of health educational program for maternity using principle of user centered design. Method: Research process includes five distinct phases: needs assessment, analysis, design, development/testing/revision, and application release. Results: This program includes an introduction, pregnancy test and fetal assessment, maternal and fetal change during pregnancy, self-care methods during pregnancy, complication of postpartum period, process of labor, complication of pregnancy, self care during the postpartum period, information for parenting skill, father page, FAQ, helping resources and institutions. Conclusion: The program will be distributed to health centers for maternal health education. The second phase of this project was evaluation this program for further development of the program. The end result of this program will be pregnant women with a high degree of usability. Author believe this program have true potential in helping maternal health promotion and successful parenting.
